  • It is generally said that primary health care is the base of health delivery system. But in Korea, the primary care have not performed its roles well. To resolve these problems, the government has elaborate some policy measures. One of them is 'Attending Hospital System'. The purposes of this study are to understand Attending Hospital System and to anticipate the change of general hospital and clinic by this regulation.

  • Purpose: This study investigates the relationship between job stress and burnout among primary healthcare practitioners during COVID-19 pandemic through mixed methods study. Methods: Data were collected from October to November 2022 using Qualtrix, a web-based survey platform. 1,082 primary health care practitioners participated in the survey. Quantitative data were analyzed using correlation analysis using IBM SPSS/WIN 27.0. Qualitative data were analyzed using content analysis through open-ended questions. Results: Job stress and burnout among primary healthcare practitioners during COVID-19 were positively correlated. Four categories and seven subcategories were identified. Conclusion: Based on these findings, it is necessary to develop a support system for primary healthcare practitioners according to the type of residential area and the number of peopleto reduce job stress and burnout.

Prevalence and Forms of Workplace Bullying Among Health-care Professionals in Cyprus: Greek Version of "Leymann Inventory of Psychological Terror" Instrument

  • Background: Workplace bulling is a pervasive phenomenon with negative consequences for the health of victims and the productivity of organizations. The aim of this study was to measure the prevalence and forms of workplace bullying among employees working at the public health-care sector of Cyprus using the Greek version of Leymann Inventory of Psychological Terror (LIPT) instrument. Methods: A translation process was followed from the French to the Greek version of LIPT questionnaire. Test-retest reliability expressed by Pearson's correlation coefficient was 0.98 indicating excellent reproducibility. Internal consistency reliability assessed by Cronbach ${\alpha}$ coefficient was 0.87 suggesting high reliability. LIPT instrument was distributed among 403 employees working at the primary health-care setting and at the largest public hospital of Cyprus with response rate of 73.4%. Results: Women comprised the majority of participants (71.4%). Mean age was 43.3 years. Prevalence of workplace bulling according to Leymann's definition was 5.9%. Most common forms of bullying were "Being continuously interrupted" (17.2%) and "continuously being given new work assignments" (13.5%). Women were significantly more often exposed to at least one mobbing behavior than men within the previous year (49% vs. 35.7%, p = 0.038), whereas nurses were significantly exposed to at least one mobbing behavior as compared to physicians (53.3% vs. 31.4%, p = 0.004). Conclusion: This was the first study that examined the prevalence of workplace bullying in the public health-care sector by elaborating the Greek version of LIPT instrument. Results showed that workplace bullying is a common and complex phenomenon among health-care organizations.

  • This study aims to evaluate the status of Korean healthcare among Organization for Economic Cooperation and Development countries and to monitor the trend of health care status since 2000. The position value for relative comparison (PARC) index was selected to gauge the level of healthcare status in demand, supply, accessibility, quality, and cost as per healthcare policy aspects. The Mann-Kendall test was conducted to allocate healthcare status of Korea since 2000. The PARC values indicate strength and weakness of Korean healthcare system by the mathematical comparisons. Korea positioned higher in demand, supply, accessibility, and quality. Yet, there are shortages in human resources and primary care. In conclusion, we suggest utilizing this study provides evidence to prioritize health care problems that can lead to establishing healthcare policy.

한국 미충족 의료 니즈 수준 및 발생 사유의 거주지역 간 격차 분석과 정책적 시사점 (Exploring Regional Disparities in Unmet Healthcare Needs and Their Causes in South Korea: A Policy-Oriented Study)

  • Background: Most developed countries are working to improve their universal health coverage systems. This study investigates regional disparities in unmet healthcare needs and their causes in South Korea. Additionally, it compares the unmet healthcare needs rate in South Korea with that of 33 European countries. Methods: The analysis incorporates information from 13,359 adults aged 19 or older, using data from the Korea Health Panel. The dependent variables encompass the experience of unmet healthcare needs and the three causes of occurrence: "burden of medical expenses," "time constraints," and "lack of care." The primary variable of interest is the region of residence, while control variables encompass 14 socio-demographic, health, and functional characteristics. Multivariable binary logistic regression analysis, accounting for the sampling design, is conducted. Results: The rate of unmet healthcare needs in Korea is 11.7% (95% confidence interval [CI], 11.0%-13.3%), which is approximately 30 times higher than that of Austria (0.4%). The causes of unmet healthcare needs, ranked in descending order, are "lack of care," "time constraints," and "burden of medical expenses." Predictive probabilities for experiencing unmet healthcare needs and each cause differ significantly between regions. For instance, the probability of experiencing unmet healthcare needs due to "lack of care" is approximately 10 times higher in Gangwon-do (13.5%; 95% CI, 13.0%-14.1%) than in Busan (1.3%; 95% CI, 1.3%-1.4%). The probability due to "burden of medical expenses" is approximately 14 times higher in Seoul (4.1%; 95% CI, 3.6%-4.6%) compared to Jeollanam-do (0.3%; 95% CI, 0.2%-0.4%). Conclusion: Amid rapid sociodemographic transitions, South Korea must make significant efforts to alleviate unmet healthcare needs and the associated regional disparities. To effectively achieve this, it is recommended that South Korea involves the National Assembly in healthcare policy-making, while maintaining a centralized financing model and delegating healthcare planning and implementation to regional authorities for their local residents-similar to the approaches of the United Kingdom and France.

