광주지역 소의 큐열 항체 보유율 조사 (Prevalence of antibody titers for Coxiella burnetii in cattle in Gwangju area, Korea)

  • The antibody titers against Coxiella burnetii (Q fever) among cattle raised in Gwangju area were analyzed from February to October in 2015. The prevalence of antibodies in collected bulk-tank milk from 7 dairy cattle farms was 57.1% and the seroprevalence of 210 dairy cows randomly selected from those farms was 7.1%. By age, the seroprevalence was 3.3% in less than 4 years of age, 9.0% between 4 and 7 years of age, and 28.6% in more than 8 years of age. On the other hand, the seroprevalence of the Hanwoo cattle was 0.4%. The result suggested that the antibodies against Coxiella burnetii increase as the age of the dairy cows increases and therefore, it is necessary to keep monitoring the prevalence of Q fever in Gwangju area.

전염병관리 관련법령의 변화 추이분석 및 향후 개정방향에 관한 연구 (The amendment tendency analysis of the Korean Infectious Disease Prevention Act and a recommendation for the next amendment)

  • This Study has been carried out to make a recommendation for the next amendment of the Infectious Disease Prevention Act with a specific focus on the kind of notifyable disease. Korean, Japanese, German, U.S, English and French acts on infectious diseases prevention were reviewed, compared with and analized in regards of numbers and kinds of notifyable infectious diseases and their tendency of amendments. An criteria was designed to assess the level of validity of diseases to be designated in the act. Four items, the fatality (greater than 10% or not), the possibility to make a big epidemic, the availability of efficient vaccination and the usefulness of isolation, are used in the assessment. This index is applied to the diseases in Korean and other countries' Infectious Disease Prevention Acts. Results are as follows: 1. The Korean Infectious Disease Preventon Act has a unique way of classifying the notifyable infectious disease, that is, the first, the second and the third class. But the author cannot find the basis of classification. No other countries reviewed have the similar classification. 2. The ten diseases, cholera, plague, yellow fever, diphtheria, typhoid fever, poliomyelitis, rabies, tetanus, malaria, and meningococcal meningitis are designated as the notifyable diseases not only in Korea but also in Japan, Germany, United States, England and france. 3. Thirty seven diseases including small pox, Lassa fever, anthrax, influenza, German measles, Legionellosis, infection with E. coli O157:H7, Q-fever, brucellosis, Lyme disease are designated as legal disease at least one of the above mentioned countries. 4. The Korea has been coped with the change of the infectious disease occurrence for last fifty years in amendment of the Infectious Disease Prevention Act. 5. Japan has a special infectious surveillance system composed of 3,880 clinics throughout the whole country. 6. Germany has classified infectious diseases in five categories which are based on seriousness of disease. Any confirmed death, cases and suspected cases in class I should be reported within 24 hours. But only confirmed death and cases in class II, but not suspected cases, are reportable in Germarny. 7. Plague, bacillary dysentery, pertussis, mumps, Japanese encephaltis and Korean hemorrhagic fevers are diseases with high credits validity index among Korean legal disease. 8. German measles, anthrax, E. coli O157 : H7 infection, Lassa fever, Q-fever, brucellosis are high in validity index among those which are not designated in Korea but designated in other countries. In conclusion, the Korean Infectious Disease Prevention Act has well been coped with the changes of infectious disease occurrence for last fifty years, but the classification basis and the validity of diseases to be designated as legal diseases is worth reevaluating.

경북 일부지역 축산(한우) 농업인의 인수공통감염병 인지도 (Awareness on Zoonoses among Livestock(Korean Native Cattle) Farmers in Gyeongsangbuk-do)

  • Objectives: General livestock farmers are known as a high riskgroup for zoonoses, but studies of general livestock farmers in regard tozoonoses have been rare in Korea. We surveyed awareness of zoonoses amonggeneral livestock farmers to suggest directions for education. Methods: A questionnaire was developed examining the workbehaviors and risk factors of general livestock farmers. We conducted a questionnairesurvey on awareness on zoonoses among 265 general livestock farmers. Results: The awareness rates for zoonosis itself,brucellosis, and q-fever were 13.0%, 65.0%, and 2.3%, respectively. The awarenessrate of zoonoses and brucellosis tended to increase with sex. Conclusions: Livestock is the principal reservoir ofzoonoses. Therefore, effective working guidelines for preventing zoonoses amonggeneral livestock farmers must be developed and an educational program onzoonoses is needed for general livestock farmers. Furthermore, publicityactivities on the prevention of zoonoses are needed for high-risk groups.

