노년기 비만군과 정상 체중군 간의 우울 성향 비교 (A Comparison of the Depressive Predisposition between Obese and Non-Obese Elderly)

  • Objective : Obesity has been considered to be associated with numerous physical, mental and psychological diseases. Depression, which is a major psychological factor affecting occurrence and treatment of obesity, can be a cause of obesity as well as can be triggered by obesity. This study aimed to find out the need of positive consideration to the existence of depressive mood in the management of obesity. Methods : A total of 101 subjects were enrolled among those who were 60 years old and over at one local clinic. Obese group (n=49) was defined as BMI${\geqq}$25 and normal weight group (n=52) as 20$5.2{\pm}2.5$) was significantly higher than that of the normal weight group ($3.8{\pm}1.7$)(p<0.05). In male subjects, there was no significant difference in the mean GDSSF-K score between both groups, but in female subjects, the mean GDSSF-K score of the obese group, $6.4{\pm}2.6$ was significantly higher than that of the normal weight group ($4.0{\pm}1.8$)(p<0.05). Conclusion : The elderly obese group showed higher depressive scale score than elderly normal weight group, especially in female elderly. Since the elderly obese female group had a depressive predisposition, comprehensive management including mental and psychological approach is required in obesity control programs.

Objecrives Although Lee Je-Ma emphasized it is important to be aware of our own Nature & Emotion(性情) and control them properly, there is few case report about Taeyangin focused on their psychological sides. This report is about a Taeyangin patient who had Hae Yuk and Yul Gyuk syndrome caused by her stress from the work place and relationship with her seniors 2. Methods We observed the female patient had hospitalized in the Oriental Hospital of Dongguk University more than twice has been followed up for the last one year. We prescripted Mi-hoo-teung-shik-chang Tang as her physical symptom. The improvement of her physical conditions were checked through her sleep, meal, menstruation, stool and urine. And MMPI has been tested three times for the last one year to analyze her psychological progress. 3. Results and Conclusions Working in a group can cause hard stress for Taeyangin who is not good at adjusting themselves to whatever environment they are in. Mi-hoo-teung-shik-chang Tang could improve Taeyangin's physical conditions such as Hae Yuk, Yul Gyuk, insomnia, nausea, anorexia and hypouresis. But as her psychological aspect, she was getting more anxious, depressed and avoiding contact with others due to the stress. It is necessary to rake care of patients' psychological sides as well as physical conditions through managing their Nature & Emotion(性情).

자살의 생물학적 기전 (Biological Mechanism of Suicide)

  • Suicide is a behavior that is intended to cause death by itself and requires medical treatment, resulting in suicidal attempt or completion. Suicide causes loss of life, damages the body, costs a lot of medical expenses, and causes families to fall into sorrow and suffering therefore this suicide is a huge loss to family and society. There have been attempts to reduce and prevent suicide by understanding the mechanism of suicide. The mechanism of suicide can be thought of as psychological mechanism and biological mechanism. In the past, if we considered the psychological and biological mechanisms separately, the development of neuroscience now connects and integrates these two. Psychological factors affect biological factors and biological temperaments also affect perception or thinking about the situation and increase psychological vulnerability. Distant factors in suicidal behavior-such as childhood adversity and family and genetic predisposition-increase the lifetime risk of suicide. They alter the response to stress and other processes through changes in gene expression and regulation of emotional and behavioral characteristics. Distant factors affect the biological system and consequently changes in these systems can increase the risk of suicide. In other words, the distal factor does not directly induce suicidal behavior but rather acts indirectly through developmental or mediating factors. These mediating factors are impulsive aggressive and anxious trait, and chronic use of substances. The mechanism of this disorder is the abnormality of the serotonin system and the abnormality of the lipid level. Proximal factors are associated with the onset of suicide events and include changes in the major neurotransmitter systems, inflammatory changes, and dysfunction of glial cells in the brain. A series of studies, including a variety of research methods and postmortem and in-vivo imaging studies, show the impairment of the serotonergic neurotransmitter system and hypothalamic-pituitary-adrenal axis stress response system for suicidal behavior. These disorders lead to suicidal behavior due to difficulty in cognitive control of mood, pessimism, reactive aggression, abnormality in problem solving abilities, excessive response to negative social signals, severe emotional distress, and cognitive dysregulation of suicidal ideation.

DYNAMIC MODEL PREDICTING OVERWEIGHT AND OBESITY IN KOREAN ADOLESCENTS

  • Adolescent obesity has a high risk of developing into adult obesity and may cause many psychological problems. This paper aims at giving a mathematical model for the obesity of Korean adolescents and predicting how much the prevalence of obesity among adolescents will increase using real data. We estimate that the obesity rate of boys will increase until about 28 ~ 29% in 2025.

