장기이식코디네이터의 감정노동, 소진의 관계에서 전문직 자아개념의 매개 및 조절효과 (The Mediating and Moderating Effect of Professional Self-concept in Relationship between Emotional Labor and Burnout of Organ Transplantation Coordinators)

  • Purpose: To determine the mediating as well as moderating effect on both the emotional labor and burnout of professional self-concept perspective organ transplantation coordinators (OTC)'. Method: A self-report questionnaire was collected from 86 OTCs nationwide. The data were analyzed using t-test, ANOVA, Scheffe' test, Pearson' Correlation Coefficients, AMOS 20.0, and multiple regression. Results: For effects of professional self-concept on emotional labor and burnout, higher emotional labor was related to higher burnout. When professional self-concept was mediated, emotional labor had a greater effect on burnout however, a partial mediating effect was detected. From the regression analysis with the independent variables of emotional labor (A) and professional self-concept (B), adding relation of both variables ($A{\times}B$) increased the explained variance to 56.3% ($p$<.05). It was confirmed that professional self-concept had a moderating effect on emotional labor and burnout. Conclusion: The results of this study indicate that, when professional self-concept was mediated emotional labor of an OTC decreased, and burnout decreased when professional self-concept was high. Also, by controlling professional self-concept, emotional labor and burnout decreased. Therefore, it's necessary to map out the environment and develop strategies to reduce emotional labor and burnout in organ transplantation coordinators.

중환자실 간호사의 소진 영향요인 (Factors Affecting Burnout in ICU Nurses)

  • Purpose: The purpose of this study was to identify factors affecting professional burnout in intensive care unit (ICU) nurses. Method: Data were obtained through questionnaires from 240 ICU nurses working in hospitals of 500 beds or more in two cities in South Korea. Data analysis was done with t-test, ANOVA, Scheffe test, Pearson correlation coefficients and multiple stepwise regression with SPSS WIN 14.0. Result: Work stress was positively correlated with level of professional burnout and there was a negative correlation between hardiness and professional burnout. Social support was negatively correlated with level of professional burnout. Hardiness had a positive correlation with social support and a negative correlation with work stress. The explained variances for burnout was 44.7% and factors affecting professional burnout in the ICU nurses' were work satisfaction, work stress, social support, and hardiness. Conclusion: The findings indicate that high levels of stress coupled with low levels of hardiness and social support in ICU nurses result in high levels of professional burnout. Reducing work stress and the level of professional burnout in ICU nurses and increasing work satisfaction can be done by enhancing the mediating factors of hardiness and social support through educational efforts and nursing and administrative support.

  • Purpose: This study was done to identify the level of stress, hardiness, and professional burnout of intensive care unit (ICU) nurses and to provide basic data for the qualitative improvement of nursing work achievement in the workplace. Method: The data were obtained from 129 ICU nurses through a questionnaire. Results: Stress showed positive correlation with the level of professional burnout and there was negative correlation between hardiness and professional burnout. Nurses in the group with a high level of stress, and a high level of hardiness reported a low level of professional burnout compared to those in the group with a lower level of hardiness (t=-2.60, p= .012). Nurses in the group with a low level of hardiness and a low level of stress showed a lower level of professional burnout than those with a high level of stress (t=3.51, p= .001). Conclusion: The results show that the high level of stress when coupled with a low level of hardiness of ICU nurses results in a high level of professional burnout. For this reason, it is important for nursing administration to support nurses to reduce levels of stress and the resulting burnout in ICU nurses.

임상간호사의 직무소진 극복과정 (The Process of Overcoming the Professional Burnout of Clinical Nurses)

  • Purpose: The purpose of this study was to identify the process of overcoming professional burnout among clinical nurses. Methods: Data were collected from 12 clinical nurses through in-depth interviews about their actual experiences. The main question was "How do you describe your experience of the process of overcoming professional burnout?" Qualitative data from the field notes and transcribed notes were analyzed using the grounded theory methodology developed by Strauss & Corbin. Results: The core category of experience of the process of professional burnout among clinical nurses was identified as "Keeping a balance by awakening-relaxing-coexisting". The participants used six interactional strategies such as 'Confronting problems', 'Exposing problems', 'Laying the mind's burdens down', 'Taking a breather', 'Seeing from a different perspective', 'Accepting realities'. The results of overcoming process of professional burnout were 'Coexisting' and 'Revitalizing'. Conclusion: The results of this study provide useful information for designing supportive programs and policies to solve professional burnout in clinical nurses.

