Evaluating the activity of N-89 as an oral antimalarial drug

  • Despite the recent progress in public health measures, malaria remains a troublesome disease that needs to be eradicated. It is essential to develop new antimalarial medications that are reliable and secure. This report evaluated the pharmacokinetics and antimalarial activity of 1,2,6,7-tetraoxaspiro[7.11]nonadecane (N-89) using the rodent malaria parasite Plasmodium berghei in vivo. After a single oral dose (75 mg /kg) of N-89, its pharmacokinetic parameters were measured, and t1/2 was 0.97 h, Tmax was 0.75 h, and bioavailability was 7.01%. A plasma concentration of 8.1 ng/ml of N-89 was maintained for 8 h but could not be detected at 10 h. The dose inhibiting 50% of parasite growth (ED50) and ED90 values of oral N-89 obtained following a 4-day suppressive test were 20 and 40 mg/kg, respectively. Based on the plasma concentration of N-89, we evaluated the antimalarial activity and cure effects of oral N-89 at a dose of 75 mg/kg 3 times daily for 3 consecutive days in mice harboring more than 0.5% parasitemia. In all the N-89-treated groups, the parasites were eliminated on day 5 post-treatment, and all mice recovered without a parasite recurrence for 30 days. Additionally, administering oral N-89 at a low dose of 50 mg/kg was sufficient to cure mice from day 6 without parasite recurrence. This work was the first to investigate the pharmacokinetic characteristics and antimalarial activity of N-89 as an oral drug. In the future, the following steps should be focused on developing N-89 for malaria treatments; its administration schedule and metabolic pathways should be investigated.

Systematic Review of the Cause of Drug-Induced Liver Injuries in Korean Literature (약인성 간손상의 원인물질에 관한 국내연구의 체계적 고찰)

  • Background: The safety of herbal products is an important issue in Korea. It is more complicated because of the concomitant use of conventional western drugs and herbal medicine, including prescribed medicine, folk herbs and food supplements. Although both western and traditional Korean medical doctors have studied on the safety of HMP, their results did not show relevant or consistent conclusions because of the poor quality of studies, including bias. Objectives: The aim of this study was to review the studies in Korea related to drug induced liver injury (DILI) systematically. We tried to estimate the proportion of causative materials of DILI and evaluate the clinical difference of DILIs from different materials. Methods: Systematic searches were performed on MedRIC and MEDLIS in Korea. The extraction of data as well as selective screening of the studies was carried out independently by two of the authors. There were no restrictions on the types of publication, including grey literature. Results: Twenty-one articles were included. Of them only 7 adapted prospective design and only 6 were published in peer-reviewed journals. The proportion of conventional drugs associated with hepatotoxic injuries in all DILI ranged from 15.8% to 83.3% and that of herbal medicine was from 28.9% to 44.7%. However, the criteria for herbal medicine were not clearly defined and concomitant medications were not fully investigated in most studies. There were limited objective data in the clinical differences of liver injuries from different materials and their results were conflicting. Conclusions: The causality assessments regarding DILI of herbal medicine were not performed properly and causative materials were misclassified in most of the studies published in Korea. These make the safety issue still ambiguous because of the limitations and lack of objectivity of the studies. More rigorous studies are required for clearly addressing these conflicting issues with cooperative investigation between traditional Korean and western medicine.

U-City Service Classification Standard & Cost Sharing Plan through the Case Studies of U-City Construction

  • Recently, the construction of U-City or of U-City service (U-service) in many kinds of urban development projects becomes the general trend. However, as the construction of U-city comes into the spotlight like the trend of the times, various service constructions are being indiscreetly implemented since it's processed focusing on local specialized projects or local landmark project by a local autonomous entity or an institute in charge being off the original purpose of the improvement of living quality of residents and the efficiency of urban management, which causes a number of problems on the process, maintenance, operation and management of those projects. Therefore, this study is to derive the standard pattern of U-service construction through the analysis of patterns and numbers of U-service and problems during the process of construction in domestic U-City construction case district, and is aimed to suggest the reasonable direction of service construction based on this. 11 projects areas in which data related to U-service are available among U-City construction project areas were selected for the subjects of this study, and it's standardized with 228 detailed unit services that were suggested by U-Eco City R&D and with 11 service domains that are regulated in "the Act of ubiquitous city construction etc.", and it is limited to services that were suggested in U-City case area of which design was completed by July, 2010 based on this. The actual state of various U-service construction is analyzed through case studies as well as reviews on the legal and theoretical concept and the method of pattern classification of U-service, the plan for the standardization (pattern classification) of U-service and the plan for the construction body of service and to share the costs are suggested as the plan to construct reasonable U-service based on this for the method and the procedure of this study For the method to construct U-service, the plan to classify the pattern of U-service into the basic service and additional service is suggested, and whether it has the publicity, whether urban infrastructure is included, whether utilities are included, the body of realization, the recipients, the urgency, the importance and the spreading, etc. are suggested for the index to determine the pattern of service. The method to get the executor of a project to construct the basic service, the method to get the executor of a project to construct the most basic service among the group classification of service and to get a private entity and a local autonomous entity to perform additional service in the future, and the method to construct the whole service and to distribute the construction costs were suggested, and the first one that determines the body of construction after dividing U-service into the basic service and the additional service was evaluated to be the most reasonable one. And the plan to solve the problem that the excessive construction costs of U-service are shifted onto the executor of a project through benefit principle, balanced distribution principle and consultation and mediation between the interested parties was suggested for the criteria to share the costs.

