Impact of the Outpatient Prescription Incentive Program on Reduction of Pharmaceutical Costs of Clinics in South Korea

  • Background: South Korea has experienced problems with excessive pharmaceutical expenditures. In 2010, the South Korean government introduced an outpatient prescription incentive program to effectively manage pharmaceutical expenditures. Therefore, we examined the relationship between the outpatient prescription incentive program and pharmaceutical expenditures. Methods: We used data from the Korean National Health Insurance claims database, which included medical claims filed for 22,732 clinics from 2011-2014 to evaluate associated pharmaceutical expenditures. We performed multiple regression analysis and Poisson regression analysis using generalized estimating equation models to examine the associations between outpatient prescription incentives and the outcome variables. Results: The data used in this study consisted of 123,392 cases from 22,372 clinics (average 5.4 periods follow-up). Clinics that had received outpatient prescription incentives in the last period had better cost saving and Outpatient Prescribing Costliness Index (OPCI) (received: proportion of cost saving, ${\beta}=6.8179$; p-value < 0.0001; OPCI, ${\beta}=-0.0227$; p-value < 0.0001; reference = non-received). Moreover, these clinics had higher risk in the provision of outpatient prescription incentive (relative risk, 2.772; 95% confidence interval, 2.720 to 2.824). The associations were higher in clinics that had separate prescribing and dispensing programs, or had professional staff. Conclusion: The introduction of an outpatient prescription incentive program for clinics effectively managed problems with rapid increases of pharmaceutical expenditures in South Korea. However, the pharmaceutical expenditures still increased in spite of the positive impact of the outpatient prescription incentive program. Therefore, healthcare professionals and health policy makers should develop more effective alternatives (i.e., for clinics without separate prescribing and dispensing programs) based on our results.

학교급식 조리종사자의 직무 만족도에 영향을 미치는 직무 소진, 직무 관여 및 조리작업안전에 대한 인식 (The Factors (Job Burnout, Job Engagement, the Workplace Safety) Influencing Employees' Job Satisfaction in School Food Service Operations)

  • The study was conducted to investigate the influences of food service employees' job burnout, job engagement, and workplace safety perception on their job satisfaction. A survey was administered on 589 school food service employees in Gyeongbuk from December 5, 2005 to February 18, 2006. The final response rate was 71% (N= 416); the data were analyzed using SPSS Windows (ver. 12.0). A majority of the respondents were females (99.0%) and 40 years old or older (75.3%); 84% were temporary contact-based or irregular workers. Among the job burnout dimensions, exhaustion ($2.98{\pm}0.59$) was rated higher than cynicism ($2.33{\pm}0.54$), while professional efficacy level ($3.61{\pm}0.47$) was relatively high. In terms of job engagement, the means of absorption ($3.67{\pm}0.49$) and dedication ($3.65{\pm}0.52$) were higher than that of vigor ($3.22{\pm}0.46$). A systematic environmental level ($3.15{\pm}0.58$) was lower than safety knowledge level ($3.63{\pm}0.49$) among the workplace safety statistics, whereas the anxiety level ($3.25{\pm}0.74$) was relatively high. The food service staff were more satisfied with 'supervisors' and 'co-workers' than with 'pay' and 'promotion'. A hierarchial regression analysis revealed that dedication, absorption, systematic environment, and safety knowledge were significant factors to increase job satisfaction and exhaustion which significantly decreased their job satisfaction.

병원 간호단위관리자의 다면평가($360^{\circ}$ feedback)에 관한 연구 (A Study on $360^{\circ}$ Feedback of Nursing Unit Manager in a Hospital)

  • Purpose: The purpose of this study was to develop 360 feedback for nursing unit manager based on the current personnel evaluation system and to evaluate the new tool according to evaluation subjects. Total of 277 subjects of nurse unit managers and staff nurses were participated in this study. Method: The study was conducted in three phases each for development, application, and analysis of 360 feedback. SAS program was utilized for data analysis with descriptive statistics, t-test, and analysis of variance. Result: The evaluation criteria of the developed 360 feedback tool consisted of 13 subscales such as professional knowledge, apprehension & judgement, job performance, applicability, creativity, leadership, responsibility, promptness & accuracy, administrative ability & sense of mission, activeness, cooperation, communicability, and general attitude. The internal consistency of the tool was Cronbach's alpha .939. The evaluation score by! peers(M=4.30) was the highest one, followed by self-evaluation(M=4.23), evaluation by supervisor(M=4.17), and evaluation by subordinate(M=4.10). The differences in the total evaluation scores among the subjects supervisor, self, peer, and subordinate were not statistically significant, but significant differences were found in some subscales scores. Conclusion: Further research is required to test the reliability and validity of the $360^{\circ}$ feedback tool, and to test the outcome and the process of $360^{\circ}$ feedback system.

