Quality of Pediatric Nursing Care: Concept Analysis (아동간호의 질에 대한 개념분석)

  • Purpose: This paper is a report of a concept analysis of 'quality of pediatric nursing care', Methods: Rodgers's evolutionary method of concept analysis was used. Data were collected from published literature related to quality of pediatric nursing care. Results: Quality of pediatric nursing care was identified with three dimensions and seven attributes: 1) nurse's character: technical competence, interpersonal competence, 2) nurse's activities: developmentally appropriate care, attentiveness, entertainment, 3) nurse-parent interaction: nurse-parent partnership, emotional support. Antecedents of quality of pediatric nursing care were 'child and parent's expectation about pediatric nursing care', 'previous caring experience of pediatric nurses'. Consequences of quality of pediatric nursing care were 'meet child and parent's needs' and 'better health outcomes.' Conclusion: 'Quality of pediatric nursing care' is a core concept in pediatric nursing practice. Appropriate instruments to operationalize the concept need to be developed.

Comparison of the Quality of Nursing Care as Perceived by Pediatric Nurses and Mothers of Hospitalized Children (아동병동 간호사와 입원아동 어머니가 인식한 간호의 질 비교)

  • Purpose: This study aimed to compare the quality of nursing care as perceived by registered nurses and mothers of hospitalized children in South Korea. Methods: This was a descriptive study that recruited 70 mothers of hospitalized children and 70 nurses in pediatric units in university hospitals as participants. The quality of pediatric nursing care was measured using importance and performance scores for 19 items describing various elements of nursing care. The collected data were analyzed using descriptive statistics and the independent t-test. Results: The hospitalized children mothers'mean importance scores were significantly higher than those of the nurses (t=2.94, p=.004). However, there were no significant differences in the mean performance scores of nurses and mothers (t=0.91, p=.363) or between nurses'and mothers'perceptions of quality of nursing care, with the exception of a significant difference for the quality of explanations (t=2.78, p=.006). The quality of explanations was assessed more positively by nurses than by mothers. Conclusion: This study suggests that when developing strategies to improve the quality of nursing care in pediatric wards, ensuring that pediatric nurses provide detailed explanations should be considered as a way to improve the quality of nursing care in pediatric units.

Levels of Partnership between Nurses and Parents of Hospitalized Children and the Quality of Pediatric Nursing Care as Perceived by Nurses

  • Purpose: This study investigated the relationship of the partnership between nurses and parents of hospitalized children with the quality of pediatric nursing care as perceived by pediatric nurses. Methods: A cross-sectional descriptive study was conducted among 90 nurses. The data collected were analyzed using the independent t-test and the Pearson correlation coefficient. Results: The level of partnership between nurses and parents showed significant differences by nurses' age (t=2.22, p=.029) and marital status (t=2.54, p=.013). The quality of pediatric nursing care was found to significantly differ by the nurses' age (t=2.62, p=.013), marital status (t=4.17, p<.001), whether nurses had children (t=2.53, p=.020), pediatric ward work experience (t=2.43, p=.020), and total work experience (t=2.51, p=.016). The level of partnership between nurses and parents was positively correlated with the quality of pediatric nursing care (r=.48, p<.001). Conclusion: To improve the quality of nursing care for hospitalized children and their parents, it is important to consider the specific characteristics of pediatric nurses, whose clinical skills should be improved.

A Survey of the Nursing Interventions Performed by Pediatric Care Unit Nurses (국내 아동간호단위 간호중재 분석)

  • The purpose of this study was to identify nursing interventions performed by pediatric care unit nurses. For data collection this study used the taxonomy of Nursing Intervention Classification (NIC : 486 nursing intervention) which was modified by McCloskey & Bulecheck(2000). The 419 nursing interventions were selected by panel group, which consist of pediatric clinical experts and nursing scholars. The data were collected 104 nurses of pediatric care unit. There were 158 nursing interventions identified as being used at least monthly 50% or more of the nurses. The 32 nursing interventions were used at least daily, indication a set of core interventions unique to pediatric care unit practice. The most frequently used nursing interventions were 'Medication administration: intravenous' & 'Medication administration: oral'. The 27 nursing interventions were rarely performed by 90% or more of the nurses. The rarely used interventions were 'Ostomy care' & 'Rectal prolaps management'. The domain received the highest mean score was Health System, followed by Physiolocal: complex, Physiolocal: Basic, Safty, Community, Family, Behavior domain. These findings will help in building of a standardized language for the pediatric care units and enhance the quality of nursing care.

