Space Usage and User Needs of Postpartum Care Centers (산후관리시설의 사용실태 및 사용자 요구에 관한 연구)

  • This study examined the current space usage of postpartum care center and postpartum mother's assessment of postpartum care center. This study was conducted questionnaire web survey. 220 women who used postpartum care center were surveyed. The data were analyzed by using SPSS Program for Win 12.0. Frequency, percentage, mean were used. The major findings were as follows: The postpartum care center was consisted of 5 areas such as living area, sanitary area, public area, service area and administration area. Living and public area were mainly occupied, but service area was insufficient. The users showed high satisfaction with mother room, and low satisfaction with nursing room and shower room. The users complained of size of mother room, collaboration with others in shower room and toilet. The users wanted guest meeting room for users' privacy of living room. The users wanted service area such as massage room, fitness room, physical care roon.

A Convergence Study on Feasibility of Expanding Establishment of Public Postpartum Care Centers: Benefit-Cost Analysis (공공산후조리원 확대의 타당성에 관한 융합 연구: 편익-비용 분석을 중심으로)

  • The purpose of this study is to analyze the economic feasibility of public postpartum care centers established for health care of low-income postpartum mothers. 6 of 8 public postpartum care centers were used to verify the economic feasibility and research data were collected through the centers. A sensitivity analysis was performed with the bed turnover rate, assuming that users in a postpartum care center will be decreased. As a result, B/C ratio was estimated to be 1.50 and the net benefit was estimated to be KRW 186,557 in million. A sensitivity analysis showed that B/C was 1.42 at 17.4 of bed turnover rate and 1.26 at 15.4 which indicates the lowest bed turnover rate among subjects. A public postpartum care center was evaluated as economically feasible as public works. Therefore it is necessary that the public postpartum care centers operated by local governments should be expanded through legal amendment.

Size Distribution and Concentration of Airborne Fungi in the Public Facilities (다중이용시설 내부에 분포하는 부유 진균의 입경별 농도 특성)

  • The aim of this study is to examine size-based concentration and genera of airborne fungi distributed in public facilities such as hospital, kindergarten, day-care center and postpartum nurse center and to provide fundamental data in order to prevent respiratory diseases caused by exposure to airborne fungi. Culturable total and respirable concentrations of airborne fungi averaged to $382\;cfu/m^3\;and\;292\;cfu/m^3$ in hospital, $536\;cfu/m^3\;and\;347\;cfu/m^3$ in kindergarten, $334\;cfu/m^3\;and\;266\;cfu/m^3$ in day-care center, and $371\;cfu/m^3\;and\;289\;cfu/m^3$ in postpartum nurse center, respectively. The ratio of respirable to total concentration of airborne fungi in the investigated public facilities was ranged from $55\%\;to\;70\%$ but there was no significant difference among them (p>0.05). The mean I/O ratio of culturable total and respirable concentrations were 0.56 and 0.64 in hospital, 0.72 and 0.91 in kindergarten, 0.33 and 0.45 in day-care center, and 0.63 and 0.73 in postpartum nurse center, respectively. Indoor concentration of airborne fungi did not correlated significantly with indoor temperature and relative humidity (p>0.05) but had a significant positive correlation with $CO_2$ concentration (p<0.01) and surrounding condition (p<0.05). Penicillium spp., Cladosporium spp., and Aspergillus spp. were estimated to over $95\%$ of total airborne fungi identified in the investigated public facilities.

A Model for Community Based Mother Infant Care Center - TMIC(transitional mother infant care center) using a Sanhujoriwon - (산후조리원의 모자건강관리 현황과 제도화방안 - 지역사회중심의 모자건강관리센터(TMIC) 개발을 위한 전략 -)

  • The purpose of this study was 1) analyze the current state of Sanhojoriwon; and 2) to suggest the new model for the community based mother infants health care delivery system: strategies of TMIC are related to Public Health policy, cost-effectiveness, mother infant care provision of medical professionalism, and so on. Method: Forty-seven workers from seventeen Sanhojoriwon participated to analyze several aspects of Sanhojoriwon. Using a questionnaire developed at Korean Sanhojori Research Forum (KSARF), such as the traditional and medical concept of the Sanhojori, postpartum care, Korean traditional postpartum care, job description on women and infant care at Sanhojoriwon, professional management, health care policy and the educational need. Results: Based on the descriptive study results, the TMIC, the community based transitional mother infants care center was suggested as a new model for the cyclic public health care system related on the reproductive health, using an already existing related center, Sanhojoriwon. Also, several strategies were presented on the TMIC.

