Evaluation of the Remineralization Capacity of Water-based Silver Fluoride

  • Silver diamine fluoride, which can arrest dental caries, is alkaline and may cause mild soft tissue irritation. Water-based silver fluoride has a neutral pH, which is closer to the physiological range, and is biocompatible for use in the oral environment. This study aimed to evaluate the effect of water-based silver fluoride on remineralizing early enamel lesions by comparing it with other fluoride agents through microhardness and quantitative light-induced fluorescence measurements. An in vitro study with intact bovine incisors was performed. Artificial enamel lesions were induced and subjected to microhardness and quantitative light-induced fluorescence testing. Specimens were randomly divided into 4 groups for treatment. The specimens in group I were treated with water-based silver fluoride and potassium iodide, group II with silver diamine fluoride and potassium iodide, group III with sodium fluoride varnish, and group IV with distilled water. After 8 days of pH cycling, the specimens were subjected to microhardness and quantitative light-induced fluorescence testing. Water-based silver fluoride and silver diamine fluoride showed the greatest increases in microhardness and quantitative light-induced fluorescence, with no significant differences between the two. Sodium fluoride varnish also exhibited a significant increase in microhardness and quantitative light-induced fluorescence, but the differences were smaller than those for water-based silver fluoride and silver diamine fluoride. Water-based silver fluoride is considered useful in a clinical setting for remineralizing enamel lesions, with the advantages of no risk of tissue burn and improved taste and smell.

  • Recently, there have been improvements in diagnostic methods for the assessment of early caries lesions. The reason is that dental professionals are seeking methods to reliably detect incipient dental caries and to remineralize them. This review examines the literature on principles, theoretical background, and history of the Quantitative Light-Induced Fluorescence (QLF) system (Inspektor Research Systems BV, The Netherlands). Furthermore, this paper discusses the potential application of QLF system to clinical practice for educational purpose, enabling dental hygiene students to perform oral health assessment using the QLF system. In addition, the clinical application of QLF system can motivate patients by providing additional visual information about caries and bacterial activity. The evidences on validity and reliability of the QLF system for detection of longitudinal changes in de/remineralization and caries were examined. The QLF system is capable of monitoring and quantifying mineral changes in early caries lesions. Therefore, it can be used to assess the impacts of caries preventive measures on the remineralization and reversal of the caries process. And the QLF system is a very promising equipment to assess educational effectiveness for dental hygiene students in their learning process. In conclusion, the QLF system is the most effective technology for more sensitive staging of caries and treatment without surgical intervention.

Detection of proximal caries using quantitative light-induced fluorescence-digital and laser fluorescence: a comparative study

  • PURPOSE. The purpose of this study was to evaluate the in vitro validity of quantitative light-induced fluorescence-digital (QLF-D) and laser fluorescence (DIAGNOdent) for assessing proximal caries in extracted premolars, using digital radiography as reference method. MATERIALS AND METHODS. A total of 102 extracted premolars with similar lengths and shapes were used. A single operator conducted all the examinations using three different detection methods (bitewing radiography, QLF-D, and DIAGNOdent). The bitewing x-ray scale, QLF-D fluorescence loss (${\Delta}F$), and DIAGNOdent peak readings were compared and statistically analyzed. RESULTS. Each method showed an excellent reliability. The correlation coefficient between bitewing radiography and QLF-D, DIAGNOdent were -0.644 and 0.448, respectively, while the value between QLF-D and DIAGNOdent was -0.382. The kappa statistics for bitewing radiography and QLF-D had a higher diagnosis consensus than those for bitewing radiography and DIAGNOdent. The QLF-D was moderately to highly accurate (AUC = 0.753 - 0.908), while DIAGNOdent was moderately to less accurate (AUC = 0.622 - 0.784). All detection methods showed statistically significant correlation and high correlation between the bitewing radiography and QLF-D. CONCLUSION. QLF-D was found to be a valid and reliable alternative diagnostic method to digital bitewing radiography for in vitro detection of proximal caries.

