목 적 : 면역치료는 흡입성 알레르겐에 의해 유발되는 천식이나 비결막염, 곤충자상 치료에 매우 효과적이다. 이중 집먼지진드기 항원에 감작된 환아를 1-2일 내에 유지용량에 도달하는 초급속면역치료(ultra-rush immunotherapy)에 대한 안전성과 효과를 알아보고자 본 연구를 계획하였다. 방 법 : 분당차병원 소아 알레르기 클리닉에서 알러지성 비염 또는 기관지 천식으로 진단받은 4세 이상의 소아 중, 피부반응 검사에서 Dermatophagoides 항원에만 양성 반응을 보이고 비강유발검사에서 양성소견을 보인 환자를 대상자로 선정하였다. 고식적면역치료는 11명, 초급속면역치료를 신청한 소아는 12명이었다. 초급속면역치료는 목표 농도의 100:1-1,000:1로 희석한 알레르겐을 30분 간격으로 2-3배 증량하며, 주사 후 15분마다 산소포화도와 심박수, 혈압 및 전신반응 유무를 관찰하였고, 이때 나타난 환아의 증상을 점수화하였다. 면역치료의 효과 판정은 초기치료 3개월이 경과한 후에 비강유발검사를 실시하여 점수 변화로써 확인하였다. 결 과 : 고식적면역치료와 초급속면역치료 받은 소아의 평균나이는 각각 $8.3{\pm}2.3$세, $9.2{\pm}2.8$세였다. 초급속면역치료를 받은 12명중 6명(50%)에게서 전신반응이 관찰되었으며, 아나필락시스와 같은 심각한 부작용은 발현되지 않았다. 고식적면역치료의 경우 11명중 1명(9%)에서만이 전신반응이 관찰되었다. 치료 전후의 비강유발검사 평균 점수는 고식적면역치료 군의 경우 치료 전 $8.2{\pm}1.5$점, 치료 후 $4.6{\pm}2.1$점이었으며(P=0.043). 초급속면역치료에서는 치료 전 $6.2{\pm}2.2$점, 치료 후 $3.7{\pm}2.5$점으로(P=0.017) 두 군 모두 치료 전후 비강유발 검사 결과가 호전된 것을 알 수 있었다. 결 론 : 소아에서 Dermatophagoides를 이용한 초급속면역치료는 3개월 후 비강유발검사상 호전된 것을 확인할 수 있었으나 전신반응이 고식적 면역치료보다 높아 소아를 대상으로 시행함에 있어 면밀한 주의가 요구된다.

신파극은 식민지라는 사회적 정황을 근거로 필연적 발생배경을 갖고 있으면서 국민들의 반일 감정을 삭이는 삶의 분출구 역할을 해왔다. 신파극에 대한 연구는 신파극이 '개화'라는 시대적 변화에 적응하며 우리 민중 기저의 흥미와 위안의 대중 문학적 역할을 하였다는 것이다. 그러나 신파극이 보여준 당시 사실주의 근대극이 추종할 수 없이 많았던 공연 횟수라든가 당대의 청관중들에게 적극적으로 다가서서 연극의 대중화 공로를 무시할 수가 없다. 그러므로 본고는 신파극이 존재했던 사회 정치 환경 속에서 신파극의 구조의 특이성들을 바탕으로 당시의 문화적인 장을 검토하여 신파극이 식민지 문화 '장' 내에서 지배이데올로기와 반응하면서 수행한 이데올로기적인 기능과 효과가 무엇이었는지를 논의하고자 한다. 그 동안 연구사에서 간과되어온 신파극의 구조와 양식적 특성 그리고 연극사 혹은 문화사적인 위치의 규정이 요구된다. 이러한 현실은 식민지시대의 국민적 정서를 도외시한 채 신파극단장들이 상업적인 측면의 번안 작품 공연에 주력하여 실질적인 우리나라 전통연극과의 단절을 가져왔으며, 한일 합방에 따라 일본 연극을 곧바로 직수입해서 모방한 오점들에 너무 지나치게 주목한 때문으로 풀이된다. 그런 점에서 이 논문은 신파극의 구조와 식민지에서의 현실인식을 연관지어 살폈다. 이를 위해 본고에서는 신파극을 우리나라 근대 연극사에서 독자적인 위치를 확보한 연극적 양식으로 보고, 이를 위해 대표적인 신파극 대본들로부터 그 구조적인 특징을 분석추출하였다. 또 그러한 신파극의 양식적 구조가 당대 사회와 어떠한 관련이 있는가를 분석했다. 그 결과, <사랑에 속고 돈에 울고>, <장한몽>을 중심으로 한 당시의 대표적 신파극은 종래의 학자들의 주장과 서양의 멜로드라마의 일반적인 '자극-고통-벌칙'의 구조와는 달리 '자극-고통-패배'의 양식적 구조를 이루며 당대 사회의 식민지의 정치-사회적 맥락과 서로 필연적 상호관계를 형성하고 있음을 파악하였다. 신파극에 의해 생산된 이미지들은 근대의 문턱에서 변화하는 사회의 서열화 된 위계구도를 수동적으로 반영하면서도, 한편으로는 그러한 위계구도를 반영하는 양식적 구조 속에서 식민지 제국주의의 정치-사회적 현실을 인식하였다. 또한 민중집단의 관계의 측면에서, 식민지 문화 '장'의 구조로서 신파극이 갖고 있는 이중성과 신파극의 사회-문화적 위치를 살펴야 한다. 신파극을 하나의 공연 혹은 문화현상으로 고려할 때, 공연 전체는 공연의 부분들 사이의 관계들로 빚어진 특징적인 귀결로 고려될 수 있다. 사회가 부분들의 유기적 관련들로 구성된 하나의 총체이듯, 신파극은 사회와의 관련 속에서 문화현상으로 표현된 또 하나의 총체이다. 이러한 신파극이라는 문화현상은 사회 안에 있는 하나의 구성물이다. 그러므로 좀 더 큰 구조들 즉 문화 '장'내에서 그것의 기능적 연관성을 밝혀낼 필요성이 존재한다.

