The Impact of New Work Organizational System on Job Strain, and Psychosocial Distress (새로운 작업조직 시스템이 직업성 긴장수준 및 직무스트레스에 미치는 영향)

  • Objectives : New organizational work systems, and their impact on the mental health of employees, are considered to beone of the most important topics in the area of industrial health. This study was conducted to compare job characteristics (job demand and decision latitude) levels, and psychosocial distress of workers in acompany introducing to new organizational work systems, to those of workers managed by traditional work systems. Methods ; A study sample of 627 shipbuilding workers (446the new work organizational system and 181 the traditional system) were recruited for this study. A structured-questionnaire was used to assess general characteristics, job characteristics(work demand, decision latitude), and psychosocial distress. Results : The decision latitude was not significantly higher in the new work system compared to the traditional system. However, the job demand was significantly higher in the new work system than in the traditional system. The psychosocial distress was higher within the new work system than the traditional system, but no significant relationships were found. The proportion of increased strain was significantly greater with the new system than the traditional system. Conclusion : These results suggest that increases in the decision latitude did not sufficiently compensate for higher job strain or increased work intensity. If the increase in the decision latitude was temporary, with the typical job demand remaining high, such work can be still be considered to have a job strain Futureresearch should consider psychosocial distress and fatigue as importantproblems caused by new work organizational systems, and should be performed to assess their impact through out industry.

A Study on Cognition of Oriental Medical Hospital Employees on the Oriental Medical Institution Assessment (한방의료기관평가에 대한 한방의료기관 근무자의 인지도)

  • Objectives: To determine the factors affecting the assessment of Oriental medical institutions under the Oriental Medical Institution Assessment System and thereupon, provide for some basic data and alternative measures for assessment of Oriental medical institutions. Methods: The researcher sampled 320 people employed by 26 Oriental medical institutions designated as model Oriental medical institutions subject to assessment in 2008 and 2009 and thereupon, used a structured and open-ended survey table for them to collect the data. The size of the final sample was n=302. Results: The Oriental medical institution staff were highly aware of the Oriental medical institution assessment (OMIA), After adjusting the other factors by multiple regression, the factors affecting such recognition were different significantly depending on age (those in their 30's), types of job (nurses and treatment assistants) and locations of hospital (GyeongSang-do). The staff expected the OMIA could helpful for improving facilities and system of hospitals, thus promoting satisfaction of patients. To do so, they felt it necessary to develop an assessment scale reflecting the special conditions besetting the Oriental medical institutions as well as the indices for improvement of Oriental medical service quality. Conclusion: It is hoped that this study will be followed up by future studies which will comparatively analyze Oriental medical institution staff's perception of the assessment system before and after its operation and thereby, suggest some ideal policy alternatives for assessment of the Oriental medical institutions. Furthermore, future studies are requested to research into Oriental medical institution staff's needs and consumers' needs as well in consideration of the characteristics of the Oriental medical institutions and thereupon, suggest some alternatives for continued education, development of the assessment tools, methods and policies.

