Varieties of literatures were reviewed in regard to the fundamental concept of day hospital, historical trends, the recipient of its care, facilities and personnel, therapeutic programmes and the follow-up care plans. Through the research the advantages of day hospital were highlighted in order to provide the reference for those who consider planning such health care institution. Since the introduction of the concept of day hospital and its implementation in 1930, many psychiatric patients world over are treated and cared. Patients with specific health problems ; alcoholism, acute or serious psychiatric disease, tendencies of humidor suicidal attempts, and with serious physical problems were excluded from the general recipient. Day hospital were annexed to the psychiatric hospitals in most in instances ; facilities, personnel, except nursing personnel, were shared. All therapeutic care were planned in daley, weekly programmes, and were focussed on socialization. The follow-up care were provided for those participating post- therapy club activities which were planned and introduced ahead. Many advantages of day-hospital care in contrast to the traditional hospitalization care were found: 1. The abrupt discontinuity of his family and other social role is prevented. 2. Therapeutic progress is faster. 3. Lessened economic burden to the family. 4. Behavioral regression is lessened and the lessened fear of hospitalization. 5. Less injury to the patients, self- respect, through lessened anxiety of hospitalization. 6. Incidents of secondary crisis believed to be existing in long term cases are decreased. 7. Therapeutic care implemented in freer atmosphere, better Patient-personnel relationships are created. 8. Varieties of group activities are Induced which enable faster recovery. 9. Patients could engage himself with social activities including getting job on part-time basis. 10. Rehabilitation of patient could be implemented.
Background and Objectives : This study aims to identify sociodemographic and disease-related variables which predicts continuity of outpatient treatment after discharge among bipolar patients. Materials and Method : The medical records of patients who discharged with the diagnosis of bipolar disorder from Department of Psychiatry, St. Mary's hospital from 2005 to 2009 were reviewed. Data on sociodemographic and disease-related variables were analyzed. Results : It showed older age, higher rate of male and higher rate of being married in 1-year follow-up group than in non-follow-up group. And it showed longer duration of index hospitalization, higher rate of previous psychiatric outpatient treatment within 3 months before index hospitalization, higher rate of involuntary admission in 1-year follow-up group than in non-follow-up group. The univariate logistic regression analysis revealed that older age, being male, previous psychiatric treatment and longer duration of index hospitalization were significantly related to an increased likelihood of 1-year follow-up visits. Conclusion : Age, sex, history of previous psychiatric treatment, and duration of hospitalization seems to have influence on continuity of outpatient treatment after discharge.
Purpose: This study was conducted to explore the mediating effects of internalized stigma on the relationship between hospitalization stress and quality of life among patients with mental illness in closed wards. Methods: A cross-sectional study design was employed. The participants were 141 hospitalized patients with mental illness in the closed wards of K university hospital and Y psychiatric hospital in Korea. Data were collected using self-report questionnaires including hospitalization stress, internalized stigma and quality of life. Data were analyzed using SPSS 21.0 and AMOS 21.0 programs and the mediating effects were examined by the method suggested by Baron and Kenny. Results: There was a significant correlation between hospitalization stress, internalized stigma and quality of life of psychiatric patients. In addition, internalized stigma was found to have a partial mediating effect on the relationship between hospitalization stress and quality of life. Conclusion: The results suggest that the internalized stigma plays an important role in improving the quality of life of the people with mental illness in a closed unit. Therefore, it is necessary to develop a program focusing on internalized stigma for improving the quality of life in hospitalized patients in a closed unit.
We studied the factors that would affect the severity of psychiatric symptoms of the Sampoong accident survivors. In this study, the analyzed factors are sex, age, marital status, educational level, loss of consciousness, witness of death, death of related person, duration until the rescue, duration of hospitalization, and psychiatric treatment after the accident. Although our study has some limitations, we found some factors that affect the severity of psychiatric symptoms after the severe traumatic accident.
