Purpose: Parents with a premature infant are usually confronted with difficulties in caring for their babies and need education on appropriate care. The purpose of this paper was to describe the components of caring education for parents with a premature infant based on an integrative literature review and to present strategies for future education. Methods: Four electronic databases were searched to identify studies. Sixteen articles were selected through full text screening of related research published in domestic journals from January 1990 to October 2012. Results: Through in-depth discussion and investigation of the relevant literature, three components emerged; (a) nurses as educators and parents as learners, (b) education content including caring on admission or discharge, and effects of education, and (c) education environment including timing, place and materials. Conclusion: Future education on caring for premature infant should include the nurse as educator and have diverse content including parent's role, interaction with healthcare providers, and special care for premature infants with special problems. Moreover, highly technical teaching materials should be provided, along with continuous support of institution and government to individual caregivers. Results of this study provide comprehensive prospective and basic standards for education on premature infant care.
Objectives : This study was performed using a longitudinal approach to explore the predictors for current smoking among male high school students. Methods : Baseline data was collected in May 2004 through a self-administrated questionnaire completed by 607 male students in a technical high school in Daegu city, Korea. Subsequently, their smoking behaviors were followed one year after. Among the 544 followed participants, data for 439 non-smokers in the first year was used in longitudinal analysis. Current smokers were defined as those respondents who had smoked one or more cigarettes within the 30 days preceding the survey. Several potential predictors for smoking were investigated including smoking history(never, experimental, former smoker), sociodemographic factors, environmental factors, attitudes toward smoking, and behavioral factors. Logistic regression was used to predict smoking with SPSS ver. 12.0. Results : According to multiple logistic regression analysis, those students who were more likely to smoke after one year were former smokers(OR: 2.12, 95% CI=1.01-4.44), current drinkers(OR: 2.55, 95% CI=1.33-4.89), who had four or five smokers among five best friends(OR: 3.43. 95% CI=1.14-10.30). In addition, those who had smokers among family members besides parents or siblings(OR: 1.66, 95% CI=0.92-2.98), exhibited a high level of subjective stress(OR: 1.77, 95% CI=0.96-3.26), or had a very good relationship with friends(OR: 1.93, 95% CI=0.99-3.75) were also more likely to smoke albeit with marginal statistical significance(p<0.1). Conclusions : A smoking prevention program aimed at high school students may be more effective with due consideration of the predictors highlighted in this study. However, further studies with larger sample size and various target populations are necessary to find potential predictors not found in this study but suggested in other longitudinal studies.
Background: The clinical significance of lip-tie, or a tethered maxillary frenulum, remains under debate. Clinicians and parents are often perplexed when deciding whether procedures available to relieve a seemingly tight or severe maxillary frenulum are needed. Purpose: No previous studies have assessed the consequences of not subjecting a tethered maxillary frenulum in newborns to surgical intervention. This study aimed to contribute the first prospective trial on this topic with a relatively extended follow-up of these newborn infants. Methods: This prospective observational questionnaire-based cohort trial was performed in a community setting and aimed to determine whether lip-tie is associated with an increased likelihood of eventual feeding or oral disorders. Results: The convenience sample comprised of 61 consecutively arriving infants with concomitant tethered frenula who were treated at the clinic for various reasons. This cohort was compared with a random sample of 66 age-matched children for a mean follow-up period of 6.42 years. Infants undergoing oropharyngeal procedures were excluded. Awareness of a deviation in oral structures was reported by 18% of the study group versus 0% of the controls. Mothers participating in the study group (24.6%) less frequently recalled painful nipples or discomfort during breastfeeding than those in the control group (47.0%) (P<0.01). There were no intergroup differences in other types of feeding difficulty, dental hygiene, pronunciation, or speech development. Conclusion: Our findings suggest that a tethered labial frenulum is not associated with an increase in breastfeeding disturbances or oral disorders. These data encourage clinicians to question the need to intervene in cases of tethered maxillary frenula.
