Related factors of late middle aged women's cancer preventive behaviors in local churches (지역교회 중년여성의 암 예방행위 관련요인)

  • This study was conducted to identify the factors related to the cancer preventive behaviors of middle aged women. The data was collected from October $1^{st}$ 2012 to February $10^{th}$ 2013 from 114 middle aged women in four churches in B city. As a result, there were significant differences in the cancer preventive behaviors according to cancer preventive education, concerns about cancer information and the utilization of cancer information. The cancer preventive behaviors were weakly related to the internal health locus of the control, chance health locus of the control and self-esteem. In addition, there was moderate correlation between the cancer preventive behaviors and the external health locus of the control. Meaningful variables that explain the cancer preventive behaviors were the external health locus of the control, self-esteem, cancer preventive education in that order. The total explanation power was 25.3% and the external health locus of the control has most explanation power. In conclusion, it is necessary to develop nursing intervention and health education for middle aged women to raise the internal health locus of the control and self-esteem, and encourage attending cancer preventive education to improve the preventive behaviors against cancer.

Literature Review about Yangseng(養生) studies in Korean Oriental Medicine (한의학 분야에서의 양생 연구 동향분석)

  • Objectives : This study was aimed to analysis the trend of Yangseng(養生) studies in Korean Oriental Medicine and to consider the scope of Yangseng research. Method : We collected 86 articles on Yangseng study result from the OASIS system using the keyword 'Yangseng'. Results : There were Yangseng study papers that were published between 1963 and 2011. Published 21 papers on The Korean Journal of Oriental Preventive Medicine were more than the other journals. Literature studies account for 78% of total studies, whereas clinical studies account for only 22%. Effects of clinical studies was evaluated mainly by survey. Conclusions : High quality experimental studies and intervention studies have not yet been published. It is necessary to research and develop duration of Yangseng effect and tools for evaluating.

The Proposal of Policies Aimed at Tacking Health Inequalities in Korea (우리나라 건강불평등 해소를 위한 정책 제안)

  • Although the New National Health Promotion Plan 2010 target to reduce health inequalities, whether the program will be effective for reducing the health inequalities in Korea remains quite unclear. More and more developed countries have been started to concentrate on comprehensive policies for reducing health inequalities. The health policies of the UK, Netherlands, and Sweden are the most wellknown. I propose that a comprehensive blueprint for tackling health inequalities in Korea should be made and that it must contain five domains: a target, structure and process, life-course approach, area-based approach, and reorganization of health care resources. The target should be based on determinants of health and more attention should be paid to socioeconmic factors. The structure and process require changes from the national health care policy based on medical services to the national health policy that involves the establishment of a Social Deputy?Prime Minister and the strengthening multidisciplinary action. A life-course approach especially focused on the early childhood years. Area-based approach such as the establishment of healthy communities, healthy schools, or healthy work-places which are focused on deprived areas or places is also required. Finally, health care resources should be a greater investment on public resources and strengthening primary care to reduce health inequalities. The policy or intervention studies for tackling health inequalities should be implemented much more in Korea. In addition, it is essential to have political will to encoruage policy action.

The Effects of a Preventive Educational Program on the Osteoporosis Knowledge among the Middle-Aged Women in Korea

  • The purpose of this study was to assess whether the osteoporosis preventive educational program has changed the middle-aged women's knowledge on osteoporosis. A one group pretest-posttest design was used. A total of 27 women (at pretest) and 27 women (at posttest) who were volunteers participated. The intervention program content included lecture and exercise entitled The Prevention of Osteoporosis. This program design consisted of one 3-hour session per week and lasted over 4-week period. Osteoporosis Knowledge Questionnaire (OKQ) was used. The OKQ contains 20 true-false items to measure the knowledge levels about osteoporosis. The Kuder-Richardson test (KR 20), used as an estimate of internal consistency for knowledge, was .7783. Overwhelming majority of the participants (96.2%) were ranging in age from 40 to 60. About half of the participants were college graduates and the majority of them (77.8%) had no jobs. The educational program significantly increased osteoporosis knowledge in middle-aged women. Results of this study shows that educational program is effective in increasing knowledge of osteoporosis. Further study using the same program with different age group is needed to measure knowledge, behavior and attitude on osteoporosis.

