Objectives : Literatures related to Bianque are studied to discover the path of development and the impact of Bianque school's pulse diagnosis system. Methods : Texts regarding Bianque were searched in history books such as Shiji and Zhanguoce, and medical texts such as the medical books of Mawangdui Han Tomb, Huangdineijng, Maijing, and Qianjinyifang to understand how the Bianque school's pulse diagnosis system was developed. Results : 1. Bianque school's pulse diagnosis system was used to inspect the distribution pattern of blood vessels and discover the location of the disease including the palpatation realm such as only hard or only fall. 2. The system of inspection was created when the diagnosis method that uses the color of the pulse by using the color of blood vessels was added to the diagnostic method of pulse condition. 3. Adding the concept of pulse to the visual information that derives from pulse condition becomes pulsation. This is a diagnostic method that falls under the realm of palpation, and it was used to discover the location of disease. 4. The qi of pulse is motor that induces pulse, and this concept is used in order to understand how normal and abnormal pulsations appear, and to treat the circulation disorder of qi and blood. 5. Cubit skin examination is a method that comprehensively take into account the upper arm skin's cold and heat, slippery and roughness, and relax and tension state. This method was used together with other diagnostic methods. As described above, it seems that the diagnostic method with blood vessels used by Bianque school seems to have developed from Bianque's special inspection ability to the stage where it uses palpation, and then to the stage of cubit skin examination which uses both palpation and inspection.
The following results obtained through studies related on nose. 1. Nose is called mingtang(明堂) mianwang(面王), as it emphasize the meaning of viewing diagnosis(望診) and is called yuanpinzhimen(元牝之門), tianpinzhimen(天牝之門), qiqizhimen(七氣之門) as it emphasize the meaning of the entrance of atmosphere flow. Because it's position is the middle of face, we call tianzhongzhiyue(天中之嶽), zhongyue(中嶽). 2. As the organ of body, nose comes under the category of lung-iron(肺金), functionally comes under the category of heart-fire(心火). Functionally we can smell oder, organically nose is the pathway of respiration and to be resonator. 3. Wuzangliufu(五臟六腑) is arranged closely around the nose. The essence of wuzangliufu(五臟六腑) reflect nose, so nose becomes the standard of coloring diagnosis(色診). 4. We call nose is feihou(肺候), but it is not connected directly between the lung channel and nose. In case of lung symptom(肺證), first of all, nose become to appear the condition of lung, the lung channel and nose is close functionally. 5. The view of channels, nose is a connection with channels of yangming(陽明), bladder(膀胱), du(督脈) directly. By the connecting of each three channels, spleen stomach heart and kidney(脾胃心腎) are connected with indirectly. As the result of the relation, nose helps for body to breathe and to circulate qi(氣) and smell. And so the nature of wuzangliufu(五臟六腑) appear to the nose. 6. Particularly, the yangming channel(陽明經), and the bladder channel(膀胱經) are important. Because air-qi(天氣) reach to kidney lung through nose, ground-qi(地氣) reach to spleen stomach through mouth and yingwei(營衛) revolve through the bladder channel(膀胱經). As the result nose becomes the essential organ of circulation of yinewei(營衛), ascent and descent of qi(氣).
Objectives : The objective of this paper is to develop a standardized patient program with a focus on diagnosis and treatment of internal damage fever in Korean Medical education. Methods : First, cases of diagnosis and treatment of internal damage fever were collected from various classical texts, then a module was developed according to pre-existing standardized patient program's protocols based on selected cases. Careful consideration was given to developing evaluation criteria on history taking and physical examination that are necessary to accurately differentiating the 9 types. Results : Nine types of differentiation models on internal damage fever were selected, which are qi deficiency from overexertion/fatigue and famish; blood deficiency from overexertion/fatigue, famish and fullness; fire stagnation from excessive eating and cold foods; food damage; yang deficiency; yin deficiency; phlegm; stagnated blood; liver qi stagnation. For each type, evaluation criteria in regards to history taking, physical examination, communication with patient, and patient education were developed. Conclusions : When developing a standardized patient program using internal damage fever cases, it would better reflect the characteristics of Korean Medicine in clinical education of Korean Medicine if the program is based on classical texts. It would also be useful in evaluating students' graduation competence in exams such as CPX.
