Long-Term Follow-up Survey of Postoperative Change of Pulmonary Artery Pressure in the VSD Patients with Severe Pulmonary Hypertension (중증 폐고혈압을 동반한 심실중격결손증 환자의 술후 폐동맥변화에 대한 장기추적)

  • At the Dept. of Thoracic and Cardiovascular Surgery of Pusan National University Hospital, postoperative cardiac catheterizations were performed in 12 patients of ventricular septal defect with severe pulmonary hypertension [Pp/Ps>0.75], who were operated during the period from July 1981 to Dec. 1986. The mean age of the patients preoperatively was 12.4 [range: 4-18] year-old and the mean follow-up duration was 25.8 [range: 8-53] month per patient. In comparison with the preoperative data, the systolic pulmonary artery pressure [SPAP] was decreased from 103.6*18.4 to 70.4*35.9 mmHg [p<0.01] and the Pp/Ps was decreased from 0.89*0.10 to 0.58*0.27 [p<0.01]. But the Rp/Rs and Rp were not meaningfully changed, from 0.31*0.16 and 7.6*0.4 unit to 0.41*0.32 and 8.0*6.6 unit, respectively. The preoperative Qp/Qs was bellow 2.0[mean: 1.6] in 3 out of 4 cases whose postoperative Rp/Rs and Rp were above 0.75 and 15 unit, respectively. On the contrary, the preoperative Qp/Qs was above 2.0 [mean: 3.5] in all of the 8 cases, whose postoperative Rp/Rs and Rp were below 0.50 and 10 unit, respectively.

  • In this paper, new polyphase sequences and a sequence generation method are suggested. These sequences are found to have good correlation properties. Since the suggested generation method consists only of integer sums and modular techniques, sequence generation is also easy. The performance of the sequences is investigated when used in QS-CDMA systems under frequency selective, time nonselective, slow Nakagami fading channel with additive white Gaussian noise.

Physiological Dead Space and Shunt Following Open Heart Surgery (개심술후 폐의 Physiologic dead space 와 Shunt 의 변화상 추적)

  • It has been recognized that the proper matching of ventilation and perfusion within the lung is essential for the efficient exchange of gases following open heart surgery. Physiologic shunt reflects the amount of blood going to lung units with inadequate ventilation and these are also areas of the lung with adequate ventilation but inadequate blood flow. This can be quantified by measuring physiological dead space. From January to August 1985, The physiologic dead space and shunt during postoperative course had been taken in 30 patients of open heart surgery in Soonchunhyang University Hospital. Twenty cases had congenital heart disease and acquired valvular heart disease were noticed in 10 cases. The physiological dead space and shunt during postoperative periods were calculated and we made 5 items of conclusion: 1. There is high probability of ventilation-perfusion mismatch in the acquired heart disease group compared to the congenital group. 2. Duration of the CPB can exert significant influences in the physiological dead space but less in the shunt fraction. 3. There is positive relationship between Qs/Qt and Vd/Vt in the group B [CPB>90 min.] but less reliable in correlation. 4. Perfusion impairment is more significant in the diminished pulmonic blood flow group compared to the increased pulmonic blood flow [Qp/Qs>2.0] group. 5. There is no significant ventilation-perfusion mismatch within the lung during all postoperative courses.

Inhibition of Quorum Sensing and Biofilm Formation by Synthetic Quorum Signal Analogues in Pseudomonas aeruginosa (합성된 쿼럼 신호 유사 물질에 의한 녹농균 쿼럼 센싱 및 생물막 형성의 제어)

