Effects of Therapeutic Exercise on Pain, Physical Function, and Magnetic Resonance Imaging Findings in a Patient with Multilevel Lumbar Disc Herniation: A Case Report

  • Background: In some clinical guidelines followed in clinical practice, nonsurgical treatments are recommended as the primary intervention for patients with lumbar disc herniation (LDH). However, the effect of a therapeutic exercise program based on stabilization of the lumbar spine for treatment of multilevel LDH has not been evaluated thoroughly. Objective: To investigate the effects of therapeutic exercise on pain, physical function, and magnetic resonance imaging (MRI) findings in a patient with multilevel LDH. Design: Case Report Methods: A 43-year-old female presented with low back pain, radicular pain and multilevel LDH (L3-L4, L4-L5, L5-S1). The therapeutic exercise program was conducted. in 40-min sessions, three times a week, for 12 weeks. Low back and radicular pain, lumbar disability, and physical function were measured before and after 6 and 12 weeks of the exercise program. MRI was performed before and after 12 weeks of the program. Results: After 6 and 12 weeks of the therapeutic exercise, low back and radicular pain and lumbar disability had decreased, and lumbar range of motion (ROM) was improved bilaterally, compared with the initial values. Also improved at 6 and 12 weeks were isometric lumbar strength and endurance, and the functional movement screen score. The size of disc herniations was decreased on MRI obtained after 12 weeks of therapeutic exercise than on the pre-exercise images. Conclusions: We observed that therapeutic exercise program improved spinal ROM, muscle strength, functional capacity, and size of disc herniation in LDH patient.

MRI Findings of a Malignant Solitary Fibrous Tumor of the Diaphragmatic Pleura: a Case Report

  • Solitary fibrous tumors (SFT) are rare mesenchymal tumors that most commonly develop in the pleura; they rarely involve the diaphragm. MRI has not been widely used to evaluate SFTs of the thoracic cavity, though it may be highly useful in assessing local invasion, predicting malignant potential, and helping in the differential diagnosis. However, MRI findings of malignant SFTs of the diaphragmatic pleura have been described in only two cases. We report a rare case of a malignant solitary fibrous tumor of the diaphragmatic pleura in an 82-year-old man. We describe the clinical and characteristic imaging features, including computed tomography, conventional MRI, and diffusion-weighted imaging. Contrast-enhanced MRI is more accurate than is CT in identifying the origin of SFTs, predicting whether they ae benign or malignant, and assessing local invasion. This imaging modality proved helpful in deciding on the treatment strategy for these rare tumors.

Quantitative NMR Analysis of PTMEG compounds

  • PTMEG(Polytetramethylene ether glycol) is a polymer compound widely used as a wide range of applications in the textile industry. PTMEG substance carrying various 1,800~2,000 molecular weight are mainly used as the raw material of the spandex production. Molecular weight and degree of polymerization value for 4 different PTMEG samples under pilot plant scale synthetic process were determined by a new quantitative NMR method. In NMR experiments, p-toluenesulfonic acid(TSOH) was used for external standard material of PTMEG quantitative analysis. were measuring The concentration of the primary standard TSOH was measured by UV/Vis spectroscopy. By using NMR peak assignments and the integral values of designated proton NMR peaks, We were able to measure the % composition of the synthetic PTMEG polymers, concentrations, molecular weight and the degree of polymerization that show the synthetic process of each manufacturing pilot plant. By utilizing a newly developed quantitative NMR method were able to obtain the molecular weight of PTMEG samples within 0.08 error % range.

선반 기어박스의 진동.소음 분석과 저감에 관한 연구 (A Study on Vibration and Noise Reduction of a Lathe Gear Box)

  • When operating lathe gear box which is equipped with geared transmission, it sometimes generates loud noise and excessive vibrations. In order to identify their causes, in this study, torsional and lateral vibration characteristics including critical speeds of the gear transmission system are firstly analyzed using lumped parameter models. Natural frequencies and mode shapes of the gear box structure are also analyzed by using the modal test. Furthermore, measured vibration and noise signals during operations are analyzed and compared with theoretical analysis results. After all, it is concluded that the primary cause of the excessive noise and vibrations is the resonance between gear meshing frequency including its side bands, the frequencies of shaft bending and torsional vibrations, and the natural frequencies of the gear box structure. Consequently the noise and vibration levels are greatly reduced by avoiding resonance between the natural frequencies and gear meshing frequencies through the rearrangement of the gears on the transmission shaft without any gear ratio change.

