Background: In chronic hepatitis B (CHB), the presence of hepatic steatosis (HS) seems to be associated with known host and viral factors which may influence the long-term prognosis of chronic hepatitis B (CHB), probably leading to cirrhosis and hepatocellular carcinoma (HCC). Different from chronic hepatitis C (CHC), factors associated with HS in CHB are not clearly explored. Materials and Methods: 160 CHB patients were divided into two groups depending on the results of liver biopsy. Group I consisted of 71 patients with confirmed steatosis. Group II comprised 89 patients without steatosis. The groups were compared in terms of basal characteristics, body mass index (BMI), liver enzymes (ALT, AST, ALP), serum fasting blood sugar (FBS) and lipids, hepatitis B e antigen (HBeAg), viral load, and histological findings. Results: In terms of host factors, male gender, older age, BMI, high serum FBS and lipid levels were associated with HS. On the other hand, ALT levels, the HAI scores of necroinflammation and stage of fibrosis did not associate with HS. On multivariate analysis, parameters of sex, BMI, cholesterol and FBS levels were independently associated with HS. Regarding viral factors, HBeAg negativity was significantly associated with HS (81.7%, p value 0.006), but not HBV DNA level (p value 0.520). Conclusions: HS in CHB appears to be unrelated to the status of HBV replication. However, fibrosis progression in CHB is related to variable host factors. HS may be enhanced through these factors in HBV chronic patients.
This study evaluated the indoor air quality of 26 government offices located in Seoul. The pollutant samples were taken from Jan. 13th to Jan. 29th and Feb. 20th to Feb. 23rd, 2010 in the offices. The target indoor pollutants for this study were $PM_{10}$, formaldehyde, carbon monoxide, carbon dioxide, total bacteria counts, total volatile organic compounds, radon, nitrogen dioxide, ozone, and asbestos which were controlled by the indoor air quality law for the multiple-use facilities management. The results of this study showed that some pollutants of the 38.5% offices exceeded the standards of the air quality guideline. The correlation analysis of the same pollutants between indoor and outdoor represented that $NO_2$ (r=0.629, p<0.05) and $O_3$ (r=0.459, p<0.01) were significant, however, $PM_{10}$ and CO were not. The correlation analysis between different pollutants showed that CO and TVOC (total volatile organic compounds: r=0.724; p<0.01), CO and $NO_2$ (r=0.674; p<0.01), HCHO and humidity (r=0.605; p<0.01), $CO_2$ and TVOC (r=0.534; p<0.01), TBC (total bacteria counts) and Asbestos (r=0.520; p<0.01) were significant. The energy-saving system of government buildings in winter caused under-ventilated and poor air quality. This study suggests that the concentrations of radon and $CO_2$ should be used as an indicator for monitoring indoor air quality and maintaining effective ventilations.
Providing free primary care to everyone is an important goal pursued by many countries under universal health care programs. Countries like India need to efficiently utilize their limited capacities towards this purpose. Unfortunately, due to a variety of reasons, patients incur substantial travel and out-of-pocket expenses for getting primary care from publicly-funded facilities. We propose a set-covering optimization model to assist health policy-makers in managing existing capacity in a better way. Decision-making should consider upgrading centers with better potential to reduce patient expenses and reallocating capacities from less preferred facilities. A multinomial logit choice model is used to predict the preferences. In this article, a brief background and literature survey along with the mixed integer linear programming (MILP) optimization model are presented. The working of the model is illustrated with the help of numerical experiments.
Purpose: This descriptive study investigated the effects of self-efficacy and self-stigma on self-care in people with diabetes. Methods: The study included a total of 377 patients with diabetes enrolled in university hospitals in D city and public health centers in S city. Data were collected from 1 July to 31 August, 2017, and were analyzed using descriptive statistics, t-test, analysis of variance, Pearson's correlation coefficient, and hierarchical multiple regression. Results: Diabetes self-care was positively correlated with diabetes self-efficacy, whereas it was negatively correlated with diabetes self-stigma. Participants' education level, marital status, perceived health status, type of medication, self-efficacy, and self-stigma explained 42.4% of the variance in diabetes self-care. Conclusion: The findings indicate that diabetes self-efficacy and self-stigma are important factors for improving self-care in patients with diabetes. Therefore, systematic programs for enhancing self-efficacy and reducing self-stigma of these individuals should be developed.
