Treatment Methods for Functional Recovery after Total Knee Arthroplasty (슬관절 전치환술 후 기능 회복을 위한 치료법)

  • Total knee arthroplasty is performed widely in degenerative arthritis of the knee joint, and the frequency of use is increasing. Efforts should be made to achieve functional recovery, such as returning to daily life, and the recovery of strength and range of motion after surgery. The procedure should be approached from a range of perspectives, such as patient factors, surgical technique and rehabilitation. The patient's age, degree of obesity, sex, and strength of the quadriceps muscle can affect the functional recovery after surgery. In addition, the patient's mental state, such as expectation value and satisfaction, can also have an effect. For functional recovery, patient education, pain control, and strengthening of the quadriceps muscle can be performed prior to surgery. Postoperative physiotherapy, such as icing and compression, transcutaneous electrical nerve stimulation therapy, neuromuscular electrical stimulation therapy, low frequency low intensity magnetic field therapy, strengthening of quadriceps muscle, and range of motion exercise can also be applied. In recent years, hydrotherapy, which restores the strength and balance of the leg, is being performed increasingly. These treatments are not only performed shortly after surgery, but are also performed continuously. In addition, the surgeon should apply it appropriately considering the patient's condition, compliance, and social and psychological conditions.

  • Fast track completion of urgent typhoon damage recovery construction project is becoming crucial for the prevention of the secondary damage from the next year typhoon, prompt psychological stabilization of flood sufferers, the early recovery of damaged infrastructure condition and so forth. As the recovery learning process has been continued for last consecutive typhoons and experts suggested, the keys in attaining the goals of the project (preventative, sustainable, cleaner and fast tracking development) are found to be the early contracting and improved competencies in project management especially during the design phase. It can be presumed that the former is attainable through administrative supports in the form of government guidelines, but the latter necessitates more research efforts. In this regards, this study aims to find a way how to lift up the current level of project management capability facing ever changing project mangement environment of urgent typhoon recovery project. By comparing the current level of application of government guidelines in flooded districts and analyzing the time required for each phase from design contracting to construction starting, several reformative ideas are illustrated in association with the necessity of adopting CM method in the deign phase. It is highly expected that CM application in the design phase can be an effective alternative in overcoming current limits in improving the quality of the project and prevent the delay due to the lack of expertise and professional workers in the owner side.

A Hermenutic Phenomenological Study of Psychological Burnout Experiences due to Emotional Contagion (정서전염으로 인한 심리적 소진 경험에 관한 해석현상학적 연구)

  • This study explored the essence of psychological burnout experiences due to emotional contagion using a hermeneutic phenomenological approach. In-depth interviews were conducted on 9 participants who work in fields that are subject to emotional contagion. Data analysis was conducted by using van Manen's methodology, insisting that the pure description of an experience can be enriched by adding interpretation. The emotional contagion experiences were identified through this process and the findings were categorized into 3 core themes, 8 essential themes, and 35 subthemes. The first core theme is "emotions in constant exchange". This theme included two essential themes: 'various channels of emotional contagion' and 'subjective states that change depending on the transmitted emotions'. The second core theme, "filtering the experience of emotional contagion" included the essential themes of 'the characteristics susceptible to the emotions of others', 'attitudes of spreading negative emotions' and 'situations that makes one feel overwhelmed by emotions'. The final core theme, "from burnout by emotional contagion to communication" was categorized into the following essential themes: 'burnout-inducing entangled interactions', 'moving toward communication and connection' and 'recovery after psychological burnout'. Finally, the implications and suggestions for future research were discussed by summarizing the core contents of each themes.