  • 본 논문은 서구의 이론을 바탕으로 개발된 직무만족도에 관한 인과모형을 한국의 병원근무자들을 대상으로 실증적으로 검증하고 직무만족도를 결정하는 인과적 요인들을 파악하는데 목적이 있다. 본 모형의 종속변수는 직무만족도이고 독립변수로는 3개집단의 변수들로 크게 분류하여 선정되었는데 첫째, 사회적 변수들로써 직무의 자율성, 직무의 모호성, 직무 갈등, 직무의 양, 상사 및 동료와의 관계, 발전가능성, 직무의 단순성, 분배정의, 승진기회, 물리적 근무환경, 임금 등이며, 둘째, 심리적 변수들로써 기대 충족도, 근무의욕, 적극적 심성, 부정적 심성 등이고, 셋째, 환경적 변수들로써 외부 취업기회와 가족에 대한 책임 등이 사용되었다. 또한 독립변수들이 종속변수에 영향을 미치는데 있어 개인의 가치관이 어떻게 작용하는지도 검증하였다. 자료수집을 위해 대구에 위치한 750병상 규모의 한 대학병원에 근무하는 전직원을 대상으로 자기가 입식 설문조사를 실시하였으며 879매가 회수되어 74.7%의 응답율을 기록하였다. 수집된 자료중 사용 가능한 836명의 응답을 바탕으로 선형구조관계분석(LISREL)과 다중회구분석기법을 이용하여 인과 모형을 검증하였다. 분석결과 직무만족도에 유의한 영향력을 가지는 변수들은 직무의 모호성, 직무의 모호성, 동료관계, 직무의 단순성, 분배정의(이상 시회적 변수), 기대의 총족도, 근무의욕, 적극적 심성, 부정적 심성(이상 심리적 변수), 외부 취업기회(환경적 변수) 등인 것으로 나타났으며 개인의 가치관과 인구학적인 변수들은 직무만족도에 큰 영향을 미치지 못하는 것으로 나타났다. 또한 본 연구에서 사용된 인과모형은 직무만족도의 변이를 75.4% 설명함으로서 기존의 다른 모형들 보다 높은 설명력을 나타내었다. 결론적으로 본 연구에서 사용된 직무만족도의 인과모형은 한국의 조직에서도 적용이 가능한 것으로 보이며, 또한 직무만족도의 일반적 모형은 구조적 변수, 심리적 변수, 그리고 환경적 변수들을 모두 포함하는 포괄적인 모형이 타당하다는 결론을 얻을 수 있었다.

A Study on Ethical Climate for Nurse's Engagement and Intend to quit (병원의 윤리풍토가 간호사의 인게이지먼트와 이직의도에 미치는 영향)

  • This study holds various implications and usefulness toward nurses as well as hospitals in that the study empirically found out the relations among hospitals' ethical climates, employee engagement (job engagement and organizational engagement) and intention to quit. The study shows that nurses' awareness of principles of responsibilities and benevolent ethical climates impact on enhancing organizational engagement and lowering intention to quit. Internally, employees are encouraged to make decisions accompanying fairness and responsibility and thus, shared values will be created between hospitals and nurses, while it will contribute to create a good image of the hospital to the public. However, it is also revealed that climates deficient in ethics, such as a climate of selfishness, have strong negative influences on workplace attitude of nurses. Therefore, recognizing importance and effectiveness of ethical climates, hospitals should underscore personal ethics of managers and nurses, and should go further to exert strategic efforts on the organizational level to create ethical hospital climates where nurses can naturally take ethical actions.

A Survey on Preliminary Dental Hygienists' Senses of Employment (예비치과위생사의 취업에 관한 의식 조사)

  • The purpose of this study was to conduct a survey on the senses that preliminary dental hygienists have on employment, who applied for the National Dental Hygienist Examination, so that it could provide a basic reference required for fostering good human resources specializing in oral health with a sense of mission and responsibility as professional. To meet these goals, a self-administered questionnaire survey was conducted for participants in a refresher education meeting for preliminary dental hygienists across three major regions(Seoul, Daejeon and Busan), which was held by the Korean Dental Hygienists Association(KDHA) on Jan. 15, 2006. Total 289 sheets of questionnaire were collected, and 262 sheets except invalid or incomplete ones were used for examination and analysis. As a result, this study came to the following conclusions: First, it was found that 40.4% respondents wanted to be employed in dental clinic or hospital. Most respondents(90.1%) answered that faithfulness is most valued as prerequisite qualification for employment in hospital, and highest percentage of respondents(39.3%) wanted to earn 16 to 17 million Korean won annually. For internship opportunities, 61.9% respondents replied that they could willingly accept the internship, provided that basic pay and switchover to regular employee are all assured. Second, as the results of survey on employment circumstances around respondents, it was found that most respondents(58.3%) were employed in dental clinic or hospital, and highest percentage of respondents(35.0%) earned 15 to 16 million Korean won. Third, it was found that interpersonal relationships among employees had most significant effects(4.81) on selection of employment, which was followed by in-house welfare benefits 1(monthly/annual vacation, resting room, etc; 4.56). For possible influential factors on the selection of employment depending on whether employed or not, it was found that there were Significant differences in pay, welfare benefit 2(seminar, orientation, refresher training opportunities, etc), post-wedding continued employment and merit system between employees and non-employees(pE0.05). Fourth, the survey on respondents' occupational senses of dental hygienist showed that major tasks of dental hygienists were represented by oral disease prevention(92.6%), case management(71.5%), oral health education(66.3%), hospital/clinic management and dental management support(10.4%) and public oral health activities(6.7%) respectively. For job satisfaction, it was found that almost half respondents(48.9%) showed satisfaction at their job and 32.6% felt satisfied at and proud of their job. That is, majority of respondents(81.5%) felt satisfied at their job as dental hygienist. For follow-up requirements to qualify for dental hygienists, it was found that 73.3% felt it necessary to give more investments to self-development, 62.2% respondents considered protection of membership's rights as one of future assignments that KDHA should be more committed to. For future social awareness about dental hygienists as occupation, 69.2% respondents expected that they would be better developed and treated as professional. And desired training courses as a part of qualification for professional dental hygienists were represented by case management(services, manners, etc; 33.3%), implant(28.9%), esthetics(correction, prosthesis, whitening; 18.9%) and so on. For an item on working years, it was noted that 75.9% respondents would keep working as dental hygienists as long as they could.

