Background: Chronic obstructive pulmonary disease (COPD) is a major cause of chronic morbidity and mortality throughout the world and is the only major disease that is continuing to increase in both prevalence and mortality. The second Korean National Health and Nutrition Survey revealed that the prevalence of COPD in Korean subjects aged ${\geq}45$ years was 17.2% in 2001. Further surveys on the prevalence of COPD were not available until 2007. Here, we report the prevalence of spirometrically detected COPD in Korea, using data from the fourth Korean National Health and Nutrition Survey (KNHANES IV) which was conducted in 2007~2009. Methods: Based on the Korean Statistical Office census that used nationwide stratified random sampling, 10,523 subjects aged ${\geq}40$ years underwent spirometry. Place of residence, levels of education, income, and smoking status, as well as other results from a COPD survey questionnaire were also assessed. Results: The prevalence of COPD (defined as forced expiratory volume in 1 sec/forced vital capacity <0.7 in subjects aged ${\geq}40$ years) was 12.9% (men, 18.7%; women, 7.5%). In total, 96.5% of patients with COPD had mild-to-moderate disease; only 2.5% had been diagnosed by physicians, and only 1.7% had been treated. The independent risk factors for COPD were smoking, advanced age, and male gender. Conclusion: The prevalence of COPD was 12.9% in the KNHANES IV data. Most patients with COPD were undiagnosed and untreated. Based on these results, a strategy for early COPD intervention is warranted in high risk subjects.

Objectives: The aim of this study was to measure income differences in smoking prevalence at the district level and to investigate correlations among area deprivation, smoking prevalence, and income differences in smoking prevalence, stratified by urbanity. Methods: Data were pooled from the Community Health Survey data of South Korea between 2008 and 2014. The age-standardized prevalence of smoking and its interquintile income differences were calculated. We conducted correlation analyses to investigate the association of the deprivation index with smoking prevalence and interquintile differences in smoking prevalence. Results: Across 245 districts, the median prevalence of smoking in men was 45.9% (95% confidence interval [CI], 43.4 to 48.5%), with an interquartile range (IQR) of 4.6% points. In women, the median prevalence was 3.0% (95% CI, 2.4 to 3.6%) and IQR was 1.6% points. The median interquintile difference in smoking prevalence was 7.4% points (95% CI, 1.6 to 13.2% points) in men and 2.7% points (95% CI, 0.5 to 4.9% points) in women. The correlation coefficients for the association between the deprivation index and smoking prevalence was 0.58, 0.15, -0.22 in metropolitan, urban, and rural areas, respectively, among men, and 0.54, -0.33, -0.43 among women. No meaningful correlation was found between area deprivation and interquintile difference in smoking prevalence. The correlation between smoking prevalence and interquintile difference in smoking prevalence was more evident in women than in men. Conclusions: This study provides evidence of geographical variations in smoking prevalence and interquintile difference in smoking prevalence. Neither smoking prevalence nor the deprivation index was closely correlated with interquintile income difference in smoking prevalence. Measuring inequalities in smoking prevalence is crucial to developing policies aimed at reducing inequalities in smoking.
The aim of this study was to assess a dietary status and to examine the factors related to the prevalence of metabolic syndrome in Korean children and adolescents. For this study, 5,576 subjects aged 10~18 years, who participated in the health and dietary intake survey (24h recall method) of the 2007~2013 Korea National Health and Nutrition Examination Survey (KNHANES), were sampled. The five components for the diagnosis of metabolic syndrome in the subjects were taken from the modified NCEP-ATP III. The total prevalence of metabolic syndrome among the subjects was 4.6%. The total prevalences of the metabolic syndrome components among the subjects were central obesity 8.4%, hypertriglyceridemia 18.8%, low HDL-cholesterol 13.4%, hypertension 22.4%, and hyperglycemia 5.2%. The gender, age, weight status, frequency of daily meals and eating-out frequency of subjects affected the prevalence of metabolic syndrome. Based on these results, There should be to improve the dietary guidelines and nutrition education to decrease the prevalence of metabolic syndrome for Korean children and adolescents.
