지역사회 거주 노인의 인지기능, 일상생활활동, 참여 및 삶의 질의 관계 (Relationships Among Cognitive Functions, Activities of Daily Living, Participation and Quality of Life in Community-dwelling Elderly)

  • 목적 : 본 연구는 지역사회 거주 노인의 인지기능, 일상생활활동, 참여 및 삶의 질의 관계를 확인하고자 하였다. 연구방법 : 지역사회 거주 노인 144명을 대상으로 인지기능, 일상생활활동, 참여 및 삶의 질을 측정하였다. 인지기능은 한국어판 간이정신상태검사, 일상생활활동은 한국형 일상생활활동 측정도구, 참여는 한국형 활동분류카드, 삶의 질은 노인 삶의 질 척도를 사용하였다. 결과 : 인지기능은 참여의 사회활동과 삶의 질 3개 영역(신체적 건강, 심리적 건강, 사회관계 및 경제수준)과 유의한 상관이 있었다. 일상생활활동은 참여(수단적 일상생활활동, 여가활동)와 삶의 질의 모든 하위영역에서 유의한 상관이 있었다. 참여의 하위영역들 중 수단적 일상생활활동과 여가활동은 삶의 질의 모든 하위영역과 상관이 있었고, 참여의 사회활동은 신체적 건강을 제외한 삶의 질의 모든 영역과 유의한 상관이 있었다. 여러 변인들 중 삶의 질과 가장 상관이 큰 변인은 참여의 여가활동인 것으로 나타났다. 결론 : 참여, 일상생활활동, 인지기능의 순서로 삶의 질과 상관이 있었고, 참여의 하위영역 중 여가활동이 삶의 질과 상관관계가 가장 큰 것으로 나타났다. 본 연구는 지역사회 거주 노인의 인지기능, 일상생활활동, 참여와 삶의 질의 상관관계를 확인하여 성공적인 노년기 삶을 위한 기초자료를 제공하였다는데 의의가 있다.

대학 도서관 서비스 품질요인에 대한 인식차이 분석 (Perception Gap Analysis on Service Quality Factors of Academic Libraries)

  • 대학 도서관의 지식문헌정보서비스는 대학 교육 및 교수의 연구를 지원하는 중요한 역할을 수행하고 있다. 이에 따라, 서비스 품질요소를 계층적 관점에서 정의하고 이들의 중요성을 분석함으로써 대학 도서관의 서비스를 개선하기 위한 연구들이 선행되어 왔다. 그러나 기존 연구들은 서비스 품질을 구성하는 하위요소들 간의 상호 영향관계, 그리고 서비스 이용자와 제공자 두 집단간의 서비스 품질요인에 대한 인식차를 파악하여 서비스를 개선하는 측면에서는 연구가 미흡한 실정이다. 따라서 본 연구는 SERVQUAL의 관점을 바탕으로 도서관 서비스를 구성하는 서비스 품질요소 간 상관관계를 네트워크 모델로 도출하고, Analytic Network Process를 활용하여 서비스 이용자와 제공자 집단간의 서비스 품질요소에 대한 인식차이를 도출, 분석하였다. 그 결과 제공자는 요소들을 비교적 균일하게 중요하게 인식하지만 이용자는 특정 몇 가지 요소들을 중요하게 인식하는 것을 확인하였다. 도서관 서비스 품질요소들간의 상관관계와 집단간의 인식차이를 파악하는 본 연구의 접근방법은 추후 대학 도서관의 지식문헌서비스 경쟁력 확보에 기여할 것으로 기대된다.

4차 산업혁명 시대의 CMMI 품질성과관리 연구 (The Quality Performance Management of CMMI in the Era of Industry 4.0)

  • Purpose: CMMI is a process model used to assess or improve an organization's software development capabilities. This paper deals with the quality indicators when using CMMI and their priorities for possible improvement. Methods: The 22 process areas and 167 practices of CMMI are matched with 60 indicators of Quality Scorecard(QSC) first to analyze the balance of CMMI in terms of prevention, appraisal, and final result categories and second to isolate a set of key areas for quality focused performance measures. Results: A total of 86.2% (144 out of 167) CMMI practices were mapped to QSC. According to the CMMI level of maturity, level 2 and 3 accounted for more than 75% of the total. The practices at the maturity level of 4 and 5 were mapped to more than 52% of the final result's measurements. It has been observed that CMMI practices need further elaboration at higher levels to consider prevention, appraisal, and final results simultaneously. Conclusion: In order to improve the quality performance of the organization by applying CMMI, the final result measures should be refined in metrics, cycles, and methods, and then corrective actions could be conducted to improve the performance of CMMI practices. This strategy would help the practitioners benefit from CMMI in fostering the overall quality level of key activities for the organization's business goals.