응급구조사의 전문직 자아개념이 임파워먼트와 소진에 미치는 영향 (The influence of professional self-concept on empowerment and burnout of paramedics)

  • Purpose: This study aimed to examine the degree of paramedics' professional self-concept, empowerment, and burnout and to verify the effects of empowerment and burnout on their professional self-concept. Method: The study subjects were 116 in-hospital paramedics who completed a structured questionnaire. Data were collected between June 18 and 25, 2013. Results: The results were as follows. First, the paramedics' mean score for professional self-concept was 2.93 (range: 1-4), and the mean scores for empowerment and burnout were 2.87 (range: 1-4) and 2.48 (range: 1-5). Second, in terms of the general characteristics of the paramedics, professional self-concept showed significant differences based on sex, religion, work experience, and medical institution classification. Third, professional self-concept had a positive correlation with empowerment, and professional self-concept and empowerment had negative correlations with burnout. Fourth, the main variables affecting paramedics' professional self-concept were gender and total years of work experience. Conclusion: It is necessary to develop program to reduce paramedics' burnout and qualitatively improve paramedic services.

응급실 간호사의 전문직 삶의 질: 공감만족, 소진, 이차 외상성 스트레스 (Emergency Nurses' Professional Quality of Life: Compassion Satisfaction, Burnout, and Secondary Traumatic Stress)

  • Purpose: Professional quality of life is the quality one feels in relation to their work as a helper. The purpose of this study was to describe professional quality of life among emergency nurses. Methods: A total of 15 emergency rooms were selected in three cities. Among 263 nurses working at these emergency rooms, 178 nurses consented to participate in this cross-sectional survey. Professional Quality of Life (ProQOL) Scale version 5 was used to measure compassion satisfaction, burnout and secondary traumatic stress. Cluster analysis was used to classify nurses according to professional quality of life. Results: The mean scores (SD) for compassion satisfaction, burnout, and secondary traumatic stress were 32.12 (5.45), 28.27 (4.28), and 28.20 (5.07), respectively. The result of cluster analysis according to standardized score of compassion satisfaction, burnout, and secondary traumatic stress yielded three clusters. Over 50% of participants showed low professional quality of life. Nurses who were included in a cluster representing low professional quality of life were younger, had shorter periods of nursing experience, and perceivedlower social support than other cluster. Conclusion: Education or support programs for emergency nurses are needed to enhance their professional quality of life.

치과위생사의 역할스트레스, 회복탄력성, 전문직정체성이 소진에 미치는 영향 (Effect of role stress, resilience, and professional identity on burnout in dental hygienists)

  • Objectives: This study aimed to identify measures that can improve the quality of dental hygienists' work by analyzing the effects of role stress, resilience, and professional identity on burnout. Methods: Self-report questionnaires were administered to 339 dental hygienists working in dental institutions located in Seoul, Incheon, and Gyeong-gi, South Korea from August 1 to September 30, 2017. Results: Out of a maximum of 5 points, role stress scored an average of 2.80, resilience scored 3.61, professional identity scored 3.27, and burnout scored 2.78. The factor that most affected the exhaustion of dental hygienists was professional identity (${\beta}=-0.397$), followed by role stress (${\beta}=0.251$), and resilience (${\beta}=-0.150$). Conclusions: The results showed that the role stress, resilience, and professional identity of dental hygienists affected burnout. Efforts to reinforce the resilience and professional identity of dental hygienists may be needed alongside efforts to reduce role stress. Hence, resilience reinforcement programs alongside supplemental education programs that reinforce the work skills required of dental hygienists according to their clinical experience after graduation should be developed.

