Relations Between the Types of Communication and, Job Satisfaction and Nursing Performance in Staff Nurses (일반간호사의 조직 내 의사소통 유형과 직무만족 및 간호업무성과의 관계)

  • This study attempts to investigate the relations between the types of communication, the job satisfaction. and the nursing performance of the staff nurses. The subjects of this study were the 114 nurses working in general wards, intensive care units, and the emergency rooms of the general hospital in Kyongbuk province. They were asked to answer the questionnaires as regards the types of communication and job satisfaction, and their performance of nursing, and these were evaluated by the head nurses, and their direct seniors. The data were collected for 10 days from the September 17 to 26, 2002. According to the purpose of the study, descriptive analysis, ANOVA. Pearson correlation coefficient, and stepwise multiple regression were applied for the data analysis using SPSS 10.0. The results are as follows : 1. The types of communication of staff nurses showed that the upward communication was $3.12\pm0.56$. the downward communication was $3.56\pm0.42$. the horizontal communication was $2.98\pm0.44$. and the informal communication was $2.59\pm0.49$. It showed that the most frequent communication was the downward communication. 2. In terms of the job satisfaction, there was significant difference according to the length of the period working for one ward (F=5.95, p=0.003), 3. The performance of nursing was significantly different according to the age (F=23.52, p=0.000), the working duration as a nurses (F=21.9, p=0.000), the types of nursing unit (F=5.33, p=0.002), and the working duration in present nursing unit (F=15.87, p=0.000). 4. In terms of communication type, the job satisfaction was associated with quality of the upward communication (F=6.23, p=0.003), the downward communication (F=5.78, p=0.004), and the horizontal communication (F=11.73, p=0.000) There were no association between the types of communication and the performance of nursing. 5. There was a positive correlation between the communication and the job satisfaction. And the downward communication (r=0.398) showed the greatest correlation with the job satisfaction. There was no statistically significant correlation between the communication and the performance of nursing. And the upward communication (r=-0.058) showed a negative, though not significant correlation with performance of nursing. 5.   • The creativity of thesis is that the significance of cyber security challenges in blockchain. The variety of enterprises, including those in the medical market, are the targets of cyberattacks. Hospitals and clinics are only two examples of medical facilities that are easy targets for cybercriminals, along with IoT-based medical devices like pacemakers. Cyberattacks in the medical field not only put patients' lives in danger but also have the potential to expose private and sensitive information. Reviewing and looking at the present and historical flaws and vulnerabilities in the blockchain-based IoT and medical institutions' equipment is crucial as they are sensitive, relevant, and of a medical character. This study aims to investigate recent and current weaknesses in medical equipment, of blockchain-based IoT, and institutions. Medical security systems are becoming increasingly crucial in blockchain-based IoT medical devices and digital adoption more broadly. It is gaining importance as a standalone medical device. Currently the use of software in medical market is growing exponentially and many countries have already set guidelines for quality control. The achievements of the thesis are medical equipment of blockchain-based IoT no longer exist in a vacuum, thanks to technical improvements and the emergence of electronic health records (EHRs). Increased EHR use among providers, as well as the demand for integration and connection technologies to improve clinical workflow, patient care solutions, and overall hospital operations, will fuel significant growth in the blockchain-based IoT market for linked medical devices. The need for blockchain technology and IoT-based medical device to enhance their health IT infrastructure and design and development techniques will only get louder in the future. Blockchain technology will be essential in the future of cybersecurity, because blockchain technology can be significantly improved with the cybersecurity adoption of IoT devices, i.e., via remote monitoring, reducing waiting time for emergency rooms, track assets, etc. This paper sheds the light on the benefits of the blockchain-based IoT market.