A study of Korean Physical Therapist's Attitudes and Beliefs on Korean Government's National Long Term Care Insurance Program (노인 장기요양 보험 제도에 대한 부산광역시 물리치료사들의 인식도 조사)

  • Objective: This study was conducted to describe physical therapists' attitudes and beliefs towards Korean Government's National Long Term Care Insurance Program implemented last July 2008. Design: A survey research. Methodology: Participants were 143 physical therapists working in General Hospitals or University Hospitals in Busan, Korea. Each of the participants completed a questionnaire which was comprised of 22 questions. Among the 22 questions, 7 of these were independent variables such as gender, age, career, etc. while 15 of these questions were dependent variables such as the subject's expectations and reactions on the said program, opinions of the participants on the impact of the program to old people's families and relatives, participant's sentiments on social effects of this program to the society, etc. Data collection was conducted from the $20^{th}$ of July to the $5^{th}$ of August, 2008. Data were analyzed by Chi-square test, frequency and cross tabs using SPSS/PC program. Results: Based on the survey conducted, it showed that 50.3% were female respondents, 53.8% were on their twenties, 31.5% of the total respondents were married and that a percentage of 82.4% had working experiences below 10 years. About 80.4% of respondents were satisfied of their jobs and 32.4%of the respondents graduated from three - year college degree, 49.3% of the respondents graduated from four-year college degree. 95.1% of the total respondents knew the existence of the national long term care insurance program of the government and almost 78% got the information of the said program via internet, news paper or T.V. About 86% of the respondents believe that there will be future improvements in the quality of life in Korea through the said program and 82% said that there will be an increase sense of responsibility among family members to take care of their old relatives. 67.2 % of respondents believe that problems regarding old people will be solved through the government's national long term care insurance program. In aspect of Physical Therapy, 50% of the respondents expected generation of more jobs especially to physical therapists and 95% of the respondents want to work in any position in the said program especially those are more experienced ones. Conclusion: Physical Therapists in Korea are aware of the existence of the National Long Term Care Insurance Program of the Korean Government. Based on the gathered results from the survey, many experienced physical therapists of the country wanted to acquire positions in the said program especially that their expertise are needed in the said field. Through this study, it is evident that many physical therapists of the country have positive vibes on the said program and are optimistic on the outcomes of the said insurance program.

An Analysis of Domestic Medicine Study Tendency on Obesity -Focused on The Korean Journal of Obesity- (비만에 대한 국내 의학계의 최근 연구 동향 분석 -대한비만학회지를 중심으로-)

  • Objectives: This study was performed to analyze the recent trend of the studies about obesity in medicine and to provide background for futher studies. Methods: All of the article were selected in "The Korean Journal of Obesity". Search were conducted through "http://kosso.or.kr" with the search word "obesity". Collected articles were classified into clinical study, experimental study, literary study, case report. Results: One hundred eighty four studies were included and analyzed in terms of study design, subject, intervention, period, obesity index and so on. The numbers of clinical studies, literary studies, experimental studies, case reports were respectively 143 (77.7%), 26 (14.1%), 2 (1.1%) and 7 (3.8%). In clinical studies, observational studies were 112 (78.3%) and intervention studies were 31 (21.7%). And most of sample sizes were more than 100 and less than 499. Body mass index, waist circumstance, body fat percent were major criteria of clinical study. Most of the subjects on obesity were about relation with another disease or experimental results and diagnosis. According to classification by the kinds of intervention, diet, exercise, drug, behavior were respectively 22, 18, 8, and 8. More than fourty percent of intervention studies were studied for 12 weeks. Conclusions: To improve the quality of Korean Medicine study for obesity, we need to recruiting big sample size and activate randomized clinical trial.

