Immunochemical Studies of Starfish Gangliosides: Production of Monoclonal Antibody against AG-2, the Major Ganglioside of Starfish Acanthaster planci, and Detecting Its Distribution in Tissues by TLC Immunostaining

  • In this study, we establish a thin-layer chromatography (TLC) immunostaining method for detecting starfish gangliosides. A new monoclonal antibody (MAb) against AG-2, the major gangliosides molecular species of Acanthaster planci, was produced by fusing hybridoma with splenocytes immunized to liposomal AG-2. BALB/c male mice were injected with liposomal AG-2 antigen, and immunized. Their splenocytos were isolated and fused with hypoxanthine-aminopterine-thimidine (HAT)-sensitive mouse myeloma cells. Hybridomas producing MAb reactive to AG-2 were cloned using the limited dilution method. Established hybridomas were cultured in eRDF medium. Crude MAb produced from clone 8D4 was purified with a magnesium pyrophosphate column. Enzyme immunoassay and TLC immunostaining of AG-2 were performed using the purified MAb. Structurally related gangliosides did not cross-react with anti-AG-2 antibodies. The detection limit of TLC immunostaining was 50 ng of AG-2. The newly established immunostaining method was further developed for detecting AG-2 distribution and qualitative analysis in tissues and/or organs. Our results show that the majority of AG-2 is present in the stomach of male A. planci, while AG-2 is distributed not only in the stomach but also in the the pyloric caeca of female A. planci.

Immunohistochemical study of the somatostatin-immunoreactive cells in the gastrointestinal tract of the native korean goat during postnatal periods (생후 발생단계에 따른 한국재래산양 위장관에 있어서 somatostatin 면역반응세포에 대한 면역조직화학적 연구)

  • The regional distribution and relative frequency of somatostatin-immunoreactive cells in the gastrointestinal tract were studied during the postnatal development(1 day old, 1 month old, 6-month-old and adult) of the Korean native goat by immunohistochemical methods. Somatostatin-immunoreactive cells were found in the entire gastrointestinal tract and were most predominant in the fundic gland region except for the pyloric gland region of the 1 day old. The number of these cells was observed to decrease along the gastrointestinal tract and to decrease according to the increase with age in all postnatal stages. In this study, immunoreactive cells revealed the differences of regional distribution and the relative frequency in the gastrointestinal tract during postnatal development. These results suggest that although the functional significance of fetal endocrine cells remains unknown, this was caused by changing of feeding habits and physiological conditions during different ages.

Pathological Studies on the Esophagogastric Ulcers in Swine (돼지의 위궤양(胃潰瘍)에 관한 병리학적(病理學的) 연구(硏究))

  • Stomachs from 3,354 fattening pigs were examined at slaughter during the period from September 1980 to August 1981. Pigs of both sexes and $Landrace{\times}Hampshire$ crossbreds were included in the present studies, and they weighed about 60-120kg. Gross pathologic alterations of the stomach were classified as normal, epitnelial change, erosion, ulcer and scar formation. Representative tissue sections were taken from the stomach lesions at random and fixed in 10% buffered formalin. Cut sections were stained with hematoxylin and eosin, and examined histopathologically. The results obtained in the present studies were as follows. 1. In the seasonal prevalence of gastric ulceration, severe ulceration with erosion was shown during the Autumn and Winter, whereas mild ulceration was mainly shown during the Spring and Summer. 2. Of the 3,354 stomachs of the pigs, 20.8% were found to be normal. Of the rest, 40% had epithelial changes, followed by erosion (24.7%), ulcer (13.6%) and scar formation (0.9%), respectively. 3. In the prevalence of ulcers in the different regions of the stomach, the fundic region had the highest rate (61.6%) of ulceration, followed by cardiac (21.7%), esophageal (15.0%) and pyloric region (1.7%). 4. The principal gross changes were severe epithelial changes with keratotic proliferation in the esophageal region, and in the fundic region severe folding of the stomach wall was covered with bloody mucous exudates. 5. Main histopathological changes were inflammatory cell infiltrations in most cases, hemorrhages in acute ulcers and prominent proliferation of granulation tissues in chronic ulcers.