Measurement of Branching Ratio for broad 27-keV Resonance of $^{19}F(n,g)^{20}F$ Reaction by using Time-of-flight Method with Anti-Compton NaI(Tl) Spectrometer

  • The neutron capture spectrum for the light nuclide was very useful to study the nuclear structure. In the present study, the capture gamma-ray from the 27-keV resonance of $^{19}F(n,g)^{20}F$ reaction were measured with an anti-Compton NaI(Tl) spectrometer and the 3-MV Pelletron accelerator of the Research Laboratory for Nuclear Reactors at the Tokyo institute of technology. A neutron Time-of-Flight method was adopted with a 1.5 ns pulsed neutron source by the $^7Li(p,n)^7Be$ reaction. In the present experiment, a Teflon(($CF_2$)n) sample was used The sample was disk with a diameter of 90mm. The thickness of sample was determined so that reasonable counting rates could be obtained and the correction was not so large for the self-shielding and multiple scattering of neutrons in the sample, and was 5mm. The primary gamma-ray transitions were compared with previous measurement of Kenny.

  • Purpose: To investigate whether there are significant changes in regional brain metabolism in patients with Parkinson's disease after thalamotomy using proton magnetic resonance spectroscopy (1H MRS). Materials and methods: Fifteen patients with Parkinson's disease of mean age 56.5 years (7 males and 8 females; mean age, 56.5 years) that have treated with levodopa were included. All patients with tremor experienced amelioration of their symptoms on the side contralateral to the thalamotomy. As a single-voxel technique, 1H MR spectra were obtained from the volume of interested regions in thalamus and primary motor cortex. Spectral parameters were: 20 ms TE, 2000 ms TR, 128 averages, 2500 Hz spectral width, and 2048 data points.

Breast Magnetic Resonance Imaging Indications in Current Practice

  • Although mammography is the primary imaging modality for the breast, it has its limitations especially with dense breast parenchyma. Breast magnetic resonance imaging (MRI) has evolved into an important adjunctive tool as it is currently the most sensitive technique for breast cancer detection. Despite this high sensitivity, overlap in the appearances of some benign and malignant breast lesions results in additional unnecessary intervention with negative results. These false positives, in addition to high cost and limited availability, necessitate establishing proper indications for breast MRI. The literature was here reviewed for recent clinical trials, meta-analyses and review papers which have studied this important subject. PubMed; the US national library of medicine, was utilized to review the literature in the last twenty years. Using the obtained information, current uses of breast MRI are discussed in this paper to determine the indications which are relevant to clinical practice.

  • Triphenylphosphine phenylimide의 비수용액에서의 전기화학적인 환원반응을 polarography, cyclic voltammetry, controlled-potential coulometry 및 electron spin resonance 법을 써서 고찰하였다. 이 유기인화합물은 one-electon transfer에 따라서 anion radical이 형성되나 순간일 뿐이고 protonation과 재차 one-electon reduction 결과 인과 질소사이의 이중결합이 끊어진다. 그 결과 아닐린이 주요 반응생성물로서 발견되었다. 또 한편 동반하는 화학반응결과 생긴 주산물의 하나인 triphenylposphine oxide의 환원결과 인과 페닐 사이의 단일결합이 끊어지는 것도 관찰할 수 있었다.

  • Intracranial involvement in multiple myeloma patients takes up around 1%, and is usually known to be present in the parietal bone or skull base in cases of skull vault involvement, while it presents in the dura and parenchyma in cases of intracranial involvement. Primary pachymeningeal invasion is even rarer with extremely rapid progression and very poor prognosis. It is our intent to report a case in which we had to differentiate multiple myeloma with other metastatic tumors, lymphoma, and leukemia with intracranial involvement. Our patient showed an osteolytic lesion of the skull with dural involvement and subdural mass formations.

  • In the design of modem rotating machinery, it is often necessary to increase the performance of rotor-bearing system. Since a critical speed range influences the performance and safety of the whole system, it should be necessary to constrain the critical speed and thus resonance response in design process to result in large vibration. Consequently the minimization of resonance response amplitudes within the operation range of the rotor system becomes the most primary design objective. In this paper, based on the assumption that the external shape of rotating-shaft, bearing supporting positions and etc, the natural frequency analysis of spindle is performed by ANSYS $10.0^{(R)}$ Optimum design is conducted using the RBF model.