  • The aim of this study was to identify the performance and requirements of the visiting nursing care using Omaha system in public health center. The highest performance were 'personal hygiene', 'pain', 'medication regimen', 'nutrition', 'physical activity', 'sanitation', 'sleep and rest patterns', 'oral hygiene', 'mental health' in order. The lowest performance were 'sexuality', 'postpartum', 'income', 'family planning', 'pregnancy', 'spirituality', 'abuse', 'reproductive function', 'neglect' in order. Problems such as 'postpartum', 'pregnancy' and 'family planning' need to strengthen the role of visiting nurses according to the region. this result will be the basis for visitung nursing care.

Field Study of Characteristics of Airborne Bacteria Distributed in the Regulated Public Facilities (규제대상 다중이용시설내 부유세균의 분포 특성에 관한 현장 조사)

  • Concentration and identification of airborne bacteria in the regulated public facilities were examined with the six-stage cascade impactor. Geometric mean total and respirable concentrations of airborne bacteria were $404cfu/m^3$ and $194cfu/m^3$ in hospital, $931cfu/m^3$ and $358cfu/m^3$ in kindergarten, $294cfu/m^3$ and $134cfu/m^3$ in day-care center, and $586cfu/m^3$ and $254cfu/m^3$ in postpartum nurse center, respectively. As a result, culturable total and respirable concentrations of airborne bacteria were significantly highest in kindergarten and lowest in day-care center (p<0.05). The ratio of respirable to total concentration of airborne bacteria in the investigated public facilities was ranged from 30% to 40% but there was no significant difference among them (p>0.05). The mean I/O ratio of culturable total and respirable concentrations were 0.58 and 0.66 in hospital, 0.71 and 0.83 in kindergarten, 0.28 and 0.41 in day-care center, and 0.63 and 0.78 in postpartum nurse center, respectively. Day-care center showed the lowest I/O ratio of culturable total and respirable concentration of airborne bacteria (p<0.05) but a significant difference was not found among other facilities. Indoor concentration of airborne bacteria did not correlated significantly with indoor temperature and relative humidity (p>0.05) but had a significant positive correlation with $CO_2$ and surrounding condition (p<0.05). Staphylococcus spp., Micrococcus spp., Corynebacterium spp., and Bacillus spp. were dominant genera and amounted to over 95% of total airborne bacteria identified in the investigated public facilities. Size distributions of four dominant genera did not observed inconsistently regardless of type of public facility.

Effects of Postpartum Depression and Temperament of Infant on Child-care Stress among Mothers of Newborn Infants (영아기 어머니의 산후 우울과 아기 기질이 양육 스트레스에 미치는 영향)

  • Purpose: This study was designed to identify mother and infant related factors that influence child-care stress among the mother of newborn infants. Methods: Data were collected through self-administered questionnaires and descriptive statistics, correlation and multiple regression were used to analyze the data. The data survey was conducted with 957 conveniently selected mothers of infants when they visited a public health center in Seoul to have their children immunized. Results: The average item score for the Childcare Stress Inventory was 38.03, for the Edinburgh Postnatal Depression Scale, 9.31 and for the Degree of Bother Inventory, 23.42. The CSI was positively correlated to EPDS (r=.44, p<.001) and DBI (r=.40, p<.001). Also these two variables explained 30.0% of CSI in infants' mothers. Conclusion: These findings are expected to expand the understanding about postpartum mothers' child-care stress and can contribute to the development of comprehensive interventions based on community health nursing.