연구배경: 심인성 폐부종은 임상실험이나 동물실험에서 비특이적 기도과민 반응을 증가시킨다는 많은 보고가 있다. 또한 만성 신부전 환자는 혈관밖 폐수분 증가에 의하여 무증상에서 명확한 폐부종까지 다양한 임상 양상을 보이고 있다. 여러 보고에 의하면 만성 산부전 환자에서 혈관 밖 폐수분 증가로 폐활량이 감소하며 혈액투석 후는 폐 간질성 부종의 감소 혹은 소설로 폐활량이 호전 된다고 한다. 그런데, 이 폐 간질성 부종이 비특이적 기관지 과민 반응을 야기하는 한 기전으로 생각하고 있다. 그래서 연구자들은 만성신부전 환자에서 폐간질성 부종에 의하여 기관지 반응이 증가되는지 여부를 알기 위하여 본 실험을 시행하였다. 방법: 다른 동반질환이 없는, 1주에 3번씩 혈액투석을 하고 있는 18명의 만성신부전 환자를 대상으로 하였다. 이 환자들은 혈액투석전 폐기능 검사와 메타코린 기관지 유발검사를 하였고, 기관지 과민반응을 나타낸 환자는 혈액투석후 폐기능검사와 메타코린 기관지 유발 검사를 반복 시행하였다. 결과: 1) 18명중 12명에서 혈액투석전 메타코린 기관지 수축 유발검사에서 양성 반응을 하였고(66.7%), 이 기관지 과민반응은 혈액투석후 감소되었다. 2) 폐기능은 혈액투석후 호전되었고, 혈액투석 전후 $FEV_1$ 변화와 체중 변화는 유의한 상관 관계가 있었다(r=-0.62, p<0.01). 3) $PD_{20}$와 $FEF_{25}$ 사이에도 유의한 상관관계가 있었다(r=0.59, p<0.05). 결론: 만성 신부전 환자에서 관찰된 폐기능 저하와, 기관지 과민성 반응은 폐 간질성이 부종에 의할 것으로 사료된다.

