An Evaluation of Accountable Care Organization in USA and Policy Implications for Korean Health Care System (미국의 책임의료조직(Accountable Care Organization) 운영현황 분석과 국내 의료정책에서 정책적 함의 평가)

  • Background: As a reform plan of health care system, Accountable Care Organization (ACO) has became an object of attention in the United States after Patient Protection and Affordable Care Act was enacted. ACO is a group of various health care providers and provide coordinated care to its assigned beneficiaries. If ACOs improve the quality level and reduce the cost of care, they can get financial incentives. Under the discussion for a quite long time and demonstration projects, ACO has been established. We aimed to analysis and discuss the history, policy mechanism, contents, status and outcomes of ACO. Also, we intended to suggest political implication Korean health care system with regard to ACO. Methods: We searched the articles related ACO in PubMed and selected several available papers about ACO. Total 56 studies were reviewed and categorized three parts; demonstration projects for formation of ACO, policy mechanism and agenda, empirical results of ACO performance. Results: As a result, establishment of ACO was successful partly in the US. It seems to be due to various project and pilot test for verification in the long time. The empirical effect of ACO was also identified in a few study but it needs more evidences to judge its positive effect. Conclusion: In Korea, there are arguments for the application of ACO. However it is difficult to implement a ACO by different political conditions between Korean and US. Nevertheless ACO proposed us the necessity of paradigm shift in our health policy and could be significant to national policy orientation in the future.

  • The purpose of this research is to develop a standard model for the compilation of production results in molding business to establish the informatization system for the production facility among informatizadon projects which can generate the performance of medium and short term introduction of the project in implementing the system to small and medium industry. The theory on the development method for the standardization of informatization for production facility in molding business applies PSDM (Production System Development Method) for which a number of researches were already done while developing a standardization model by standardizing two processes of raw material demand/supply management and making sum total for production quantity which are main processes for production management process. On the basis of the result of this research, small and medium companies in molding business and relevant specialized IT companies which desire to establish production facility informatization systems will be able to establish more efficient system by applying standardized model, and the result of research will enable to facilitate the establishment of the system for them while providing reliability of the system. Through the application of the result of this research, it will be possible to accomplish the elimination of unreasonable factors in production process, the enhancement of product quality and the saving of production cost.

  • This study investigates the risks that can occur during the development stage of IPP project in developing countries. In case that ECA and MLA cannot participate due to poor credit rating of the country, the diversification of power purchaser in marginal states can be a great help to reduce both market risk and country risk at the same time. In case of thermal power plants and combined cycle power plants, the effect of performance degradation as time passed will be considered and expected profit of sponsors should be maintained. Recently, developing countries are expanding IPP projects to reduce the financing cost and Korean power companies are positively participating in IPP projects. Accordingly, the loss of Korean companies should be minimized by risk management through the risk mitigation methods of this study.

  • This study analyzed the economic analysis on the Cambodian inland waterway from Phnom Penh to Chong Kneas. The social discount rate of 3.5% was applied for the cost and benefit of projects and converted to the current values in 2009. The benefits were supposed as the triangle distribution with minimum, mode, and maximum value corresponding to pessimistic, moderate and optimistic prospect separately. And the distributions of costs were the normal. As the result of probabilistic simulations, the average of B/C for scenario A showed relatively the highest with 0.25 and its 90% confidence interval 0.16~0.35. The average B/C of scenario B is 0.10 with the 90% confidence interval 0.06~0.13 and the one of scenario C is 0.15 with 90% confidence interval 0.12~0.19. Therefore it was concluded as low economic feasibility to install inland waterway aids to navigation along the surveyed waterway. However, the performance of the project should be determined by its political analysis as well as the economic.

  • According to revision of Construction Technology Management Act (2005), the government ordering construction which cost is more than 10billion won should investigate the value engineering(VE) of a construction. In this sense, the government actively promote plans in 2008 after identifying the examining standards of the investigation on the VE(2007). It is substantial issue to introduce Pre-Qualification(PQ) in oder to restrain unlicensed VE consulting companies or VE consultants. This system will contribute to the quality and the settlement of the value engineering in our country. The reason why the Korea VE Society founded in 2006 is one of the afore-mentioned reasons. Therefore, we try to establish the value engineering of Build-Transfer-Lease Scheme by providing the practical application of construction value engineering and definite procedures through the example of Andong Culture and Art Center's research in one of the various construction fields.