A Literature Review of Clinical Studies on Korean Medicine Treatment on Tension-type Headache: Focused on Domestic Case Reports (긴장형 두통의 한의학적 치료에 대한 임상연구 동향 분석 : 국내 증례보고를 중심으로)

  • Objectives : This study was conducted to analyze the Korean Medicine treatment on Tension-type Headache (TTH) in Korean clinical studies. Methods : Articles were searched from four Korean databases(KISS, RISS, OASIS, ScienceON). Study type, publication year, demographic information of participants, intervention type and details, outcome measurements and treatment results of selected articles were analyzed. Results : 17 articles were selected among those published until January 15th 2023. 'Cheonghyulgangki-tang', acupuncture at 'GV20', moxibustion at 'CV12', pharmacopunture at 'GB20', 'ByeolGab' were most frequently used in treatment on TTH. 'Numeric Rating Scale(NRS)' and 'Visual Analog Scale(VAS)' were most frequently used in outcome measurements. All of the articles exhibited that Korean Medicine treatments improved symptoms of TTH. Conclusion : This study showed that various Korean Medicine treatments for TTH were effective. This finding could be used in clinical practice and build the basis for the direction of clinical practice guidelines in future, Nevertheless, this study has few limitations. Further systematic reviews and meta analyses of randomized controlled trials are required for more evidence on Korean Medicine.

A Community-Based Integrated Preventive Program of Depression and Its Effectiveness in Caring for Vulnerable Elderly (취약계층 노인의 우울예방을 위한 지역사회기반의 통합프로그램 개발 및 효과검증)

  • The purpose of the study was to develop an integrated prevention program to strengthen elders self-care capability and to examine its effectiveness on their psychological condition. This study used one group pre- and post-test design. Subjects were 85 elderly residents (over 65 years of age) who lived alone, and received free basic medical care and social welfare services in a rural community in Korea. Subject eligibility criteria for this study were to an elders who 1) is not currently taking any anti-depressant medication 2) is able to communicate, and 3) agrees to participate in this study. The integrated program was composed of horticulture, reminiscence, and friendship activities. Twelve sessions were provided for 12 weeks in community-based partnerships to achieve better outcomes. The intervention was case-managed by a public health nurse and aided by six volunteers. The main outcome variable was depression, which was assessed by using 15 items selected from the Geriatric Depression Scale-short form Korean version. Socio-demographic characteristics, functional status, and satisfaction with social support were used as covariates. Results showed that there was a significant intervention effect at post-intervention time point compared to pre-intervention time point(E.S. 0.94). Multiple linear regression analysis showed significant interaction effects between intervention and satisfaction with social support. These findings must be interpreted within the context that an effects of an integrated program could be more synergistically increased when social support factor is considered in the program. A community-based integrated prevention program of depression is effective for vulnerable rural elderly. It is suggested that randomized controlled trials within community setting for better methodological strength as well as multi-level outcomes on community need to be conducted in future.

Study on the Development of Home Care Nursing Intervention Protocol for Stroke Patients (뇌졸중 환자의 가정간호중재 프로토콜 개발)

  • Stroke patient needs rehabilitation after receiving an acute treatment in a hospital. When stroke patient gets involved in an early discharge program, home care nurse plays a pivotal role to make them to gain a full strength and to come back to his/her prior life before he/she is sick. In spite of the importance of home care nursing intervention protocol for home care nurses to perform home care nursing autonomously, home care nursing intervention protocol for stroke patient is rarely developed. Therefore this study was conducted to develop home care nursing protocol that is applicable for stroke patients in home care nursing area. 41 home care nursing charts for stroke patients registered in home care nursing agencies from December 1st 1994 to August 31st 1999 at Y hospitals in Seoul and Won-Ju city were analyzed. 44 home care nurses who were having over three years' experience on stroke patients were participated in this study as a user validity validation group. The results of this study are as follows. 1. 28 nursing diagnoses were selected on the basis of evaluation of nursing diagnoses of stroke patients presented in a previous literature and case studies on home care nursing. 2. 17 nursing diagnoses were classified through the frequency analysis of home care nursing charts for 41 stroke patients who had received home care nursing. The order of sequence was like these: impaired skin integrity, risk for infection, nutritional deficit, impaired physical mobility, constipation, knowledge deficit, ineffective airway clearance, anxiety in family members, risk for aspiration, self care deficit, altered urinary elimination, ineffective individual coping, social isolation, risk for injury, self-esteem disturbance, impaired verbal communication, fatigue of family caregiver. 3. Based on validation on expert and user validities, 44 nursing interventions which were above ICV=.80 were chosen. 4. Nursing intervention protocols which showed above ICV=.90 were developed and were like these; pressure ulcer care, position change, preventive care for circulatory dysfunction, tube care : catheter, vital sign monitor, constipation/impaction management, artificial airway management, suction of airway secretion, environmental management : safety, and fall prevention.

