청소년 범죄 재범방지를 위한 경찰 다이버전 프로그램 활성화 방안 (Police Diversion Program Active Plan For Juvenile Delinquency Repeat Prevention)

  • 청소년 범죄는 사소한 비행에서 시작되어 재범을 반복하는 생활을 하기 쉬운 첫 단계로 초기 단계에서 적극적인 선도대책을 시행함으로써 청소년 범죄를 감소시킬 수 있는 방안 마련이 시급하다. 비행 소년 범법 소년 사건처리에서 가장 중요한 것은 법적인 절차로 판단을 내리는 것 보다는 선도절차에 초점을 맞추어 재범을 방지하고 사회의 정상적인 구성원으로 복귀시키는 것이 최상의 방안이다. 이러한 맥락에서, 비행소년 범법 소년의 재범률 감소를 위한 다이버전 프로그램 개발 및 활성화 방안으로 전문가 활성화 및 초범에 대한 적절한 대응, 부모교육의 필요성, 멘토의 활성화, 후속 프로그램 개발을 제시하였다.

청소년의 자살생각 관련 요인: 자아존중감과 우울의 매개효과를 중심으로 (Factors Associated With Adolescents' Suicidal Ideation: Focus on Self-Esteem and Depression as Mediators)

  • This study examined the effects of eco-system factors on adolescents' suicidal ideation and whether self-esteem and depression mediate in the process to provide theoretical basis for prevention and treatment for problems of adolescents who have suicidal ideation. 2,088 high school students were asked to complete self-administering questionnaires on eco-system factors and adolescents' suicidal ideation. Data were analyzed with frequencies, t-test, ANOVA with Scheffe ad-hoc test, correlation analysis using SPSS 12.0 program and AMOS 5.0 program. Study results were ; First, adolescents' suicidal ideation showed significant differences according to their gender, socio-economic financial status, family structure, and the type of school they attended. Second, self-esteem and depression were proved to have significant mediating effect. Child abuse victimization, subjective school achievement and influence of media coverage of celebrity suicide were most important variables which influenced adolescents' suicidal ideation. These results reinforce importance of understanding adolescents' suicidal ideation with eco-system perspective. Moreover, prevention programs for adolescents' suicide require differentiated strategies and tactics according to the target group characteristics.

  • MicroRNA expression is a research focus in studies of tumors. This article concentrates attention on potential links between tumors caused by mouse mammary tumor virus (MMTV) and human breast cancer, in order to provide theoretical basis for using mouse model to search for miRNA effects mediated by Wnt/beta-catenin signaling in human breast cancer. By analyzing interactions between miRNAs and the Wnt/beta-catenin signaling pathway in breast cancer, we hope to casts light on more biological functions of miRNAs in the process of tumor formation and growth and to explore their potential value in cancer diagnosis, prognosis and treatment. Our endeavor aimed at providing theoretical basis for finding safer, more effective methods for treatment of human breast cancer at the miRNA molecular level.

Communication Competencies of Oncology Nurses in Malaysia

  • This paper reports on part of a large study to identify competencies of oncology nurses in Malaysia. It focuses on oncology nurses' communications-related competency. As an important cancer care team member, oncology nurses need to communicate effectively with cancer patients. Literature shows that poor communication can make patients feel anxious, uncertain and generally not satisfied with their nurses' care. This paper deliberates on the importance of effective communication by oncology nurses in the context of a public hospital. Four focus group discussions were used in this study with 17 oncology/cancer care nurses from Malaysian public hospitals. The main inclusion criterion was that the nurses had to have undergone a post-basic course in oncology, or have work experience as a cancer care nurse. The findings indicated that nurses do communicate with their patients, patients' families and doctors to provide information about the disease, cancer treatment, disease recurrence and side effects. Nurses should have good communication skills in order to build relationships as well as to provide quality services to their patients. The paper concludes by recommending how oncology nursing competencies can be improved.

Evidence for U.S. Preventive Services Task Force (USPSTF) Recommendations Against Routine Mammography for Females between 40-49 Years of Age

  • Breast cancer is the most common cancer among females, worldwide, accounting for 22.9% of all cancers (excluding non-melanoma skin cancer) in females. Mammography is a sensitive (77-95%) and specific (94-97%) screening method for breast cancer. Previously, females between the 40-50 years old were recommended to have mammograms every one to two years. However, based on current evidence, in 2009, USPSTF recommended that the decision to start regular, biennial screening mammography for females younger than 50 years should be an individual decision and take patient context into account, including the patient's values regarding specific benefits and harms. This decision was based on findings regarding radiation exposure, false-positive and false-negative rates, over-diagnosis, and pain and psychological responses. The goal of this paper is to focus on evidence for updating the U.S. Preventive Services Task Force (USPSTF) recommendation against routine mammography for females between 40-49 years of age.