Occupational Therapists' Perception and Communication of the Elderly (작업치료사를 대상으로 한 노인 인식 및 의사소통 현황)

  • Objective : Due to the rapid aging of the country, the elderly population is increasing and the ratio of face-to-face meetings with the elderly in major areas of occupational therapy is high. The purpose of this study was to investigate the overall perception of the elderly and the status of communication with the elderly. Methods : This survey was conducted From April to June 2020. Occupational therapists who provide interventions for the elderly in hospitals and public health centers across the country were collected using online questionnaires. A total of 91 questionnaires were collected through a self-administered questionnaire, of which 82 were used for data analysis, excluding 9 with insufficient responses. Results : Occupational therapists generally had a lot of negative perceptions of the elderly. In addition, as factors that hindered communication with the elderly, they responded in order of hearing-related, language understanding related, and articulation related. Lastly, occupational therapists were found to induce smooth communication with the elderly in the order of conversation response management, emotional expression, understanding enhancement, and relationship control. Conclusions : Occupational therapists are health care professionals who play a major role in improving the subjects' social participation and quality of life. Therefore, it is necessary to provide professional therapeutic interventions and related education using improved communication skills along with an understanding of the elderly's communication skills and their level of communication.

Effects of Koryo Hand Therapy on Menstrual Cramps and Dysmenorrhea in College Women Students (고려수지요법이 여대생의 생리통 및 월경곤란증에 미치는 효과)

  • Purpose: This study attempted to verify the effect of Koryo hand therapy on menstrual cramps and dysmenorrhea among college students. Method: This study performed the quasi-experimental design with nonequivalent control group and the pre and post-test design from August 28 to November 4, 2002. The subjects of this experimental study consisted of 64 college students in the nursing college of K University in D city and K college students in K city, who had more than 5.0 GRS score of menstrual cramps. Among them, 16 people belonged to the experimental group A by using Ceramic Seo Am moxa therapy, 16 to the experimental group B by using Seo Am pellet therapy, 16 to the experimental group C by using combination of Ceramic Seo Am moxa therapy and Seo Am pellet therapy and 16 to the control group. Three different kinds of methods were used three times per week for $5{\sim}6$ weeks(a total of $15{\sim}18$ times) interventions were completed. For the experimental group, A Ceramic Seo Am moxa therapy was given for 40 minutes per each treatment; for the experimental group, B Seo Am pellet therapy was given for 4 hours: for the experimental group, C combination of Ceramic Seo Am moxa therapy and Seo Am pellet therapy was given. To measure menstrual cramps, the graphic rating scale (GRS) was used and to measure dysmenorrheal, a dysmenorrhea scale (15 contents) was used, which was modified from Han &Hur's scale (13 contents). Cronbach's was 0.78 in the pre-test, 0.83 in the first post-test, 0.89 in the following post-test. Data were analyzed by one-way ANOVA, 2 test, repeated measures ANOVA, time contrast test and Sheffe test with the SPSS/Win 11.0 program. Results: ? The first hypothesis, 'Among the experimental group A by using Ceramic Seo Am moxa therapy, the experimental group B by using Seo Am pellet therapy and the experimental group C by using combination of Ceramic Seo Am moxa therapy and Seo Am pellet therapy will have different graphic rating scores of menstrual cramps', was supported (F=6.77, p=0.000, Interaction: p=0.000). ? The second hypothesis, 'Among the experimental group A by using Ceramic Seo Am moxa therapy, the experimental group B by using Seo Am pellet therapy, the experimental group C by using combination of Ceramic Seo Am moxa therapy and Seo Am pellet therapy and the control group will have a significantly different level of dysmenorrhea', was supported (F=6.88, p=0.000, Interaction: p=0.000). From the above results, it can be an effective nursing intervention to give Koryo hand therapy to college students who have menstrual cramps and dysmenorrhea. Conclusion: These findings indicate that Koryo hand therapy could be applied to improve the quality of life and to prevent drug misuse among college students who are physically, mentally and psychologically suffering from menstrual cramps and dysmenorrhea. Furthermore, Koryo hand therapy could be developed as an effective Korean alternative and complementary care in the future. and it could also provide a guideline to apply Koryo hand therapy to other pain and difficulties.

A Study on the Korean Medical Institution's Strategies for Advancing into Free Port of Vladivostok (한국 의료기관의 블라디보스톡 자유항 진출에 관한 연구)

  • The Russian government enacted the 'Federal Law on the Free Port of Vladivostok' to improve the medical industry in the Far East in 2015. As a result, Korean medical institutions are increasingly interested in advancing into Free Port of Vladivostok. Increased demand for high-quality Korean medical services from Russian patients and the active government policy direction of the Russian government serve as strengths and opportunities for advancing into Free Port of Vladivostok. On the other hand, the legal imperfections caused by the delay in the establishment of sub-regulations for Federal Law on the Free Port of Vladivostok and the low reliability of the Russian government are both weaknesses and threats. Therefore, Korean medical institutions will need to carefully consider the timing of advancing into Free Port of Vladivostok by closely monitoring the process of establishing the sub-regulations of Federal Law on the Free Port of Vladivostok. And it is the most realistic model for Korean medical institutions to establish 'Professional clinic model linked with health screenings(1st stage model)'. The Korean government needs to conclude a 'MOU for Cooperation on Health and Medical Care between Korea and Russia' with the Russian government and should also carry out the 'Korean Minnesota Project'.

