Comparing locus of control in orthodontic patients of a university dental hospital with those of a private dental clinic. (치과의원과 대학치과병원 교정환자의 통제소재 특성)

  • Objective: Many factors are considered when a patient chooses the type of dental institution they wish to receive treatment from. Numerous studies have been conducted regarding this issue. However, more research is needed in analyzing the patient's psychological aspects when he/she decides to choose the dental institution they wish to receive treatment from. Methods: A locus of control test was utilized to analyze the psychological aspects in orthodontic patients of a university dental hospital and a private dental clinic. The locus of control test is known to be useful in predicting a human being's behavior. Both intrinsic and extrinsic locus of control tests were carried out in 934 orthodontic patients attending Seoul National University Dental Hospital and 1466 orthodontic patients from a private dental clinic. Results: The orthodontic patients at the university dental hospital showed a stronger extrinsic locus of control compared with those from the private dental clinic. Conclusion: The orthodontic patients at the university dental hospital value the doctor as the authority figure and the treatment quality more than those at the private dental clinic.

Dentists' Opinions in The Dental Field of Present Health Insurance Claim and Review (건강보험중 구강요양급여의 청구 및 심사에 관한 치과의사의 견해)

  • The study was intended to investigate how dentists in private dental clinic thought on the present claim and review of dental insurance to reflect it in future establishing dental insurance policies. 1,465 dentists who were running own dental clinic in Pusan Metropolitan City and the south part of Kyungsang province were surveyed in February, 2004. A total of 406 copies of finished questionnaire were finally retrieved and analyzed. The findings are as follows. 1. About insurance claim affairs : Most of the subject of insurance claim was by dentist himself or dental hygienist(nurse). Agency claiming was carried under 20% of total insurance claim. 2. The degree of attendance on insurance lecture : The degree of attendance on insurance lecture was relatively low. 3. Filing a protest against insurance claim : Filing a protest against insurance claim was reavealed about half-and-half for "have been" or "have not been". 4. Private clinic dentist,s opinion about the regulations affecting review of dental insurance : Private clinic dentists opinion about current guide for insurance review of dental fee was“the guidance is difficult and unfair cutback of claim fee may be carried”. 5. The affairs about health insurance review agency : About 70% of private clinic dentists have dissatisfaction on health insurance review agency. 6. Standpoint of private clinic dentists about issuance of receipt for dental fee : About 70% of private clinic dentist have an difficulty in issuance of receipt for dental fee. 7. The affairs about change insurance noncoverage treatment to insurance coverage treatment : Most of private clinic dentists hoped that insurance coverage about full mouth scaling, pit and fissure sealant, fluoride application. But they do not hoped that insurance coverage about geriatric denture, prothodontic treatment except precious metal, photopolymerization resin treatment.

Short-term, Multi-center Prospective Clinical Study of Short Implants Measuring Less Than 7mm

  • Objective : This prospective study sought to verify the stability of three types of short implants measuring 7mm or less. Materials and methods : Implants measuring 7mm or less were placed in patients at multicenter dental clinics in Korea, China, Taiwan, and Singapore. Initial stability, intraoperative and postoperative complications, crestal bone loss, and survival rate of the implant were prospectively evaluated. Results : The primary stability of a 6-mm implant was lower than that of a 7-mm implant. The marginal bone loss of short implants measuring less than 7mm was minimal. Complications such as wound dehiscence, implant mobility, and peri-implant mucositis developed, and these were associated with initial implant failure. The short-term survival rate of 6-mm implant was 93.7%, and that of 7-mm implant, 96.6%. Conclusion : Short implant for the mandible with insufficient height for the residual ridge can be selectively used. Poor primary stability and wound dehiscence can cause osseointegration failure and alveolar bone loss.

  • The design concept of this dental clinic Interior design is emotional color expression for the medical facility. This dental clinic is showing off the new adoption of the Korean primary traditional color pallette for the medical interior design. The graphic wall of the corridor is strongly attracting the eyes of customers while wrapping the corridor wall and partition with rather unconventional primary colored graphic shaping a tooth. Also the vivid and the dynamic atmosphere are emphasized by using the red color in the treatment area and the powder room. For the space programing, I focused on the functionality of each space and the efficiency of the traffic line. Centering around the information desk, the principal traffic line Is divided Into the staffs' private movement and the public movement.

Longitudinal and Epidemiological Study for Nationally Insured Patients of Private Dental Clinic in Korea

  • Purpose: This study want to draw conclusion based on data taken from national health insurance and determined the distribution and direction of patients treated at private clinics. Methods: The author's research spanned and compared the nationally insured patients by sexes, diagnoses, age groups and cases per year (2005, 2009, and 2013). Subjects were 3,536 patients of a private clinic in Seoul that were covered under national insurance. Results: There was no disparity across sexes nor cases, but both were on the decline. The most common dental conditions were pulpitis and dental caries at 38.4% and 16.4%, respectively. Both have decreased. Despite a drop in overall patients, the percentage of patients under 10 years old jumped substantially. Of overall age groups, teens were most prevalent at 33.5%, second and third being those in their forties and fifties (14.2% and 12.0%, respectively). Conclusions: The major illnesses that plague patients are pulpitis, dental caries, eruption disorder, gingivitis and periodontitis: the wane of pulpitis cases (a considerable percentage) and the actual numbers of patients has contributed to the general decrease in cases.