Relationship between Psychosocial Stress and Coronary Risk Factors of Employees in a Research Institute (일부 남성 연구원들의 사회심리적 스트레스와 관상동맥질환 위험인자의 관련성)

  • The purpose of the present study was intended to assess psycosocial stress of male researchers working in a Research Institute, and to reveal the relationship between psycosocial stress and coronary risk factors. Data were obtained from 356 male researchers who underwent health package check-up at the health promotion center of a university hospital from May to June 2007, using by self-administered questionnaire & health check-up. For psycosocial stress, we used the questionnaire with 18 items developed as Psychosocial Well-bing Index(PWI). For coronary risk factors, systolic and diastolic blood pressure(SBP, DBP), triglyceride(TG), total cholesterol(TC), high and low density lipoprotein cholesterol(HDL, LDL), fasting blood sugar(FBS) and % fat rate were measured. As a results, the level of psycosocial stress is influenced by socio-demographic characteristics, job-related characteristics and health-related behaviors. Especially, it was found to have the inter-relational effects that the higher the blood pressure, the higher the level of psycosocial stress.

Laboratory-Acquired Infections with Hantavirus at a Research Unit of Medical School in Seoul, 1996 (의과대학 연구동에서 발생한 유행성출혈열 역학조사)

  • Background : In April 6, 1990, a male researcher who has worked at a research unit at the Basic Research Building of Seoul National University(SNU) College of Medicine admitted to SNU Hospital due to persistent fever. He was diagnosed serologically as hemorrhagic fever with renal syndrome(HFRS). Another female researcher in the same unit was also diagnosed as HFRS at the same hospital several days later. Epidemic investigation of HFRS was conducted to determine the magnitude of the problems since these two cases were strongly suspected to have laboratory-acquired infections of HFRS. Methods : All researchers and employees working at the Basic Research Building(BRB) of SNU College of Medicine as of April 1, 1996 were recruited for the study, information on symptoms of HFRS and history of contact to experimental animals were collected by self-administered questionnaires and serological tests among study subjects were also conducted by indirect immunofluorescent antibody(IFA) to hantavirus. The experimental animals were also serologically tested for infection with hantavirus by IFA. Results : Among 218 surveyed, six researchers and an animal caretaker had hantavirus antibodies above 1:20 in IFA titer. Five of seven sero-positive subjects had antibodies above 1:640 in IFA titer and had shown clinical symptoms compatible to HFRS during Jan. 1 to Apr, 20, 1996. The sero-positive persons had handled animals more frequently than sero-negative persons (OR, 19,68; 95% Cl, 1.11-350.40) and handling animals at the animal quarter at School of Public Health(SPH) had shown consistently higher risk to get infected with hantavirus irrespective of types of animals handled (OR, 4.90-6.37). Sero-positivity of rats of the aniamal quarter at BRB was 30-60%, whereas 80% of rats at SPH tested were shown sero-positivity. Conclusion: There was a epidemic of HFRS in research units of a medical school during the period from Jan. through Apr, Further investigation is needed to determine the extent and the mode of transmission of the laboratory-acquired infection with hantavirus in other research facilities.