사회복지사의 소진과정(Burnout Process) 모델 - 직업관련 스트레스 요인, 직무스트레스와 전문직효능성에 의한 소진 경로분석 - (Burnout Process Model of Social Workers: Analysis of the Paths from the Job-related Stressors, Job-stress and Professional Self-efficacy to Burnout)

  • 사회복지사들의 소진결정과정에 영향을 미치는 요인을 조사하고자, 직업과 관련된 8가지 스트레스요인(업무모호성, 업무과중, 문제해결의 어려움, 클라이언트와의 갈등, 수퍼바이저의 지지결여, 기관내 의사소통의 폐쇄성, 관료주의, 무책임성), 4종류의 매개요인(직무스트레스, 전문직역할수행, 기관정책 준수, 클라이언트에 대한 편파성)과 소진의 4구성요인(신체적 탈진, 심리적 탈진, 비인간화, 개인전 성취)으로 이루어진 포괄적인 연구모델을 개발하였다. 본 연구에서 개발된 모델은 사회복지사 207명으로부터 수집된 설문조사 자료를 통하여 검증되었다. 경로분석을 사용하여 변인들 간의 구조관계를 설명할 수 있는 최적 이론구조모델이 선정되었다(df=66, $x^2$=63.250, GFI=0.962, CFI=1.00). 자료분석 결과, 소진은 업무모호성과 업무과중, 문제해결의 어려움, 클라이언트와의 갈등, 수퍼바이저의 지지결여, 기관내 의사소통의 폐쇄성 등의 직업관련 스트레스요인에 의해 증가되는 것으로 나타났다. 한편, 소진을 결정하는 감정적 경로의 매개변인인 직무스트레스는 전문직 역할수행과 기관정책 준수의 정도를 증가시키며, 소진(신체적 탈진, 심리적 탈진, 비인간화)을 증가시키는 것으로 나타났다. 반면, 소진과정의 인지적 경로를 구성하고 있는 전문적 효능성은 전문직역할수행의 정도가 높고 기관 정책준수의 정도 및 클라이언트에 대한 편파성이 낮을 경우 증대되며, 소진을 감소시키는 것으로 나타났다.

수술실 간호사의 직무스트레스, 회복탄력성, 전문직 정체성이 소진에 미치는 영향 (Influence of the Job Stress, Resilience, and Professional Identity on Burnout in Operation Room Nurses)

  • Purpose: The purpose of this study was to identify the factors influencing on burnout in operating room nurses. Methods: Using a cross-sectional design, a total of 109 operating room nurses working at 7 general hospitals with 300 beds or more in B city were analyzed. The instruments used for this study assessed job stress, resilience, professional identity, and burnout. Data was analyzed using descriptive statistics, a t-test, an ANOVA, a Pearson's correlation coefficient and a multiple regression analysis. Results: There was a statistically significant correlation between burnout and job stress (r=.53, p<.001), resilience (r=-.59, p<.001), and professional identity (r=-.47, p<.001). The factors influencing burnout include job stress (${\beta}=.27$, p<.001), resilience(${\beta}=-.37$, p<.001), dissatisfaction with the nursing job (${\beta}=.32$, p<.001), and moderate satisfaction with the nursing job (${\beta}=.19$, p=.014), and the explanatory power was 53.0%. Conclusions: The results suggest that intervention to reduce job stress and to improve resilience, which were the factors influencing burnout in operating room nurses, is necessary.

  • 본 연구의 목적은 임상간호사의 업무소진, 대인관계 능력, 전문직 자아개념 및 우울에 대해 파악하고, 이들 변수와 우울간의 관련성을 파악하는 것이다. 본 연구는 충청도 소재의 A종합병원 8개의 간호단위에서 근무하는 임상간호사 150명을 대상으로, 구조화된 설문지를 이용한 자가보고식 설문조사로 2012년 12월 10일부터 2013년 1월 30일까지 시행되었다. 연구결과, 업무소진은 대인관계능력, 전문직 자아개념과 음의 상관관계를, 우울과는 양의 상관관계를 나타냈다. 대인관계 능력은 전문직 자아개념과 양의 상관관계를, 우울과는 음의 상관관계를 나타냈고, 전문직 자아개념은 우울과 음의 상관관계를 나타냈다. 본 연구에서 일반적 직업관련 특성 중 우울에 유의한 차이를 보였던, 연령, 결혼상태, 근무경력, 근무부서와 주요변수인 업무소진, 대인관계 능력, 전문직 자아개념 중 업무소진 만이 우울에 유의한 영향을 미치는 변수로 파악되었고, 이 변수의 우울에 대한 설명력은 43%로 나타났다.