Surgical Removal of the Lung Lobe Metal Foreign Body in a Dog

  • A 4-year-old, weighing 7.6 kg, castrated male, Pug presented with ingestion of gastric cavity foreign body. Physical examination revealed panting, retching and hyper-salivation. Blood chemistry and complete blood cell count were normal, but hypophosphatemia was observed. An abdominal radiograph revealed the foreign body (FB), round shape and 2 cm length, at the pyloric region of stomach. A thoracic radiograph revealed an incidental metal FB, 3.5 cm length, at the cranial portion of the diaphragm. An upper gastrointestinal endoscopy was performed to remove the FB in the stomach and then a peach-pit was removed. However the metal FB was not found in the esophagus therefore a lateral thoracotomy was performed. A right lateral thoracotomy through the $7^{th}$ intercostal space was accomplished to expose the right caudal lung lobe. After open the thoracic cavity, foreign body was not observed by gross evaluation and caudal lung lobe was attached to the diaphragm. The FB was identified inside the lung lobe and surrounded by granulation tissue. The metal FB (sewing needle) was removed with blunt dissection and incised lung lobe was sutured using absorbable suture material PDS 4-0 with interrupted suture. A thoracotomy tube was inserted into the thoracic cavity during surgery. Patient's respiration became stable after surgery. A chest tube was removed 3 days after surgery. No complications were noted and the dog was discharged 4 days after surgery. In small animal, foreign body ingestion is a common reason for emergency. After ingestion of the FB, perforation through the esophagus and migration to inside the lung lobe is not common in small animals. In this case, thoracic metal FB was identified incidentally and removal of a thoracic FB with thoracotomy was performed successfully.

Disruption of the Tff1 gene in mice using CRISPR/Cas9 promotes body weight reduction and gastric tumorigenesis

  • Trefoil factor 1 (TFF1, also known as pS2) is strongly expressed in the gastrointestinal mucosa and plays a critical role in the differentiation of gastric glands. Since approximately 50% of all human gastric cancers are associated with decreased TFF1 expression, it is considered a tumor suppressor gene. Tff1 deficiency in mice results in histological changes in the antral and pyloric gastric mucosa, with severe hyperplasia and dysplasia of epithelial cells, resulting in the development of antropyloric adenoma. Here, we generated Tff1-knockout (KO) mice, without a neomycin resistant ($Neo^R$) cassette, using the clustered regularly interspaced short palindromic repeats/CRISPR-associated nuclease 9 (CRSIPR/Cas9) system. Though our Tff1-KO mice showed phenotypes very similar to the previous embryonic stem (ES)-cell-based KO mice, they differed from the previous reports in that a reduction in body weight was observed in males. These results demonstrate that these newly established Tff1-KO mice are useful tools for investigating genetic and environmental factors influencing gastric cancer, without the effects of artificial gene insertion. Furthermore, these findings suggest a novel hypothesis that Tff1 expression influences gender differences.

Preemptive pyloroplasty for iatrogenic vagus nerve injury in intrahepatic cholangiocarcinoma patients undergoing extensive left-sided lymph node dissection: a retrospective observational study

  • Background: Intrahepatic cholangiocarcinoma (ICC) of the left liver often shows left-sided lymph node (LN) metastasis. If gastric lesser curvature is extensively dissected, it can induce an iatrogenic injury to the extragastric vagus nerve branches that control motility of the pyloric sphincter and lead to gastric stasis. To cope with such LN dissection-associated gastric stasis, we performed pyloroplasty preemptively. The objective of this study was to analyze our 20-year experience of preemptive pyloroplasty performed in 10 patients. Methods: We investigated clinical sequences of 10 patients with ICC who underwent preemptive pyloroplasty following left hepatectomy and extended left-sided LN dissection. Incidence of gastric stasis and oncological survival outcomes were analyzed. Results: All 10 patients were classified as stage IIIB due to T1-3N1M0 stage according to the 8th edition of American Joint Committee on Cancer staging system. The overall patient survival rate was 51.9% at 1 year, 25.9% at 2 years, and 0% at 3 years. Seven patients showed uneventful postoperative recovery after surgery. Two patients suffered from gastric stasis, which was successfully managed with supportive care. One patient suffered from overt gastric paresis, which was successfully managed with azithromycin administration for 1 month. Conclusion: We believe that preemptive pyloroplasty is an effective surgical option to prevent gastric stasis in patients undergoing extensive left-sided LN dissection. Azithromycin appears to be a potent prokinetic agent in gastroparesis.

Prevalence and treatment of gastric ulcers in Thoroughbred racehorses of Korea

  • Background: Gastric ulcer is one of the prevalent diseases in racehorses. However, it has not been recognized as important in Korea, and drugs used to treat gastric ulcers are included in the doping test list, so they are not allowed to be administered to racehorses in training. Objectives: This study was performed 1) to investigate the prevalence and the severity of gastric ulcers in Thoroughbred racehorses in Korea, 2) to confirm the therapeutic effect of ranitidine and omeprazole, and 3) to compare the efficacy between ranitidine and omeprazole. Methods: Forty-nine horses were randomly recruited, and gastroscopy was performed within two days after racing. Twelve horses with a sum grade of five or higher were randomly assigned to two treatment groups. Seven horses were administered ranitidine, and five horses were administered omeprazole. Follow-up gastroscopy was scheduled within one to five days after finishing the treatment. Results: The prevalence of gastric ulcer in Korean Thoroughbred racehorses after racing was 100%, and the grade was more severe in the non-glandular region than in the pyloric region. There was no correlation between the severity of gastric ulcer in the two regions. Omeprazole had a greater therapeutic effect than ranitidine. Conclusions: This study shows the importance of recognizing gastric ulcers as an important factor, and omeprazole as a possible treatment option in Korea, as it has been removed from the list of prohibited substances for racehorses. Thus, the use of omeprazole is currently recommended until one day before the race.