An Evolutionary Concept Analysis of Pediatric Hospice and Palliative Care

  • Purpose: This study aimed to clarify the concept of pediatric hospice and palliative care through conceptual analysis. It also sought to identify the differences between related concepts such as pediatric death care and pediatric spiritual care, in order to provide foundational data for the development of nursing theory and knowledge. Methods: A conceptual analysis of pediatric hospice and palliative care was conducted using Rodgers' evolutionary method. Out of 5,013 papers identified, 28 were selected for detailed reading and analysis. Results: Pediatric hospice and palliative care encompasses physical, psychological, social, mental, spiritual, and family care for children with acute and chronic diseases with uncertain prognoses ahead of death, as well as their families. Effective pediatric hospice and palliative care will require multidisciplinary team nursing, effective communication, and supportive policies. Conclusion: The findings of this study suggest that providing pediatric hospice and palliative care will lead to improvements in pain relief for children and families, the efficiency of responses to death in children, and the quality of life for children and families. The significance of this study is that it clearly clarifies the concept by analyzing pediatric hospice and palliative care using an evolutionary method.

An Analysis on Clinical Education of Pediatric Nursing (아동간호학 임상실습교육 현황)

  • This study was conducted to analyse the current clinical education of pediatric nursing in baccalaurate nursing program, then to give basic data for enhancing the quality of future clinical education of pediatric nursing. Data were collected through self-reported questionnaire by mail from December 2001 to February 2002. The subjects were 29 schools of 50 baccalaurate nursing education programs. The data were analysed by double raters, researcher and assistant researcher. The results were summarized as follows: 1. Twenty-eight schools had the objectives of the clinical education of pediatric nursing, and 28 schools in pediatric ward, 23 schools in nursery, 22 schools in neonatal intensive care unit(NICU), 15 schools in objectives related to profession by clinical site. 2. Credits on clinical education of pediatric nursing were most 15 schools of 3 credits. 3. The clinical sites were mainly the hospital that sick children were admitted in. 4. The clinical teacher were 9 types including pediatric professor and field nurse. 5. On teacher's role, the professor instructed the case study and conference, and field nurse instructed the patient assignment and nursing procedures. 6. All of schools used explanation and conference as a method of clinical education, 1 or 2 schools used PBL or role play or field study. 7. On clinical education content, most of school included Apgar scoring system, physical examination in newborn assessment, respira- tion maintenance, temperature maintenance, infection prevention, nutrition, and bath in newborn care. 8. On clinical education content, most of school included care of incubator, phototheraty, infusion, gavage feeding and how to use the instruments in NICU. Eighteen schools included attachment promotion, and 20 schools case study. 9. On clinical education content, most of school included a checklist of nursing procedures, case study, assessment of growth and development in pediatric ward and other sites. 10.There were various evaluation types in scores, measuring items. In conclusion, the results of this study revealed that there were some discrepancy in the objectives and contents, clinical sites on hospital focused, teacher's role, and diversity of measurement items and ratings in clinical education of pediatric nursing. There is a need for a standardization of content, clinical site, and evaluation tool to improve a quality of clinical education of pediatric nursing based on this study.