Objectives : This study aims to improve the diagnosis and treatment of contemporary insomnia by examining Zhang Jiebin's discussion on treating insomnia. Methods : The classical texts from the 'Insomnia' chapter of the Jingyue Quanshu were examined threefold in terms of symptom, treatment, and prescription analysis, after which the treatment discussion part was examined within the historical context of discussions on insomnia in major medical texts starting from the Huangdineijing. Results : According to Zhang, the cause of insomnia could be divided into two, after which criteria for diagnosis and treatment were set as excess pathogen and vital qi deficiency. He argued that insomnia could be naturally resolved through improvement of various pathogenic situations. Discussions on insomnia from various medical texts since the Huangdineijing suggest that pathology related to psychological function and emotions gradually increased and expanded over time. Conclusions : Zhang's discussion on symptom, treatment and prescriptions of insomnia suggests a new framework that could improve treatment effects through a Korean Medical Mind-Body approach, rather than the contemporary classification of organic insomnia and non-organic insomnia.
Objectives : The Aim of this study was to investigate the clinical characteristics of male and female patients who have been suffered from headaches. The investigation was undertaken based on Qi-section(Methods of prescribing oriental herbal medicine) from DongEuiBoGam(東醫寶鑑). Methods: A statistical survey was conducted to compare the difference in clinical characteristics between male and female patients experiencing headaches. Oriental medical Diagnosis was used to classify all parients under the following categories Qi-deficiency (氣虛), Jing-dificiency (精虛), Food-stagnation (食積), Damp-accumulation (痰飮), Qi-stagnation (氣滯). Patients were treated using acupuncture therapy and herbal medicines. Results : 1. The ratio between male and female patients was about 1:3. 2. It showed that periods of illness were longer in female patients than in male patients. 3. The total number of treatments received was higher in female patients than in male patients. 4. Statistically, most female patients had Metopodynia and Migraines(Rt.), while male patients suffered mostly from Laryngalgia. 5. Male patients had feelings of strain in the head and female patients had splitting headaches. 6. Musculo-skeletal System and General Symptoms were frequently observed in male patients. Digestive System and Nervous System were frequently observed in female patients. 7. Results from oriental medical Diagnosis showed that male patients mostly had Qi-deficiency, ling-deficiency while the majority of female patients had Food-stagnation, Damp-accumulation, Qi-stagnation. 8. The recovery time for male patients was shorter than it was for female patients. The recurrence rate of the headaches were higher for female patients. Prognosis was better than in female patients in comparison to male patients. Conclusions : The statistical survey conducted was based on Qi-section of DongEuiBoGam. It had significant differences in clinical characteristics between male & femal patients.
1. Objectives This study was to investigate correlation between the diagnosis of Sasang Constitutional Medicine(SCM) and the differentiation of syndromes according to the state of Qi, Blood, Body Fluid, Phlegm retention and the five viscera in storke inpatients. 2. Methods Stroke inpatients were investigated through questionaire of differentiation of syndromes and QSCC2. The results were analyzed statistically by one-way ANOVA on SPSS 12. 3. Results and Conclusions 1) Soeumin had a high grade on Qi-insufficiency than other consititutions. 2) Soyangin had a low grade on Qi-insufficiency, spleen disease than other consititutions. 3) There were not significant results in the investigation item of the sweating, constipation, diarrhea, anorexia, abdominal pain among the constitutions.