  • Q fever is a zoonotic disease caused by the obligate intracellular bacterium Coxiella (C.) burnetii and affects wild and domestic animals worldwide. The aim of this study was to evaluate the seroprevalence of C. burnetii in native Korean goat (Capra hircus coreanae) in Gyeongbuk province, Korea, using ELISA. A total of 256 goat blood samples from 56 farms in Gyeongbuk province were collected between May 2012 and March 2013. Among them, 22 (8.6%) samples from 10 (17.9%) farms were seropositive for C. burnetii by ELISA. According to regional analysis, the seroprevalences among goat farms in eastern, western, southern, and northern areas of Gyeongbuk province were 0%, 18.2%, 36.8%, and 6.3%, respectively, showing the highest seroprevalence in the southern region. Among 22 counties in Gyeongbuk province, 10 (45.5%) counties had one or more farms positive to C. burnetii antibody. Accordingly, the seroprevalence of C. burnetii in high-risk humans and animals are constantly demanded by regional investigation.

Prognostic Factors and Survival in Acute Myeloid Leukemia Cases: a Report from the Northeast of Iran

  • Background: Acute myeloid leukemia (AML) is a clonal hematopoietic disorder resulting from genetic alterations in normal hematopoietic stem cells. The aim of this study was to evaluate prognostic factors and survival of AML patients in the Northeast of Iran. Materials and Methods: This retrospective study covered 96 patients with AML referred to Emam Reza Hospital, Mashhad city, Iran, from 2009 to 2015. Age, sex, blood group, type of AML, fever, consumption of amphotericin B, cytogenetic forms and survival were analyzed. Also, WBC, hemoglobin and platelet levels were checked. Mean follow-up was 30.5 months (60.4% mortality). Survival was plotted by GraphPad Prism 5 with Log-rank test. Results: The mean age for all AML patients at diagnosis was 40.4 years (range, 17-77 years). Some 42.7% patients were aged <35 years and 40.6% were male. In all patients, 76% had fever and 50% consumed amphotericin. T(15;17)(q22;q21) had the most prevalence (37.7%) compared to other forms. Out of 92 patients, O+(30.4%) was the most common blood group and AML-M5 (28.3%) the most common subtype. There was a significant difference in survival based on WBC and consumption of amphotericin B (P<0.05). Conclusions: WBC level, fever and consumption of amphotericin B proved to be factors for survival of AML patients. The mean age for patients in Iran is lower than other areas in the World and also survival in this study was higher than in other studies.

Simple and rapid colorimetric detection of African swine fever virus by loop-mediated isothermal amplification assay using a hydroxynaphthol blue metal indicator

  • In this study, a simple loop-mediated isothermal amplification (LAMP) combined with visual detection method (vLAMP) assay was developed for the rapid and specific detection of African swine fever virus (ASFV), overcoming the shortcomings of previously described LAMP assays that require additional detection steps or pose a cross-contamination risk. The assay results can be directly detected by the naked eye using hydroxynaphthol blue after incubation for 40 min at 62℃. The assay specifically amplified ASFV DNA and no other viral nucleic acids. The limit of detection of the assay was <50 DNA copies/reaction, which was ten times more sensitive than conventional polymerase chain reaction (cPCR) and comparable to real-time PCR (qPCR). For clinical evaluation, the ASFV detection rate of vLAMP was higher than cPCR and comparable to OIE-recommended qPCR, showing 100% concordance, with a κ value (95% confidence interval) of 1 (1.00~1.00). Considering the advantages of high sensitivity and specificity, no possibility for cross-contamination, and being able to be used as low-cost equipment, the developed vLAMP assay will be a valuable tool for detecting ASFV from clinical samples, even in resource-limited laboratories.