FIT OF FIXTURE/ABUTMENT INTERFACE OF INTERNAL CONNECTION IMPLANT SYSTEM (내측연결 임플란트 시스템에서 고정체와 지대주 연결부의 적합에 관한 연구)

  • Purpose : The purpose of this study was to evaluate the machining accuracy and consistency of implant/abutment/screw combination or internal connection type. Material and methods: In this study, each two randomly selected internal implant fixtures from ITI, 3i, Avana, Bicon, Friadent, Astra, and Paragon system were used. Each abutment was connected to the implant with 32Ncm torque value using a digital torque controller or tapping. All samples were cross-sectioned with grinder-polisher unit (Omnilap 2000 SBT Inc) after embeded in liquid unsaturated polyester (Epovia, Cray Valley Inc). Then optical microscopic and scanning electron microscopic(SEM) evaluations of the implant-abutment interfaces were conducted to assess quality of fit between the mating components. Results : 1) Generally, the geometry of the internal connection system provided for a precision fit of the implant/abutment into interface. 2) The most precision fit of the implant/abutment interface was provided in the case of Bicon System which has not screw. 3) The fit of the implant/abutment interface was usually good in the case of ITI, 3I and Avana system and the amount of fit of the implant/abutment interface was similar to each other. 4) The fit of the implant/abutment interface was usually good in the case of Friadent, Astra and Paragon system. The case of Astra system with the inclined contacting surface had the most Intimate contact among them. 5) Amount of intimate contact in the abutment screw thread to the mating fixture was larger in assembly with two-piece type which is separated screw from abutment such as Friadent, Astra and Paragon system than in that with one-piece type which is not seperated screw from abutment such as ITI, 3I and Avana system. 6) Amount of contact in the screw and the screw seat of abutment was larger in assembly of Friadent system than in asembly of Astra system of Paragon system. Conclusion: Although a little variation in machining accuracy and consistency was noted in the samples, important features of all internal connection systems were the deep, internal implant-abutment connections which provides intimate contact with the implant walls to resist micro-movement, resulting in a strong stable interface. From the results of this study, further research of the stress distribution according to the design of internal connection system will be required.

Effect of supplementation and withdrawal of selenium-enriched kale sprouts on productivity and egg selenium concentration of laying hens

  • Objective: The aim of this trial was to investigate the effect of supplementation and withdrawal of selenium-enriched kale sprouts (SeKS) on productivity and egg Se concentration of laying hens. Selenium from commercial Se-enriched yeast (SeY) was used as a comparative Se source. Methods: One-hundred and eighty 61-week-old laying hens were randomly divided into 5 treatment groups with 4 replicates (9 hens each) in a 2×2+1 Augmented Factorial Experiment in a completely randomized design. The experimental diets were basal diet, basal diet supplemented with 0.2 and 0.4 mg Se/kg from SeKS and SeY, respectively. The 8-week feeding trial was divided into 2 periods, namely the Se supplemental period (week 1 to 4) and the Se withdrawal period (week 5 to 8). Results: Productive performance, egg quality and egg Se concentration of laying hens were not affected by sources of Se (SeKS and SeY) during both, the Se supplemental and withdrawal periods. Egg production and egg Se concentration increased (p<0.05) with increasing levels of Se supplementation. The egg Se concentration increased and reached a peak 1 week after Se supplementation. However, concentration of Se in eggs of hens fed Se from both sources decreased rapidly from the second week of the Se withdrawal period to reach the same egg Se concentration of hens fed the basal diet by the fourth week of the Se withdrawal period. Conclusion: The efficacy of Se from SeKS on productivity and egg Se concentration in laying hens was comparable to commercial SeY. Thus, SeKS can provide an alternate organic Se source for production of Se-enriched eggs.