Vitamin D deficiency is a risk factor for metabolic syndromes. We examined whether vitamin D deficiency altered the prevalence of cardiovascular disease (CVD) in older Koreans. Cross-sectional analysis of data from the Korean National Health and Nutrition Examination Survey IV 2008-2009 was used to examine the association between serum 25-hydroxyvitamin D (25(OH)D) levels and the prevalence of CVD in a representative population-based sample of 5,559 men and women aged ${\geq}50$ years. CVD was defined as angina pectoris, myocardial infarction, or stroke. The prevalence of CVD (7.0%) in the older Korean population was lower than that in the older US population, although average serum 25(OH)D levels were much lower in the Korean population. Additionally, serum 25(OH)D levels did not differ significantly between the CVD and non-CVD groups. However, subjects in the lowest category (< 25 nmol/l) of serum 25(OH)D level had the greatest prevalence of CVD, about two-fold higher than subjects in the highest category (> 75 nmol/l), after adjusting for age, gender, body mass index, education level, residence location, and region. The prevalence of other risk factors for CVD, including higher waist circumference, fasting glucose, low-density lipoprotein (LDL) cholesterol, and triglyceride levels and lower high-density lipoprotein (HDL) cholesterol levels, was also higher among subjects in the lowest category than among those in the highest category. In conclusion, low serum 25(OH)D may be an independent risk factor for CVD in older Koreans.
Objectives: Self-reported anthropometric values, such as height and weight, are used to calculate body mass index (BMI) and assess the prevalence of obesity among adolescents. The aim of this study was to evaluate the validity of selfreported height, weight, and BMI of the Korea Youth Risk Behavior Web-based Survey questionnaire. Methods: A convenience sample of 137 middle school students and 242 high school students completed a selfadministered questionnaire in 2008. Body height and weight were directly measured after self-reported values were obtained from the questionnaire survey. Sensitivity, specificity, and kappa statistics were computed in order to evaluate the validity of the prevalence of obesity (BMI $\geq$ 95th percentile or $\geq$$25;kg/$m^2$) based on self-reported data. Results: Self-reported weight and BMI tended to be underestimated. Self-reported height tended to be overestimated among middle school females and high school males. Obese adolescents tended to underestimate their weight and BMI and overestimate their height more than non-obese adolescents. The prevalence estimate of obesity based on selfreported data (10.6%) was lower than that based on directly measured data (15.3%). The estimated sensitivity of obesity based on self-reported data was 69.0% and the specificity was 100.0%. The value of kappa was 0.79 (95% confidence interval, 0.70 - 0.88). Conclusions: This study demonstrated that self-reported height and weight may lead to the underestimation of BMI and consequently the prevalence of obesity. These biases should be taken into account when self-reported data are used for monitoring the prevalence and trends of obesity among adolescents nationwide.
Objectives: The purpose of this study was to estimate the prevalence of metabolic syndrome (MS), metabolic abnormalities, and nutrient intakes in Korea children and adolescents using the Korea National Health and Nutrition Examination Survey from 2008. Methods: A sample of 838 children and adolescent males (n=442) and females (n=396) aged 10-18 was used from the 2008 Korea National Health and Nutrition Examination survey. The diagnosis of the metabolic syndrome subjects was adapted from modified National Cholesterol Education Program-Adult Treatment Panel III by Ford. To compare nutrient intakes, we used a judgment sampling. The first group was composed of all children and adolescents (n=46) with MS. The second one along with the first group had children and adolescents with the same age, sex, and body mass index (BMI) but without MS (n=46). The control group like the first two had children and adolescents with same sex and same age but with normal BMI and without MS (n=46). Results: In this randomized controlled controlled trial, the prevalence of the metabolic syndrome was 5.8%. The risk factors was associated with the MS were abdominal obesity 9.4%, hypertriglyceridemia 25.0%, low HDL-cholesterol 10.3%, hypertension 23.4%, and hyperglycemia 7.1%. Among metabolic abnormalities, blood pressure was significantly affected by sex, age and obesity. On the other hand, HDL-cholesterol, triglycerides, and waist circumference were directly linked to obesity. There were no significant differences in nutrient intakes among the three groups. Conclusions: The prevalence of MS was higher in children (10-11 years old) than in adolescents (12-18 years old). There was a difference in hypertension among risk factors by gender, and there were no significant differences in nutrient intakes.