  • Hepatocellular carcinoma (HCC) is the most frequent type of malignant liver tumor and a high impact health problem worldwide. The prevalence of HCC is particularly high in many Asian and African countries. Some HCC patients have no symptoms prior to diagnosis and many of them therefore present at late stage and have a grave prognosis. The well-established causes of HCC are chronic hepatitis B virus (HBV) or chronic hepatitis C virus (HCV) infection or alcoholic cirrhosis and nonalcoholic steatohepatitis. The Barcelona Clinic Liver Cancer (BCLC) Staging System remains the most widely used for HCC management guidelines. To date, the treatments for HCC are still very challenging for physicians due to limited resources in many parts of the world, but many options of management have been proposed, including hepatic resection, liver transplantation, ablative therapy, chemoembolization, sorafnib and best supportive care. This review article describes the current evidence-based management of HCC with focus on early to advance stages that impact on patient overall survival.

지역사회 중년의 당뇨병 지식과 건강증진행위 사이의 자기효능감의 매개효과 : 성별차이를 중심으로 (The Mediating Effect of Self-efficacy in the Relationship between Diabetes Knowledge and Health Promoting Behaviors: Focus on Gender Differentiation)

  • Purpose: This study examined diabetes knowledge, self-efficacy, and health promoting behaviors, and investigated factors affecting health-promoting behaviors of middle-aged people. Methods: From August to September 2014, a convenience sample of 264 subjects was recruited from B city and G province. The data analysis was done by ${\chi}^2$ test, t-test, Pearson's correlation coefficient, and stepwise multiple regression. Results: Diabetes knowledge was $11.3{\pm}3.63$ in men and $12.7{\pm}3.74$ in women. The average levels of self-efficacy and health promoting behaviors of subjects were similar with other middle-aged individuals. Diabetes knowledge, self-efficacy, and health promoting behaviors had a positive correlation with each other in both men and women. To test the mediating effect of self-efficacy in the relationship between diabetes knowledge and health promoting behaviors, a Sobel test was performed and Z-scores of 3.698(p<.001) in men and 2.748(p=.006) in women were obtained. Conclusion: Diabetes prevention education is recommended for the middle-aged community. When developing diabetes prevention program for middle-aged people, such programs should consider self-efficacy, especially in women.

Structural Factors of the Middle East Respiratory Syndrome Coronavirus Outbreak as a Public Health Crisis in Korea and Future Response Strategies

  • The recent Middle East respiratory syndrome coronavirus (MERS-CoV) outbreak has originated from a failure in the national quarantine system in the Republic of Korea as most basic role of protecting the safety and lives of its citizens. Furthermore, a number of the Korean healthcare system's weaknesses seem to have been completely exposed. The MERS-CoV outbreak can be considered a typical public health crisis in that the public was not only greatly terrorized by the actual fear of the disease, but also experienced a great impact to their daily lives, all in a short period of time. Preparedness for and an appropriate response to a public health crisis require comprehensive systematic public healthcare measures to address risks comprehensively with an all-hazards approach. Consequently, discussion regarding establishment of post-MERS-CoV improvement measures must focus on the total reform of the national quarantine system and strengthening of the public health infrastructure. In addition, the Korea Centers for Disease Control and Prevention must implement specific strategies of action including taking on the role of "control tower" in a public health emergency, training of Field Epidemic Intelligence Service officers, establishment of collaborative governance between central and local governments for infection prevention and control, strengthening the roles and capabilities of community-based public hospitals, and development of nationwide crisis communication methods.

Regulatory Network of MicroRNAs, Host Genes, Target Genes and Transcription Factors in Human Esophageal Squamous Cell Carcinoma

  • Abnormally expressed microRNAs (miRNAs) and genes have been found to play key roles in esophageal squamous cell carcinoma (ESCC), but little is known about the underlying mechanisms. The aim of this paper was to assess inter-relationships and the regulatory mechanisms of ESCC through a network-based approach. We built three regulatory networks: an abnormally expressed network, a related network and a global network. Unlike previous examples, containing information only on genes or miRNAs, the prime focus was on relationships. It is worth noting that abnormally expressed network emerged as a fault map of ESCC. Theoretically, ESCC might be treated and prevented by correcting the included errors. In addition, the predicted transcription factors (TFs) obtained by the P-match method also warrant further study. Our results may further guide gene therapy researchers in the study of ESCC.

Status of Helicobacter pylori Infection among Migrant Workers in Shijiazhuang, China

  • Background: Helicobacter pylori infection leads to many upper gastrointestinal diseases. Migrant workers are the main part of floating population in China. However, up to now, their health status has not been a focus of attention. Methods: In order to assess the status of H. pylori infection among migrant workers in Shijiazhuang, over five years we interviewed 324 individuals between 2007 and 2011. Each underwent a rapid urease test to identify H. pylori infection and socio-demographic indicators were collected using a survey questionnaire. Results: Our results showed that family income (P = 0.003), dietetic hygiene (P = 0.005), education (P = 0.004) and marital status (P = 0.007) were associated with H. pylori infection. Conclusion: We found that migrant workers had little basic knowledge of H. pylori and their prevalence of infection remains high. Therefore, we need to promote education and awareness of H. pylori and to ensure access to diagnosis and treatment for infected workers.
