수용개작방법을 활용한 욕창간호 실무지침 개발 (Development of Pressure Ulcer Management Guideline by Adaptation Process)

  • Purpose: This study was done to develop an evidence-based nursing clinical practice guideline (PU CPG) for pressure ulcer prevention and management in Korea. Methods: The guideline adaptation process was used and conducted according to the guideline adaptation manual developed by Gu et al. (2012) which consists of three main phases and 9 modules including a total of 24 steps. Results: The newly developed PU CPG included the introduction, pressure ulcers, summary of recommendations, recommendations, references, appendices, and glossary. The total number of recommendations was 148 in 4 sections (organizational policy, assessment, pressure ulcer prevention and management, and education) and 16 domains. Of the recommendations 4.7% were graded A, 16.9%, B, and 78.4%, C. Conclusion: Results indicate that this new PU CPG is an evidence-based practice guidance for pressure ulcer prevention and management and can be recommended for dissemination and utilization by nurses nationwide to improve the quality of pressure ulcer prevention and management. Regular revision is recommended.

체력관리 수업이 여자 대학생의 신체구성, 혈압 및 건강관련 체력에 미치는 영향 (The Effect of Fitness Management Class on Body Composition, Blood Pressure and Health-related Fitness in College Women)

  • 본 연구는 체력관리 수업이 비만도가 각기 다른 여자 대학생들의 신체구성, 혈압 및 건강관련 체력에 미치는 영향이 다른지에 대해 검증하는 것이다. 본 연구의 대상자는 본교 2012~2016년 9학기 동안 체력관리 수업을 수강한 여학생 317명을 대상으로 하였으며, 그룹 구분은 체질량지수와 체지방률(%) 기준으로 저체중, 마른 비만, 정상, 정상 비만 그리고 비만 그룹으로 구분하였다. 체력관리 수업은 저항성 운동과 유산소 운동의 복합운동으로 일일 75분, 주 2회로 구성하여 진행하였다. 본교의 체력관리 수업은 비만 그룹을 제외한 네 그룹의 체중, 체질량지수, 체지방율, 근육량 그리고 허리둘레를 긍정적으로 개선시키는 효과가 있는 것으로 나타났다. 하지만 혈압(SBP, DBP)과 건강관련 체력에서는 다섯 그룹 모두에서 대부분 긍정적으로 개선되는 효과가 있었다. 따라서 체력관리 수업은 비만 여자 대학생의 신체구성의 개선을 제외하고 여자 대학생들의 신체구성과 혈압의 개선 및 건강관련 체력을 향상시키는 것으로 나타났다.

고혈압-뜸 임상시험 참여자들의 혈압 관리행동에 대한 질적연구 (A Qualitative Study on Management Behaviors about Blood Pressure of Participants in Moxibustion Clinical Trial for Hypertension)

  • Objectives : The purpose of this study which adopted a qualitative method was to evaluate blood pressure management behaviors of pre- and stage 1 hypertension patients and to provide fundamental data for developing a strategy about proper oriental medical treatment through analyzing factors related to a moxibustion clinical trial. Methods : Semi-structured interviews that focused on personal experiences with hypertension and its management were conducted with 10 hypertensive patients. The interviews lasted for approximately 50 minutes. Results : Most participants recognized that the direct cause of high blood pressure was unhealthy behavior rather than inheritance. Thus, the hypertensive patient believed they could recover their blood pressure to a normal level through removing the direct cause of hypertension instead of taking drugs. The reasons for these statements were that the drugs for controlling hypertension are not natural and they may have side effects. On future management plan of patients, most of them will continue to keep moxibustion and healthy behavior in the well-controlled blood pressure group. However in the uncontrolled blood pressure group, there was an increasing tendency to begin medication. Conclusions : For developing a strategy for an individual approach to hypertension management, we need to develop a client-centered attitude and strategy. That is, we need to tailor our approach to individual cases to avoid generalizations and stereotyping when developing an adherence increasing strategy.

Work Pressure and Safety Behaviors among Health Workers in Ghana: The Moderating Role of Management Commitment to Safety

  • Background: safety and healthy working environment has received numerous research attention over the years. Majority of these researches seem to have been conducted in the construction industry, with little attention in the health sector. Nonetheless, there are couple of studies conducted in Africa that suggest pressure in hospitals. Therefore the aim of the study was to examine how pressure influence safety behavior in the hospitals. With reference to the relevance of safety behavior in primary health care delivery, there was the need for the study. Method: Data was obtained from 422 public hospital employees. Respondents were assured that all information would be kept confidential to increase the response rate and acquire more accurate information. Collection of questionnaires from participants took four weeks (20 working days), after which the data was analyzed. Results: The result of the study showed that work pressure correlated negatively with safety behavior. General safety climate significantly correlated positively with safety behavior and negatively with work pressure, although the effect size for the latter was smaller. Hierarchical regression analysis showed management commitment to safety to moderate the relationship between work pressure and safety behavior. Conclusion: When employees perceive safety communication, safety systems and training to be positive, they seem to comply with safety rules and procedures than voluntarily participate in safety activities.

