A Retrospective Analysis of Characteristics of Probiotics Associated with Invasive Bacterial Infections in Children (소아청소년에서 발생한 유산균 제제에 포함된 균주에 의한 침습성 감염증의 후향적 분석)

  • Purpose: The purpose of this study was to analyze the clinical features and risk factors of invasive infections caused by Lactobacillus spp. and Saccharomyces spp., components of commercially available probiotics. Methods: We analyzed demographic and clinical data from children ${\leq}18$ years of age with an invasive infection caused by Lactobacillus spp. or Saccharomyces spp. at the Asan Medical Center Children's Hospital from January 1998 to June 2016. Probiotic consumption data were also analyzed. Results: During the study period, a total of 24 episodes of invasive infections were caused by Lactobacillus spp. (n=16) and Saccharomyces cerevisiae (n=8). Along with the increase of probiotic use (755,594 [days/1,000 patient-admission days] in 2001 to 2005, 1,444,066 in 2006 to 2010, and 6,904,736 in 2011 to 2016), the incidence of probiotic-associated invasive infection increased ($R^2=0.70$). The median age of the patients was 1.8 years (range, 2 months to 17 years), and most of them had underlying medical conditions. The 30-day mortality rate was 20.8% (5/24), and 11 (45.8%) of these patients resulted from a severe invasive infection. We determined the risk factors for invasive infection to be: previous intensive care unit stay (odds ratio [OR], 3.0; 95% confidence interval [CI], 1.5 to 6.1] and the presence of a central venous catheter (OR, 2.2; 95% CI, 1.2 to 4.3). Conclusions: Although the probiotic-associated invasive infections rarely occurred in children, the incidence has increased along with probiotic pressure. Judicious use of probiotics is mandatory, especially in young children with underlying medical conditions and continuous surveillance will be needed to minimize the safety concerns.

Effect of Electrolytic Material Feeding on Blood and Carcass Traits of Broiler under Intense Heat Condition in Summer (폭염 시 육계 출하 전 전해질 급여가 닭고기 도체 및 혈액에 미치는 영향)

  • This study was conducted to investigate the effect of feeding the electrolytic materials on blood and carcass traits of broiler during transportation exposed under intense heat condition in summer. The broilers were selected on the day when the outside temperature was about $32^{\circ}C$ to provide heat stressed environment. Broilers reared for 33 d were selected and fed with the electrolytic materials ($NaHCO_3$, NaCl, KCl) for 2 days. Treatments were as follows; feeding the underground water for control, $NaHCO_3$ (1.0%) + NaCl (0.5%) for treament 1, KCl (0.5%) + NaCl (0.5%) for treatment 2, KCl (1.0%) + NaCl (0.5%) treatment 3, KCl (0.5%) + $NaHCO_3$ (1.0%) + NaCl (0.5%) for treatment 4 and KCl (1.0%) + $NaHCO_3$ (1.0%) + NaCl (0.5%) for treament 5. pH of chicken meat increased for treatments group of electrolytic material, especially, that of treatment 3 was highest when compared to the other treatments. The frequency rate (%) of $1^+$ quality grade were 33.3, 60.0 and 83.3% at control, treatment 3, 4 and treatment 5, respectively. Occurrence rates of PSE were 50% for control and 13.3% for treatment 5. Corticosterone increased at the post-harvest period compared to the pre-harvest period of broiler and have small disparity between pre-and post-harvest only except treatment 3 when compared to control. $pCO_2$ partial pressure of blood at the pre-harvest period was low in all treatments by heat stress, the disparity value of control was high for control, and those of treatment 4 and 5 were low compared to other treatments.

