A Study of General Population's Awareness and Attitudes Toward Medical Records : Focusing on Open Notes (진료기록에 대한 일반인의 인식과 태도 : 오픈노트(Open Notes) 운동을 중심으로)

  • The purpose of this study was to investigate general population's awareness and attitudes toward medical records and an 'Open Notes' system which allows the general public to access their medical records anytime on the hospital website. This study also examines the possibility of making the 'Open Notes' system available to Korean medical community and the general public. The results of this study shows that the general population usually used internet for health information. They obtained their medical records from the hospital mostly for the purpose of submitting to insurance company. They also believed that medical records that hospital and doctors provided might be forged or falsified. The majority of them responded that they could trust their doctors and hospitals more if they could have access to their own medical records anytime. Most of the respondents agreed that the Open Notes system would be beneficial for the general public and that it should be implemented in Korea. And they would be willing to participate in the Open Notes system if it is introduced. In conclusion, if the Open Notes system which emphasizes transparency in medical records is introduced, it could enhance the trust between doctor and patient. The trust doctor-patient relation would make patients more likely to comply and be satisfied with doctors.

  • Effects on Health Center and Hospital Visit Patient Characteristics and Overall Satisfaction As a result of comparing the characteristics of visiting patients in the public health centers and hospitals, the number of patients at the public health centers was relatively higher than the hospital patients, the number of medical benefits patients was higher, and there were more vaccinations or physiotherapy patients than the purpose of disease treatment.There was a small number of private insurances,The older adults were more. The most influential variable on the overall satisfaction level of the public health center is the facility satisfaction And doctor satisfaction had a weak effect.In the case of hospitals, the facility satisfaction Also, the degree of satisfaction with doctors, satisfaction with medical expenses, and satisfaction with treatment environment were in order.

Survey of Korean Medicine School Doctor for activation Korean medicine school doctor program (한의사 교의사업 활성화를 위한 한의사 교의 대상 설문조사)

  • Objectives : This study aimed to investigate the satisfaction of Korean medicine doctors (KMD) who conducted Korean medicine school doctor program (KMSD). We are going to use study as a foundation for activation KMSD program. Methods : Seoul Korean medicine association and Seoul metropolitan office of education conducted KMSD program for 22 schools in 2017. The program included health lessons, health counseling, and health care programs. After program finished, we carried out self-administered questionnaire survey to KMD who participated in KMSD program and we analysed it. Results : A total of 45 people answered the questionnaire, and 56% of respondents answered as 'Good progress' and 44% answered as 'Bad progress'. The reason for good progress was 'Good cooperation of School' (47.4%). 'Personalized program for participant' (23.68%), 'Support of Seoul Korean Medicine Association' (21.1%). The reason for 'Bad progress' was 'Bad cooperation of School' (37.8%), 'Lack of personal circumstances' (32.4%), 'Lack of motivation' (16.2%). The advantage of KMSD program included 'It is helpful for positive perception of the Korean medicine' and 'Korean medicine can contribute to improving public health, which is the health of schools' was 25%, 'It is helpful in expanding the services of Korean medicines to children and adolescents' was 17.6%, 'Highly satisfaction in participations' was 15.7%. The question of prerequisites for activation KMSD Program included 'Active cooperation of school' was 35.05%, 'Expand teaching materials and programs' was 20.62%, 'Support of Seoul Korean Medicine Association' was 15.46%. Conclusions : We will organize and activate programs of KMSD program and conduct program based on harmony of local community and school support, research of school health program. The program will contribute to improve student health and develop Korean health support program.

  • The author surveyed overall obesity indicies and factors concerned with obesity such as dietary intake, physical activity, stress and life style with the subject of doctors. The number of subjects was total 508 with 396 men and 112 women. They were subgrouped into surgical part, medical part and service and basic part by speciality. And also subgrouped into intern and resident, pay doctor, and practitioner by working type. The results were as follows. 1) Obesity indices: BMI of total doctor was $23.1{\pm}2.8$, and WHR was $0.87{\pm}0.08$ and overweight prevalence(BMI>25.0) was 23.6%. It was within normal limit but slightly over the Korean standard. The degree of obesity indices of subgroups by speciality was 'surgical part > medical part > service and basic part', and by working type was 'practitioner > pay doctor > intern and resident'. 2) Dietary intake and Physical activity: Average dietary intake was $2148{\pm}451kcal/day$. The degree of dietary intake by speciality was 'surgical part > medical part > service and basic part'. By working type it was 'practitioner > pay doctor > intern and resident'. Average physical activity was $29{\pm}5$ METs/day. The degree of physical activity also showed similiar pattern. But there was no significant difference among each groups. 3) Comparision between over-weight and non-over-weight group: The items that showed significant difference between two groups were dietary intake, skip breakfast, regular exercise, smoking, heavy drinking, chronic disease etc.