Determinants of Mental Health Care Utilization in a Suicide High-risk Group With Suicidal Ideation

  • Objectives: The suicide rate in Korea is increasing every year, and is the highest among the Organization for Economic Cooperation and Development countries. Psychiatric patients in particular have a higher risk of suicide than other patients. This study was performed to evaluate determinants of mental health care utilization among individuals at high risk for suicide. Methods: Korea Health Panel data from 2009 to 2011 were used. Subjects were individuals at high risk of suicide who had suicidal ideation, a past history of psychiatric illness, or had utilized outpatient services for a psychiatric disorder associated with suicidal ideation within the past year. The chi-square test and hierarchical logistic regression were used to identify significant determinants of mental health care utilization. Results: The total number of subjects with complete data on the variables in our model was 989. Individuals suffering from three or more chronic diseases used mental health care more frequently. Mental health care utilization was higher in subjects who had middle or high levels of educational attainment, were receiving Medical Aid, or had a large family size. Conclusions: It is important to control risk factors in high-risk groups as part of suicide prevention strategies. The clinical approach, which includes community-based intervention, entails the management of reduction of suicidal risk. Our study identified demographic characteristics that have a significant impact on mental health care utilization and should be considered in the development of suicide prevention strategies. Further studies should examine the effect of mental health care utilization on reducing suicidal ideation.

Converged Relationship between Oral Health Beliefs, Oral Disease Preventive Intention and Oral Disease Preventive Activities in Partial Middle Aged Adults (일부 중년층의 구강건강신념과 구강병 예방행동의도, 구강병 예방행동에 관한 융합적 관계)

  • In this study, the amalgamative relationship associated with oral health belief and preventive behavior against oral disease subject to middle aged people was analyzed. 20 persons in their middle age living in U city were selected as final subjects for this analysis. It was found that a factor with the strongest effect on intention to prevent oral disease and preventive behavior among oral health belief was importance. In the study, it was identified that the parameters such as importance and benefit had a partial mediating effect on the intention to prevent oral disease and self-efficacy had complete mediating effect. Through these results, it seems necessary to develop an intervention program for middle aged people able to recognize importance and benefits of oral health by themselves in order to promote their oral health.

The Risk Factors Associated with Increased Blood Pressure, Sugar and Lipids in Multi-phasic Health Checkup Examinee (일부 종합검진 수진자에서의 혈압, 혈당 및 혈중지질과 관련된 위험요인)

  • Objectives : This study was conducted to evaluate the risk factors associated with cardiovascular disease. Methods : By the questionnaire, the informations of education level, monthly income, alcohol consumption and smoking habit were obtained. Height, weight and blood Pressure(BP) were measured by a trained nurse. The level of lasting blood sugar(FBS), total cholesterol, hight-density lipoprotein(HDL) cholesterol and triglyceride were tested by enzyne method about a group of 2888 males and 1696 females ages ever 20 who had never taken the medication for hypertension. Statistical analysis, ANOVA, stepwise multiple regression, correlation analysis were peformed using SAS package program. Results : There were significant differences among age groups in systolic and diastolic blood pressure, body mass index(BMI), FBS, triglyceride and cholesterol except HDL-cholesterol. The risk factors associated with systolic and diastolic BP significantly in male were BMI, age, alcohol intake, but in female BMI, age. Smoking habit had a significant negative correlation with BP in both sex. In the group of mild hypertension ($\geq140\;mmHg\;or\geq90\;mmHg$ defined by JNC-VI) and hypertension ($\geq160\;mmHg\;or\;\geq95\;mmHg$ defined by WHO), the percent of diabetes $(FBS\geq140\;mg/dl)$, hypercholesterolemia $(\geq240mg/dl)$, and hypertriglyceridemia $(\geq200\;mg/dl)$ were significantly higher compared with normal group in male and female. BMI, and alcohol consumption had a positive correlation with FBS, total cholesterol and triglyceride. Smoking had a positive correlation with cholesterol and triglyceride but negative correlation with HDL-cholesterol in male. The educational attainment had a negative correlation with BMI, FBS, total cholesterol and triglyceride in female. Conclusions : This study suggested that drinking, smoking, and BMI and lower educational level were associated with hypertension, hyperlipidemia and diabetes, but further evaluation was needed by prospective intervention study.