심리적 부검을 통한 전북지역 자살 실태에 관한 연구 (Suicide in Jeonbuk region: Psychological autopsy study)

  • 본 연구의 목적은 심리부검을 통해 전북지역의 자살실태를 파악하는 것이었다. 연구대상은 2013년 5월부터 2014년 4월까지 1년간 전북지역에서 발생한 자살사망자 중 심리적 부검에 동의한 42건의 사례로, 사망자의 직계가족을 대상으로 반구조화된 심리면담을 실시하였다. 그 결과 자살 사망자중 71%가 과거의 자살시도 경험이 있었으며, 50%에서 자살가족력이 보고되었다. 자살의 추정원인으로는 정신 질환(38%)이 가장 빈번하였으며, 그 다음으로는 대인관계 문제(31%), 가정 문제(11%)순이었다. 사망자가 진단받은 정신 질환은 우울증이 가장 빈번하였는데, 대부분이 과거에 치료를 받았지만 사망 당시에는 치료를 받지 않았던 것으로 나타났다. 이러한 결과는 자살이 정신 질환과 정신질환에 대한 적절한 개입 실패와 연관되어 있음을 시사한다. 또한 과거 자살시도나 자살 가족력이 있는 대상에 대한 집중적 자살예방개입의 중요성을 시사한다. 끝으로 본 연구의 의의, 제한점에 대해 논의하였다.

  • Stroke is the leading cause of adult disability and the third leading cause of adult death in the world. Stroke has an impact on the physical, psychological and social dimensions of quality of life. An increasing number of authors have reported a deterioration of the quality of life in a large proportion of patients after a stroke. Just as satisfaction with each dimension varies to everyone, the importance of each dimension also varies and does not impact equally on the quality of life. So assessment on the quality of life has to adopt a multidimentional approach and to make use of the subjective and objective indicators.

영역특정론과 영역일반론에 따른 유아의 인과추론 - 물리, 심리 영역을 중심으로 - (The Role of Domain-specific Causal Mechanism and Domain-general Conditional Probability in Young Children's Causal Reasoning on Physics and Psychology)

  • The role of domain-specific causal mechanism information and domain-general conditional probability in young children's causal reasoning on physics and psychology was investigated with the participation of 121 3-year-olds and 121 4-year-olds recruited from seven child care centers in Seoul, Kyonggi Province, and Busan. Children watched moving pictures on physical and psychological phenomena, and were asked to choose an appropriate cause and justify their choice. Results showed that young children's causal reasoning differed depending on domain-specific mechanism. In addition, their causal reasoning on physics and psychology differed by the developmental level of causal mechanism. The interaction of domain-specific mechanism and domain-general conditional probability influenced children's causal reasoning : evident conditional probability between domain-appropriate cause and effect helped children make more inferences based on domain-specific causal mechanism.

  • On the basis of the equity theory, as the couple perceives their relationship is equitable, they tend to make decisions jointly and are satisfied with their family lives. Therefore this study is aimed to identify the relationship among perception of equity, decision-making styles and family life satisfaction in dual-career couples. As a result of analyzing the cause-effect relationship, it shows that many socioeconomic$.$psychological variables have direct indirect impact on family life satisfaction through medium factor, so this study proves the application of the cause-effect modes. However the influence of the perception of equity of wives were higher than husbands.

고침무우산으로 개선된 중풍 환자 불면 1례 (Oriental Treatment of Insomnia in Stroke Patient)

  • A growing number of people are concerned about their sleep. There are many people with chronic sleep disorders. As there are various causes in a disease, proper treatment according to each cause is necessary for a more effective treatment. In general, insomnia is classified into five categories of physical, physiological, psychological, psychiatric and pharmacological aspects. Sedative-hypnotics including benzodiazepine and non-benzodiazepine have widely been used in chronic insomniacs. However, most hypnotics including non-benzodiazepine cause some of dependence, tolerance, impaired daytime function and rebound insomnia. Therefore, we are looking forward to proposing an effective oriental treatment for insomnia. A 71-year-old male who had suffered from cerebral infarction was admitted to our department for oriental treatment of stroke and insomnia. Initial treatment modalities with administration of paroxetine were not effective. However administration of oriental medicine' Gochimmuwoo-san(Gaozhenwuyou-san)' achieved a desirable effect.

이갈이의 진단과 치료 (Diagnosis and Management of Bruxism)

  • Bruxism is defined as 'diurnal or nocturnal parafunctional activity including clenching, bracing, gnashing, and grinding of the teeth'. Bruxism and clenching are two of the most common contributing factors in patients with temporomandibular disorders and craniofacial pain disorders. Even though many studies report the high prevalence of bruxism, its cause is still not clear. Occlusal interference has been regarded as a major etiologic factor. Nowadays, psychological stress and sleeping disorders are generally regarded as major possible etiologic factors. More than likely, the cause is multifactoral and overlapping, which makes it difficult for the practitioner to apply comprehensive and effective management strategies. Although dentists and psychologists generally believe that effective treatment is best achieved with a better understanding of the etiology of a given disorder, for now treatment for this type of disorder must proceed without a clear understanding of etiology. To overcome this obstacle, evidence-based comprehensive management protocols based on accumulated scientific findings should be provided. In this presentation, epidemiology, etiology, and the characteristics of bruxism are reviewed. Diagnostic procedures and management strategies focused on occlusal appliances and behavioral approaches are also discussed.
