The use of granulation tissue for the esthetic implant restoration for missing tooth due to alveolar bone loss (치조골 소실로 발치하게 된 치아의 심미적인 임플란트 수복을 위한 granulation tissue의 활용)

  • When maxillary anterior tooth is extracted due to alveolar bone loss, the augmentation of alveolar ridge is very important for esthetic implant restoration. Because alveolar bone loss increases after extraction, the ridge preservation performed right after tooth extraction is meaningful for esthetic implant restoration. However, no achievement of primary closure during ridge preservation can negatively affect bone regeneration. To overcome this problem, we can use granulation tissue in the extraction socket for primary closure. This case report confirmed that primary closure using granulation tissue resulted in not only ridge preservation but also ridge augmentation by providing an environment more advantageous of bone regeneration than the open wound.

  • Supernumerary teeth are dental abnormalies in the permanent dentition and the primary dentition. The etiology is unclear, but it may occur due to dichotomy of the tooth bud or hyperactivity of dental lamina. They occur more in the permanent dentition than in the primary dentition, with the most common site being the premaxillary area. Supernumerary teeth can be classified by morphology and position. Supplemental tooth refers to normal shape tooth. The treatment of supernumerary teeth depends on its shape, position, effect on dentition, and child's physiological condition. In this case, supernumerary primary tooth in the maxillary molar area was revealed by radiographical and clinical examination, but it was difficult to determine which is the supernumerary tooth. The tooth on the mesial side was extracted to induce the formation of adequate space and to prevent excessive space loss, and the result was favorable.

  • The effect of primary tooth infection on permanent teeth was reported by many previous study. It is seen histologic change in the enamel forming organ and in the surrounding sac, thus produce the defect as either hypoplasia or hypocalcification and alter the eruption of the permanent teeth. The periapical inflammation cause permanent tooth displacement in various direction and can be classified into the three categories : rotation, deflection inversion. This study reported case which displacement of succedaneous teeth may be caused by periapical infection of primary dentition and concluded that treatment procedure of infected primary tooth is selected and performed by importance to retain the tooth for space maintain, possibility to successfully restored, evidence of abnormal development of the succedaneous tooth.

Effect of mother's oral health knowledge and behaviour on dental caries in their preschool children (어머니의 구강보건지식 및 행동이 자녀의 유치우식증에 미치는 영향)

  • The purpose of this study was to analyze the influence of child's general property, mother's social and economic property, mother's knowledge and behavior in oral hygiene upon the appearance and treatment of child's primary tooth decay. For the purpose, oral examination was applied to one hundred three(103) small children who were at the age between four(4) and seven(7) and went to two(2) places of day care centers located in Seoul, and questionnaire was done to their mothers. The results of the study are as follows. 1. The number of children's dt is 1.55, the number of their ft is 1.42, dft index for primary tooth is 2,98, ft rate is 45.61%, and the higher child's age is, the higher their value is. 2. Mother's age, educational background, and occupation does not show significant difference with dft index for primary tooth. The higher mother's monthly average income is, the higher child's ft rate is. 3. dft index for primary tooth does not show significant difference according 10 mother's knowledge in oral hygiene. And, the child of mother using dental floss does show higher it rate in comparison with the one of mother who does not use dental floss, 4. Experience using dental clinic to treatment tooth decay does show significant difference with dft index for primary tooth. And experience using dental clinic for the purpose of oral examination and preventive treatment does show significant difference with ft rate. 5. From the result of multiple regression with dependent variable of dft index for primary tooth, there is no variable having significant influence. From the result of multiple regression with dependent variable of ft rate, explanatory variable is 43%, child's age, mother's occupation, mother's monthly average income, and experience using dental clinic to prevent tooth decay are significant explanatory elements. Through the above results, we can know that mother should practice positive behavior in oral hygiene for child to improve oral health. Under the reason, oral health education should be applied toward mothers as soon as possible, and governmental support should be followed so that mothers can participate in the education.

  • Author have measured the depth of gingival sulcus of the primary teeth. 333 teeth were selected from the children who attended on the department of pedodontic, College of dentistry, Yonsei University, and kindergarten children at Won Ju city aged from 1 year 8 months to 10 year 2 months. 1998 point were measured. The results were as follows. ; 1. Facial surface was the shallowest in sulcus depth compared with other surfaces. It was 1.56 mm. The depth of sulcus in lingual surface was deeper than facial. 2. The deepest part of each surface was mesial and distal, There were no statistical differences between mesial and distal part of the sulcus depth. 3. The mean sulcus depth of each tooth was as follows. Primary central incisor.....1.71mm. Primary canine....1.75mm. Primary second molar.....2.03mm. 4. The mean sulcus depth of upper was 1.86mm, and the lower was 1.76mg. The upper tooth was deeper than lower in sulcus depth. 5. The lower primary central incisor had the shallowest sulcus and the upper 2nd primary molar had the deepest compared with other tooth. 6. There wasn't specific statistical differences between the age groups of primary dentition and mixed dentition.