Protective Effects of Adenosine-enriched Cardioplegic Solution in Ischemic Myocardium (Adenosine을 함유한 심정지액의 심근보호 효과)

  • Ischemic myocardial damage is inevitable to cardiac surgery. Myocardial damage after initiation of reperfusion through the coronary arteries is one of the most important determinants of a successful surgery. Adenosine is a potent vasodilator, and is also known to induce rapid cardioplegic arrest by its property of antagonizing cardiac calcium channels and activating the potassium channel. Thus, we initiated this study with adenosine to improve postischemic recovery in the isolated rat heart. We tested the hypothesis that adenosine could be more effective than potassium in inducing rapid cardiac arrest and enhancing postischemlc hemodynamic recovery. Isolated rat hearts, connected to the Langendorff appratus, were perfused with Krebs-Henseleit buffer and all hearts were subjected to arrest for 60 minutes. Three groups of hearts were studied according to the composition of cardioplegic solutions : Group A (n=10), adenosine 10mmo1/L+potassium free modified St. Thomas cardioplegia : Group B (n=10), adenosine 400mo1/L+S1. Thomas cardioplegia:Group C(control, n=10), St. Thomas cardioplegia. Adenosine-treated groups (group A & B) resulted in more rapid cardiac arrest than control group (C) (p< 0.01). There was greater improvement in recovery of coronary blood flow at 20 and 30 minutes of reperfusion in group A and at 20 minutes in group B when compared with control group(p<0.01). Recovery of systolic blood pressure at 10 minutes after reperfusion in group A and B was significantly superior to that in group C (p<0.01). Recovery of dp/dt at 10 minute after reperfusion in group A was also significantly superior to group C (p<0.05). Group A and B showed better recovery rates than control group in aortic blood flow, cardiac output, and heart rate, but there were no statistical differences. CPK levels of coronary flow in group A were significantly low (p< 0.01). We concluded that adenosine-enriched cardioplegic solutions have better effects on rapid cardiac arrest and postischemic recovery when compared with potassium cardioplegia.

Study on Nucleation and Evolution Process of Ge Nano-islands on Si(001) Using Atomic Force Microscopy (AFM을 이용한 Si (001) 표면에 Ge 나노점의 형성과 성장과정에 관한 연구)

  • The nucleation and evolution process of Ge nano-islands on Si(001) surfaces grown by chemical vapor deposition have been explored using atomic force microscopy (AFM). The Ge nano-islands are grown by exposing the substrates to a mixture of gasses GeH4 and H2 at pressure of 0.1-0.5Torr and temperatures of $600-650^{\circ}C$. The effect of growth conditions such as temperature, Ge thickness, annealing time on the shape, size, number density, and surface distribution was investigated. For Ge deposition greater than ${\sim}5$ monolayer (ML) with a growth rate of ${\sim}0.1ML/sec$ at $600^{\circ}C$, we observed island nucleation on the surface indicating the transition from strained layer to island structure. Further deposition of Ge led to shape transition from initial pyramid and hut to dome and superdome structure. The lateral average size of the islands increased from ${\sim}20nm$ to ${\sim}310nm$ while the number density decreased from $4{\times}10^{18}$ to $5{\times}10^8cm^{-2}$ during the shape transition process. In contrast, for the samples grown at a relatively higher temperature of $650^{\circ}C$ the morphology of the islands showed that the dome shape is dominant over the pyramid shape. The further deposition of Ge led to transition from the dome to the superdome shape. The evolution of shape, size, and surface distribution is related to energy minimization of the islands and surface diffusion of Ge adatoms. In particular, we found that the initially nucleated islands did not grow through long-range interaction between whole islands on the surface but via local interaction between the neighbor islands by investigation of the inter-islands distance.

  • The influence of high temperature storage on the chemical composition and color intensity of the concentrated red ginseng extract(RGE) was investigated. The concentrated RGE was prepared by extraction of red ginseng tails with water and concentrated under reduced pressure. Changes in free sugars, saponin patterns and brown color intensity were measured during 96 hours of heat treatment at various temperature. A decrease in the contents of glucose, fructose and sucrose was resulted as the brown color intensity increased during the storage. The sugar contents and color intensity showed rapid initial change followed by slowing down at higher temperature. A significant relationship was found between sugar content and browning rate. The saponin pattern measured by high performance liquid chromatography, particularly in the region of protopanaxtriol, was also affected significantly. The peak heights of ginsenoside -Re and $-Rg_1$ were decreased while those of ginsenoside $-Rg_2$ and -Rh group were increased.

