경요도전립선전기기화술의 초기 임상경험 : 경요도전립선절제술 및 레이저 전립선절제술과의 비교 (Early Clinical Experience with Transurethral Electrovaporization of the Prostate for Benign Prostatic Hyperplasia: Comparison with Transurethral Resection of the Prostate and Visual Laser Ablation of the Prostate)

  • 최근에 Roller loop electrode를 이용한 경요도전립선전기기화술에 관심이 집중되고 있다. 저자들은 전립선비대증에서 전립선전기기화술의 가치를 알아보기 위해 Roller loop electrode를 이용한 전립선전기기화술(TEVP, 17례)을 레이저전립선절제술(VLAP, 39례)과 전통적 경요도전립선절제술(TURP, 59례)과 비교 분석해 보았다. TEVP는 효과 면에서는 전통적인 경요도전립선절제술에는 미치진 못하지만 VLAP과는 유사한 효과를 보였으며 안전성면과 경제적인 면에서는 VLAP을 능가하는 치료법으로 사료된다. 본 연구가 적은 환자를 대상으로 10개월까지 관찰한 결과이고 경요도전립선전기기화술의 초기 경험이라 명확한 결론을 내리기에는 다소 성급한 면이 있으나 전통적인 전립선절제술에 비해 짧은 입원기간, 짧은 요도관유치 기간 및 적은 출혈 등을 보여주며 레이저 시술의 단점인 고가장비가 필요 없다는 점에서는 전통적 전립선절제술의 대체 술기로 높이 평가되어진다.

땅콩나물 추출물이 남성갱년기 증상에 미치는 영향 (Effects of Peanut Sprout (Arachis hypogaea L.) Extract on Partial Androgen Deficiency in Aging Males (PADAM))

  • 본 연구는 땅콩나물 열수 추출물이 남성 갱년기 관련 증상인 전립선 비대증과 발기부전에 대해 어떠한 영향을 미치는지 알아보기 위하여 phosphodiesterase(PDE) 저해 효과, 5-alpha reductase 활성 억제능, RAW264.7 세포에서 NO 함량 측정 및 TM3 세포에서 testosterone 함량을 측정하였다. 땅콩나물 추출물은 높은 PDE 저해능을 확인되었다. 또한, 5AR2 활성을 억제하였다. 전립선 비대와 관련된 염증에 대한 영향으로 RAW264.7 세포에서 lipopolysaccharide에 대한 NO 함량 억제능을 확인한 결과 땅콩나물 열수 추출물에 의해 NO 생성이 억제되었고 염증 인자가 감소하는 것으로 나타났다. TM3 세포에서 과산화수소로 유도된 산화적 손상에서는 땅콩나물 추출물 처리 시에 높은 세포보호능을 보였으며, 땅콩나물 추출물에 의해 testosterone 함량도 증가하는 것을 확인할 수 있었다. 결과적으로 땅콩나물 열수추출물은 5AR2 억제 활성을 증대시키고 항염증 효과가 전립선 비대증의 유발을 억제시킬 수 있다는 선행연구로 볼 때 땅콩나물 추출물은 전립선 비대 유발에 직접 관계된 효소와 유전자를 제어함과 동시에 항염증 효능을 가짐으로써 전립선 비대 증상 개선에 매우 효과적일 것으로 판단된다. 또한, 땅콩나물 추출물은 PDE 활성을 저해함으로써 발기부전 증상을 완화시키고 고환 내 세포의 산화적 스트레스를 완화하며, testosterone과 같은 호르몬 합성을 증가시켜 전립선 비대증 외에 발기부전 및 호르몬 저하로 인해 발생하는 남성갱년기 증상에도 효과를 보일 것으로 기대된다. 이상의 결과로 볼 때 땅콩나물 열수 추출물은 전립선 비대를 비롯한 남성갱년기 증상을 완화시키는 기능성 소재로 활용할 수 있을 것으로 보인다.

Expression and Significance of Microsomal Prostaglandin Synthase-1 (mPGES-1) and Beclin-1 in the Development of Prostate Cancer

  • The aim of this study was to investigate the expression and significance of microsomal prostaglandin synthase-1 (mPGES-1) and Beclin-1 in the development of prostate cancer (PCa). Immunohistochemistry was performed on paraffin-embedded sections with rabbit polyclonal against mPGES-1 and Beclin-1 in 40 PCa, 40 benign prostatic hyperplasia (BPH) and 10 normal prostate specimens for this purpose. Quantitative real-time polymerase chain reaction (qRT-PCR) was applied for mRNA expression of mPGES-1 and Beclin-1, while MTT assays were used to ascertain the best working concentration of the mPGES-1 inhibitor (CAY10526). The effect of CAY10526 treatment on expression of Beclin-1 in DU-145 cells was studied using Western blot analysis. Localization of Beclin-1 and mPGES-1 was in endochylema. Significant differences in expression was noted among PCa, BPH and normal issues (P<0.05). Beclin-1 expression inversely correlated with mPGES-1 expression in PCa tissue (P<0.05). CAY10526 could significantly block mPGES-1 expression and the proliferation of DU-145 cells (P<0.05), while increasing Beclin-1 levels (P<0.05). Overexpression of mPGES-1 could decrease the autophagic PCa cell death. Inhibiting the expression of mPGES-1 may lead to DU-145 cell death and up-regulation of Beclin-1. The results suggest that inhibition of mPGES-1 may have therapeutic potential for PCa in the future.