국내 중소 과학관 현황 분석: 과학관의 조직, 운영, 교육을 중심으로 (A Study on the Current Status of Domestic Science Museum: Focus on Organization, Operation, and Education)

  • 이 연구의 목적은 지방 과학관의 조직, 운영, 교육 등의 실태를 분석하는 것이다. 이를 위하여 과학관 분석을 위한 평가틀을 개발하고, 28개의 과학관을 선정하여 관계자와의 심층면담을 실시하였다. 주요 연구결과는 다음과 같다. 첫째, 과학관의 전문 인력이 부족하여 원활한 과학관 운영이 어려웠다. 둘째, 과학관의 재정상태가 매우 좋지 않았다. 셋째, 과학관의 시설이나 전시물 관리에서 부족한 측면이 다수 발견되었다. 넷째, 과학관의 관람객을 이끌 수 있는 다양한 홍보나 마케팅이 소극적으로 이루어지고 있었다. 다섯째, 과학관의 연구 및 교육프로그램에 미흡한 부분이 많이 발견되었다. 실태조사 결과를 바탕으로 지방과학관의 발전 방안을 위한 방안을 논의하였다.

영유아 교육·보육 통합시설의 필요 공간과 면적기준에 관한 연구 (A Study on the Necessary Spaces and Area Standards of Integration Facility for Early Childhood Education and Care)

  • 본 연구는 '유치원 어린이집 간 격차 완화'를 위한 하나의 방안으로 '교육시설 질 균등화'를 위해 우선적으로 영유아 교육 보육 통합시설이 갖추어야 할 필요공간과 면적기준(안)을 제시하는 데 목적을 두었다. 그 결과 첫째, 영유아 교육 보육 통합시설에 필요한 공간의 종류를 표15와 같이 반드시 필요한 필수공간과 교육 보육 활동의 질적 수준을 높이기 위해 추가되면 좋을 권장공간으로 구분하여 제시하였다. 둘째, 영유아 교육 보육 통합시설의 필요 공간별 면적기준(안)을 표23과 같이 제시하였다. 아울러 영유아 교육 보육 통합시설에 필요한 영유아 1인당 대지면적과 연면적, 연면적 대비 공용면적의 비율을 각각 제시하였다.

전기로 인한 화재 원인 통계와 고찰 (A statistics and study of fire cause by electrics)

  • 급격한 경제발전과 더불어 전기로 인한 화재가 발생하면 막대한 인명과 재산피해가 발생하여 국가적으로 크나큰 손실이 아닐 수 없다. 이러한 전기화재를 예방하기 위해서는 원인과 장소별 등으로 분류된 세부적인 통계를 생산하고 정확한 화재원인을 규명하여 화재예방 자료로 활용하는 것이 가장 근본적인 대책이라 할 수 있다. 이 논문에서는 최근 10년간 화재발생 현황 및 2006년 전기화재 원인별, 장소별, 월별로 분석하였고 또한, 2005년 전기화재 8,554건에 대하여 전국의 소방서를 우리공사 직원이 직접 방문하여 심층적이고 전문적인 전기화재 실태조사를 실시하였다. 따라서 이번 논문이 전기화재를 예방하는 자료로 활용되고 정확한 화재통계 생산에 디딤돌이 되었으면 하는 바람으로 제시하였다.

Sustainable Park Management with Citizen Participation of the Awaji Island Regional Park

  • Many efforts have been made to improve the management of large-scale green spaces. How to manage large-scale green spaces and their active uses, as well as how to build relationships with local communities have been important issues. For this research, I reviewed the actual status of management, use and citizen participation at large-scale regional parks in Hyogo Prefecture. In addition, I studied the sustainable management through citizen participation of the Awaji Island Regional Park, where I have been involved for several years. I conducted various projects related to the use and management of the park, and examined the direction of citizen participation by conducting questionnaires and interviews. (1) Through interviews about the park, I collected opinions, including good points, problems, and potential solutions through physical and programming measures. (2) I examined what kinds of activities should be conducted in the park in order to revitalize park use and stimulate the surrounding communities. (3) I examined the current status of citizen participation while citizens carried out activities of their own planning. (4) I studied what is necessary to sustain park events and other activities. As a result, I came to the following conclusions. (1) Provision of information that is easy to access, including signs in the park, explanation of routes in large parks, and other techniques that help people become familiar with park facilities, is very important. (2) Local community events, and programs that draw out the willingness and capabilities of volunteers are effective. (3) Several different types of participation exist, including volunteers, guests, staff who work continuously for the project, coordinators, and professional specialists. (4) To sustain citizen involvement in the use and management of large-scale parks, a system that includes coordinators should be developed.