  • Economic evaluations in the healthcare are used to assess economic efficiency of pharmaceuticals and medical interventions such as diagnoses and medical procedures. This study introduces the main concepts of economic evaluation across its key steps: planning, outcome and cost calculation, modeling, cost-effectiveness results, uncertainty analysis, and decision-making. When planning an economic evaluation, we determine the study population, intervention, comparators, perspectives, time horizon, discount rates, and type of economic evaluation. In healthcare economic evaluations, outcomes include changes in mortality, the survival rate, life years, and quality-adjusted life years, while costs include medical, non-medical, and productivity costs. Model-based economic evaluations, including decision tree and Markov models, are mainly used to calculate the total costs and total effects. In cost-effectiveness or costutility analyses, cost-effectiveness is evaluated using the incremental cost-effectiveness ratio, which is the additional cost per one additional unit of effectiveness gained by an intervention compared with a comparator. All outcomes have uncertainties owing to limited evidence, diverse methodologies, and unexplained variation. Thus, researchers should review these uncertainties and confirm their robustness. We hope to contribute to the establishment and dissemination of economic evaluation methodologies that reflect Korean clinical and research environment and ultimately improve the rationality of healthcare policies.

  • Purpose: The purpose of this study is to find the relationship between Michell Foucault's the primary, secondary, and tertiary spatialization of disease and spatial composition for the development of architectural planning of the healthcare architecture. Methods: Literature review of spatialization of disease and comparison between medical and dental disease have been conducted. The synchronic structure and diachronic change of spatialization process have been analyzed through spatial composition and history of Seoul National University, School of Dentistry and Dental Hospital. Results: The result of this study can be summed up in three points. First of all, spatialization of dental disease is similar to that of medicine but it should be more focused on the tertiary spatialization. The second one is that the process of spatialization of dental disease started the secondary spatialization first and the primary spatialization followed after a short interval and spatial composition has been followed the process of spatialization of dental disease in Korea. The third one is that the tertiary spatialization has not been actively gone along until recently and it has to be developed in near future. Implications: It is necessary to analyze spatialization of dental disease in other dental facilities to develop the relationship between spatial composition and program in healthcare system.

  • This study investigated the healthcare status of South Korea and member states of the Organization for Economic Cooperation and Development (OECD). By employing the position value for relative comparison index, healthcare status was measured through the following components: demand, supply, accessibility, quality, and cost. Statistical analysis was conducted through the Mann-Kendall test from analyzing trends from 2000 onwards. Results showed that while Korea, on average, scores higher than the OECD average in most of the investigated components, it is below average in certain indexes including primary care and mental health care. Considering the various health issues that have been raised about these indexes, it is important these components be improved upon by policy-makers.

Determinants of Satisfaction in the Usage of Healthcare Information Systems by Hospital Workers in Hyderabad, India: Neural Network and SEM Approach

  • This study focuses on the adoption of Healthcare Information System (HIS) in India's healthcare services, which has led to an increased use of HIS software for managing patient information in hospitals. The study aims to evaluate the factors that influence hospital workers' satisfaction with HIS usage and its impact on their intention to continue in the use of HIS. Primary data was collected through a survey questionnaire from 265 hospital workers. A new framework was developed, and Structural Equation Modeling (SEM) was used for analysis. Sensitivity analysis was also conducted on demographic data using an Artificial Neural Network (ANN) approach. The results indicated that all hypotheses were significant (p < 0.05). Effort expectancy was the most significant factor influencing hospital workers' satisfaction (p < 0.01). Sensitivity analysis showed that education (Model-A) and experience in use of HIS (Model-B) were the most important factors. The study contributes by proposing a new theoretical framework and extending the previous research on HIS usage satisfaction. Overall, the study highlights the importance of easiness and usefulness in predicting HIS usage satisfaction.

  • The objective of this study is to investigate the healthcare status of South Korea and other member countries of the Organization for Economic Cooperation and Development (OECD) using OECD health statistics 2020. We employed the position value for relative comparison index to measure the healthcare status in five following components: demand, supply, accessibility, quality, and cost. The Mann-Kendall test was used to analyze for increasing or decreasing trend of the position value for relative comparison values from 2000 to the recent year. Results showed that Korea was positioned above than the OECD median values in most of components, but lower than the median values in certain indices including healthcare employment, primary care, and mental health care. This study sheds some light on healthcare issues to be improved and the policy-makes can take into account for prior setting process.