서울지역 일부 노인집단에 대한 만성질환관리 교육의 효과 (The Effects of Education of Chronic Diseases Management for the Elderly Group in Parts of Seoul)

  • This study was conducted to evaluate the effects of health-behavioral change for the elderly group after community based education of chronic diseases management. We measured self recognition of health status, medication administration of hypertension and diabetes, regular check for blood pressure and blood sugar level, recognition of body indicators (weight, hight, blood pressure, blood sugar etc), knowledge level for chronic diseases management and smoking and alcohol habitation before and after education of chronic diseases management for participants. The subjects of this study consist of 432 people with community-dwelling Seoul citizen being active churches. Education programs designed essential parts of fundamental chronic diseases management, physical exercises for health promotion, diet and nutrition etc. All data collection completed for 5 months from Aug. 2008 to Dec. 2008 by trained surveyors via interview survey. The data obtained were analyzed using descriptive statistics, Wilcoxon Singed Rank test, McNemar test and Paired t-test. The results showed that self recognition of health status, knowledge level for chronic diseases management, recognition of body indicators were statistically significantly increased after the education of chronic diseases management. Also, blood pressure were statistically significantly decreased in elderly with hypertension and blood sugar were statistically significantly decreased in elderly of high-risk group. Based on these results, it was suggested that preventive education policy of chronic diseases management should be considered with priority coming true for successful aging society.

산업단지 고압매설배관 안전관리 향상방안 연구 (A Study on the Safety Management of High Pressure Underground Pipeline in Industrial Estate)

  • 국내산업단지 내 고압 매설배관은 장기사용배관이 많고 독성 가연성 불활성 등 고압가스는 물론 다양한 유해화학물질이 한도로 내에 복잡하게 매설되어있어 다양한 타공사 등 사고 발생 시 인접하여 매설된 다른 배관에 손상을 미칠 수 있어 높은 수준의 안전관리가 요구되고 있다. 이에 본 연구에서는 고압가스매설배관과 도시가스매설배관 안전관리실태를 심층비교분석을 통하여 고압가스 매설배관의 효율적인 안전관리방식 도출에 활용하고자한다.

운전석 체압분포 기반의 착좌전략 분류 (A Classification of Sitting Strategies Based on Seating Pressure Distribution)

  • Drivers' sitting strategies are considered in car seat design. Existing research has identified representative sitting strategies by visual inspection of seating pressure measurements collected from a small sample size of drivers. The present study recruited 20 female and 20 male participants, measured seating pressure, and classified sitting strategies by cluster analysis. The participants' sitting postures were classified based on seating pressure distribution into mid-back and scapular, mid-back and lumbar, and lumbar sitting strategies for the upper body and hip concentrated, hip and mid-thigh concentrated, and hip and mid-thigh distributed sitting strategies for the lower body. The effects of gender and occupant package layout (OPL) on sitting strategy were nout found significant. The identified sitting strategies would be of use for the design and evaluation of an OPL and a seat.

화재시 제연 및 피난을 위한 기능성 차압 조절장치 개발에 관한 연구 (The Development of a Functional Pressure Controller in Smoke Management Systems for the Fire Safety of Buildings)

  • The smoke and toxic gases generated from the building fires are proved to cause human death. Therefore, the necessity and significance of smoke control have been emphasized, and lots of studies for developing improved smoke management system have been carried out. In this study, the experiments were conducted to evaluated a function and performance in newly developed smoke management system using mechanical pressurization. As a result of this experiment, the differential pressure was 40Pa${\sim}$60Pa and the air velocity through the door was 0.7m/s between safety zone and fire zone. The functional pressure control equipment which could make proper pressure and maintain differential pressure between safety zone and fire zone was developed. And it will give a lot of helps to evacuation activity for peoples in building and fire fighting.

초고압 커먼레일 연료분사튜브 원재료 강성 최적화를 위한 인발 공정에서의 Die와 Plug 각도 변경에 따른 해석적 연구 (An Analytical Study by Variation of Die and Plug Angle in Drawing Process for the Strength Optimization of Ultra High Pressure Common Rail Fuel Injection Tube Raw Material)

  • The study is actively being performed to increase fuel injection pressure of common rail system among countermeasures to meet the emission regulation strengthen of the Diesel engine. The common rail fuel injection tube in such ultra high pressure common rail system has the weakest structural characteristics against vibration that is generated by fuel injection pressure and pulsation during engine operation and driving. Thus the extreme durability is required for common rail fuel injection tube, and the drawing process is being magnified as the most important technical fact for strength of seamless pipe that is the raw material of common rail tube. In this respect, we analyzed the characteristic of dimension and stress variation of the ultra high pressure common rail fuel injection tube by variation of Die and Plug angle in drawing process. Based on the analysis, we tried to obtain the raw material strength of common rail fuel injection tube for applying to the ultra high pressure common rail system. As a result, Plug angle is more important than entry angle of Die and we could obtain the target dimension and strength of the ultra high pressure common rail fuel injection tube through optimization of Plug angle.

욕창관리 모바일 어플리케이션 구조 설계 및 사용자 인터페이스 구현 (Design of Pressure Injury Management Mobile Application Structure and User Interface)

  • Purpose: This study aimed to design user interfaces of a mobile application for managing pressure injury patients in a long-term care hospital based on the user's needs. Methods: To reflect users' needs in the mobile application, the user interfaces in this study were designed in five steps: brainstorming and mind mapping, persona and scenario, needs list and priority, a draft version of flow chart and user interfaces and expert review. These steps were conducted with a step nurse at a long-term care hospital, a professor who majored in nursing informatics, a professor who had lots of research experiences about pressure injury and a wound ostomy continence nurse. Results: Two personas, scenarios and needs' lists were derived. Listed Needs included the followings; Accurate staging of pressure injury; Appropriate management by staging; Acquisition of professional knowledge about pressure injury; Acquisition of easy pressure injury information through text, picture and video; and Sharing pressure injury information in unit. The structure, menus and features of the pressure injury mobile application were visualized with user flow based on two personas' scenarios and needs' lists. Conclusion: Our study suggests and visualizes the key features of the 'Pressure Injury Guide', a pressure injury management mobile application for nurses in a long-term care hospital, which can be utilized by nurses, application developers, and related researchers.