Health related practices and morbidity among adult in rural area (농촌지역 주민의 건강관련 행위와 질병이환과의 관계)

  • This research was carried out to determine the performance rate of health related practices, to measure the agreement between morbidity by doctor's diagnosis and morbidity by subject' self-reported and the degree of association between health related practices and morbidity rate by doctor's diagnosis, to identify their effects on morbidity among rural area populations. The data were gathered by volunteer residents (over the age of 20) of Haman Myeon, Haman Gun, Kyeongsangnam Do in Korea, from June 10, 1993 to June 12, 1993 (369 male and 516 female). Face to face interview, lab, chest P-A, EKG and physical examination were completed. Descriptive statistics, agreement analysis and multiple logistic regression procedures were employed for analyses. The results of the study were summarized as follows : 1) Age adjusted morbidity rates by doctor's diagnosis and self-reported were 38.5% (male:37.3%, female:36.5%), 26.4% (male:33.3%, female:27.5%), respectively. Kappa coefficient between morbidity by doctor's diagnosis and morbidity by self-reported was 0.21 (male:0.21, female:0.22). 2) The frequency of disease by doctor's diagnosis was as follows: hypertension(15.3%), gastritis (9.6%), diabetes mellitus (8.5%), live. disease (8.1%), and degenerative arthritis (6.2%) in the study population. 3) Order of health practice performance rate was as follows: Males-normal body weight (62.1%), non-heavy alcohol consumption (57.5%), 7-8 hours of sleeping (50.1%), non-smoking (21.7%), and exercise (19.8%). Females- non-heavy alcohol consumption (97.3%), non-smoking (84.7%), normal body weight (57.8%), 7-8 hours of sleeping (45.0%), and exercise (9.9%). 4) There was no significant relationship between health related practice and morbidity except exercise among health related practices. 5) Health related practice index which was recategorized by high, medium, and low had effects on the probability of developing morbidity.

Study on The Regulation on Poisonous Medicinal Herbs (독성 한약재의 법적 규제에 관한 연구)

  • Objective: Poisonous Medicinal herbs can be considered as a risk factor to public health unless they are prescribed by Doctor of traditional Korean medicine. The proper method to manage them should be prepared to prevent risk factors caused by misuse and abuse of the poisonous medicinal herbs and enhance public health. Methods: In this dissertation (paper), the definition, scope, management status, data about pharmacology and toxicity and media release regarding adverse reaction were understood after organizing documents, laws and regulations concerning poisonous medicinal herbs. Also, management methods are suggested by analyzing related examples and regulations in China, Japan and Hong Kong, where the use of herbal medicine is general. Results: Methods for items for poisonous medicinal herbs, safety information management, management based on standardization of traditional processing methodology and reorganization and revision of related laws and regulations are established. Conclusion: Proper laws and regulations are not yet established to manage poisonous medicinal herbs in Korea. In this regard, it is urgent to establish laws and regulations which can apply independently. The purpose of the laws and regulations should be to enhance management of poisonous medicinal herbs and prevent incidence of addiction and death, improving the public health.

The Effects of Aquatic Excercise Program on the Physical Function and Depression in Community Elderly (수중운동 프로그램이 지역사회 노인의 신체기능과 우울에 미치는 영향)

  • This study was designed to investigate the effects of an aquatic exercise program on the physical function and depression among the community elderly. The subjects were 41 cases of aged over 65 who went to elderly college in Hawsung-Kun, Kyeongkee province. Seventeen of them were assigned to control group, while twenty four subjects to the experimental group. The period of data collection was from June., 1999 to November., 1999. The aquatic exercise program was developed by the author with the assistance of rehabilitation medicine doctor and physical therapist in the public health center. The program was carried out once a week for 6 months in regular swimming pool. Data were analyzed with t-test, $X^2$ test, ANCOVA. paired t-test using SAS 6.12. Results were obtained as follows: 1. The increments in the range of motion of the shoulder and knee joints in experimental group after exercise was significantly greater than those in the control group. 2. The physical pain score decreases significantly after the exercise in the experimental group only. 3. The experimental group revealed lower level of depression after exercise than the level before the exercise, whereas control group showed no significant changes in this level. These findings may indicate that the aquatic exercise program is effective in increasing the range of motion of shoulder and knee joints and also effective in decreasing pain, the level of depression. Accordingly. the aquatic exercise program can be adopted as an health promotion program for community elderly.

The Statistical Analyses of Oriental Medical Office in a Public Health Center of Dalseong-gun, Daegu Metropolitan City During Recent 3 years (최근 3년간 대구 달성군 보건소 한방진료실의 진료현황에 대한 통계적 연구)

  • Objective : We investigated the actual condition of oriental medical office in a public health center. Methods : We classified patients who visited an oriental medical office iin a public pealth center of Dalseong-gun, Daegu Metropolitan City during recent 3 years by KCPJMAIN(data processing system), according to location, age, time(month, year), sex, disease, insurance, etc. Results : As a local category, the residents in Hyeunpung-Myeon higher than 80% of the patients who visited the public health center. The patients aged over 60 occupied 80% of the patients. The frequency of the treatment was more than 10 in those patients aged over 60. The number of the patients was the highest in May when there was a change for the past 36months, whereafter the number has sustained decline for 3-5 months and repeated decreasing. As a gender category, the female inpatients were 10732(82%). This statistics shows that these musculoskeletal system disease occupied large part in them. As a heath insurance category, the patients who were insured by health care were 12454(96.30%). Conclusion : It should need to enable the rural residents who have difficulty benefitting from medical service to reach the service by making their access to the oriental medical office in public health center easier. Most of all, the support from both government and municipality should be urged to accomplish it. plus, it should be included not only boosting doctors' reliance but also improving the capability and services of doctors in public heath center. In conclusion, the treatment service in public health center should be diversified beyond musculoskeletal system disease and the identity transform of public health center should be needed to appeal to young generation.