Frequency of Unnecessarily Biopsies among Patients with Suspicion of Prostate Cancer in Syrian Men

  • Background: The prevalence of prostate cancer is considered high in many countries, and screening tests are very important in order to detect prostate cancer in its early stages; however false positivity with these screening tests means that a lot of patients undergo unnecessary biopsy, which is an invasive procedure, for the confirmatory test. The purpose of this study was to estimate the frequency of unnecessary biopsy cases in patients referred for prostate biopsy in one of the most important and overload cancer centers in Syria. Materials and Methods: Retrospective data for a period of four years between January 2009 and December 2012 were collected in Al-Bayrouni University Medical hospital in Damascus, Syria. The patients from whom data were collected were referred to our histopathological department because of elevated prostate specific antigen (PSA) serum or an abnormal digital rectal examination (DRE). All patients underwent prostatic TRUS-guided biopsies. Diagnosis of prostate cancer (PCa) or benign prostatic hyperplasia (BPH) was based on histopathological examination and prostate cancers cases were graded and scored according to the Gleason score system. Results: For the 406 patients referred to biopsy, the $mean{\pm}SD$ age was $58.4{\pm}23.3$ years. The $mean{\pm}SD$ PSA level was $49.2{\pm}21.5ng/ml$. Of the total we found 237 patients diagnosed with PCa (58. 4%), 166 patients with BPH (40.9%) and 3 cases were unable to be diagnosed (0.7%) because of biopsy collection errors. Conclusions: Our study shows that a high percentage of patients are undergoing unnecessary biopsy, which suggests that the performed screening tests had a high level of false positive and may need re-evaluation.

개에서 발생한 양성 전립선 비대증의 약물 치료 1례 (Medication for Benign Prostatic Hyperplasin in a Dog)

  • An 8-year-old, male Shih-tzu dog with clinical signs of hemorrhage from penile orifice and pain on lifting hindlimb was brought to the Veterinary Medicine Teaching Hospital, Chungnam National University. The patient was shown polyuria/polydipsia (PU/PD), more than 10 times per day, and gasterointestinal signs such as intermittent vomiting and melena. On laboratory examination, BUN, creatinine and amylase level were all highly increased. Radiographic and ultrasonographic findings included that both kidneys were abnormal, and both prostates were hyperplastic bilaterally and cysts were presented in parenchyma. Additionally, hyperechoic pancreas and intact urinary bladder were observed from ultrasonographic examination. The dog was diagnosed as pancreatitis, acute renal failure and benign prostatic hyperplasia (BPH). After the appropriate treatment for pancreatitis and acute renal failure, He dog was treated with administration of azasteroid for BPH. On day 71 after azasteroid treatment, good response to the treatment was defined by ultrasonography and clinical signs cured completely.

전립선 비대유도 쥐의 전립선 조직에서 울금 급여에 따른 염증인자의 변화 (Effect of Hot Water Extract from Curcuma longa L. on Inflammatory Mediators in Prostate Tissue of BPH-Induced Rats)

  • 본 연구에서는 테스토스테론으로 전립선 비대를 유도한 래트에 울금(Curcuma longa L.)의 열수추출물을 다양한 농도로 급여하여 그에 따른 염증인자의 변화를 검토하였다. 조직학적 분석에서 울금 열수추출물(CL) 급여에 따라 전립선비대가 완화되는 것을 검토할 수 있었으며, 염증성 사이토카인인 TNF-${\alpha}$, IL-6 및 IL-$1{\beta}$이 전립선 비대 유도군에서 그 발현이 현저히 증가하였으나, 각 농도의 CL 군에서는 농도의존적으로 감소하였다. 또한, CL 급여에 따라 finasteride enzyme인 COX-2와 염증 전사인자인 NF-${\kappa}b$의 활성화의 감소를 확인하였다. 이와 같은 결과는 전립선 비대에서 울금 열수추출물이 항염증 효과로써 전립선 비대의 유발 및 발전을 억제시킬 수 있다는 것을 부분적으로나마 설명할 수 있는 기초 자료가 될 수 있다고 사료된다.

