Objective: To examine the effects of Gagam-Samryoungbeakchul-san (加減 蔘苓白朮散) on a premature ventricular contraction patient with dizziness and chest discomfort. Methods: A patient diagnosed with premature ventricular contraction was treated with herbal medicine and acupuncture. The period of admission was 15 days, and we measured the electrocardiogram before and after treatment. We evaluated the improvement in symptoms by Global Assessment (G/A), and checked the pulse rate by oximetry three times a day. We estimated the efficacy of treatment by analyzing the relationship between the average pulse rate and symptoms. Results: After Gagam-Samryoungbeakchul-san treatment and acupuncture therapy, the average pulse rate increased from 36.5 to 58. This increase in average pulse rate was accompanied by a reduction in dizziness of 40%, chest discomfort of 30%, and frequency of bigeminy in the electrocardiogram. Conclusions: This case report confirmed the effectiveness of Gagam-Samryoungbeakchul-san on premature ventricular contraction, but further study is warranted.
Pacemaker therapy has been established as a routine procedure in a variety of disturbances of :cardiac rhythm. Cardiac pacing has greatly reduced the mortality rate in patients suffering from bradyarrhythmias complicated by Stokes-Adams attacks. However, in spite of the simplicity of the treatment and reliability of the devices available today, patients and physicians alike are invovled in a multitde of pacemaker specific problems. Clinical experience with permanent transvenous pacing during a 3 year period is presented. A total of 20 pacemaker operations were performed in 13 patients. The complications in our 20 pacemaker operations were headed by skin ulceration of 3 generator pockets and early dislocation of 3 electrodes. In an additional 2 patient, stimulation of diaphragm due to a position of electrode in the coronary sinus was noticed in one and battery failure of pulse generator was detected in the other. Literatures were reviewed.
The incidence of nephritis associated with Henoch-Sch nlein purpura varies, but glomerulonephritis consistently accounts for most of the associated morbidity and mortality. A very small number of Henoch-Sch nlein purpura develop rapidly progressive glomerulonephritis. A three-year old male patient who showed acute nephritic nephrotic syndrome developed abdominal pain, arthralgia and multiple purpurae on lower extremities later. Peritoneal dialysis was done at the 6th hospital day and continued for 7 months. Renal biopsy disclosed crescentic glomerulonephritis (with 81% crescent formation) and methylprednisolone pulse therapy was done. These days, his general condition is good, but serum creatinine levels are 1.2-1.3 mg/dL. This case was reviewed briefly with the literatures.
Primary radiation force on ultrasound contrast agents (UCA) in a propagating and standing acoustic field was explored. A specific ultrasound contrast agent $Albunex^{(R)}$ and $Optison^{(R)}$ were chosen for simulation. The model was developed based on a shelled bubble model proposed by Church. The numerical simulation suggests that bubble translational motion is more significant in therapeutic ultrasound due to higher intensity and long pulse duration. Even a single cycle of a propagating wave of 4 MPa at 1 MHz can cause a bubble translational motion of greater than $1{\mu}m$ which is approximately one tenth of capillary. Hence, UCA characteristics can be significantly changed in therapeutic ultrasound without rapid bubble collapses.
We report an extremely rare case of traumatic cervical spinal subdural hematoma not related to intracranial injury. There has been no report on traumatic cervical spinal subdrual hematoma not related to intracranial injury. A 27-year-old female patient was admitted to our emergency room due to severe neck pain and right arm motor weakness after car collision. On admission, she presented with complete monoplegia and hypoesthesia of right arm. Magnetic resonance imaging (MRI) revealed subdural hematoma compressing spinal cord. Lumbar cerebrospinal fluid (CSF) analysis revealed 210,000 red blood cells/$mm^3$. She was managed conservatively by administrations of steroid pulse therapy and CSF drainage. Her muscle power of right arm improved to a Grade III 16 days after admission. Follow-up MRI taken 16th days after admission revealed almost complete resolution of the hematoma. Here, the authors report a traumatic cervical spinal SDH not associated with intracranial injury.