Influence of Individual and Job Characteristics, Professional Job Perception, and Group Cohesiveness on Organizational Citizenship Behavior of Physical Therapists in Hospitals (물리치료사의 개인 및 직무특성, 전문직업성, 집단응집성이 조직시민행동에 미치는 영향)

  • The main objective of this paper is to investigate the factors affecting organizational citizenship behavior of hospital employees, and based on the investigation, to suggest some implications for effective human resources management of hospitals. For this purpose, physical therapists were selected as the subject of the research. Using their individual characteristics, job characteristics, professionalism and group cohesiveness as the variables affecting organizational citizenship behavior, an empirical model was constructed and tested. A survey was conducted through structured and self-administered questionnaire for the physical therapists working at hospitals of Busan-Kyongnam area, and data from 240 therapists were utilized in the final analysis. Major results of the empirical analysis are as follows: First, perception on professionalism and the degree of organizational citizenship behavior were higher for male, older, relatively more-educated and higher-grade employees. It is necessary to develope some measures to educate and motivate the employees who are in lower state of professionalism and organizational citizenship behavior. Second, among the individual characteristics, need for growth was found to have significant, positive influence on professionalism and group cohesiveness, but no direct effect on organizational citizenship behavior. On the other hand, extroversion had direct, positive effect on organizational citizenship behavior, as well as on professionalism and group cohesiveness. This result suggests that personnel selection and personality education should be conducted carefully. Third, job characteristics appeared to have very large, positive effect on professionalism, but not directly on organizational citizenship behavior. Fourth, professionalism was found to have very large, positive influence on group cohesiveness and direct, positive effect on organizational citizenship behavior. This implies that enhancing professionalism of physical therapists can strengthen organizational citizenship behavior in hospitals, and hence top management should actively support the programs for job re-design, skill education and quality improvement to enhance professionalism of their employees. Fifth and last, as an intervening factor, group cohesiveness appeared to have the largest, direct, positive effect on organizational citizenship behavior. It is, therefore, important for top management to improve group cohesiveness by exploring ways toward greater harmony and solidarity among the members of physical therapy department.

Rutin Improves Bone Histomorphometric Values by Reduction of Osteoclastic Activity in Osteoporosis Mouse Model Induced by Bilateral Ovariectomy

  • Objective : Osteoporosis is a disease of unbalanced bone metabolism that results in low bone mineral density with increased bone fragility and propensity for fractures. The increased rate of bone fracture due to osteoporosis places a significant burden on public health care expenditures. Therefore, numerous studies have been designed and performed to identify the drugs or health foods that can improve the bone quality or quantity. This study was designed to evaluate and analyze the therapeutic effects of rutin on histomorphometric values of the spine and femur in an osteoporotic mouse model induced by bilateral ovariectomy. Methods : Thirty female ICR mice (8 weeks old) underwent either a sham operation (only abdominal incision, sham group, n=10) or bilateral ovariectomy (n=20). The ovariectomized (OVX) animals were randomly divided into two groups : untreated OVX group (OVX-C, n=10), or rutin-administered group (OVX-R, n=10). The OVX-C group received weight-adjusted doses of saline vehicle and the OVX-R group received 50 mg/kg of rutin intraperitoneally, starting 1 day after surgery. At 4 and 8 weeks after surgery, serum estrogen, osteocalcin, alkaline phosphatase (ALP), and the telopeptide fragment of type I collagen C-terminus (CTX-1) were analyzed. Interleukin (IL)-1β, IL-6, IL-10, and tumor necrosis factor (TNF)-α were also analyzed. Bone histomorphometric parameters of the 4th lumbar vertebra and femur were determined by micro-computed tomography. Results : In OVX-C group, ALP, osteocalcin, CTX-1, IL-1β, IL-6, and TNF-α levels were significantly increased at 4 and 8 weeks compared to sham operation group. Rutin administration after OVX statistically significantly reduced ALP, CTX-1, IL-1β, IL-6, and TNF-α levels at 4 and 8 weeks. Rutin administration also improves bone histomorphometric parameters including trabecular bone volume fraction, trabecular thickness, and trabecular number. Trabecular separation was also decreased in OVX-R group compared to OVX-C group. Conclusion : The present study demonstrated that rutin has therapeutic effects on improving bone histomorphometric values in an OVX mouse model. The improvement in histomorphometric values may be associated with the reduction of osteoclastic activity via inhibition of IL-1β, IL-6, and TNF-α. In future studies, the mechanism for the effect of rutin on osteoporosis should be demonstrated more clearly to use rutin in human osteoporosis.