  • The purpose of this study was to compare the microleakage pattern of glass ionomer with resin restoration in microleakage pattern of primary and permanent teeth. Microscopical observation of interface between tooth structure and restoration was also performed. 80 and 8 sound molar teeth were used for the microleakage test and SEM study respectively. Data were analyzed statistically using ANOVA test and/or t-Test. The results of the present study were as follows: 1. According to the result of microleakage pattern between primary and permanent tooth, primary tooth generally showed more micro leakage than permanent tooth in all groups (p<.05). 2. In the resin-filled groups, occlusal margin was shown to have more microleakage than gingival margin(p<.05). Whereas the glass ionomer-filled groups showed no statistically significant differences between them(p>.05). 3. No statistically significant differences in microleakage could be found between two different resin groups(p>.05), while Fuji II LC group showed less microleakage than Ketac-Fil group(p>.05). 4. The various type hybrid layer was evident under SEM in resin-filled groups both in primary and permanent teeth with generally thicker layer in primary group. Among glass-ionomer group, Fuji II LC group showed more intact adhesion to tooth surface than Ketac-Fil group

Triplication of upper right primary anterior teeth : a case report (상악 우측 유전치부에 발생한 삼중치 : 증례 보고)

  • The fusion is an anomalous union of two or more tooth germ at some stage in the development process resulting in a decreased number of teeth in the dental arch. Fusion is common odontogenic anomaly but triplication of primary teeth is a rare phenomenon. We report a case of a 3 - year - old girl who visited our clinic for the pain on maxillary anterior tooth. The patient was diagnosed by the fusion of a the maxillary primary right central and lateral incisor with a supernumerary tooth and caries of dentine. And then, Endodontic treatment and composite resin restoration were performed on the triple teeth. After follow-up of 6 months period, there was no marked complications.

Reasons for primary teeth extraction in children : Retrospective study (유치 발거 원인에 대한 후향적 연구)

  • The purpose of this study was to investigate the principal reasons for primary teeth extractions and the tooth type extracted in children. 1159 patients were selected in this study. Dental records and radiographs were reviewed and age, gender, medical history, type of tooth extracted and the reasons for extraction were collected. The data were statistically analyzed using Chi-square test. Total 2078 primary teeth were extracted. Central incisors(34.1%) were most frequently extracted. Extractions due to physiological mobility(77.5%) were the most frequent followed by caries(13.8%), orthodontic(3.9%), trauma(1.7%). Reason for the extraction was different according to age (p = 0.000), but there was no difference according to gender (p = 0.109). While extractions due to physiological mobility predominated overall, reason for extraction was different according to the tooth type. There was no previous treatment in 54.6% of primary teeth extracted due to caries. Excluded physiological mobility, caries are the most common reason for extraction of primary teeth. The importance of preventive care should be emphasized in order to preserve primary teeth and improve children's oral health.

  • The infection of primary teeth occurs by caries or trauma. But, if it is not treated on time, the complication may occur such as enamel hypoplasia, discoloration, impaction, displacement and dilaceration of permanent successors. The periapical lesion on primary tooth could displace the permanent successors in any direction. Treatment options of displaced tooth are observation after extracting the primary teeth, surgical exposure orthodontic traction, transplantation and extraction. In these cases, displacement of permanent central incisor caused by the periapical lesion of primary teeth was observed. The spontaneous eruption after extraction of primary teeth was expected considering degree of displacement, development of root and direction of eruption. This case report shows spontaneous eruption of displaced permanent tooth germ was achieved with periodic examination after extraction of primary teeth.

  • Patient with alveolar abscess due to dental caries with severe alveolar bone loss, severe tooth mobility, root resorption need extraction of tooth because it is impossible to carry out pulp treatment and restoration by using conventional method. Early loss of primary molar might cause masticatory interference, extrusion of opposing tooth, problem in maintaining space and interference on eruption of permanent tooth. Especially, early loss of primary second molar before the eruption of permanent first molar might cause space closure by mesially erupted permanent first molar and impaction of second premolar. In such a case, distal shoe space maintainer and removable space regaining appliance was the first choice of treatment. But, distal shoe space maintainer need precise adaptation and might cause chronic inflammation if the oral hygiene is poor. In a case using removable space regaining appliance, patient's cooperation is most important. If the distal root of primary second molar is comparably sound and alveolar abscess with alveolar bone loss is localized at mesial root, hemisection should be carried out for precise guide to eruption of the permanent first molar, restoration of masticatory fuction and solution to the discomfort of the patient