This study was performed to investigate the biostimulation effects of low level laser therapy (LLLT) on the fungus, Candida alvicans, according to the interval of irradiation during the cell cycle. Samples were divided into 5 groups which were P7-2Hr, P7-4Hr, P15-2Hr, P15-4Hr and Co. Sample was irradiated for 1 minute with 2 hours or 2 minutes with 4 hours of elapsed time during 28 hours of the cell cycle of Candida albicans, and the optical density was assessed by spectrophotometry every 2 hours. It was found that there was significant difference, in the cell growth, between 2 hour and 4 hour interval laser irradiation, although the total energy of each group using identical pulse during exam was equal.
We report the case of a 7-year-old boy who showed treatment-nonresponsive hypotension (59/29 mmHg) and decreased left ventricular systolic function (fractional shortening 22%) in the acute stage of Kawasaki disease (KD). The present case serves to highlight that methylprednisolone pulse therapy should be considered in patients with intravenous immunoglobulin nonresponsive symptomatic myocarditis during the acute stage of KD.
Squamous cell carcinoma (SCC) is a form of neoplasm that origins from the epithelial surface of many organs. Ocular occurrence in small animals is rare, especially in the central cornea without the involvement of limbus or conjunctiva. In the current case, a 10-year-old, spayed female Shih-Tzu was presented with a central corneal mass. Through an ophthalmic examination the sign of corneal scarring around the mass was found. Pink fleshy lesion that protruded outward was removed through superficial keratectomy. The mass with the size of $8.5{\times}6.5$ millimeter was histopathologically diagnosed as corneal SCC infiltrating not only the epithelium, but also the superficial corneal stroma with surrounding superficial lymphoplasmacytic and neutrophilic chronic keratitis. Also, the adjacent, non-affected corneal epithelium was markedly hyperplastic and keratinized; the adjacent stroma was moderately vascularized and fibrotic. The pulse-dose therapy using 1% topical 5-flourouracil were applied for five cycles. The tumor has not recurred through nine months of follow-up time.

This study examined effects of the communication accompanied with music therapy on the anxiety of patients during a MRI examination. In the experiment group consisting of thirty patients and the control group consisting of thirty patients, the experiment was performed from February 1st 2014 to March 31st 2014 in hospital C located in Daegu Korea. The research design was a non-equivalent control group quasi-experimental study. The results showed that the anxiety score (VAS score) on communication with music therapy of the experimental group was significantly lower (p=.001), than that of the control group. Among the vital signs, the blood pressure was similar in both groups. On the other hand, the pulse rate of the experiment group was significantly lower (p=.001). Four categories of the physical discomfort (perspiration, facial change, change in physical posture, and change in vocal sound) did not show any statistically significant difference between the control and experiment group. Among the mental discomforts, dizziness did not have a significant difference between the experiment group and the control group, but the experimental group had significantly low fear (p=.001) and anxiety (p=.001). From the above results, communication accompanying music therapy proves is an effective alternative examination method that reduces the anxiety of patients.

Purpose: During the pelvic radiation therapy, it is performed with controlling bladder volume because the range of irradiation is changed depending on the bladder volume. Therefore in this study, we evaluate the accuracy of BVI by tracing the change of bladder volume using ultrasonic pulse diagnosis equipment(BVI 6100, Medical supply Co. LTD) Material and Methods: From January 2017 to September 2018, 19 patients who received pelvic radiation therapy at Pusan National University Hospital were included. To treat the patient, we compared that the bladder volume obtained from the BVI and the bladder volume obtained from the CBCT image then we evaluated for significance. Results: There was a significant correlation of r=0.773, BVI and CBCT in the whole volume section. However, based on the bladder volume in the RTP Image the p value was shown to be 0 at 125~175cc and lower than 0.05 at 175~275cc, And more than 0.05 at 275~375cc. Conclusions: In this study, the patient whose bladder volume is above than 275cc, there is a significance of bladder volume between BVI and CBCT image. However, we could make a decision to be undermined the reliability of BVI measurement in the case of the patient with emptied his urine. Therefore, it is possible to acquire a significant value for 175~275cc patients to use the correction value of BVI and the appropriate tolerance of volume.