Morphological Characteristics of Seed Coat and Enzyme Activity of Aleurone Layers in Soybeans (콩 종피의 형태적 특성과 호분층 세포의 효소활성 연구)

  • Soybean seed coat consisted of three layers, and the aleurone layer was attached to the innermost part of seed coat. It showed the different morphological characteristics with single cell layer compared with many cell layers in barley aleurone layer. The structural difference in aleurone cell among varieties was not detected in this experiment. The hole of middle part of hilum consisted of net formed material in order to pass water and gas. In the experiment, it was not studied whether the varieties with hard seed consist of the same structure or not, but the detailed study on this would be necessary. The activity of acid phosphatase showed a tendency to increase in process of imbibition in distilled water. There was no significant difference in the enzyme activity among the varieties tested, but the enzyme activity of Danyoupkong was slightly higher than that of Hwanggeumkong. In germinability, Danyoupkong is higher than Hwangkeumkong, so it might be attributed to the higher activity. There was no difference in acid phosphatase activity between released from the aleurone cell and accumulated in the aleurone cell from 6 to 12 hours incubation of the medium in the absence of GA$_3$, but the difference was detected after 12 hours incubation. And enzyme activity was the highest in the 18 hours incubation. The increase in the release of acid phosphatase from the aleurone cell would be considered as passive diffusive effect due to the increase in turgo pressure of aleurone cell. The acid phosphatase released from aleurone layer increased greatly after 24 hours incubation of the medium in the presence of GA$_3$ and the accumulation within the aleurone cell decreased linearly after 18 hours incubation. The result indicates that GA$_3$ enhance the rate of enzyme release from aleurone layer, suggests that the aleurone cell wall be digested by the introduction of GA and the digested wall act as the channels for enzyme release.

Numerical Study on Spring-Neap Variability of Net Volume Transport at Yeomha Channel in the Han River Estuary (한강하구 수로별 순 수송량과 대.소조기 변화에 따른 염하수로의 순 수송량 변동에 관한 수치해석적 연구)

  • The EFDC model with find grid resolution system connecting the Gyeong-Gi bay and Han River estuary was constructed to study on spring-neap variability of net volume transport at each channel of the Han River estuary. The simulation time of numerical model is 124 days from May to August, 2009 with freshwater discharge at Han, Imjin and Yeseong River. The calibration and verification of model results was confirmed using harmonic components of water level and tidal current. The net volume transport was calculated during 30 days with normal freshwater conditions at Seokmo channel and Yeomha channel around Ganghwado. The ebbing net volume transport of 44% and 56% is drained into Gyeong-Gi bay through Yeomha and Seokmo channel, respectively. The ebbing net volume transport nearby Seodo at Yeomha channel convergence flooding net volume transport at Incheon harbor, and drain (westward direction) through channel of tidal flat between Ganghwado and Yeongjongdo to the Gyeong-Gi bay. The averaged net volume transport during 4 tidal cycles was compared to variation of spring-neap periods of the Yeomha channel. The convergence position is moved up- and down-ward according to spring-neap variability. The movement of the convergence zone is appeared because 1) increasing of discharged rate tidal flat channel between Ganghwado and Yeongjongdo at the spring period, 2) The growth of barotropic forcing with downward direction at the spring tide, and 3) The strength of the baroclinic pressure gradient is greater than spring with mixing processes.

Acupuncture for Prehypertension and Stage 1 Hypertension in Postmenopausal Women: Protocol for a Randomized Controlled Pilot Trial (폐경 후 여성의 전단계 및 1기 고혈압에 대한 침 치료: 다기관 무작위 대조 예비연구)