Skipping breakfast is associated with diet quality and metabolic syndrome risk factors of adults

  • The aim of the present study was to assess the effects of skipping breakfast on diet quality and metabolic disease risk factors in healthy Korean adults. Subjects included 415 employees (118 men, 297 women; 30-50 years old) of Jaesang Hospital in Korea and their acquaintances. Data collected from each subject included anthropometric measurements, 3-day dietary intake, blood pressure, and blood analyses. The subjects were classified into three groups based on the number of days they skipped breakfast: 'Regular breakfast eater', 'Often breakfast eater', or 'Rare breakfast eater'. Participants in the 'Rare breakfast eater' group consumed less rice, potatoes, kimchi, vegetables, fish and shellfish, milk and dairy products, and sweets than did participants in the other two groups (P for trend <0.05) and ate more cookies, cakes, and meat for dinner (P for trend <0.05). Participants in the 'Rare breakfast eater' group consumed less daily energy, fat, dietary fiber, calcium, and potassium than did participants in the other groups (P for trend <0.05). The percent energy from carbohydrates was lower and fat intake was higher in the 'Rare breakfast eater' group than in the other groups (P for trend <0.01). When diets were compared using the Acceptable Macronutrient Distribution Range for Koreans, 59.1% of subjects in the 'Rare breakfast eater' group consumed more energy from fat compared with the other two groups (P <0.005). According to the Estimated Average Requirements for Koreans, intake of selected nutrients was lower in the 'Rare breakfast eater' group than in the other two groups (P <0.05). The risk of elevated serum triglycerides was decreased in the 'Rare breakfast eater' group (OR, 0.3 [0.1-1.0], P for trend=0.0232). We conclude that eating breakfast regularly enhances diet quality, but may increase the risk of elevated serum triglycerides.

A Convergence Study on the Senior Employment Policy and Senior Job Awareness (노인 일자리 정책과 노인 일자리 인식에 관한 융합 연구)

  • The purpose of this study was conducted with suggesting a clue to improve the perception of jobs for the elderly, and the final study subject was 110 people. As a research methods were student t-test, ⲭ2-test, one-way ANOVA(Scheffe). The result of the study are as follows. The perception and economic status of the young elderly(65-74 years old) were significantly higher. The perception of public jobs was significantly higher when married, religious, and monthly income was more than 4 million won. The perception of social service-type jobs was significantly higher when married and when there was a religion. The perception of private jobs was high when the monthly income was more than 4 million won (p<0.05). The preference for "parking order service" in the public sector, "support for elderly facilities" in the social service sector, and "fast food restaurant employees, kitchen, restaurant assistants, and food delivery service workers" in the private sector was the highest. In conclusion, there is a need for an education program to improve the perception of jobs for the elderly in order to raise the awareness of job policies for the elderly, which decrease with age. It is expected to be used as basic data for job creation and job creation that can be done according to the age of the elderly.

Evaluation of the Effectiveness of a Salt Reduction Program for Employees (직장인 대상 저염화 교육 프로그램의 효과 평가)

  • The purpose of this paper was to evaluate the effectiveness of a salt reduction education program. Subjects participating in this study were 251 employees (166 in the "educated" group, 85 in the "non-educated" group) at 8 hospital and industry food service operations in Daegu. After the salt reduction education program was carried out, a salty taste assessment of both groups was conducted. The educated group had statistically significant differences and the noneducated group did not have statistically significant differences. In terms of nutrition knowledge, while the nutrition knowledge of the educated group was increased (p < 0.001), that of the non-educated group rose at a rate of 0.92. In terms of dietary attitude, the educated group exhibited increased preference toward less salty foods when compared to the noneducated group (p < 0.001). Regarding dietary behavior, the score of the educated group was improved (p < 0.001), thereby indicating a preference for less salty taste. This means that nutrition education had influence on dietary behavior. However, after education, sodium excretion for the educated group was not significantly decreased, compared to before education. The results show that there was a positive correlation between salty taste assessment and dietary attitude and behavior for a high-salt diet. There was a positive relationship between attitude for a high-salt diet and sodium intake; when people prefere a more salty taste, they eat more sodium. Therefore, in order to change dietary preference away from salty taste and to decrease sodium intake, a nationwide, systematic and continuous salt reduction education program is needed.