Background: Lower-limb varicose veins (VVs) are common and known to have a higher prevalence among people who work in occupations requiring prolonged standing. In the Republic of Korea, however, VV-related occupational factors have seldom been examined. This study was conducted to assess the prevalence of VVs among nurses, an occupational group considered to be at high risk of VVs, and determine the occupational risk factors of prolonged standing. Methods: Between March and August 2014, a questionnaire survey coupled with Doppler ultrasonography was conducted on the nurses working at a university hospital. Results: A total of 414 nurses participated in the survey and diagnostic testing. From the survey analysis and test results, the prevalence of VVs in nurses was estimated to be 16.18%. Significant factors for venous reflux were age [odds ratio (OR) = 1.06, 95% confidence interval (CI) = 1.03-1.10], pregnancy (OR = 2.15, 95% CI = 1.17-3.94), and delivery (OR = 2.02, 95% CI = 1.08-3.78). The statistical significance of these factors was verified after risk adjustment for sociodemographic factors (OR = 3.40, 95% CI = 1.27-9.09). Conclusion: Factors significantly associated with venous reflux were increasing age and prolonged working hours (${\geq}4hours$) in a standing position (OR = 2.80, 95% CI = 1.08-7.25), even after risk adjustment for sociodemographic factors. This study is significant in that an objective diagnosis of VVs preceded the analysis of the risk factors for VV incidence, thus verifying objectively that VVs are associated with occupations requiring prolonged hours of working in a standing position.
Objectives: This study examined trends in inequality in cigarette smoking prevalence by income according to recent anti-smoking policies in Korea. Methods: The data used in this study were drawn from three nationally representative surveys, the Korea National Health and Nutrition Examination Survey, the Korea Community Health Survey, and the Social Survey of Statistics Korea. We calculated the age-standardized smoking prevalence, the slope index of inequality, and the relative index of inequality by income level as a socioeconomic position indicator. Results: Smoking prevalence among men decreased during the study period, but the downward trend became especially pronounced in 2015, when the tobacco price was substantially increased. Inequalities in cigarette smoking by income were evident in both genders over the study period in all three national surveys examined. Absolute inequality tended to decrease between 2014 and 2015 among men. Absolute and relative inequality by income decreased between 2008 and 2016 in women aged 30-59, except between 2014 and 2015. Conclusions: The recent anti-smoking policies in Korea resulted in a downward trend in smoking prevalence among men, but not in relative inequality, throughout the study period. Absolute inequality decreased over the study period among men aged 30-59. A more aggressive tax policy is warranted to further reduce socioeconomic inequalities in smoking in young adults in Korea.
Purpose: The aim of the study was to assess and identify gender differences in factors associated with prevalence, awareness, and treatment of osteoporosis. Methods: Data for 3,071 men and 3,635 women ($age{\qeq}50$) from the Korea National Health and Nutrition Examination Survey 2008~2011 were included. Osteoporosis was defined by World Health Organization T-score criteria. Impact factors and odds ratios were analysed by gender using multivariate logistic regression. Results: Osteoporosis prevalence rates were 7.0% in men and 40.1% in women. Osteopenia rates were 45.5% and 46.0% respectively. Among respondents with osteoporosis, 7.6% men and 37.8% women were aware of their diagnosis. Also 5.7% men with osteoporosis and 22.8% women were treated. Higher prevalence was found among respondents who were older, at lower socioeconomic levels, with lower body mass index and shorter height in both genders, and among women with fracture history, and non-hormonal replacement therapy. Awareness and treatment rates for the risk groups were similar compared to the low risk controls for both genders. Fracture history increased awareness and treatment rates independently for both genders. Women with perceived poor health status and health screening had increased awareness and treatment rates, but not men. Conclusion: Results indicate that postmenopausal women have a higher prevalence of osteoporosis than men and awareness and treatment rates were higher than for men. Despite gender difference in prevalence, osteoporosis was underdiagnosed and undertreated for both genders. Specialized public education and routine health screenings according to gender could be effective strategies to increase osteoporosis awareness and treatment.
The purpose of this study was to examine the influence of maternal socioeconomic status, maternal oral health behaviors, and oral health behaviors of children, on the prevalence of early childhood caries in children aged 5 years. The Korean National Health and Nutrition Examination Survey data collected between 2007 and 2014 were applied to this study, and the study sample included 824 children who received oral examinations and participated in the health behavior survey. The factor that affected the prevalence of early childhood caries were confirmed by maternal and child factors. The data were analyzed using multiple linear regression. The mothers' age, income level, and job status affected the prevalence of early childhood caries. There was a significant difference in the analysis considering the factors of motherhood and children in the prevalence of early childhood caries according to mother's age, education level, income level, and the child's oral examination. The prevalence of early childhood caries was higher in children who received oral examinations than in those who did not. When the mother's educational level was higher than college education, it was found that the index of child, i.e., there was a difference in the prevalence of early childhood caries according to the mother's educational and income levels. These results indicate that maternal socioeconomic characteristics are correlated with the oral health of children. Therefore, oral health education programs that include mothers for the prevention of early dental caries in children may improve the dental health of children. In addition, specific oral health policies are necessary to address the differences in the oral health between the income groups.