Purpose: The aim of this study was to identify levels of health literacy and reported health behaviors among older adults with cardio-cerebrovascular disease residing in rural areas. Methods: A cross-sectional survey was conducted with a convenience sample of 134 older people (mean age=75.5 years, 71.6% Female) registered at seven health centers. The structured questionnaires were used to measure levels of health literacy and health behaviors. Results: The average functional health literacy score was $6.08{\pm}3.04$, and the health behavior score was $62.92{\pm}6.45$. Respectively health behavior was positively associated with health literacy. Education, economic status, number of family members, number of social activities and health literacy were contribution factors explaining 40.64% of the variance in health behavior. Health literacy specifically explained 12.5% of health behavior. Conclusion: These findings suggest that strategies for improving health behaviors and reducing health inequalities may benefit from adopting a stronger focus on health literacy within prevention, patient education, and other public health interventions. Further longitudinal studies are needed to help confirm that improving health literacy in the elderly may be effective in changing health behaviors.
Purpose: The purpose of this study was to identify the relationships between health status including objective health status and subjective health perception and Health-Promoting Lifestyle Profile II in North Korean immigrants. Methods: During August 2011 to November 2011, a convenience sample of 96 subjects aged 19 to 65 years were recruited from two centers and one public health center in Gyeongnam. The subjects were interviewed face to face by trained people with a survey questionnaire. Results: Approximately 40% of North Korean immigrants rated their health status as 'poor', and the prevalence of health problems in them was 81.3%. The average level of health-promoting lifestyle profile II was $1.78{\pm}0.35$. Females with normal weight or overweight, and have stayed for over 1 year in South Korea were related to the lower score of health-promoting lifestyle profile II. Conclusion: Considering North Korean immigrants' health background and health behaviors, customized health care program focusing on musculoskeletal disorder and psychological problems are required, and at the same time health behavior change program to achieve lifetime wellness should be developed.

Background: The purpose of this study was to investigate longitudinal changes of the utilization of operational and surgical medical care inside and outside a metropolitan area over 10 years, analyzing the residential areas of patients and the locations of medical facilities for major cardiovascular surgery. Methods: Data analysis was conducted by classifying the addresses of patients and the locations of medical care facilities of metropolitan cities and provinces, using data from the National Health Insurance Corporation from January 2003 to December 2013. Results: There is serious concentration of major heart surgery to medical facilities in Seoul; this problem has not improved over time. There were differences in percentages of surgical procedures performed in the metropolitan areas according to major diseases. In the case of Busan and Daegu provinces, at least 50% of the patients underwent surgery in medical facilities in the city, but there are other regions where the percentage is less than 50%. In the case of provinces, the percentage of surgical procedures performed in medical facilities in Seoul or nearby metropolitan cities is very high. Conclusion: Policies to strengthen the regional capabilities of heart surgery and to secure human resources are required to mitigate the concentration of patients in the capital area. Many regional multi-centers must be designated to minimize unnecessary competition among regional university hospitals and activate a win-win partnership model for medical services.
Objectives: Subjects of this study were technical high school students. It researched students' smoking status, smoking attitude, smoking perception and willingness to quit smoking, providing them with a five-day-smoking cessation program. Methods: Data collection proceeded from April to August 2008. The subjects were the whole number, 38 students, of 1EA technical high school located. The five-day-smoking cessation program took 2 successive hours a day for five days. Results: Average age for them to start smoking was 13.6 and to form smoking habits was 15.5. Average smoking period was 31.6 months and average smoking quantity was 11.2 cigarettes. As a result of the five-days smoking cessation program, 10.5% of the participants quit smoking and smoking quantity also significantly reduced. After instruction, willingness to quit smoking was generally higher than before. Smoking attitude, especially toward adolescent smoking, changed to be more negative than before. Perception of smoking also changed to be positive. Conclusions: The results from the comparative analysis of before and after instruction revealed a decrease on smoking quantity of the participants who attended the smoking cessation program. It also showed a significant change on smoking attitude, willingness to quit smoking and smoking perception.