  • A series of images whose sized and resolutions differ by a constant factor are called an image pyramid. Because the images at high levels are small, large object can be detected on high levels of the pyramid at low cost, But in this way, the boundaries of objects are not accurately localized. Therefore the pyramid algorithms extracte the objects by segmentation the constructed image using bottom-up method and description it in an original resolution using inverse bottom-up method. In this paper, we can project an object down to the next lower level of the pyramid and apply to the boundary adjustment algorithm at that level to localize it more precisely. We repeat the process at successively lower levels. In this paper, we present a method of boundary adjustment using an image pyramid to obtain optimal boundary. The performance of the proposed algorithm is compared to those of the conventional method in term of subjective quality of object boundary.

  • This study have been carried out as a part of R&D project to develop a low cost high efficiency combined grain drying & storage system. The design of sweeping auger must meet the various conditions : capacity and dimensions of silo, discharging capacity of auger, operation conditions of auger such as revolution and rotation and density of grains and even the first-in & first-out of grains in the system. Through the experimental observations with the apparatus which enables the direct observation of discharging behavior of grains, the limitations of the performance of existing straight type auger was observed. Generalized mathematical model for the profile of new variable section sweeping auger was obtained, which ensures the uniform descending of grains and also meets various operating conditions. The experimental results with a prototype sweeping auger showed that the mathematical model for the variable section sweeping auger was quitely correct.

  • Since human genome project finished, the cost for human genome analysis has decreased very rapidly. This results in the sharp increase of human genome data to be analyzed. As the need for fast analysis of very large bio data such as human genome increases, non IT researchers such as biologists should be able to execute fast and effectively many kinds of bio applications, which have a variety of characteristics, under HPC environment. To accomplish this purpose, a biologist need to define a sequence of bio applications as workflow easily because generally bio applications should be combined and executed in some order. This bio workflow should be executed in the form of distributed and parallel computing by allocating computing resources efficiently under HPC cluster system. Through this kind of job, we can expect better performance and fast response time of very large bio data analysis. This paper proposes a workflow-based data analysis system specialized for bio applications. Using this system, non-IT scientists and researchers can analyze very large bio data easily under HPC environment.

  • For FA (factory automation) and ATE (automatic test equipment) in the industrial area, the standard bus needs to increase the system performance of multiprocessor environment. VME(versa module european package format) bus is appropriated to the standard bus but has features of small package and low board density. Beside, the density of board and semiconductor have grown to become significant issues that affect development time, project cost and field diagnostics. To fit this trend, in this paper, we composed Revision C.1 (IEEE std. P1014-1987) of the integrated environment for the main function such as arbitration, interrupt and interface between, VMEbus and several control modules Also the designed, VME system controller is implemented on FPGA that can be located even into slot 1. The control and function modules are coded with VHDL mid-fixed description method and then those operations are verified by simulation. As a result of experiment, we confirmed the most important that is the operation of Bus timer about Bus error signal should occur within $56{\mu}m$, and both control and function modules have the reciprocal operation correctly. Thus, the constructed VHDL library will be able to apply the system based VMEbus and ASIC design.

  • The company invests its time and money for temporary maintenance to satisfy the fast change of the computer use environment and the user's demands. Therefore, various problems occur including low performance because of duplication of codes and unstable structures from the restructuring and redevelopment. Furthermore, if a developer, who did not participate in initial process of development, wrote additional program codes to upgrade or restructure, it would cause many problems such as lack or lose of development documentation, understanding of documentation and reuse of existing program language. This study, Object-oriented Refactoring Process, suggests that the developers can reuse object unit to overcome the limit of the reusing code. In addition, this paper shows case study to verify our process by adjusting the project called "D2D", which is a case tool for developing windows system from Company D. Our works get positive results such as improvement of system pelfonnance, decreased cost of development and maintenance and optimizing structure and class.