A Study on the Changes of Dental Health System's Expertise and Spatial Composition according to Dental Specialist System Enforcement in Korea (한국 치과 전문의 제도 시행에 따른 치과 의료체계의 전문성과 공간구성의 변화에 관한 연구)

  • The purpose of this research is to provide the basic information for the development of dental healthcare system's expertise and spatial composition in diverse dental spaces in Korea according to dental specialist system enforcement. Literature review of dental expertise especially dental specialist system and investigation on current status of spatial composition of dental spaces have been conducted. After the review, the spatial composition with field survey and architectural drawings of selected dental spaces have been analyzed. The result of this study is arranged with three points. First of all, the main clinical dental spaces are the dental school's dental hospital, dental department in medical center/hospital and private dental clinic in Korea. The second one is that dental school's dental hospital had differentiated to 10-11 departments in 1970-1980's and dental department in medical center/hospital has been changing to dental hospital with 5-7 departments in 2010's, and dental clinic is changing to specific dental clinic according to dental specialist system in 2018. The third one is that spatial composition is related with dental expertise at diverse levels. Spatial composition in dental school's dental hospital had been highly differentiated with dental expertise, dental department in medical center/hospital is partially subdivided with dental specialist system, but that of dental clinic is not changed according to dental specialist system. In addition to current spatial composition according to dental expertise, it is necessary to analyze the change of spatial composition with diachronic methods to develop the dental healthcare system.

  • Special attention should be directed toward the preservation of preoperative condylar position during orthognathic surgery because their positional change may leads to postoperative skeletal relapse as well as TM joint problem. Various condylar positioning devices, therefore, have been introduced and utilized in orthognathic surgery. Even though most of them provided us with improvement of surgical results, we also found some problems including limited indication, etc. For more accurately repositioning the maxilla and the mandible and its wide versatility, a newly designed maxilla and condylar positioning device based on the fixed part of cranium is introduced.

Evaluation of Appointment Policy and Scheduling Rule for a Dental Clinic Based on Computer Simulation (시뮬레이션을 이용한 치과의원의 예약정책과 스케줄링 규칙 평가)

  • In today's competitive dental markets, it is of paramount importance to improve service quality and at the same time to use scarce resource efficiently. In this study, we present appointment policies and scheduling rules for private dental clinics to reduce the waiting time of patients and to increase the revenue by utilizing resource more effectively. This study validates the proposed appointment policies and scheduling rules based on simulation models. We show that the bottleneck-based appointment policy is the most effective among appointment policies, followed by the multiple-block appointment one. The shortest processing time among scheduling rules contributes most to the performance of the appointment system.

The determinants of purchasing private health insurance among middle-aged and elderly Korean adults (중.고령자의 민간의료보험 가입 여부의 결정 요인)

  • Objectives : The coverage of Korean National Health Insurance is limited to basic level. Korean government encourages private health insurance for covering medical securities. So, many studies examined the determinants of purchasing private health insurance. However, 11% of Korean population is older than 65 in 2011. Considering the elderly is important to establish a health policy. The aim of this study is to examine factors determining the purchase of private health insurance among middle-aged and elderly Korean adults. Methods : We used the second Korean Longitudinal Study of Ageing (KLoSA), selected 8,688 sample of the aged 47 or older for the analysis. KLoSA collected information on demographic characteristics, income, health- related factors. KLoSA data include in the number of outpatient, inpatient, oriental hospital visit, dental clinic visit for two years. Logistic regression was used to examine the relationship between the determinants of purchasing private health insurance and the factors which include age, gender, education, residential district, marital status, smoking, drinking, physical exercise, economic activity status, national health insurance type, income, the number of chronic disease, and the number of outpatient, inpatient, oriental hospital visit, dental clinic visit for two years. Results : People who were older, did not live in a city, had higher IADL, currently drunk alcohol, did exercise regularly and had chronic diseases more than three were inclined not to purchase private health insurance. Females, the married, well-educated, past & currently smokers, the employed, high income earners, national health insurers, metropolitan citizens and someone who got high MMSE were more likely to purchase private health insurance. The more people experienced outpatients, inpatients, dental clinics and Chinese medicine clinics, the more private health insurance was purchased. The elderly people over 75 had more private health insurance than the aged 65-74. The strongest factors for private health insurance is gender, and economic status such as income. Conclusion : In this study, we found healthy-high income people were more likely to purchase private health insurance. In contrast, unhealthy-low income and older people did not. The economic factors were strongly related with private health insurance in aged over 75. These mean inequality exists in the using private health insurance. Therefore, the government should consider vulnerable social group before expanding private health insurance.