경기도 낙농업자의 주요 인수공통감염증 인지도 (Awareness of Major Zoonoses among Dairy Farmers in Gyeonggi Province)

  • 2007년 경기도 낙농업 농가 482개소의 716명을 대상으로 인수공통감염증(브루셀라증, 큐열, 장출혈성대장균감염증)에 대한 설문연구를 실시하여 인수공통감염증에 대한 인지도를 분석하였다. 브루셀라증 인지율은 90.2%(646명), 큐열의 인지율은 2.5%(18명), 장출혈성대장균감염증의 인지율은 56.6%(405명)이었다. 성별에 따른 인지율은 브루셀라증 인지율은 남자 92.4%, 여자 86.4%, 큐열의 경우 남자 2.2%, 여자 3.1%, 장출혈성대장균감염증은 남자 60.7%, 여자 49.2%이었다. 연령에 따른 인지율은 브루셀라증의 경우 39세 이하가 94.3%, 40대가 92.2%, 50대가 89.5%, 60세 이상이 85.6% 이었으며, 큐열은 각각 5.7%, 2.9%, 1.9%, 2.1%, 장출혈성대장균감염증은 각각 64.2%, 63.8%, 52.9%, 46.4%이었다. 학력에 따른 인지율은 브루셀라증은 초등학교 졸업 이하 84.4%, 중학교 졸업 88.4%, 고등학교 졸업 93.3%, 기술(전문)학교 졸업 이상 96.1% 이었으며, 큐열은 각각 1.3%, 3.2%, 1.9%, 5.3%, 장출혈성대장균의 경우 각각 41.6%, 52.8%, 64.4%, 69.7%로 브루셀라증과 장출혈성대장균감염증에서 학력별로 유의한 차이를 보였다. 브루셀라증과 장출혈성대장균감염증에 대해 연령, 성별로 보정하고 학력과 사육하는 소중 젖소가 50%를 넘는 낙농업자 중 사육 젖소 두수를 이용해 로지스틱 회귀분석을 시행한 결과, 장출혈성대장균감염증에서 초등학교 졸업 이하 학력군에 비해 고등학교 이상의 학력군이, 사육 젖소 49두 이하 군에 비해 사육 젖소 70두 이상 군이 장출혈성대장균감염증 인지도가 유의하게 높았다. 브루셀라증, 큐열, 장출혈성대장균감염증을 들어본 적이 있는 연구 대상자를 상대로 각 인수공통감염증에 대하여 알게 된 경로를 묻는 질문에서는 모두 텔레비전을 응답한 경우가 가장 많았다. 전파경로에 관한 질문에서 브루셀라증의 경우 '소 분만과정 참여 및 태반의 비위생적 처리'라고 응답한 대상자가 63.2%로 가장 높았으며, 큐열과 장출혈성대장균감염증의 경우 '생으로 된 소고기나 부산물'이라고 응답한 대상자가 각각 66.7%, 64.2%로 가장 높았다. 인수공통감염증 예방이 어려운 이유를 묻는 질문에는 '보호구가 불편하고 거추장스러워' 46.8%, '예방법을 몰라서' 34.1%, '보호장비가 없어서' 21.8%, '예방의 필요성을 못 느껴서' 18.9% 등의 순이었다. 이 연구는 인수공통감염증의 고위험군인 낙농업자의 브루셀라증, 큐열, 장출혈성대장균감염증의 인지도를 연구하였고, 인수공통감염증의 교육 및 홍보에 기초자료로 기여할 수 있을 것이다.

A Novel Marker for the Species-Specific Detection and Quantitation of Shigella sonnei by Targeting a Methylase Gene

  • Shigella sonnei is a causal agent of fever, nausea, stomach cramps, vomiting, and diarrheal disease. The present study describes a quantitative polymerase chain reaction (qPCR) assay for the specific detection of S. sonnei using a primer pair based on the methylase gene for the amplification of a 325 bp DNA fragment. The qPCR primer set for the accurate diagnosis of Shigella sonnei was developed from publically available genome sequences. This quantitative PCR-based method will potentially simplify and facilitate the diagnosis of this pathogen and guide disease management.
