Purpose: To evaluate the inter-fractional position and respiratory reproducibility of lung and liver tumors using pressure conserving type(P-type) abdominal compressor in volumetric modulated arc therapy(VMAT). Materials and methods: Six lung cancer patients and three liver cancer patients who underwent VMAT using a P-type abdominal compressor were included in this study. Cone-beam computed tomography(CBCT) images were acquired before each treatment and compared with planning CT images to evaluate the inter-fractional position reproducibility. The position variation was defined as the difference of position shift values between target matching and bone matching. 4-dimensional cone-beam computed tomography(4D CBCT) images were acquired weekly before treatment and compared with planning 4DCT images to evaluate the inter-fractional respiratory reproducibility. The respiratory variation was calculated by the magnitude of excursions by breathing. Results: The mean ± standard deviation(SD) of overall position variation values, 3D vector in the three translational directions were 1.1 ± 1.4 mm and 4.5 ± 2.8 mm for the lung and liver, respectively. The mean ± SD of respiratory variation values were 0.7 ± 3.4 mm (p = 0.195) in the lung and 3.6 ± 2.6 mm (p < 0.05) in the liver. Conclusion: The use of P-type compressor in lung and liver VMAT was effective for stable control of inter-fractional position and respiratory variation by reproduction of abdominal compression. Appropriate PTV margin must be considered in treatment planning, and image guidance before each treatment are required in order to obtain more stable reproducibility

Temperatures of engine head and liner depend on many factors such as spray and combustion process, coolant passage flow and engine related structures. To estimate the temperature distribution of engine structure, multi-dimensional computational fluid dynamics (CFD) codes have been mainly adopted. In this case, it is of great importance to obtain the realistic wall temperature distribution of entire engine structure. In the present work, a CFD-FEM coupling methodology was presented to address this demand. This approach was applied to a real large-size marine diesel engine. CFD combustion and coolant flow simulations were coupled to FEM temperature analysis. Wall heat flux and wall temperature data were interfaced between combustion simulation and solid component temperature analysis via translator by a commercial CFD package named FIRE by AVL. Heat transfer coefficient and surface temperature data were exchanged and mapped between coolant flow simulation and FEM temperature analysis. Results indicate that there exists the optimum cell thickness near combustion chamber wall to reasonably predict the wall heat flux during combustion period. The present study also shows that the effect of cell refining on predicting in-cylinder pressure during combustion is negligible. Hence, the basic guidance on obtaining the wall heat flux needed for the reasonable CFD-FEM coupling analysis has been established. It is expected that this coupling methodology is a robust tool for practical engine design and can be applied to further assessment of the temperature distribution of other engine components.

We have analyzed 247 patients who were admitted to the 3rd ICU for intensive care between January 2003 and December 2003 to investigate characteristics of ICU patients and obtain better guidance and management in the ICU. The results are as follows: 1) The total number of patients was 247. The proportion of males was 59.1 %, of females was 40.9%, and the ratio of male to female was about 1.45:1. 2) The most prevalent age group is those in their seventies, the proportion is 33.6%. 3) The total admission days was 3,438days, and mean patient stay being 13.9days. 4) The total number of expired patients was 26, the mortality rate was 10.5%. The mortality in distribution by disease was higest in Cerebral infarction, rate was 53.8%. the proportion of raised intracranial pressure was 42.3%, of hemorrhagic transformation was 11.5%. 5) In distribution by admission channel, Oriental hospital E.R. was 60.7%, Oriental hospital ward was 38.5%, and etc. was 0.8%. 6) The number of patients of Cerebral infarction was 146(59.1%), being the highest in distribution by chief disease and the proportion of MCA territory infarction is highest in the Cerebral infarction(61.6%). 7) In distribution by Oriental Medicine therapy, the proportion of Acupuncture treatment was 15.8%, of Acupuncture treatment combination Herb medication was 78.5%, and of Acupuncture, Herb medication plus subcutaneous Acupuncture treatment was 5.7%.
It's used to be said that tsunami is a rare event. The recurrence time of tsunami in Sumatra area is approximately 230 years as CalTech Research Group‘s study from paleocoral. However, the tsunami occurred in Indian Ocean on 26 December 2004, 28 March 2005 and 17 July 2006, because the earthquakes still release the energy. To cope with the tsunami disaster, we have to put the much effort on better disaster preparedness. The Tsunami Reduction Of Impacts through three Key Actions (TROIKA) was suggested by Eddie N. Bernard, the director of NOAA/PMEL (Pacific Marine Environmental Laboratory). They are Hazard Assessment, Mitigation and Warning Guidance. The satellite remote sensing has potential on these actions. The medium and high resolution satellite data were used to assess the degree of damage at the six-damaged provinces on the Andaman seacoast of Thailand. Fast and reliable interpretation of the damage by remote sensing method can be used for inundation mapping, rehabilitation and housing plans for the victims. For tsunami mitigation, the satellite data can be used with GIS to construct the evacuation map (evacuation route and refuge site) and coastal zone management. It is also helpful for educational program for local residents and school systems. Tsunami is a kind of ocean wave, therefore any satellite sensors such as SAR, Altimeter, MODIS, Landsat, SPOT, IKONOS can detect the tsunami wave in 2004. The satellite images have shown the characteristics of tsunami wave approaching the coast. For warning, satellite data has potential for early warning to detect the tsunami wave in deep ocean, if there are enough satellite constellation to monitor and detect the first tsunami wave like the pressure gauge, seismograph and tide gauge with the DART buoy can do. Moreover, the new methods should be developed to analyse the satellite data more faster for early warning procedure.