This study was done to determine the relationship between the anxiety levels of hospitalized psychiatric patients and various influencing variables. The purpose of this study was to determine factors that may help hospitalized psychiatric patients to experience lower levels of anxiety in relation to changing situations and provide the basic data for a dynamic approach which is important in the field of modern psychiatric nursing that understands and analyses the meaning of patients behavior. The anxiety may produce stress, which is a common experience among all human beings. Patients may merely feel uncomfortable in the state of mild anxiety, however, the severe state could be an obstacle to treatment and recovery from disease. The anxiety of the psychiatric patient is a factor which greatly influencing the patient's behavior, so his disorderd behavior is an expression of defence or pathologically fixed behavior. According the psychiatric patient's anxiety at the time of admission is the concern of the health team. The nurse's special concern has to do with understanding and supporting the patient and meeting his individual needs by frequent close contact during the entire hospitalization period, compared to other teamembers the nurse's responsibility in this regard is greated. So this study emphasizes the necessity of creating conditions these, but above all the psychiatric nurse should create a therapeutic environment by not only regarding the patient's behavior or symptoms but understanding the meaning of them. The subjects of this study were 57 psychiatric patients selected from the K neuropsychiatric hospital located in Kunsan city. Data were collected twice from the same patients within a 24 hour period after admission and 10 days after admission. (September 18th to November 8th, 1980). The data collected method was through direct interview, and the interview time was 20 minutes for each patient. Data analysis included Item Analysis & Internal Consistency Reliability Tests, Percentages, t-test, analysis of variance and stepwise multiple regression analysis. In Korea, exposure to stress has been accompanied by mental pain in the process of achieving many growth along with rapid development, various social problems, and the frequency of emergency hospitalization is increasing.. In the case of mentally ill patients, "unwanted hospitalization" is a problem, and police and 119 paramedics try to suppress the body of mentally ill patients, and many problems are exposed This is because the constituent requirements of the provisions of emergency hospitalization under the Mental Health and Welfare Act do not reflect reality, and each institution has a different position on one mentally ill person, and emergency hospitalization does not proceed smoothly or leads to friction between related organizations, and the safety of the mentally ill or others is not secured. Emergency hospitalization is defined as "a person who finds a person who is presumed to be mentally ill and is at high risk of harming his or her health or safety or others," and if the situation is so urgent that he or she cannot afford time to go through the hospitalization procedure to decide on his or her own hospitalization, he or she can request emergency hospitalization with the consent of a doctor and a police officer. In this case, 119 paramedics are escorted to a psychiatric institution. This provision of emergency hospitalization poses many problems in the process of transferring to psychiatric institutions. If a police officer or 119 paramedics in charge of practice use "physical force" during the emergency hospitalization process, side effects will inevitably occur, and professional negligence can be a problem. Specifically, when exercising physical force, the minimum necessary physical restraint based on laws and regulations and proportional principles is required, and the lack of the duty of care of 119 paramedics or police officers under the laws and regulations will eventually be resolved by applying other laws and regulations. Accordingly, it will be an opportunity for mentally ill patients to be transferred to psychiatric institutions in a safe environment by changing the subject of emergency hospitalization provisions under the Mental Health Welfare Act, defining and prescribing the use of physical protection guards as the enforcement regulations of the Mental Health Act, setting the duty of care for 119 paramedics and police officers, and creating an environment for transportation so that mentally ill patients can be treated safely.
Purpose: This study was done to develop a measurement for stress experienced by patients with schizophrenia during hospitalization. Methods: The preliminary tool was developed through in-depth interviews and a validity verification test of content. For data collection, 15 inpatients with schizophrenia were selected as participants for in-depth interviews and 195 patients admitted to one of eight psychiatric hospitals in four provinces were recruited as participants to test reliability and validity of the preliminary tool. Results: The questionnaire was developed as a four-point Likert-type scale in a self-report form with 28 items. Factor analysis showed 28 items in six factors. Factors were named 'Unjust human rights infringement', 'Futureless life', 'Alienation from other family members', 'Infringement of basic needs', 'Infringement of personal preference' and 'Inconvenience of shared living'. The six factors explained 63.5% of the total variance. Cronbach's alpha for the total items was .93 and for the factors ranged from .65 to .87. Conclusion: A tool to measure stress in patients hospitalized with schizophrenic was developed based on identified hospitalization stress experiences. Study results indicate that this tool can be used to evaluate hospitalization stress in these patients and will contribute to establishing nursing interventions for relief of hospitalization stress.