The Cause-and-Effect Relationship between Perceptual Satisfaction and Expectation Factors for Special Hospitals by Busan and Gyungnam Medical Consumers (부산·경남지역 의료소비자들의 전문병원에 대한 지각 만족도와 기대요소 간의 인과관계)

  • This study was performed to examine the strategy method for the sustainable stability and effectiveness of small and medium hospital, where the relationship between perceptual satisfaction and expectation factors for special hospital was analyzed. The structural questionnaire was given to the residents of Busan and Gyungnam over the age of 20 and the square contingency and the logistical regression were employed for the analysis of the results. First, the respondent with 68.0% has realized the hospital claiming to be characterization as the special hospital and the Ministry of Health and Welfare designated hospital was identified to the respondent of 21.3%. Second, perceptual satisfaction with expectation level was satisfied to the most visitor in case of visiting special hospitals with subjective perception and was confirmed to the intention of revisiting consistently. Third, for the perceptual satisfaction of visiting with expectation level, the safety and the quality of treatment affected significantly the cause-and-effect. The result has provided the useful information with the establishment of a strategy for the long-term and continued effect through the perceptual satisfaction and the emotional attachment.

Study of Sleep Patterns on Depression and Cognitive Difficulties among Older People Living in the Community (재가 노인의 수면 양상과 우울 및 인지기능의 관계에 대한 연구)

  • This study focuses on the sleep pattern of the elderly people living in the community and its relationship to the occurrences of the depression and deterioration of the cognitive function. Our primary data is the raw data gathered by the Korean Ministry of Health and Welfare in 2008 in the National Senior Living Conditions and Well-being Needs Assessment Survey. The survey contained data from 12,087 people over 65 years of age living in the community. We have used the secondary data analysis method on this raw data to see if there exists correlation between age, gender, soundness of the sleep, total sleep time and the depression and the cognitive difficulties. Our study finds that the older a person is, the more trouble she has in sleeping. It also shows that too much sleep (in excess of 9 hours) and too little sleep (less than 6 hours) can both be linked to more occurrence of depression. Lack of restful sleep could also be linked to more frequent occurrence of depression and cognitive difficulties. Changes in the sleep pattern is not always pathological in elderly people. However, our study shows that it is important the primary health-care givers understand the role of sleep in elderly person's daily life. They should examine the elderly person's sleep pattern focusing on the quantity and the quality of sleep and develop programs suited for individuals to prevent and intervene sleep disorder.

  • Along with the development of ICT technology, wearable devices of various sizes and shapes have been developed. In addition, performance and specifications are rebuilt with IOT fusion products so that they can connect with the current smartphone. This is one of the general-purpose technologies of the 4th industrial revolution, which is spot-lighted with technology that changes the quality and environment of our lives. Along with this, as new technology products combining health care technology increases, various functions are provided to users who need it. Wearable technology is ongoing trend of technology development. It also sells products developed as products in the form of smart watches. At present, various related products are made in various ways, and it is recommended to use the Arduino processor in accordance with the application. In this study, we developed wearable physical activity monitoring system using open source hardware based TinyDuino. TinyDuino is an ultra-compact Arduino compatible board made on the basis of Atmega process Board, and it can be programmed in open source integrated development environment(named Sketch). The physical activity monitoring system of the welfare body can be said to be a great advantage, as a smart u-Healthcare system that can perform daily health management.