Objective: The aim of this study was to investigate enuretic children attending oriental hospital and classify subtypes and evaluate possible factors that enhance or hamper of continence. Methods: Children attending kyeongju dongguk oriental hospital pediatrics between January 2000 and December 2003 with chief complaint of nocturnal enuresis were enrolled. The data of present symptoms and improvement progress was collected via telephone interviews with their parents. Results: The total number of children was 61. 32 of them were male and 29 female. Sex ratio was 1.1:1. Patient under 6 years of age was 68.8%. Using acupuncture, acupressure or chimsband with herb medicine, treatment frequency was increased. Patients with nocturnal enuresis(NE) was 48, and primary nocturnal enuresis(PNE) 39, secondary (SNE) 9. 13 was impossible to diagnose. The ratio of male to female was 1:1.09. PNE was 4 times as many as SNE. Male was predominent in PNE, and female in SNE. In the progress of PNE, male and female were similiar. but SNE, male was all cured, female remained 50%. And elapsed time of male to improve was longer than that of female. In the progress of improvement, many parents mentioned that their child was improved spontaneously regardless of subtype. Conclusion: We consider age, sex distinction, subtype, bladder symptoms, frequency, time of progress as influencing factor of prognosis, but failed to verify significance. To develop guideline of NE and find influencing factor of prognosis, more prospective study through taking history carefully and using questionnaire is needed.
Purpose: This pilot study aimed to describe children's social contact patterns and to analyze factors related to their social contacts. Methods: The participants were 30 children aged ≥13 months to <7 years, whose teachers at childcare centers and parents at home were asked to maintain diaries of their social contacts prospectively for 24 hours. Data were collected from November 30, 2018, to January 7, 2019. Results: The 30 participating children were in contact with 363 persons in a 24-hours period (mean, 12.1±9.1). The number of contacts showed significant relationships with day of the week (p<.001), number of family members/cohabitants (p=.015), area of residence (p=.003), and type of housing (p=.002). A multiple regression model showed significantly higher numbers of contacts on weekdays (B=10.64, p=.010). Physical versus non-physical types of contact showed significant differences in terms of duration, location, and frequency (p<.001). The duration of contacts showed significant relationships with their location and frequency (p<.001), while the frequency of contacts was significantly related to their location (p<.001). Conclusion: This is the first survey describing the characteristics of Korean preschool children's social contacts. Further large-scale social contact studies of children should be conducted.
Purpose: High risk women with congenital heart disease decide to get pregnancy is determined by not individual autonomous intention but complex interaction with their physical status and socio-psychological environments. This study tried to the answer to the question. : "What is experience high risk women who have congenital heart disease during transition to parenthood?". Method: A micro-ethnographic research method and oral historic research approach were done at the Grown-Up Congenital Heart Disease Clinic in one Korean metropolitan city from July 2002 to September 2003. Result: It was discovered that high risk women's experience of transitional parenthood is accounted as the process of lonely and fearful self-accomplishment. Their need for self-accomplishment creates them seek more opportunities to increase enduring abilities for their parenthood. Conclusion: We suggest that from the time of beginning of patient's making decisions about becoming pregnant, collaborative efforts must be considered that priority level of patient's needs be reviewed and find appropriate advices for their situation. Special counseling program should be provided to all the prospective parents with understanding their meaning of parenthood.
Objective: This study aimed to examine the prospective links among perspective taking, attachment security, and executive function and the mediating role of executive function on the relationship between attachment security and perspective taking. Methods: The participants included 147 preschoolers living in Gyeonggi. The data-analysis methods included descriptive statistics, independent t-tests, one-way ANOVAs, Pearson's correlation analysis, hierarchical multiple regression analysis, and the Sobel test. Results: First, preschoolers' perspective taking was significantly related to attachment security, and executive function. Furthermore, there was a significant association between attachment security and executive function. As expected, the correlations were positive in all cases. Second, the effects of attachment security on intention and emotion perspective takings were fully mediated by executive function when holding children's age and vocabulary constant. However, executive function did not predict thought perspective taking, whereas attachment security did. Conclusion: Given that parent-child attachment security might play an important role in child cognitive and socio-cognitive development, parents' endeavors to enhance the quality of the attachment relationship could be a fruitful path for preschoolers' developing executive capacities and perspective taking.