Objectives : The purpose of this paper is to study the correlation between cheekbone and life span prediction. Methods : The 『Huangdineijing』 was searched for verses that include terms that refer to the zygoma such as '顴骨', '䪼', '頄', '目下', '墻'. Terms such as '大骨' that are directly related to life span were searched as well, of which the results were analyzed. The relationship between bone shape and life span, the characteristic of facial bone diagnosis, the relationship between zygoma diagnosis and life span, and zygoma related contents in physiognomy texts such as the 『Mayixiangfa』 were examined. Results & Conclusions : Dagu[大骨, big bone] refers to bones in major joints that reflect the condition of Essence Qi, which is why the diagnosis of Dagu is key to determining one's life span. The zygoma is the big bone of the face, and a bad complexion in this area reflects pathogenic heat penetration into the Kidney, which is the foundation of Yin. As Kidney water as Yin Essence is directly connected to life, complexion change in the zygomatic area is highly relevant to life span. Moreover, as one of the main bones where the Kidney Essence is concentrated, the zygoma is the last to stand when the body is deteriorating, as it is the manifestation of heightened bone qi that is rooted in Yin Essence, thus an important site that provides clues to determine one's life span.
Objectives: The aim of this study is the development of the questionnaire for pattern identification of Liver-qi stagnation infertility. Methods: We conducted a literature review and retrieved the symptoms and diagnosis from Korean and Chinese literatures which mentioned pattern identification of Liver-qi stagnation infertility. Based on the findings, We conducted three times expert Delphi surveys on selection of question items and determination of weight. Results: 12 questionnaire items for subjects and 4 questionnaire items for researchers were selected from 19 references. From expert delphi surveys, we finally determined 14 questionnaire items (10 items for subjects, 4 items for researchers) which are clinically significant and obtained weight of question items. Conclusions: Questionnaire for pattern identification of Liver-qi stagnation infertility was developed through experts' discussion. Further study is required to identify the validity and reliability of this pattern identification instrument for Liver-qi stagnation infertility.
Purpose: The objective of this study was to obtain improved susceptibility weighted images (SWI) of the cervical spinal cord using respiratory-induced artifact compensation. Materials and Methods: The artifact from $B_0$ fluctuations by respiration could be compensated using a double navigator echo approach. The two navigators were inserted in an SWI sequence before and after the image readouts. The $B_0$ fluctuation was measured by each navigator echoes, and the inverse of the fluctuation was applied to eliminate the artifact from fluctuation. The degree of compensation was quantified using a quality index (QI) term for compensated imaging using each navigator. Also, the effect of compensation was analyzed according to the position of the spinal cord using QI values. Results: Compensation using navigator echo gave the improved visualization of SWI in cervical spinal cord compared to non-compensated images. Before compensation, images were influenced by artificial noise from motion in both the superior (QI = 0.031) and inferior (QI = 0.043) regions. In most parts of the superior regions, the second navigator resulted in better quality (QI = 0.024, P < 0.01) compared to the first navigator, but in the inferior regions the first navigator showed better quality (QI = 0.033, P < 0.01) after correction. Conclusion: Motion compensation using a double navigator method can increase the improvement of the SWI in the cervical spinal cord. The proposed method makes SWI a useful tool for the diagnosis of spinal cord injury by reducing respiratory-induced artifact.
Objective : This study is aimed to application possibility and status of pattern identification in middle and high school students. Method : This study was cross-sectional study. We recruited 277 students through school doctor program of Seongnam city in 2017. Male students are 66 in middle school, 32 in high school. Female students are 117 in middle school, 62 in high school. For pattern identification, we used the tool of Qui Xui Shui pattern identification in students. Students fill questionnaires in inquiry of pattern identification and Korean medicine doctor diagnosis inspection and palpation of students. Results : Among 277 students, no pattern identification were 114(41.2%). It appeared in the order of phlegm, blood deficinecy, qi deficiency, qi stagnation and qi reversal. Qi deficiency, qi stagnation, qi reversal, blood deficiency and phlegm pattern are statistically significant by gender except blood stagnation. of 277 students, 105 (37.9%) had one more pattern identification. Conclusions : This study showed possibility of Qui Xui Shui pattern for student health management. In the future, large scale follow up study will be needed to clarify the relationship of pattern identification and student's health.