  • Pseudomonas aeruginosa is an opportunistic pathogen that causes various infections on urinary track, cornea, respiratory track, and burn wound site, and mainly relies on quorum sensing (QS) for its virulence. To control the infectivity of P. aeruginosa, we previously synthesized the structural analogues of a major QS signal, N-3-oxododecanoyl homoserine lactone (3OC12-HSL) to use as a QS inhibitor. Two of them (5b and 5f) had been confirmed to have an inhibitory effect on LasR, a major QS signal receptor of P. aeruginosa in the screening by the recombinant Escherichia coli reporter. To further evaluate these compounds, we tested their efficacy to control the QS and virulence of P. aeruginosa. Unlike the result from E. coli reporter, both 5b and 5f failed to affect the LasR activity in P. aeruginosa, but instead they selectively affected the activity of QscR, another 3OC12-HSL receptor of P. aeruginosa. Interestingly, their effect on QscR was complex and opposite to what we obtained with E. coli system. Both 5b and 5f enhanced the QscR activity at the low concentration range (< 10 ${\mu}m$), but high concentration of 5f (${\approx}$1 mM) strongly inhibited QscR. While 5b and 5f didn't affect the production of proteases, the key virulence factor, they significantly reduced the biofilm formation that is important in mediating chronic infections. Especially, 5f inhibited the initial attachment of P. aeruginosa, rather than the biofilm maturation. Based on our results, we suggest that 5f can be applied for an anti-biofilm agent without increasing virulence of P. aeruginosa.

Monitoring of Bifenazate Resistant Two-spotted Spider Mite, Tetranychus urticae Using Molecular Detection Method (분자학적 진단방법을 이용한 bifenazate 저항성 점박이응애 모니터링)

  • In 2010, two-spotted spider mite, Tetranychus urticae was collected from the rose greenhouse and apple orchards in Cheongju (CJ), Chungju (CUJ)-1, CUJ-2, Kangjin (KJ), Yesan (YS), and Yeongju (YJ). Among them, KJ and YS strain showed high resistance to bifenazate of 964.5- and 1l30-fold, respectively. The other strains showed low resistance to bifenazate. By analyzing the mitochondrial cytochrome b (cytb) sequence, G126S point mutation was detected in KJ and YS strain. Thus, G126S point mutation in the mitochondrial cytb was available molecular detection marker for selection of bifenazate resistant T. urticae. Two molecular detection methods, quantitative sequencing (QS) and PCR amplification of specific alleles (PASA) were well detected specific G126S point mutation. Therefore, these methods can be used to monitor the resistance allele in field population of T. urticae and bifenazate resistance management strategy.

Inhibitory Effects of Stewartia koreana Extracts on Pseudomonas aeruginosa Biofilm Formation (노각나무 추출물이 Pseudomonas aeruginosa의 바이오필름 형성에 미치는 영향)

  • This study was conducted to investigate the potential of Stewartia koreana as anti-microbial materials. The branches, stems and leaves of S. koreana were extracted into 70% ethanol and their antibacterial activity against P. aeruginosa was confirmed. The leaf, branch and stems extracts (1 mg/disc) showed the antibacterial activity against P. aeruginosa and leaf extracts showed higher antibacterial activities than those from branch extracts. The MIC against P. aeruginosa was 0.8 mg/ml and showed bacteriostatic action. The inhibitory effects of extract on biofilm formation and gene expression related to biofilm formation of P. aeruginosa was determined by biofilm biomass staining, SEM and qRT-PCR analysis. The biofilm biomass and cell growth of P. aeruginosa in the cultures treated with 0.2~2.0 mg/ml of S. koreana leaf extracts were significantly decreased in a concentration-dependent manner. We observed that the extract had an inhibitory effect on the formation of P. aeruginosa biofilms at concentrations of 0.8 mg/ml by SEM. qRT-PCR analysis showed that the lasI and rh1I gene expression associated to quorum sensing (QS) in the cultures treated with 0.2~2.0 mg/ml of S. koreana leaf extracts were suppressed in a concentration-dependent manner. Based on the above results, it can be concluded that S. koreana leaf extracts can be used as anti-microbial material derived from natural materials, as demonstrated by the antibacterial action and inhibition of biofilm formation of P. aeruginosa by QS inhibition.

