Health Impact Assessment of Free Immunization Program in Jinju City, Korea

  • Objectives: This study was conducted to assess the potential health impacts and improve the quality of the free immunization program in Jinju City by maximizing the predicted positive health gains and minimizing the negative health risks. Methods: A steering committee was established in September 2010 to carry out the health impact assessment (HIA) and began the screening and scoping stages. In the appraisal stage, analysis of secondary data, a literature review, case studies, geographic information systems analysis, a questionnaire, and expert consultations were used. The results of the data collection and analyses were discussed during a workshop, after which recommendations were finalized in a written report. Results: Increased access to immunization, comprehensive services provided by physicians, the strengthened role of the public health center in increasing immunization rates and services, and the ripple effect to other neighboring communities were identified as potential positive impacts. On the other hand, the program might be inaccessible to rural regions with no private clinics where there are more at-risk children, vaccine management and quality control at the clinics may be poor, and vaccines may be misused. Recommendations to maximize health gains and minimize risks were separately developed for the public health center and private clinics. Conclusions: The HIA provided an opportunity for stakeholders to comprehensively overview the potential positive and negative impacts of the program before it was implemented. An HIA is a powerful tool that should be used when developing and implementing diverse health-related policies and programs in the community.

Analysis of the Seoul public bikes usage for new rental locations (서울 공공자전거 신규 대여소를 위한 수요량 예측 분석)

  • Seoul public bike program facilitates access to bicycles and offers potential for greater mobility and health for users. Furthermore, it would have positive impacts on transport congestion, energy use, and the environment. Hence, it is important to find future rental locations by taking to account both bike-demand and regional imbalance. This paper first finds eligible candidates of rental locations with the required spatial conditions such as a sufficient sidewalk width and accessibility of bike pick-up vehicles. And then, estimates public bike daily usage for each selected location via random forest based on Seoul public bike historical usage, Seoul geographical features, regional characteristics, and populations. This study contributes to a better comprehension of the Seoul public bike program, and would be useful in determining new public bike rental locations.

Application of Program Theory and Logic Model to Evaluate Immunization Disparity Program for Children under 3 Years

  • With the outbreak of coronavirus disease 2019 (COVID-19) pandemic, health policymakers are adopting new policies regarding the issue of immunization disparities, especially for children in low-income communities of color who lack awareness and thereby access to vaccines. The purpose of this paper is to propose an evaluation framework using program theory-based evaluation approach and logic model to analyze and evaluate the immunization disparities in children aged 19-35 months. Data is collected from New York City department of Health and the U.S. Census Bureau for Northern Manhattan Start Right Coalition program which consists of 19,800 children, and the community-provider partnership includes 26 practices and 20 groups. Program theory is used to evaluate this community-based initiative with the logic model which is a visual depiction that illustrations the program theory to all stakeholders. The logic model highlights the resources, activities, outputs, outcomes, and impacts of the program to guide to planners and evaluators and to call attention to the inadequacies or flaws in the operational, implementation and service delivery process of the program in offering a new perspective on the program. This framework adds to the literature on evaluations of immunization disparities in determining whether evaluators can definitively attribute positive immunization outcomes in the community to the program and conclude whether it has potential in expanding or duplicating it to other similar settings, especially in other rural areas of the United States, and abroad, where routine immunization equity gaps are wide due to income, racial and ethnic diversity, and language barrier.

Family Function and Internet Addiction in Lower Grade Elementary School Students (초등학교 저학년 아동의 가족기능과 인터넷 중독)

  • Purpose: This study explored the degree of internet addiction and its influencing factors in lower grade elementary school children in rural areas. Methods: Subjects included 1,137 elementary school children. A structured questionnaire consisting of questions internet addiction, family function and school life satisfaction was used for data collection. Descriptive statistics, ${\chi}^2$-test, ANOVA and Correlation and Regression were performed using a SPSS program. Results: Among the subjects, 85.8% were regular users, 8.1% were potential risk users, and 6.1% were high-risk users. Career of internet use, daily internet access time, the number of conversations per week, the number of times spent eating out and traveling with parents within the last month and activities with friends, and satisfaction on school life differed significantly according to the degree of internet addiction. Finally, among the factors studied, daily internet access time, conversations within family, activities with friends and school life satisfaction in children in early elementary school was 20.7% of the explanatory power of the subjects' degree of internet addiction, collectively. Conclusion: Use of the internet by early elementary school children should be monitored and strategies to promote familial conversations, activities with peers and school life satisfaction should be developed.

FEDERAL DISABILITY LAW AND ITS IMPACT ON HEALTH CARE FOR PERSONS WITH DISABILITIES IN THE UNITED STATES (미국 연방 장애법과 동법이 장애인의 의료서비스에 미친 영향)

  • Federal disability law has evolved from several laws geared to protect people with disabilities since the late 1960s and early 1970s. When U.S. Congress passed the Americans with Disabilities Act (ADA) in 1990, no federal statute prohibited the majority of employers, program administrators, owners and managers of places of public accommodation and others from discriminating against people with disabilities. Toward the ends to assure equality of opportunity, full participation, independent living, and economic self-sufficiency for individuals with the disabilities, the ADA pursues three major strategies: Title I addresses inequality in employment, Title II, inequality in public services, and Title III, inequality in services and accommodations offered by private entities. The purposes of the study were to analyze the impact of the ADA on health care for persons with disabilities and to review the ongoing health policy reforms at the federal and state governments. Essential remedies that the ADA contemplates are based on two principles, simple discrimination and reasonable accommodation, which significantly improved access to quality care, especially long-term care, by persons with disabilities. However, the ongoing Medicaid policy reforms to control rising health care costs in the U.S. could threaten the access to care by persons with disabilities in optional groups and to optional care services by persons with disabilities in mandatory groups.