Social Adaption of Persons With Spinal Cord Injury by Modified Barthel Index

  • The purpose of this study was to evaluate the general characteristics, complications and level of social adaptation of spinal cord injured patients. The subjects were one hundred forty five members who were inpatients or outpatients from October 1, 2004 to April 30, 2005 in general hospitals and municipal welfare centers for the handicapped located in the metropolitan city of Gwangju. The following results were obtained using Modified Barthel Index (MBI). 1) Gender distribution was 77.9% male and 22.1% female. The mean age at the time of injury was 35.4 and the mean age during the study was 44.2. 2) The degree of paralysis among the subjects was as follows: 49.0% had complete paralysis and 51.0% suffered incomplete paralysis. The most frequently injured lesion among the subjects was cervical (49.0%), followed by thoracic (35.9%), and lumbar (15.2%). 3) The mean MBI score was 63.5. There was statistically significant difference in the MBI score in the relation between complete and incomplete paralysis, the relation between cervical, thoracic, and lumbar injury, and the relation between a recovery period of less than three years and more than three years according to the characteristics of injury (p<.05). 4) There was statistically significant difference in the MBI score of subjects who had complications concerning spasticity, deformity, urinary tract infection, and sexual dysfunction (p<.05). 5) The most serious emotional pain after spinal cord injury resulted from economic issues, which affected 35.2% of the subjects. The group having a shorter recovery period after spinal cord injury complained of psychological matters, the group having a longer recovery period complained about the surrounding environment (lack of convenient facilities), suggesting statistically significant difference (p<.05). 6) The most common activities of the group with injuries more than ten years old included meeting schoolmates and working, while most common activities of the group with injuries less than three years old included attending religious functions and miscellaneous others (watching TV, spending time with family), suggesting statistically significant difference (p<.05).

Design Requirements by Evaluating Comfort while Wearing Korean Naval Duty Uniforms for Summer and Winter (대한민국 동하절기 해군 함상복의 착용쾌적성 평가를 통한 디자인 요소 요구성능 분석)

  • The purpose of the present study was to suggest the design requirements for Korean naval duty uniforms by evaluating the physiological and psychological comfort while wearing the uniforms. Two sets of wear trials were conducted with summer uniforms(eight young males) and winter uniforms(seven other young males). The summer wear trial consisted of 10-min rest, 60-min exercise, and 10-min recovery at an air temperature($T_{air} $) of $33^{\circ}C$ and 62%RH, followed by 10-min recovery at a $T_{air} $ of $23^{\circ}C$ and 64%RH(total 90 min). The winter wear trial consisted of 20-min rest at $T_{air} $ $20^{\circ}C$ and 55%RH, 25-min rest, 30-min exercise, and 35-min recovery at a $T_{air} $ of 0oC and 43%RH(total 110 min). Rectal and seven skin temperatures, clothing microclimate, heart rate, oxygen consumption, total sweat rate, and subjective perceptions were measured during the wear trials. By evaluating the experimental results from the wear trials, we extracted the following psycho-physiological design requirements to improve the current Korean naval uniforms: (1) It is important to maintain the skin temperatures within their comfort range, which depends on the body region (higher than $30^{\circ}C$ in winter, but less than $35^{\circ}C$ in summer). (2) In summer, the feet should be protected from the high heat of the ship floor surface. (3) In summer, sweat from the back should be sufficiently absorbed and allowed to dry quickly.

  • The purpose of this study is to suggest a professional consensus on the topics that should be dealt with importantly in the movement program for the recovery of cancer patients in Korea. As a result of calculating the importance of the finalized movement program component, First, the opinions about the environment for the promotion of movement were suggested as pleasant facilities, natural lighting, rest room space, facilities aspect of environment friendly space, education about diseases, participation programs, health feedback, information handouts. Second, the opinions on the purpose of the movement program were suggested to promote prevention and early screening of cancer, education, sharing effective treatment methods of cancer patients, cognitive aspect and psychological support for accurate information transmission, stress relief, and improvement of quality of life. Third, the opinions on the consideration of the movement program suggested stress relieving, cancer patient's needs, correct information delivery, quality of life, and participation of cancer patients. Fourth, the problems of the movement program were presented with different physical characteristics, physical inconvenience, the patient side of passive participation, space narrowness and lack, space aspect of program exclusive space etc.