This article examined relationships between selected variables, such as demographic background, care, treatment variables, environmental characteristics, and patient's daily behavior and mood change. Relationship were determined between independent variabltherapeutic-rapeutie approach, demographic data, environmental management approach-,and dependent variable-patient's daily behavioral and mood change. 35 patients selected within some criteria in a psychiatric ward, were obserbed during 5 weeks by use of Wyatt's Behavior & Mood Rating Scale ac-cording to the object of the study. At the same time, the frequence of the care and treatment were collected. Criteria for sample selection and independent variables as an influential factor to the patient behavioral change, based on a literature revienw and clinical experiences. Pearson's correlation and multiple regression analysis were used to determine the influfntial factors to the patient behavioral change. Systematic reading (r=.8324), Psychiatrist's individual interview (r=.5764), tranquilizer (r=.3441) and hospitalization processing date (r=.4143) were related with patient's behavioral change. That is these 4 variables can be said to influence to the patient's behavior and mood. A stepwise multiple regression analysis of the effect of the independent varibles of systematic reading, psychintrists individual interview, tranquilizer and hospitalization processing date on the dependent variable, patient's behavioral change was carried out. Systematic reading with on R²of. 69 revealed to be the main influential factor to the patient's behavior and mood change, as the next factor psychiatrist individual interview. A total inclusion of these factors revealed a 73% prediction for the patient's behavior and mood change. But the most influential factor was the interaction of the systematic reading and psychiatrist's individual interview.
The purpose of the present research is to evaluate the therapeutic ingredients-empathy, warmth, and intimacy - as perceived by patient in the relationship with nurses during his hospitalization. "Relationship Questionnaire"developed by C. B. Truax during 1963 was administered from September 1976 to December 1976 to a sampling of 61 discharged patients, 20 were received by mail questionnaire, 19 were successfully interviewed by their doctor at a regular psychotherapy meeting, 22 were contacted by nurses or psychiatric residents at discharge day. Findings showed that 1) the total mean score of therapeutic ingredients of nurses was 9. 54 ; 2)the total mean score of empathy, warmth and intimacy was 10. 10, 9.57, and 9.00 : 3) there was the significant relationship between therapeutic ingredients and other variables-sex, education, number of admission, helpfulness ratings of admission and, present health condition- : 4) there was no significant. relationship between therapeutic ingredients as patient′s perception and age, duration of admission, or motivation of admission, or follow - up rare ; 5) there was no relationship among empathy, warmth and intimacy ; 6) tile ranking of the important psychiatric personnel who were marked as the helper at the treatment and understanding for patient′s recovery, was psychiatric doctor and nurse and then nurse student and then nurse aid.

Objectives : The objective of this report is to identify the utilization of hospital school service during hospitalization among patients in their childhood and adolescence with psychiatric disorders. Methods : We retrospectively reviewed the medical record of child and adolescent psychiatric who were hospitalized during March 2009 through October 2012. We compared the one-year successful schooling and outpatient follow up rate between users and nonusers of the inpatient hospital school service. The hospital schooling experiences of the users were investigated upon follow-up visits to the outpatient clinic. Results : Sixty-three students received hospital school service during hospitalization among total 122 child and adolescent inpatients. Hospital school participants showed a significantly higher school reentry rate (61.9%) than non-participants (40.7%). However, there was no difference on follow up rate between the two groups. More than 60% of the 22 interviewed participants expressed an above-average level of satisfaction about hospital school service. Conclusion : Many patients with mental illness experience difficulty in receiving school education during treatment. That induces deterioration in disease, academic failure, poor social skills, low self-esteem, economic difficulties, and future job opportunities. The results of this study emphasize the importance of hospital school service and offer useful guidance for hospital school operation.