  • Encryption and authentication system in National Defense is divided into three system; KMI, MPKI, and GPKI. In this paper, we report inherent problem and security threaten in MPKI and propose an attribute-based authentication scheme using attribute-based signature in order to improve user authentication. In our scheme, access structure is used by Monotone Span Program, and system server provides service after user authentication.

Limite of Integration and Discrimination of Paraguayan Inmigrants in Argentina (아르헨티나 거주 파라과이 이민자에 대한 차별과 통합의 한계)

  • This article has analyzed the limite of integration and discrimination of Paraguayan inmigrants in Argentina. The most numerous collectivity is the Paraguayan inmigrants, with about 325,046 members that reside mainly in the Gran Buenos Aires in 2001. Paraguayan inmigrantes and their families usually undergo abuses by security forces and the over exploitation and the illegal work to a large extent work informally and for low wages. They also point out numerous problems of discrimination with respect to access to the services of public of health, education and housing. For the reason, Argentine government has established the program of Patria Grande in April 17, 2006. This program is that issue a permanent residence and DNI for Latin American immigrants from bordering countries. But the Argentine government does not maintain the program Patria Grande. Paraguayan inmigrants are still situated in the increase of the ilegal work and social insecurity.

Disclosure of Digitalized Information by Public Agencies (정보공개의 새로운 지향 - 전자정보공개제도(電子情報公開制度)를 중심으로 -)

  • Digitalization of public administration information shall be accelerated more. When information exists both in the form of electronic and paper record, even the disclosure of paper record make it possible the access to public administration information itself, but there may some needs for the disclosure of electronic record in applicant's situation. Similarly, when only electronic record exists, there may be some problem about whether to disclose the record as print-out or as being electronic format itself. Thus, the method and format of disclose are very sensitive issues, and it is very important to clarify who has the competence to decide the method and format of disclosure, applicant or the public agency. In making any record available to an applicant under the EFOIA in America, the public agency shall provide the record in any form or format requested by the applicant, if the record is readily reproducible by the agency in that form or format. And for the convenience of the applicant with sensory disability, the AIA in Canada also permits the right to access to information in an alternative format. It is desirable also in our country that disclosure of information is done by public agencies in the format that applicant wants, as possible. In the meantime, we should consider the costs and technological restrictions corresponding to the change of format of information to the format that applicant specifies. In the case of electronic record, efforts required for searching cause some hard problems. Information disclosure system requires disclosure of record that exists at the demand point, and creation of new record that does not exist at that them is not required on the public agency. For the search of electronic information, we need some code or program. So, if we evaluate that act of coding or programming as creation of new record, demand on disclosure of electronic record becomes impossible, in fact. Therefore, when we include electronic record as the object of information disclosure system, we need to clarify the degree of reasonable efforts for searching the information included in that record, as long as possible, although it is very difficult problem. Also, we should consider the way to make it permitted to demand the disclosure of electronic record by FAX or E-mail. Disclosure of electronic record itself by E-mail is not generalized yet, even in America or Canada. There are many technological and legal problems to solve, before permitting or enforcing the disclosure of electronic record by E-mail. But, it is desirable to expand the method of disclosure to including disclosure by E-mail in possible spheres. Also, as well as disclosure on demands, we need to expand electronic access to information, so far as possible, in the process of information offer.

Improvement Plan for Internet Reference Services of National Assembly Records (국회기록 인터넷 정보서비스 개선 방안)

  • This study aims at the fundamental factors needed to make the policy for internet reference service of public records were presented, considering the methods for improvement of internet reference service of national assembly records. For this purpose, reference service theory of recorded informations and examples of foreign congress reference services were analyzed. From the result of analysis, three tasks were proposed as an improvement plan for internet reference services of national assembly records: establishment of access policy, improvement of reference service programs, and activation of communication with users.

Impact of increasing the level of copayments on the number of physician visits (의료보험 본인부담금 인상에 따른 외래이용 변화)

  • The level of copayment increased in order to stabilize the financial condition of the health insurance on 1986. An important question regarding the policy was whether the increase in the level of copayments reduced the utilization of medical services in the poor selectively. In spite of the importance of the research question, no study has been reported. This study was designed to find out changes in numbers of physician visits, to explain characteristics influencing the difference of utilization before and after the program. Finally the interaction effect between the program and the level of income was examined for the abover question. A total of 10,421 persons from eight institutions was selected as the study sample. Research findings are as follows. 1. The number of physician visits decreased by ten percent as a result of increasing the level of copayment. 2. The decrease was remarkable in some groups such as children, rural area and large family. 3. The most important factor which explained the difference was the number of physician visits before the introduction of the new program. The more numbers of physian visits during the last year were, the more numbers of physian visits decreased after the program. 4. The interaction term between the program and the level of income was statistically significant in the multiple regression model which explained physician visits and its coefficient was negative. It means that an increase in copayment did not reduced the number of physician visits in the poor, selectively. 5. It can be concluded that imposing adequate copayment reduces the use of medical services as well as medical costs without serious damage in access especially for the poor people.