영화 <남산의 부장들>은 다른 10.26 사건 소재의 콘텐츠와 달리 대립되는 인물 간의 치열한 심리 대결을 표방한 연극적 성격이 짙은 '분노 캐릭터' 영화라는 점에 착안해 본 연구에서는 라코프와 코브시스(Lakoff & Kövecses) 등이 제시한 '분노의 5단계 모델'을 적용해 장면을 단위로 설정해 서사구조를 내용 분석하였다. 그 결과 이 영화는 심리학의 분노 5단계 이론을 잘 반영해 주인공의 분노 결행에 대한 인과성과 개연성을 한층 높인 것으로 나타났다. 특히 분노조절(25분)에 가장 긴 시간을 할애해 응징·보복의 동기를 정당화한 반면 통제력 상실(14분)에 가장 짧은 시간을 부여해 응징 행동의 긴박감을 극대화 하고자한 것으로 분석되었다. 각 단계에서 분노를 유발하는 기폭제 역할은 무시, 모멸, 오만, 과시, 놀림, 조롱, 무단 개입, 수군거림, 비아냥, 말 가로채기 언행으로 각 단계마다 2~6회씩 배치되었다. 특히 '경호실장'역의 경우 조롱 발언이, '박통'역의 경우 친구도 죽인 무도한 인간이란 극심한 모멸감을 야기하는 발언이 주인공으로 하여금 복수를 실행하도록 하는 결정적인 '분노 방아쇠'로 작용하였다. 결론적으로 이 영화는 서사구조가 분노이론을 정교하게 따랐다. '정치극'이라서 서사의 전개에서 분노의 공적(公的) 사유를 많이 설정하는 것이 일반적인데도 분노유발목록(PI)과 24%나 일치할 정도로 관객들이 흔히 겪는 일상의 분노사례를 적잖이 반영한 것으로 확인되었다. 이상과 같이 제작진이 이 영화에 심리학의 분노이론을 정교하게 반영하고 분노의 공적·사적 동인(動因)을 균형 있게 안배한 것은 분위기가 무겁고 낯설기 마련인 정치영화에 대한 관객들의 이해도와 흡인력을 높이려는 소통강화 의도에서 비롯된 것이라 평가된다.

서해 북방한계선에 관한 논쟁은 1953년 8월 유엔군사령관이 한반도 해역에서 남북간의 우발적 무력충돌을 예방한다는 목적으로 설정한 이후부터 지금까지도 첨예하게 대립하고 있는 문제이다. 2022년에는 분단이후 최초로 NLL이남으로 북한이 미사일 도발을 한 바 있다. 이처럼 북한의 실질적 도발이 진행중인 NLL에 대한 연구는 어떻게 진행되어 왔는지 확인해보고 나아갈 방향제시를 하는데 본 연구의 목적이 있다. 본 연구는 1998년부터 2023년까지 NLL에 관해 연구된 학술논문을 중심으로 RISS, 스콜라 등 총 5개의 학술정보 DB를 활용하여 연구의 경향을 살펴보았다. 연도별, 분야별, 연구 방법별 현황을 살펴본 결과 연도별로는 정부별 대북관계 기조에 따라 연구량의 유의미한 차이를 확인할 수 있었으며, 연구 분야는 NLL에 대한 개념소개와 역사적 배경 등이 가장 많아 힘의 논리에 따라 변화하는 국제 환경을 고려할 때 NLL의 국제법적 당위성과 명분을 구비하기 위해 더욱 다양한 분야로 확대될 필요성을 확인했다. 마지막 연구방법면에서는 문헌연구가 대부분이어서 면접, 설문, 빅데이터 등을 활용한 양적연구 등의 필요성도 알 수 있었다. 이러한 본 논문의 분석결과가 현재도 여전히 진행중인 국제정치환경의 이해관계 속에 향후 NLL의 국제적인 대응을 위한 연구방향설정에 긍정적인 역할을 하길 기대한다.

Leptin, a 16-kDa cytokine, is secreted by adipose tissue in response to the surplus of fat store. Thereby, the brain is informed about the body's energy status. In the hypothalamus, leptin triggers specific neuronal subpopulations (e.g., POMC and NPY neurons) and activates several intracellular signaling events, including the JAK/STAT, MAPK, PI3K, and mTOR pathway, which eventually translates into decreased food intake and increased energy expenditure. Leptin signal is inhibited by a feedback inhibitory pathway mediated by SOCS3. PTP1B involves another inhibitory pathway of leptin. Leptin potently promotes fat mass loss and body weight reduction in lean subjects. However, it is not widely used in the clinical field because of leptin resistance, which is a common feature of obesity characterized by hyperleptinemia and the failure of exogenous leptin administration to provide therapeutic benefit in rodents and humans. The potential mechanisms of leptin resistance include the following: 1) increases in circulating leptin-binding proteins, 2) reduced transport of leptin across the blood-brain barrier, 3) decreased leptin receptor-B (LRB), and/or 4) the provocation of processes that diminish cellular leptin signaling (inflammation, endoplasmic reticulum stress, feedback inhibition, etc.). Thus, interference of the cellular mechanisms that attenuate leptin signaling improves leptin action in cells and animal models, suggesting the potential utility of these processes as points of therapeutic intervention. Various experimental trials and compounds that improve leptin resistance are introduced in this paper.