  • Objectives : This study aims to evaluate the effectiveness and safety of acupuncture and explore the appropriate number of treatment for postmenopausal women diagnosed with prehypertension and stage 1 hypertension. Methods : A 4-arm randomized open label pilot trial will be performed at 2 centers. Sixty participants will be divided into 2 treatment groups and 2 control groups. Treatment groups will receive acupuncture at 8 points(bilateral GB20, LI11, ST36, SP6) for 4 weeks(treatment group A, 10 total sessions) or 8 weeks(treatment group B, 20 total sessions), while maintaining usual care. Control groups will not receive acupuncture but will be under usual care for 16 weeks(control group C) or 20 weeks(control group D). Each patient's living habits will be corrected and drugs that may affect blood pressure(BP) will be prohibited. Treatment group A and control group C will be evaluated at 4, 8, 12, and 16 weeks after randomization, while treatment group B and control group D will be evaluated at 4, 8, 12, 16, and 20 weeks after randomization. The major outcome variable is the magnitude of change in diastolic BP levels at 4 weeks after randomization; auxiliary outcome variables are (1) diastolic BP change at 8, 16, and 20 weeks, (2) systolic BP change, (3) BP control rate, (4) lipid profiles, and (5) high-sensitivity C-reactive protein. Patient safety will be assessed at every visit. Results and Conclusions : The study findings may help develop evidence for the effectiveness and safety of acupuncture for BP control.

An Association of Changed Levels of Inflammatory Markers with Hematological Factors during One-time Aerobic Exercise in Twenty-aged Young Men (20대 젊은이들에 있어 1회성 유산소운동 시 염증 표지자와 혈액변인들 간의 상관관계)

  • This study was carried out on thirty men to define the association of inflammatory markers with physiological factors on one-time aerobic exercise (for 15 min. Post-exercise interleukin-6 (IL-6), mean corpuscular hemoglobin concentration (MCHC), heart rate (HR), systolic blood pressure (SBP), and pulsatility and resistance index of middle cerebral artery (PI and RI, respectively) levels were elevated compared to those measured pre-exercise. Total leukocyte and platelet counts, high-sensitivity C-reactive protein (hs-CRP), free radical (FR), and low density lipoprotein cholesterol (LDL) levels tended to decrease after exercise. Pre-exercise IL-6 levels were positively correlated with pre-exercise SBP levels, while post-exercise IL-6 level was positively correlated with post-exercise PI and RI levels. Post-exercise, hs-CRP levels were negatively related to SBP and HR. Pre-exercise, FR levels were positively associated to SBP, DBP, and HR. Post-exercise FR levels were negatively related to the post-exercise blood flow velocity in middle cerebral artery. Pre-exercise erythrocyte indices (RBC, MCV, MCH, and MCHC levels) were in inverse proportion to pre-exercise IL-6 levels. Post-exercise FR levels were inversely related to post-exercise total leukocyte, lymphocyte, monocyte, and MCH levels. Pre-exercise $Mg^{++}$ levels were in inverse proportion to pre-exercise IL-6, hs-CRP, or FR levels. These findings suggest that one-time aerobic exercise offers a significant relationship between inflammatory markers and some biochemical markers or electrolytes. Further studies need to be carried out for investigation of differences between genders or age groups following one-time or regular aerobic exercise.

Effects of a 12-week, school-based obesity management program on obese primary school children (12주간의 학교 비만 관리 프로그램이 초등학교 비만 아동에 미치는 효과)

  • Purpose : This study was designed to determine the effects of a school-based obesity-management program on obese primary school children. Methods : A total of 995 children (6-2 years old) in a primary school were screened in March 2008, and of those, 101 obese students (44 boys and 57 girls, body mass index (BMI) ${\geq}95$ percentile) were enrolled for a study group. The schoo-lbased, obesity management program, which includes physical exercise and nutritional education, was conducted as part of an extracurricular program for 12 weeks. The measurement of height, weight, waist circumference, blood pressure (BP), and bioelectrical impedance analysis (BIA) was performed before and after the program.Results : Height and weight increased significantly (P <0.05). The BMI and obesity index decreased significantly (P <0.01). Systolic and diastolic BP decreased significantly (P <0.01). BMI decreased in 61.4% of boys and 66.7% of girls. Protein and basal metabolic rate (BMR) increased significantly on the BIA (P <0.01). Fat decreased significantly (P <0.05). The total body water (TBW) and percent body fat (PBF) decreased significantly (P <0.01). The changes in protein, fat, TBW, PBF, and BMR significantly correlated to the change in BMI (P <0.05). In a multiple logistic regression analysis, BMI change was significantly correlated to the changes in protein and fat content (P <0.01). Conclusion : The school-based obesity management program is a very effective way to manage obesity for obese primary school children.