A Case of Long Term Survival of Gastric Cancer on Trastuzumab Based Treatment (Trastuzumab으로 장기 생존한 진행성 위암 증례 1예)

  • We report a case of long term survival on trastuzumab based treatment. A 51-year-old man with dyspepsia received esophagogastroduodenoscopy on another hospital and was transferred for further evaluation under the impression of advanced gastric cancer, Borrmann type III, antrum, lesser curvature. After further studies in our hospital, the patient was diagnosed with advanced gastric cancer, adenocarcinoma, moderately differentiated with pancreas invasion and lymph node metastasis. Though he was recommended with chemotherapy, he refused and left for oriental herbal medicine. After 4 months, the patient was admitted through emergency room for hematemesis. Diagnosed with gastric outlet obstruction due to gastric cancer in the antrum, he underwent the placement of pyloric metal stent insertion. Immunohistochemical staining showed HER2-positive finding, and he was treated with palliative chemotherapy of trastuzumab, capecitabine, and cisplatin, 16 times during 11 months. The patient showed neutropenia after the therapy, so cisplatin was left out, and he received combination chemotherapy of trastuzumab and capecitabine, 34 times during 25 months. Response evaluation showed no remarkable change in extent of primary stomach cancer, lymph node metastasis, and regression of metastasis site, and the patient is continuing chemotherapy.

Clinicopathological and endoscopic features of Helicobacter pylori infection-negative gastric cancer in Japan: a retrospective study

  • Background/Aims: Helicobacter pylori infection-negative gastric cancer (HPNGC) has not been systematically investigated in consecutive patients. Hence, this study aimed to investigate the clinicopathological and endoscopic features of HPNGC. Methods: This single-center retrospective study selected participants from patients with gastric cancer who were treated at the Fukuoka University Chikushi Hospital between January 2013 and December 2021. Only patients diagnosed with HPNGC were enrolled, and their clinicopathological and endoscopic features were analyzed in detail. Results: The prevalence of HPNGC in the present study was 2.6% (54/2,112). The types of HPNGC observed in each gastric region were as follows: advanced gastric cancer was observed in the cardia; gastric adenocarcinoma of fundic-gland differentiation, gastric adenocarcinoma of foveolar-type presenting with whitish elevation and raspberry-like foveolar-type gastric adenocarcinoma, gastric adenocarcinoma arising in polyposis, and gastric adenocarcinoma with autoimmune gastritis were observed in the fundic gland region ranging from the gastric fornix to the gastric body; signet-ring cell carcinoma was observed in the gastric-pyloric transition region ranging from the lower gastric body to the gastric angle; and well-differentiated tubular adenocarcinoma with low-grade atypia was observed in the antrum. Conclusions: This study revealed that tumors from each gastric region exhibited distinct macroscopic and histological types in HPNGC.

Morphological Variations in Relation to Geographical Distribution of Pond Smelt, Hypomesus olidus Pallas (한국산 빙어의 지리적 형태변이에 대하여)

  • The authors made an investigation concerning the geographical distribution and some major differentiations in morphological characteristics associated with the each habitat, of the pond smelt, Hypomesus olidus, which was introduced into Korea more than 50 years ago. Major characteristics utilized for the comparison study were upper jaw (maxillary), number of fin rays, scale structure, vertebral count, number of pyloric caeca, number of gill rakers, and the relative measurements of these and other internal characters. 1. The main areas of natural distribution are Paro Lake, Soyang Lake, Han River, Euirim Lake, and Ogjeong Lake (or Unam Lake) and the tributaries of these waters. 2. Morphological variations varied when the mean values of measurements from each area were compared with the standard measurement value by mosaic comparison, and in some instances a significant variation was found. 3. The most significant variations were found in the number of fin rays of pectoral and anal fins; the lengths from the tip of snout to the origin of pectoral fin, from snout to ventral fin, and from snout to anal fin, relative to standard body length; eye depth relative to head length; upper jaw: the number of pyloric caeca and the scale structure. Variations, on the other hand, by each habitat were not clear in the numbers of ventral and dorsal fin rays, gill rakers, vertebrae and lateral line scale : and the depth of caudal peduncle and distance from the tip of snout to the origin of dorsal fin relative to standard body length. 4. The scales of pond smelt showed some differences in the shape of scales, the shape and position of the focus and the number of ridges according to the scale positions on the body. No radii were present. The relations between longitudinal and transverse diameters of the scale suggest that the pond smelts of Soyang Lake and Un-am Lake are closely related, and those of Han River and Euirim Lake are also closely related. 5. The geographical variations in morphology of the pond smelt seem to have resulted from the variations in turbidity, water temperature, salinity (rather conductivity) and currents. 6. From the results obtained, it may be concluded that such factors as supramaxillary, relation between scale length and its breadth, number of caudal vertebrae and eye depth relative to head length may be used as the key characters for the classification of geographical varieties of Pond smelt.