Patient's Factors Correlated with Prostate Volume Recovery after 5 Alpha Reductase Inhibitor Discontinuation

  • Purpose: The 5 alpha reductase inhibitor (5ARI) reduces the size of the prostate and alleviates lower urinary tract symptoms. After stopping 5ARI, the prostate quickly recovers to its pre-medication size. The purpose of this study was to investigate the factors affecting the restoration of prostate size after 5ARI discontinuation. Materials and Methods: Between March 2009 and May 2017, patients who visited an outpatient clinic and were diagnosed with benign prostatic hyperplasia were selected and start 5ARI medication. After 6 months of medication, the patients stopped medication for 1 year. Meanwhile, we measured the prostate volumes of patients 3 times (before and after medication, after discontinuation) and divide the patients into 3 groups (maintained, intermediate, and restored) with recovered prostate volume ratio. After classification, we investigated the relationship between the variable factors (age, serum prostate-specific antigen, initial volume, reduced volume after medication) between groups. Results: Among the 147 selected patients, the mean age and plasma PSA level were $61.6{\pm}7.9$ and $0.8{\pm}0.6$, respectively. The mean initial prostate volume was $32.3{\pm}4.2ml$, which reduced to $23.2{\pm}3.2ml$ after medication. After one year of discontinuation, the mean volume was $31.4{\pm}6.4ml$, with restoration to 101.5% of the reduced size. We noticed a tendency that patients with faster prostate volume recovery were generally older than those with slower recovery; however, this was not statistically significant. Other factors showed no relationship with prostate recovery. Conclusions: When using 5ARI in elderly patients, continuous treatment seems better than intermittent treatment. If discontinuation is needed, short term follow-up is recommended.

전립선 기질세포의 증식과 COX-2 발현에 대한 프로게스테론의 영향 (Effect of Progesterone on COX-2 Expression and Proliferation of Prostate Stromal Cell)

  • 전립선비대증은 노인 남성에서 흔히 유발되는 질환이며, 노화가 진행될 수록 빈도가 높아지는 특징을 가진다. 이 질환의 원인은 전립선 기질세포의 과도한 증식으로 유발된다고 알려져 있지만 그 자세한 기전에 대해서는 잘 알려져 있지 않다. 전립선비대증에서 progesterone 수용체 양성 세포가 다른 전립선 종양에 비해서 많고, progesterone은 testosterone에서 DHT로 전환되는 것을 감소시키는 역할을 가진다고 알려졌다. 또한 남성 전립선 평활근의 과증식에 의한 질환이므로 평활근 세포의 증식과 관련성이 있다고 보고된 COX-2의 전립선비대증에 대한 영향에 대한 연구가 필요하다. 전립선 기질세포에 progesterone을 3일간 투여하여 배양한 경우 기질세포 증식은 차이가 없었다. Progesterone을 단독 또는 DHT와 같이 투여한 기질세포에서 남성호르몬 수용체 mRNA 발현은 비처리군과 비교하여 유의한 차이가 없었다. 또한 progesterone과 DHT 동시 투여에 의한 COX-2 mRNA 발현에도 차이가 없었다. 그러나 progesterone에 의한 남성호르몬 수용체와 COX-2 단백 발현에서는 대조군과 비교하여 유의하게 감소시켰다. 이상의 결과는 progesterone은 남성호르몬 수용체에 대해 전사 후 반응 (post-transcriptional response)에 효과를 나타내어 남성호르몬 수용체 발현을 감소시키는 작용은 가지며, COX-2 발현 억제효과를 나타내므로 전립선비대증의 치료에 이용될 수 있을 것으로 사료된다.

하부요로 폐색진단을 위한 비침습적 방광내압 계측 시스템의 구현 및 평가 (Implementation and estimation of the noninvasive vesical pressure measurement system for diagnosis of lower urinary tract symptom)

  • Lower urinary tract symptoms (LUTS) affect self-assessed quality of life. The prevalence of LUTS is commonly high in the elderly. The purpose of urodynamic investigation is to obtain information on the function of the urinary system. One of the most frequently used measurement procedures in urodynamics is filling and voiding cystometry. But in this system transurethral catheter is used and it makes patients uncomfortable. The aim of this study is to implement the system that could evaluate noninvasively the function of urinary tract. A new system has been developed to analyze urine flow rate and vesical pressure during voiding. These signals were recorded simultaneously and transmitted to a PC. For system evaluation, a model for the lower urinary system of men was constructed. From the evaluation of the model, vesical pressure was correlated with the occlusion degree. In a pilot study with five male subjects, maximum of standard deviation was 1.26, error rate was 3.49 and coefficient of variation was 3.48.

삼점 신호 평균기법에 의한 요속신호의 잡음 축소 기법 (Noise Reduction Technique by Three-Points Ensemble Averaging in Uroflowmetry)

  • Uroflowmetry is a convenient clinical test to screen the benign prostatic hyperplasia(BPH) common in the aged men. A load cell is located beneath the urine container to measure the weight of urine. However, it is sensitive to the impact applied on the bottom of the container by the urine stream, which could be a noise source lowering the reliability of the system. With this aim, our study proposed a noise reduction technique by computing ensemble average of the weighted signals that were acquired from three-load cells forming a regular triangle beneath the urine container. Simulated urination experiment was performed with three different collection methods, all of which demonstrated significant noise reduction by ensemble averaging. Furthermore, the best results can be obtained without any special urine collection devices. Thus, our novel method can be usefully applied to uroflowmetry for enhancing measurement in terms of accuracy and reliability.