Turner syndrome (TS) is a genetic disorder in phenotypic females that has characteristic physical features and presents as partial or complete absence of the second sex chromosome. Growth hormone deficiency (GHD) is a condition caused by insufficient release of growth hormone from the pituitary gland. The concomitant occurrence of TS and GHD is rare and has not yet been reported in Korea. Here we report 2 cases of TS and GHD. In case 1, GHD was initially diagnosed. Karyotyping was performed because of the presence of the typical phenotype and poor response to growth hormone therapy, which revealed 45,X/45,X+mar. The patient showed increased growth velocity after the growth hormone dose was increased. In case 2, a growth hormone provocation test and chromosomal analysis were performed simultaneously because of decreased growth velocity and the typical TS phenotype, which showed GHD and a mosaic karyotype of 45,X/46,XX. The patient showed spontaneous pubertal development. In female patients with short stature, it is important to perform a throughout physical examination and test for hormonal and chromosomal abnormalities because diagnostic accuracy is important for treatment and prognosis.

Epicondylitis, as a tendinopathy characterized by fibroblast and microvascular hyperplasia, is a common musculoskeletal problem especially related with repetitive hand and wrist motion. It has a prevalence of between 0.2% and 5% in general population depending on the amount of exposure to manual labor jobs. Although it is known that the pathological lesions lie in the flexor or extensor common tendons, there could be collateral ligament lesions and/or reactive synovitis accompanied, which may make a case unresponsive to the treatment aimed only at the tendinopathy. Epicondylitis is easy to diagnose with typical pain, tenderness, and positive provocation tests. However, many conditions can mimic epicondylitis that further imaging or electrodiagnostic studies should be undertaken to exclude other possible problems. Ultrasonography provides information about the existence and extent of tendinopathy with relatively high specificity. Magnetic resonance imaging is often required to rule out other problems and confirm the diagnosis of the cases intractable to long term treatment. Many options of treatment are available for epicondylitis while numerous conflicting evidences have been noted, debating one treatment method is better than the others. Since it was reported that over 80% of epicondylitis improved within a year no matter what was done as treatment, it is a challenge to make accurate diagnosis and combine effective therapeutic regimens for the 20% of intractable cases.

The change of society causes the change of art necessarily. The experiments of plastic art which as been done by avant-garde movements at early 20th century have pursued ″Object drama″ consistently as trials of more or less abstract drama. As these plastic arts have used genre or media like circuses, sports, variate, films, screen play, newspapers and a quick method which had not been considered suitable for conventional aesthetic plastic arts, these arts looked provocative and, these provocative plastic arts have ignored universality shown in conventional arts and have caused extreme ″sensation″ accordingly. As a result, abstract art based on style which is against aestheticism and naturalism has been generalized. This style does not deny customary laws of art wholly but it requires new concept. Having ignored traditional styles, a new compositive and responsive way of composition which does not deviate from tradition has been pursued. A new and general understanding of arts which do not deny all conventionally-effective standards extremely has been developed. In this regard, this study analyzes characteristics of Bauhaus Theater which has tried to apply new technology engineering to plastic art as an integrated concept of art and technology amongst the directions of development of provocation, innovation and revolution in case of the plastic art of stage painting, and analyzes how this influences on modern painting in terms of avant-garde arts reviewing the meaning of this theory.

As the types of North Korea's provocation are diverse and unexpectable in the costal area, ROK navy needs to develop countermeasures, such as costal defence guided rockets. Recently ROK navy developed the PKX-B which is equipped with the new 130 mm guided rocket. The most popular rockets are LOGIR for short range targets, 130 mm guided rocket for middle range targets and Spike-NLOS for long range targets. As various guided rockets are developed, it is required to develop a guided rocket analysis model and it's analysis methodology. In addition, these guided rockets can be installed on any platforms; ground vehicle, aircraft and warship. The paper proposes systematic methodology to estimate the operational effectiveness of costal defence guided rockets. A case study exploiting the ARENA simulation model is explained to demonstrate the implementation of the proposed methodology.