The Effect of Child's Mother and Nurse Partnership on the Anxiety and Perceived Quality of Nursing Care of Hospitalized Child's Mother (일 종합병원 아동병동의 환아 어머니와 간호사의 파트너십이 환아 어머니의 불안과 간호의 질 인식에 미치는 영향)

  • This study was conducted to investigate child's mother and nurse partnership, anxiety, and quality of pediatric nursing care, and to identify factors influencing anxiety and quality of pediatric nursing care of the hospitalized child's mother. A descriptive survey was conducted from September 21 to October 4, 2015. Data were collected using a self-reporting questionnaire of 140 the mothers of a hospitalized child in a general hospital. Completed data were analyzed using descriptive statistics, t-test, ANOVA, and multiple regression analysis by IBM SPSS 18.0. The results showed that the frequency of hospitalization, genito-urinary disease, other diseases, and child's mother and nurse partnership significantly influenced anxiety. Additionally, the frequency of hospitalization, genito-urinary disease, other diseases, and child's mother and nurse partnership significantly influenced the quality of pediatric nursing care. Based on these results, child's mother and nurse partnership had a significant predictor of the anxiety and the quality of pediatric nursing care. These finding showed that there was a need to develop educational and supportive strategies to enhance and improve the partnership between hospitalized child's mother and nurse to ensure qualified pediatric nursing care.

Factors influencing the quality of nursing care as perceived by mothers of hospitalized children in South Korea

  • Purpose: This study aimed to identify the factors affecting the quality of nursing care as perceived by mothers of hospitalized children and provide basic data for the development of nursing care quality improvement programs. Methods: The participants consisted of 167 mothers, each of whom had a child hospitalized at a specific children's hospital. Data were collected from June 22 to August 8, 2019. Results: The factors that affected how mothers perceived the quality of nursing care were the communication ability of nurses (β=.44, p<.001) and the mother-nurse partnership (β=.33, p=.001). The total explanatory power of these factors was 54.1%. Conclusion: To improve the quality of pediatric nursing care, it is necessary to improve the communication abilities of nurses and to promote partnership between nurses and the mothers of hospitalized children.

A Convergence Study on Nursing Needs of Hospitalized Children's Mothers and Quality of Care in Pediatric Unit (입원 아동 어머니의 간호요구와 소아병동 간호의 질에 관한 융합 연구)

  • This study aimed to investigate the nursing needs of hospitalized children's mothers and the quality of care in a pediatric unit. A secondary data analysis was conducted using mixed methods design. The qualitative data from 8 mothers of hospitalized children gathered through an in-depth interview were analyzed to investigate nursing needs. The quantitative data from 194 hospitalized children's mothers were analyzed to examine the quality of care in the pediatric unit using IBM SPSS 24.0. The results of qualitative research were categorized into 4 themes: dignity and respect, explanation, participation and collaboration, and nursing practice. The importance of quality of care, performance, and quality index were $8.76{\pm}1.19$ (0~10), $.30{\pm}.19$ (0~1), and $7.52{\pm}1.62$ (0~10), respectively. The quality of care scores differed according to the stage of development of the children (F=2.75, p=.044), and the score of the quality index of nursing skill varied according to the age of the mother (t=-2.08, p=.039). The results of this study are expected to contribute to the development of strategies to improve the quality of nursing care for children.

Transfer anxiety in parents of children transferred from pediatric intensive care units to general wards in South Korea: a hybrid concept analysis

  • Purpose: This study aimed to analyze the concept of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Methods: The hybrid model by Schwarz-Barcott and Kim was used to analyze the characteristics of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Results: Transfer anxiety was defined by the following attributes: 1) stress concerning the adaptation process, 2) concern about the child's condition worsening due to the parent's caregiving, and 3) involuntary changes in daily life due to the treatment. Transfer anxiety has the following antecedents: 1) uncertainty; 2) a lack of knowledge about the illness, medical devices, and caregiving; and 3) a lack of social support. It resulted in 1) caregiver burden, 2) a decrease in the capacity for coping with caregiving, 3) delays in the child's physical and psychological recovery, and 4) decreased quality of life. Conclusion: It is necessary to develop an assessment scale that considers the attributes of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Furthermore, an effective nursing intervention should be developed to reduce transfer anxiety.