  • This study has purpose to perform leaving the hospital nursing plan during hospital treatment of patient with the knee ligament Injury on the basis of the concerns between the time of leaving the hospital after 2 weeks in hospital and the concerns through practical life after leaving the hospital systematically. This study is also tried to make exercise plan to prevent atrophy of Quardriceps muscle of the thigh caused by the limitation of activities. The subjects of study are 12 people who were in the hospital in affiliation to a university in Seoul, were operated on the knee ligament, and were discharged from the hospital within 2 weeks. Data collection time was for about 4 months from December, 94 to March, 95. Research method was performed 2 times by unorganized open questions. The collection of first data is through direct interview about the concerns before leaving the hospital, and secondary data is through direct interview about the concerns by practical life and experiences after leaving the hospital. The data was categorized by classifying several common elements among similar contents according to data by the situational analysis of Giorgi. The result of this study is as follows : 1) The concerns before leaving the hospital are like that the concerns of recovery possibility, the limitation of activities, the grim realities of life, the lack of confidence about self-nursing, the eyes of the surroundings, the burden to the others, and so on. 2) The concerns after leaving the hospital are like that the demands of the inconvenience by the limitation of self-nursing, the support about self-nursing by the difficulties of performing treatment order, the support of physical activities limitation by physical tiredness and the limitation of social activities, the support of information by the necessities of education in using crutches before leaving the hospital and continuing informational needs, psychological and mental support of normal physical recovery by the worries of physical recovery and physical change, and social support by thankfulness for his family and relatives. I suggest to make more systematic leaving the hospital nursing plan and exercise education plan for nursing plan after leaving the hospital of the patients with the knee ligament injury on the basis of this content.

The Grounded Theoretical Study on Negative Emotion Recovery Process of Female College Students Through Bibliotherapy (독서치료를 통한 여대생의 부정적 정서 회복과정에 대한 근거이론적 연구)

  • Today. the mental health has a great deal of impact on individuals and the society surrounding. Therefore. this research is to propose a way of bibliotherapical intercession by stages and patterns as to examine the recovery process of students with negative emotion by using the grounded theoretical method. This research collected materials through focus-interview, participant observation and book reports of 4 experiments involving female college students aged from 21 to 23, all having emotional and psychological difficulties. The recovery process of female students with negative emotion involved causal factors such as "recognition of hidden wound" "Sense of inferiority", and "difficulty in forming relationships", as well as contextual factors like "sympathetic attitude". which altogether brought about the core phenomenon. "pursuit for healthy ego". This core phenomenon induced to recovering the bright and positive attitude through using the strategies of 'self insight' and application of 'management plan', influence by such incoherent conditions as 'intimate surroundings' and $\setminus$ 'support system patterns'. And as time passed. the recovery process appeared tin four stages: 'issues recognition'. 'emotional responses', 'issue solution' and 'self application'.

One Case of Female Patient with Suspected Kleine-Levin Syndrome (여성 Kleine-Levin syndrome 의증 환자 1례)

  • Kleine-Levin syndrome is a rare disorder characterized by recurrent episodes of hypersomnia, hyperphagia, hypersexuality and delusion and hallucination with spontaneous recovery. A 17-year-old female suddenly showed hypersomnia, severe fatigue, and compulsive hyperphagia. She was diagnosed with suspected Kleine-Levin Syndrome. The precipitating factor was an upper respiratory tract infection that the patient had undergone 2 weeks prior. Past psychiatric history included somnambulism and ADHD; her symptoms of ADHD included attention deficit, impulsiveness, hyperactivity, and irritability. This was her third episode. Her hypersomnia was longer in duration but lower in frequency compared with usual KLS. Her low LF/HF ratio in heart rate variability measurement showed high parasympathetic nerve activity. A MMPI report showed some psychological problem. The patient was diagnosed of gastric fever by pattern identification. Her hypersomnia episode improved 6 weeks after onset. This is the first reported case of suspected Kleine-Levin Syndrome in the field of Oriental Medicine in Korea.

Treatment and Management of Conversion Disorder (전환장애의 치료와 관리)

  • Conversion disorder is a psychologically produced alteration or loss of physical functioning suggestive of a physical disorder. Conversion symptoms are often superimposed on organic disease and can be overlooked. Psychological techniques are central to the management include the following: avoiding confrontation with the patients; avoiding reinforcement or trivializing the symptoms; reviewing results of tests and exams and creating an expectation of recovery; educating the patient before a treatment is begun; evaluating the patient's emotional adjustment and considering it at a treatment; using caution in labeling the condition; considering referral for psychotherapy; establishing particularly a treatment plan and making a definite treatment program; adjusting patient' s environment; letting participate a family at appropriate time; developing a reinforcement program for a treatment of chronic symptoms; developing a home program for outpatients. Use behavior therapy reinforcement may be helpful with more chronic or resistant symptoms, especially when there is a history of vague or excessive somatic complaints or significant secondary gain.