Comparison of Clinical Characteristics of Fluorescence in Quantitative Light-Induced Fluorescence Images according to the Maturation Level of Dental Plaque

  • Background: Proper detection and management of dental plaque are essential for individual oral health. We aimed to evaluate the maturation level of dental plaque using a two-tone disclosing agent and to compare it with the fluorescence of dental plaque on the quantitative light-induced fluorescence (QLF) image to obtain primary data for the development of a new dental plaque scoring system. Methods: Twenty-eight subjects who consented to participate after understanding the purpose of the study were screened. The images of the anterior teeth were obtained using the QLF device. Subsequently, dental plaque was stained with a two-tone disclosing solution and a photograph was obtained with a digital single-lens reflex (DSLR) camera. The staining scores were assigned as follows: 0 for no staining, 1 for pink staining, and 2 for blue staining. The marked points on the DSLR images were selected for RGB color analysis. The relationship between dental plaque maturation and the red/green (R/G) ratio was evaluated using Spearman's rank correlation. Additionally, different red fluorescence values according to dental plaque accumulation were assessed using one-way analysis of variance followed by Scheffe's post-hoc test to identify statistically significant differences between the groups. Results: A comparison of the intensity of red fluorescence according to the maturation of the two-tone stained dental plaque confirmed that R/G ratio was higher in the QLF images with dental plaque maturation (p<0.001). Correlation analysis between the stained dental plaque and the red fluorescence intensity in the QLF image confirmed an excellent positive correlation (p<0.001). Conclusion: A new plaque scoring system can be developed based on the results of the present study. In addition, these study results may also help in dental plaque management in the clinical setting.

  • Planar laser induced fluorescence(PLIF) has been widely used to obtain two dimensional fuel distribution. Preliminary investigation was performed to measure quantitative air excess ratio distribution in an engine fueled with LPG. It is known that fluorescence signal from acetone as a fluorescent tracer is less sensitive to oxygen quenching than other dopants. Acetone was excited by KrF excimer laser (248nm) and its fluorescence image was acquired by ICCD camera with a cut-of filter to suppress Mie scattering from the laser light. For the purpose of quantifying PLIF signal, an image processing method including the correction of laser sheet beam profile was suggested. Raw images were divided by each intensity of laser energy and profile of laser sheet beam. Inhomogeneous fluorescence images scaled with the reference data, which was taken by a calibration process, were converted to air excess ratio distribution. This investigation showed instantaneous quantitative measurement of planar air excess ratio distribution for gaseous fuel.

Application of quantitative light-induced fluorescence to determine the depth of demineralization of dental fluorosis in enamel microabrasion: a case report

  • Enamel microabrasion has become accepted as a conservative, nonrestorative method of removing intrinsic and superficial dysmineralization defects from dental fluorosis, restoring esthetics with minimal loss of enamel. However, it can be difficult to determine if restoration is necessary in dental fluorosis, because the lesion depth is often not easily recognized. This case report presents a method for analysis of enamel hypoplasia that uses quantitative light-induced fluorescence (QLF) followed by a combination of enamel microabrasion with carbamide peroxide home bleaching. We describe the utility of QLF when selecting a conservative treatment plan and confirming treatment efficacy. In this case, the treatment plan was based on QLF analysis, and the selected combination treatment of microabrasion and bleaching had good results.

유치 및 영구치에서 QLF를 이용한 항우식 제품의 재광화 효과 비교 (Evaluation of Remineralization Effects on Enamel Demineralization by Anti-cariogenic Agents using Quantitative Light-induced Fluorescence-digital (QLF-D) in vitro)

  • 본 연구의 목적은 quantitative light-induced fluorescence-digital(QLF-D) 시스템을 이용하여 유치와 영구치에 형성된 탈회된 법랑질에 서로 다른 세 종류의 항우식 제품 적용 후 각각의 재광화 효과를 비교하는 데 있다. 연구를 위해 각 48개의 건전한 유치 및 영구치를 대상으로 대조군(1군), 불소 함유 치약(2군), functionalized tricalcium phosphate(fTCP) + 불소 함유 치약(3군), casein phosphopeptide-amorphous calcium phosphate(CPP-ACP) 함유 치아 크림(4군)으로 무작위 분류하였다. 유치 및 영구치에 법랑질 탈회를 유발한 후, 14일 동안 1일 2회 항우식 제품을 처리하였으며, quantitative light-induced fluorescence(QLF)를 이용하여 재광화 효과를 비교 분석하였다. 연구 결과, 세 종류의 항우식 제품 모두 유치와 영구치에서 대조군인 1군에 비해 우식 병소의 무기질 소실량을 유의하게 감소시켰으며, 치약 제품 간 비교에서는 3군과 4군에서 2군에 비해 유의하게 높은 재광화 효과를 나타냈다. 이를 통하여 fTCP와 불소 함유 치약 및 CPP-ACP 함유치아 크림이 유치와 영구치 모두에서 우수한 재광화 효과를 보인다는 것과, QLF-D가 초기 우식 병소와 항우식 제제의 재광화 효과를 정량적으로 평가함에 있어 유용한 장비임을 알 수 있다. 그러므로 임상가들은 유치와 영구치에서의 탈회 및 재광화 평가 시 QLF-D 시스템의 임상적 적용을 고려할 수 있을 것이다.