의료 서비스에 대한 만족도 측정 도구의 개발 (The Development of Satisfaction Tool to Health Care Services - focused on Patients and their families -)

  • Background : In these days, the health care organizations have concerned about customer-centered care in order to empower the competitiveness on the health care markets. The departments working for quality management of the hospitals have used health care quality indicators in terms of medical areas as well as service areas of the hospitals. However, there were insufficient efforts to develop the credible measurement to seek the customers' needs, their expectations and their satisfaction levels related to health care services because various kinds of challenges were in the process of scale development to measure customers' satisfaction in health care. The purpose of this study was to develop the satisfaction scale to health care services in a Korea health care organization and to test its tool with validity and reliability. Method : The concept of this tool was acceptability that one of the components of health care quality defined by Donabedian. Acceptability has the five dimensions of concept: Accessibility; Patient-Practitioner Relations; Amenities; Patient Preferences as to the effects of care; and Patient Preferences as to the costs of care. The Satisfaction Tool to Health Care Services was reviewed by expert panel with five researchers, including hospital managers and a professor related to quality management of the hospitals. As a result, the content validity index was .84 in the outpatient satisfaction tool. The inpatient satisfaction tool had .87 of the content validity index. The Satisfaction Tools to Health Care Services finally consisted of 44 items for outpatients/their families and of 60 items for inpatients/their families. Study subjects of the construct validity test were 479 outpatients/their families and 561 inpatients/their families who visited or admitted at a University hospital from July 1, 1996 through August 10, 1996. The data were examined by Factor Analysis with SPSS. Result : The items of Satisfaction tools for outpatients/their families were categorized by eleven factors with eigenvalue greater than 1.0 accounting for 64.2 percent of the variation in item scores. Also, the items of inpatient tool had eleven factors with eigenvalue greater than 1.0 accounting for 60.3 percent of the variation in item scores. The reliability of overall scale were .95 and .96 for the outpatients/their families satisfaction scores and inpatient/their families satisfaction scores. The internal consistency reliability with eleven factors was ranged from .30 to .94 for inpatients/their families. The Satisfaction Tool with eleven factors for inpatients/their families had internal consistency reliability ranged from .53 to .89. Conclusion : The Satisfaction Tools to Health Care Services focused on outpatients/their families and inpatients/their families developed in this study had a high reliability and the strong evidence of content validity and construct validity based on quality concept. Therefore, this tool would be utilized as a credible quality indicator of health care services to assess the quality problems and to monitor the quality improvement activities in Korean Health Care Organizations.

기관내관 소독 방법 개선에 따른 간호업무 시간단축 및 비용 절감 효과 (Saving Effects Cost and Time in Nursing through Improving Sterilization Method of Inner Cannula)

  • Background : The purpose of inner cannula is to protect the upper air way and permit air to pass freely, in addition, to provide endotracheal suction, artificial respiration and to maintain adequate oxygen saturation. The tube needs to be sterilized for maintenance and cleanness of air way and for prevention of bronchospasm. However, it has been reported that there is no guideline for sterilization and many hospitals conduct their own sterilization methods, for example, once a day(13's general hospital), three times a day(The Catholic University of Korea ST Mary's hospital) or even no cleansing. Consequently, the QI team of our hospital suggested the SOP(standard operating procedure) of sterilization and evaluate cost and time effect in nursing. Method : 1) Benchmarking of 13's neurosurgery department of general hospital in Seoul 2) Investigation of test records of sputum culture from patients with intubation for tracheotomy 3) Check of results of O2 Sat. monitoring to confirm of maintaining opened air way Result : 1) Improvement of process: decrease of excess sterilization of inner cannula (from 3 times a day to once a day) 2) Cost effects: saving over 10 million won per one year 3) Providing better nursing: time effects (30 min a day) permit to conduct more nursing activities Conclusion : It can get Cost and time effects in nursing with improved sterilization method of inner cannula. It needs to do research on improvement of the monthly exchange protocol of outer cannula and provide supporting data for the proper exchange schedule. The result of additional microorganism detection from patients with new process needs to be evaluated further more.