The Predictors of Postpartum Depression (산후우울 예측요인)

  • Purpose: This study was designed to determine the predictors of postpartum depression. Method: One hundred-sixty one women within one year after delivery from one public health center located in the northern area of Seoul were used in this study. The instruments were a survey of general characteristics, the Edinburgh Postnatal Depression Scale, recent life events index, perceived social support from family, Quality of marriage index, parenting stress index, and Rosenberg's self-esteem inventory. Data was analysed using descriptive statistics, Pearson correlation coefficients, and logistic regression. Result: The average item score of the EPDS was 6.67. 12.4% of respondents, who scored above a threshold 12, were likely to be suffering from a depression of varying severity. The fitness of the model for explaining postpartum depression from six variables, plan for pregnancy, family support, quality of marital relation, perceived social support, life events, childcare stress, and self-esteem, was statistically significant and the predictive power of these variables was 90.9%. The significant predictors of postpartum depression were family support and child care stress. Conclusion: Further research is needed to identify the prevalence rate of postpartum depression using more reliable sampling methods from a large general population. Nursing interventions need to be developed for promoting family support and reducing childcare stress.

Study on the Comparison of Emotion between the Mothers with Low-birth Weights and Normal Infants and the Effect of Home Visiting for the Low-birth Weights (저체중출생아 어머니와 정상신생아 어머니의 정서와 지지 비교 및 보건소 저체중출생아 가정방문간호의 효과에 대한 연구)

  • This study was conducted to compare the emotional state between the mothers with low-birth-weights and mothers with normal infants, and to analyze the effects of home visiting for the low-birth-weights in one city. Data were collected from 51 mothers with low-birth-weights and 90 mothers with normal infants to compare emotional state, and from 26 mothers with low-birth weights to evaluate the effect of home visiting care. Summaries of results were as follows; 1. In mothers with low-birth-weights, social support form others was significantly lower than those of mothers with normal infants. Although the differences were not significant, mothers with low-birth-weights have more stress and child rearing burden, and less maternal self-esteem than those of mothers with normal infants. 2. Mothers with low-birth-weights, the more burden, postpartum depression, and the less husbands' support they felt. When they had lower maternal self-esteem and lower husbands' support, child rearing burden was higher. Also there was significant negative correlation between maternal self-esteem and postpartum depression. 3. In mothers with low-birth-weights, the score of post-intervention stress, care-giving burden, and postpartum depression were somewhat decreased, and maternal self-esteem was increased than pre-intervention data, although they were not statistically significant. 4. Mothers' satisfaction on the home-visiting care was considered to be high. In summary, mothers with low-birth-weights had lower social support even though they experienced more stress than mothers with normal infants. Therefore, public health nurse in community should pay more attention to them.

Postpartum Depression and Maternal Role Confidence, Parenting Stress, and Infant Temperament in Mothers of Young Infants (영아기 어머니의 산후 우울 정도와 모성역할 자신감, 양육 스트레스, 영아기질에 관한 연구)

  • Purpose: The purposes of this descriptive survey study were to describe postpartum depression level and to identify the influence of postpartum depression on maternal role confidence, parenting stress and infant temperament, in order to develop a base for nursing intervention programs for mothers who have postpartum depression. Method: Data were collected from 183 mothers of infants who visited one public health center in Goyang city. The instrument used for this study was a self-report questionnaire to identify the postpartum depression (BDI), maternal role confidence, parenting stress (PSI) and infant temperament (WBL). Results: Of the mothers 42.1% were in the normal range for postpartum depression, 33.3% in the mild group and 24.6% were in the moderate to severe group. There were significant differences in parenting stress, and infant temperament according to level of postpartum depression. There were correlations between maternal role confidence (r=-0.13), infant temperament (r=-0.20), parenting stress (r=0.51) and postpartum depression, and weak negative correlation between parenting stress and infant temperament (r=-0.30). Conclusion: Postpartum depression in the infant period when the mother's role is very important and the effect is not only on the mother's role but also on the infant's growth. It is essential to assess and provide immediate care to mothers who have postpartum depression.