Quantitative Light-induced Fluorescence를 이용한 법랑질 탈회 속도에 관한 비교 연구 (COMPARATIVE STUDY ON THE RATE OF DENTAL ENAMEL DEMINERALIZATION USING A QLF)

  • 치아 우식증을 조기에 발견하여 기존의 외과적 와동 형성을 피하고 건전한 치질을 최대한 보존하고자하는 예방적 시술 개념에 대한 관심과 연구가 최근 유럽을 중심으로 확산되고 있다. 치아 우식증의 임상적 조기 진단을 위해 새로운 장비들이 개발되고 있으며, 이미 실험실 연구를 통해 성능의 우수성이 입증된 바 있다. 본 연구의 목적은 치아 우식증의 조기 진단을 목적으로 새로 개발된 quantitative light induced fluorescence analysis (QLF) 시스템을 이용하여 우치, 사람의 유치 및 영구치의 법랑질 탈회 속도를 비교 평가하고자 하였다. 각 군당 25개씩 배정된 실험 대상 치아들은 표면을 약 $50{\sim}100{\mu}m$정도 제거한 후 활택하여 탈회 용액(0.1mol/L 젖산, 0.2% 카보폴 907 그리고 50%로 농축된 수산화 인회석으로 제조)에서 1, 2, 4 그리고 8일 동안 탈회시켰다. 모든 시편들은 QLF분석을 시행하였으며 실험 일정에 따라 5개의 시편들을 선택하여 gold standard로 사용하기 위해 Transverse Microradiography analysis (TMR)를 시행하였다. TMR 분석을 통하여 무기질의 탈회량(IML)과 병소 깊이(LD)를 측정하였다. QLF 분석에서 측정된 정상 법랑질의 fluorescence radiance (FRs) 값에 대한 탈회 법랑질의 fluorescence radiance $(FR_D)$의 비율인 FR ratio $(FR_D/FR_s)$를 산출하였다. 우치에서는 FR Ratio와 병소 깊이 간에 통계적 유의성이 있었으며(p<0.05), 유치에서는 FR Ratio와 광물질 소실 및 병소 깊이 간에는 유의성이 없었다(p>0.05). 영구치에서는 FR Ratio와 광물질 소실 및 병소 깊이 간에 통계적 유의성이 있었다(p<0.05). 탈회 시간에 대한 FR Ratio의 회귀 분석 결과에서 얻은 회귀 계수 값은 우치에서는 -4.643(p<0.05), 유치에서는 -5.421(p<0,05), 영구치에서 -4.435(p<0.05)로 나타났다.

초기 우식 병소에서 광원에 따른 형광효과 비교 (A COMPARISON OF FLUORESCENCE EFFECT OF VARIOUS LIGHT SOURCES IN EARLY ENAMEL CARIES)

  • 본 연구의 목적은 초기 우식 병소에서 광원에 따른 형광효과를 비교하는 것으로 정량 분석형 laser/light 형광법(Quantitative laser/light-induced fluorescence, QLF)을 이용하여 아르곤 레이저광, 할로겐광, 발광다이오드광(LED), 플라즈마광의 형광효과를 비교하였다. 발거된 60개의 치아를 선정한 후, 인공우식용액에 노출되지 않을 부분에 nail varnish를 도포하였고 치아는 24, 48, 72시간 동안 인공우식용액에 보관되었다. 건조 후 QLF영상으로 초기 우식 부위의 광밀도 차이에 의한 탈회정도가 기록되었다. 또한 조영제를 이용하여 동일한 방법으로 실험하였고 다음과 같은 결과를 얻었다 염료를 이용하여 같은 방법으로 실험하여 또 다른 결과를 얻었다. 1. 각 군의 평균 광밀도를 볼 때, 플라즈마광이 세 군 모두에서 다른 광원들 보다 높았다(p<0.05). 2. 세 군 사이에서 평균 광밀도를 비교해 볼 때, 플라즈마광과 할로겐광이 차이를 보였다(p<0.05). 3. 조영제를 사용하였을 경우 평균 광밀도를 비교해 볼 때, 플라즈마광이 세 군 모두에서 다른 광원들보다 높았고 (p<0.05), 발광다이오드광(LED)와 아르곤 레이저광을 제외한 모든 광원이 차이를 보였다(p<0.05). 4. 조영제를 사용하지 않은 경우와 사용한 경우 평균 광밀도를 비교해 볼 때, 사용한 경우에 모든 광원의 평균 광밀도 차이가 컸다.