인구동태조사 개선을 위한 방법론적 고찰 (A Study on Vital Statistics Survey : its Type, Source of Errors and Improvement Scheme)

  • It is well known that vital statistics is of great importance as basic data for establishing various range of national policies. Especially, vital statistics is important among demographic information for monitoring and evaluating the population policy, for constructing life table, for making population projection, and for studying various aspects of the society. In principle, the production of vital statistics is based on the registration system. It is, however, still observed that there are some limitations in utilizing fully the registration system due to the inherent problems such as problems in its coverage, accuracies and timeliness. Thus, as an alternative, many countries conduct survey on vital statistics in order to supplement the registration system and obtain in-depth data. Korea is no exception in this aspect. The National Bureau of Statistics carries out the so-called Continuous Demographic Survey. This is a kind of multi-round retrospective survey, covering 32, 000 households and having reference period of one month. The survey has also characteristics of multi-subject sample. Thus, surveys on economic activity status of population, house-hold income & expenditure, and social indicators are together conducted with the same sample. It is, however, found that the survey itself tends to have some quality problems. Especially, the quality problems connected with field data collection are summarized as coverage error, non-response error and response error. Although it is inevitable not to be free from these errors, we should make all our efforts to reduce the errors. The probable schemes pointed out in this paper are as follows : 1) the strengthening formal quality control activities, 2) the review of the survey method, i. e., the combining interview method with mail-sending and mail-back method or pick-up method, 3) well documentation for various cases found in every stage of data collection, and 4) the strengthening the analytical activities. It is, also, emphasized that sincerity of planners and interviewers is the most important factor among other things.

Different Effect of Sodium Chloride Replacement with Calcium Chloride on Proteolytic Enzyme Activities and Quality Characteristics of Spent Hen Samgyetang

  • Sodium chloride (NaCl) replacement with calcium chloride (CaCl2) effect on protein solubility, proteolytic enzyme and quality characteristics of a chicken soup prepared from spent hen (SH) chicken were investigated. By means of immerse marination prior to cooking, a total of 60 skinless SH breast meat were randomly allocated into ten groups admitted to treatments with marinade solution containing sodium tripolyphosphate (STPP) and reduced percentage of NaCl with CaCl2 at 0%, 25%, 50%, 75%, and 100% at 4±2℃ for 20 h. STPP was adjusted to 0.5% for all treatments and NaCl replacement at 0% was used as control. The different methods, particularly boiling at 100℃ and retorting at 121℃, 1.5 kgf/cm2 for 60 minutes, were applied following marination. An upregulation of cathepsin-B and caspase-3 enzymes were a consequences from a higher percentage of CaCl2 within meat environment. Accordingly, modified the protein solubility in particular the myofibrillar and total protein solubility. In addition, a significant increase in water holding capacity (WHC), pH value, myofibril fragmentation index (MFI), and moisture content was obtained due to salt replacement (p<0.05). Limited effect was observed for shear force value, collagen content and cooking yield. Eventually, this study implied that although protelytic enzyme and protein solubility was upregulated by the replacement of NaCl with CaCl2 at >75%, extensive effect on texture properties was not observed. Therefore, NaCl replacement at 75% could be a promising strategy for quality improvement of SH chicken soup.