제약회사 영업사원의 면담 품질(Call Quality) 향상을 위한 퍼실리테이션(Facilitation) 교육법의 효과 (The Effectiveness of Facilitation Education for Call Quality of Medical Representative in Pharmaceutical Industry)

  • Objectives: The purpose of this study is to figure out that Facilitation education can affect significant improvement in sales staff's understanding of precise pharmacokinetics, benefits, safety, and side effects, etc. and eventually lead to increase in call after taking courses. Methods: Data utilized in this study was collected from 413 sales staffs who completed Facilitation course for 5 months. This study used statistical methodologies, paired t-test, exploratory factor analysis, and logistic regression model in order to identify change in Call and after Facilitation courses. Results: The result shows that there are statistically significant increases in CALL quality after Facilitation courses based on the result of pared t-test. Moreover, Facilitation education is more effective in average time of one-time detail than average number of visits per day and average number of doctors per day from the result of logistic regression. Conclusions: In order for MR in pharmaceutical company to improve CALL quality, the education for precise pharmacokinetics, benefits, safety, and side effects is necessary. In addition, various professional training required for detail, including disease education, Selling Skills education, and literacy education are essential. Therefore, Facilitation education would be desirable choice in terms of pharmaceutical marketing strategy.

대한수혈학회 창립 30주년을 기념하며 (History of the Korean Society of Blood Transfusion and Blood Services in Korea)

  • The Korean Society of Blood Transfusion (KSBT) was established in Seoul in 1982 by a group of physicians of clinical pathology (currently laboratory medicine), internal medicine, paediatrics, anaesthesiology, and surgery, who were members of the Korean Society of Hematology, in which they had been actively engaged as transfusion specialists. However, these physicians were dissatisfied with the lack of public recognition of blood transfusion and the unrealistic blood service system and therefore attempted to build an influential organization for improvement of blood services and transfusion medicine. The mission of the KSBT is to advance the science and practice of transfusion medicine. The KSBT advocates for education and research in the fields of transfusion medicine and for safe blood in blood services. The inaugural KSBT conference, with approximately 50 participants, was held in November, 1982; since then, annual conferences and educational workshops have been held regularly once or twice a year. Currently, the KSBT has approximately 800 members, who are practitioners of blood transfusion (holders of M.D.) or staff members of blood services (holders of other professional degrees). The Society also publishes the Korean Journal of Blood Transfusion (first issue published in 1990) and guidelines for blood services. A Society web site (http://www.transfusion.or.kr) was started in 2001, with the hope of increasingly providing information and feedback to members on a real-time basis. The KSBT has been an active participant in international activities. In 1995, the KSBT hosted the Sixth ISBT Western Pacific Regional Congress in Seoul and will host the $33^{rd}$ ISBT Congress in Seoul in 2014. Since its inauguration, the KSBT has had key roles not only in education and research in the fields of transfusion medicine but also in development of safe-blood services during the last 30 years.

장애인활동보조서비스 갈등에 관한 에이블뉴스 보도 내용분석 (A Qualitative Content Analysis on the Ablenews Reports on Personal Assistance Service Conflicts)

  • 연구의 목적은 장애뉴스에 특화된 인터넷 독립언론매체 에이블뉴스의 장애인활동보조서비스 갈등 보도에 나타난 주요 주제를 분석하고, 활동보조서비스 갈등에 관한 보도에 내재된 관점을 비판적으로 분석하는 데 있다. 이를 위해 본 연구는 에이블뉴스의 장애인활동보조서비스 갈등 관련 기사들을 자료로 하여 귀납적 질적 내용분석, 인과요소와 책임귀인분석, 장애패러다임분석을 실시하였다. 내용분석결과 에이블뉴스는 장애인활동보조서비스 갈등을 충실히 보도하고 있었으나 책임귀인 분석에 의하면 갈등의 원인과 해결을 장애인이용자와 활동보조인에게 귀인하는 경향이 두드러졌다. 또한 장애패러다임 분석결과 재활모델의 관점을 반영한 텍스트들이 적지 않아 자립생활모델과 밀접한 활동보조서비스 갈등을 보도하면서 독자들에게 재활모델의 관점을 전달할 우려가 있는 것으로 나타났다. 이러한 연구결과를 토대로 장애에 특화된 독립언론매체로서 보다 전문적인 보도를 위해 전문기자의 활용, 외부 전문기고자의 활용, 편집회의를 통한 기사 게재 등을 제안하였다.