The purpose of this study is to suggest a response and solution through Christian educational care to the crisis and change of the era of low birth rate faced by Korean society and the Korean church. This study proposes to find an alternative to the biblical aspect of pregnancy, childbirth, and parenthood as God's blessing for the demographic cliff and low birth rate problem that have become a reality in Korean society and churches. Being a parent in an age of low birth rate is very difficult, but on the other hand, it gives happiness and joy. Being a parent is a blessing from God, and is the most important and valuable thing in life. However, modern society emphasizes the right and necessity to choose one's own parenthood status. In the nuclear family, the decrease in the number of children, and the development of child research, parents feel more responsibility and economic burden for raising children than ever before. Therefore, it is a reality that the number of people who delay becoming parents or voluntarily do not have children is gradually increasing. To improve the perception of becoming a parent due to a decrease in responsibility for raising children, it is necessary to shed light on marriage, pregnancy, childbirth, and childrearing from a Christian educational point of view. In addition, it is necessary to understand the recognition of being a parent and the characteristics of childbirth and rearing, and to analyze past and present value changes. This study will also discuss the causes of low birthrate and try to provide Christian educational care for childcare including solving the low birthrate problem.
Purpose: During the recent decades, the age at menarche continued to decline in Korea and worldwide. Prior studies have suggested that early menarche may increase the risk of various social, psychological, and physical health problems in young adolescent girls, but little is known about the determinants associated with early menarche. The purpose of this study is to evaluate independent determinants of early menarche among 5th~6th female graders in South Korea. Methods: Our analysis was conducted in 95 menarcheal girls and 95 age-matched pre-menarcheal girls residing in Daegu, South Korea. Demographic and lifestyle characteristics were collected using survey questionnaires for children and parents. Dietary information was assessed by 2 day~24 hour food records and survey questionnaires, which were completed by both children and their parents. Anthropometric data were obtained from the student health check-ups at the school. Results: A multiple logistic regression analysis using a conditional likelihood method was performed for simultaneous evaluation of several risk factors. There were significant differences in that higher proportion of obesity (OR, odds ratio = 5.60, 95% CI, confidence interval = 1.34~23.42), shorter sleep duration (OR = 0.45, 95% CI = 0.23~0.87), and younger mother's age at menarche (OR = 0.64, 95% CI = 0.44~0.93) were observed in the menarcheal group compared to the pre-menarcheal group. Conclusion: These findings indicate a possible association of sleep duration, mother's menarcheal age, and obesity with age at menarche. A well-planned, prospective cohort study is warranted to examine causal relationship.
Purpose: Childhood obesity is an increasing public health issue worldwide. We examined dietary patterns among adolescents in a dormitory school, identified obese adolescents and tried to intervene to improve food habits and physical activity. Methods: We conducted an experimental prospective longitudinal study based on 36 obese (body mass index $[BMI]{\geq}95th$ percentile) adolescents (aged 12-18 years) compared with controls (healthy children: normal age-appropriate BMI ($BMI{\leq}85th$ percentile). Six months' intervention included lifestyle-modification counseling (once a week by a clinical dietician), and an exercise regimen twice a week, 60 minutes each time, instructed by a professional pediatric trainer). Both groups underwent baseline measurements at the beginning of the study and 6 months later (arterial stiffness, blood pressure, pulse, weight and height, hemoglobin, creatinine, liver enzymes, highly sensitive C-reactive protein and complete lipid profile). Results: Twenty-one participants completed the study. Low compliance from participants, school staff and parents was observed (participation in planned meetings; 71%-83%). BMI significantly decreased from $32.46{\pm}3.93kg/m^2$ to $30.32{\pm}3.4kg/m^2$ (P=0.002) in the study group. Arterial stiffness was not significantly different between the 2 groups and did not change significantly after 6 months' intervention (P=0.494). No significant changes in CRP and lipid profile were observed after the intervention. Conclusion: Making lifestyle modifications among adolescents in a dormitory school is a complex task. Active intervention indeed ameliorates BMI parameters. However, in order to maximize the beneficial effects, a multidisciplinary well-trained team is needed, with emphasis on integrating parents and the school environment.