수공예활동을 이용한 그룹 작업치료 중재프로그램이 뇌졸중 환자의 일상생활 수행 능력과 삶의 질 향상에 미치는 효과 (Effects of Stroke Arts and Craft Group Occupational Therapy Intervention Program on Improving the Quality of Life and Activity of Daily Living)

  • 목적 : 본 연구는 수공예활동을 이용한 그룹 작업치료 중재프로그램이 뇌졸중 환자의 일상생활 수행능력과 삶의 질 향상에 미치는 효과를 알아보고자 한다. 연구방법 : 뇌졸중 환자 10명을 대상으로 진행하였으며, 두 집단 사전-사후 설계(Pretest-Posttest control group design)를 사용하였다. 실험은 사전검사 1회, 중재 10회, 사후 검사 1회로 총 12회기로 주 2회 6주간 실시하였다. 인지기능은 한국판 간이 정신상태검사 (Mini-Mental State Examinatin for Korean: MMSE-K)와 일상생활평가로 한글판 수정 바델 지수 (Korean Version of Modified Bathel Index: K-MBI)와 캐나다 작업수행 측정 (Canadian occupational performance measure: COPM)을 사용하였고, 삶의 질 평가는 한국판 세계 보건기구 삶의 질 간편형 척도(The World Health Organization Quality of life- BREF: WHOQOL-BREF)를 중재 전 후에 시행하였다. 기술통계와 비 모수 통계를 이용하여 결과분석을 하였다. 결과 : 실험군과 대조군의 사전 - 사후 결과를 비교한 결과 일상생활 활동 수행능력 부분에서 K-MBI의 점수 증진은 있었으나, 유의미한 차이는 없었다. COPM은 실험군에서만 수행도 전 후에 유의미한 차이가 있었으며, 실험군, 대조군 모두 만족도에 유의미한 차이가 있었다. WHOQOL-BREF에서 실험군에서 실험 전 후 및 집단 내에 유의미한 차이가 있었으며, 집단 간에도 유의미한 차이가 있었다. 결론 : 본 연구 결과를 통하여 뇌졸중 환자에게 수공예활동을 적용한 그룹 작업치료 중재프로그램이 작업수행능력과 삶의 질 증진에 효과가 있음을 확인 할 수 있었다.

국내 학술지 평가모형에 관한 연구 (A Study of a Korean Academic Journal Evaluation Model)

  • 학술지는 학문교류에 매우 중요한 수단이기 때문에 한 나라의 학문 발전에 매우 중요한 역할을 담당하고 있다. 그러나 대부분의 국내 학술지는 이의 질에 대한 낮은 인식으로 인해 학술지의 기본적인 기능을 제대로 수행하지 못하고 있다. 국내 학술지가 학술지로서의 역할을 수행하기 위해 이루어져야 할 것이 많겠지만 가장 근본적인 것이 학술지에 대한 평가이다. 본 연구의 목적은 국내 학술지를 공정하고 효과적으로 평가할 수 있는 평가 모형을 개발하는 것이다. 즉, 학술지의 질을 정확하게 측정할 수 있고 학술지의 장점과 단점을 보여줄 수 있으며, 장점을 살리고 단점을 보완할 수 있는 방법을 제시할 수 있고 그리고 국내 학계의 사회문화적인 환경에 맞아 그 기능을 원활하게 수행할 수 있어 평가의 근본 기능을 효과적으로 수행할 수 있는 평가모형을 개발하는 것이다. 국내 연구자들에 대한 면담과 설문조사, 외국 사례 분석 등의 과정을 통해 평가모형을 개발하였다.

개원의의 개방병원 참여에 대한 의견 (The Private Physicians' Opinions of Being Attending Physicians in Teaching Hospitals)

  • A mailed survey with structured questionnaire was conducted to study the demand of private physicians who were operating their own clinics in the community to be a attending physician at the general hospital. The responding proportion was 21.6 percent of the 960 private physicians. A total of 207 responders; 65.2 percent wanted to be a attending physician. In particular, the physicians who were male, young, surgeon and teaching hospital careered after specialist were more highly motivated. The major activities what they wanted as a attending physician were medical care for the admission patients. They responded that the hospital charges for the medical services and the responsibility of malpractice issues should be fairly shared by attending physician and hospital according to their contributions. There is growing consensus that the need of attending physician at the general hospital will become wide spread, but little organizational preparation to assure the quality of medical care of attending physicians including training of resident physicians and students. In addition, the effective reimbursement system should be develop to compensate appropriately according to the medical achievement of the attending physicians.