Descending Aorta Index and Pulmonary Index in Infants Comparison between Atrioventricular Septal Defects, At ial Septal Defects and Ventricular Septal Defects (심방실 중격 결손증에서의 하행대동맥, 폐동맥 지수)

  • To clarify the apparent hypoplasia of the descending aorta in infants with atrioventricular septal defect[AVSD] patients, we reviewed the catheterization data and angiograms of 34 consecutive patients with AVSD less than 1 year of age who underwent repair at our institution since 1985. We compared them to 10 patients with Atrial Septal Defect[ASD] and 10 patients with Ventricular Septal Defect[VSD] who were matched for age, size and Qp/Qs. The Descending Aorta Index [DAI] of the AVSD group was not different from the VSD or ASD groups, [147.9$\pm$ 34.8 mm2/m2 versus 158.6$\pm$ 31.5 mm2/m2 and 153.2$\pm$ 43.1 mm2/m2].However, the Pulmonary Artery Index [PAI] of the AVSD group was significantly larger than the other groups [684.3$\pm$ 170.7 mm2/m2 versus 454.1$\pm$ 109.1 mm2/m2 and 534.9$\pm$ 148.4 mm2/m2][p<0.05], as was the ratio of PAI/DAI in the AVSD group [4.99$\pm$ 1.77 versus 2.89$\pm$ 0.81 and 3.6$\pm$ 0.92][p<0.05]. Despite similar Qp/Qs ratios, both the mean PA pressure and the Rp/Rs in the AVSD group was higher than the VSD and ASD groups: 43.1$\pm$ 15.6 mmHg versus 29$\pm$ 11.6 mmHg and 24$\pm$ 18.1 mmHg [p<0.05], and 0.27$\pm$ 0.22 versus 0.14$\pm$ 0.03 and 0.11$\pm$ 0.05 [p<0.05] respectively. The apparent hypoplasia of the descending aorta in infants with AVSD is an illusion created by the abnormally large pulmonary arteries, which are significantly larger than in patients with ASDs or VSDs.

Aoric Valve Lesion in Type I Ventricular Septal Defect (제1형 심실중격결손에서 대동맥판막 병변)

  • Background: In this study, we investigated the risk factors for the development or progression of aortic regurgitation(AR) in patients with type I ventricular septal defect (VSD) to determine the optimal surgical timing and strategy. Material and Method: Three-hundred and ten patients with type I VSD with or without AR were included. The mean of age was 73.7$\pm$114.7 (1-737) months. One hundred and eighty six patients (60%) had no AR, 83 (27%) had mild AR, 25 (8%) had moderate AR and 16 (5%) had severe AR. Aortic valve was repaired in 5 patients and replaced in 11 patients with closure of VSD in the first operation. Four patients required redo aortic valve repair and 11 patients required redo aortic valve replacement. Age at operation, association with aortic valve prolapse, Qp/Qs, systolic pulmonary arterial pressure, VSD size and systolic pulmonary artery to aortic pressure ratio(s[PAP/AP]) were included as risk factors analysis for the development of AR. The long-term result of aortic valve repair and aortic valve replacement were compared. Result: Older age at operation, association with aortic valve prolapse, high Qp/Qs, and s[PAP/AP] were identified as risk factors for the development of AR (p<0.05, Table 2). The older the patient at the time of operation, the higher the severity of preoperative AR and the incidence of postoperative AR (p<0.05, Table 1, Fig. 1). For the older patients at operation, aortic valve repair had higher occurrence of AR compared to those who had aortic valve replacement (p<0.05, Fig. 2). Conclusion: From the result of this study, we can concluded that early primary repair is recommended to decrease the progression of AR. Aortic valve repair is not always a satisfactory option to correct the aortic valve pathology, which may suggest that aortic valve replacement should be considered when indicated.

  • In this paper, the trend of quality management system that suppliers in the automotive industry are adopting, the classification system of suppliers and car manufacturers requirements for quality are described. Due to the complexity of the requirements for quality, the introduction of an active quality management system which can meet all conditions is a difficult task. Thus, to cope with this difficulty, this paper shows the optimal requirements that suppliers have to consider when they are introducing quality management system and the discriminated strategies to assure parts quality by supplier model.