Study on the beam properties of small field sizes (소조사면 전량분포의 특성에 관한 고찰)

  • Purpose : The aim of this study is to investigate the properties of small field size and to measure the penumbra and central axis depth dose varying to the jaw setting and off axis distance for indicate this data to small field sizes radiation therapy. Material and methods : The percentage depth dose, beam profile and central axis output dose was measured by farmer type ion chamber and pinpoint chamber using Primart linac with 6MV energy. Beam quality and penumbra variations according to the central axis shift, from center to every 2cm outside increment, and field size, from $1{\times}1cm$ to $10{\times}10cm$ was investigated and compared with that of the standard geometrical condition's results Results : The differences of measured values between two ion chamber was about $37\%$ at 10cm depth with $1{\times}1cm$ field sizes but as field size increased this differences was diminished gradually. Measured data from various off axis distance with the different asymmetric collimations are not changed significantly but as size decreased the dose variation was increased and at $1{\times}1cm$ field size dose difference among off axis distance was as much as $13\%$, and as shallower the measured depth the central axis dose variations among the OAD was increased, penumbra was not changed noticeably depending on off axis distance but the percentage of penumbra from its initial field sizes was strongly dependant on field sizes and penumbra occupation rates of its own field sizes ranging from $6\%$ at $10{\times}10cm$ to $50\%$ at $1{\times}1cm$ field size. Conclusion : For imrt treatment, there are several numbers of different gentry angles with beams of nonuniform fluences are required and several complex factors involved. Among them the characteristics of beam output varying to the geometrical setting and design of collimators are of important to attaining a good treatment results. As mentioned in results the differences of measured values are changed significantly depends on ion chamber volume, depths and field size. For providing quality radiation treatment, especially at small field size, those factor's should have considering deliberately.

Current Trends in Intervention Studies of Hwabyung in Korean Medicine (화병의 한의학적 치료에 대한 연구동향)

  • Objectives: To determine the general characteristics of clinical studies about Hwabyung and assess their limitations and alternatives. Methods: Clinical studies that examined the effects of traditional Korean medicine intervention on Hwabyung were included in this study. A systematic search of English, Chinese, Japanese, and Korean databases was performed. The characteristics of included articles were described and those articles were assessed by Risk of Bias (RoB) tool or Risk of Bias for Nonrandomized Studies (RoBANS) tool. Results: Sixteen articles were selected from 1,826 articles. Most clinical studies about Hwabyung were published in Korea. The number of conducted trials was insufficient. The prevailing study design was randomized controlled trial. Traditional Korean medicine intervention used in the trials were acupuncture, herbal medicine, counselling, meditation, emotional freedom technique (EFT), music therapy, art therapy, and multi intervention program. Herbal medicine study used placebo as control while non-pharmacological intervention study mostly used no treatment as control. Most of the trials were supported by the government. Therefore, financial conflict of interest might not exist for results. We judged that some studies had a high risk of bias. In general, most of the studies with a high risk of bias were non-pharmacological intervention studies, and the risk of bias was mainly due to lack of blinding. Conclusions: More clinical studies of Hwabyung are needed. There are some issues about a suitable comparison and effective blinding strategy for non-pharmacological study. Improving methodological quality is required.

Dietary crude protein levels during growth phase affects reproductive characteristics but not reproductive efficiency of adult male Japanese quails

  • Objective: The objective was to evaluate the influence of different dietary crude protein (CP) levels during the growth phase on reproductive characteristics and reproductive efficiency as well as the body development of adult male Japanese quail. Methods: Three hundred one-day-old male quails were distributed into five treatments with diets containing different CP levels (18%, 20%, 22%, 24%, and 26%) in a completely randomized design, with six replicates of ten birds each. The CP diets were applied only during the growth phase (1 to 35 days). At 36 days of age, the birds were transferred to 30 laying cages with three males and nine females each, and all birds received the same diet formulated to meet production-phase requirements until 96 days of age. Results: The growth rate of the birds increased linearly (p<0.01) with increasing dietary CP, but the age of maximum growth decreased (p<0.05). At growth maturity, all birds had the same body weight (p>0.05). At 35 days of age, higher weight gain was obtained (p<0.05) with diets containing 22% CP or higher. No effects on feed conversion were observed in this phase. The increase in dietary CP enhanced (p<0.01) nitrogen intake and nitrogen excretion but did not affect (p>0.05) nitrogen retention. Testis size, seminiferous tubular area, number of spermatogonia, and germinal epithelial height at 35 days of age increased linearly (p<0.05) with dietary CP, while the number of Leydig cells decreased (p<0.01). The Sertoli cell number at 60 days of age increased linearly (p<0.01) with dietary CP. Dietary CP levels did not affect cloacal gland size, foam weight, foam protein concentration, semen volume, or flock fertility at 90 days of age. Conclusion: Dietary CP concentration affected body and testicular development in male Japanese quails but did not affect reproductive efficiency.