Inversion therapy has been used to relieve backpain, decrease pressure on the discs between vertebrae and stretch musdes as early as 400 BC. Gravity Guidance Inversion Table was introduced in 1960's and has been commercialized mostly for relieving backpain. in United States. But this machine needs to adjusted and possible sudden movement can cause severe injury to the users. In the 1990's, motor assisted inversion cram was introduced in Germany. But this inversion cram is too heavy and inconvenient to be used at home. In these days, the inversion machine become popular equipment in fitness clubs and health care places for relieving backpain as well as stretching muscles. And there is a growing need for home use version of motorized inversion machine. In this study, the motorized inversion machine was developed to be used for stretching exercise at home. The design became compact, simple and foldable as the results of adapting the wabble mechanism. And the machines are optimally designed by analyzing the torques applied to the machine by diverse physical size of users.
This paper will discuss about how we can foster educational support mechanisms to facilitate health promotion programs at the local level. Health promotion in Korea is in the early developmental stage; it has only been since the Health Promotion Act was legislated in 1995, the health promotion programs have been planned and implemented. In the context of the recent decentralization process, local health departments have a major responsibility for developing and implementing health promotion programs at the local level. The short history of health promotion in Korea has meant that local public health departments have limited experience and organizational capacity for health promotion planning & practice. The results of one survey for investigating the progress of health promotion at the local level are instructive. The survey demonstrated that the public health workers recognized that the lack of personnels, insufficient budget, the lack of policy & the organizational support, the lack of skill & knowledge to be effective health promotors, the lack of guidance for health promotion practice were major barriers to implementing health promotion programs at the local level. The object of this paper is to suggest some ways of overcoming barriers to implementing health promotion programs at the local level This paper emphasizes on the importance of educational supports as well as environmental supports - legislative, policy, organizational, economical - in building the organizational capacity and infrastructure of local health department for health promotion. It suggests some ways of providing educational supports to the public health workers at the local level. and supports the positions that educational opportunities for training in health promotion can be better provided to the public health workers at the provincial level rather than at the national level. It argues that the educational & training programs should be developed and based on the educational need assessment; that the application of the main educational principles & theoretical models for health promotion be used to develop educational programs for the public health workers; and that professional health organizations should make plans to provide more educational programs at their annual conferences or at other convenient times. These kinds of educational supports facilitate the ability of public health workers to improve their capacity for health promotion practice at the local level and help to alleviate some of the pressure on state resources.
An ensemble Kalman filter (EnKF) with Weather Research and Forecasting (WRF) Model is applied for Typhoon Wukong (200610) to investigate the performance of ensemble forecasts depending on experimental configurations of the EnKF. In addition, the ensemble sensitivity analysis is applied to the forecast and analysis ensembles generated in EnKF, to investigate the possibility of using the ensemble sensitivity analysis as the adaptive observation guidance. Various experimental configurations are tested by changing model error, ensemble size, assimilation time window, covariance relaxation, and covariance localization in EnKF. First of all, experiments using different physical parameterization scheme for each ensemble member show less root mean square error compared to those using single physics for all the forecast ensemble members, which implies that considering the model error is beneficial to get better forecasts. A larger number of ensembles are also beneficial than a smaller number of ensembles. For the assimilation time window, the experiment using less frequent window shows better results than that using more frequent window, which is associated with the availability of observational data in this study. Therefore, incorporating model error, larger ensemble size, and less frequent assimilation window into the EnKF is beneficial to get better prediction of Typhoon Wukong (200610). The covariance relaxation and localization are relatively less beneficial to the forecasts compared to those factors mentioned above. The ensemble sensitivity analysis shows that the sensitive regions for adaptive observations can be determined by the sensitivity of the forecast measure of interest to the initial ensembles. In addition, the sensitivities calculated by the ensemble sensitivity analysis can be explained by dynamical relationships established among wind, temperature, and pressure.
Purpose : The purpose of this study was to examine the stressors and coping patterns of dental hygiene students in a bid to boost the efficiency of guidance and counseling. Subjects and Method : The subjects in this study were 359 dental hygiene students from some colleges. After a survey was conducted, the collected data were analyzed with SPSS 13.0 program. Statistical data on frequency and percentage were obtained, and ANOVA and t-test were utilized. Findings : 1. The dental hygiene students investigated suffered the most stress with excessive workload, and anxiety about the national examination was the greatest stressor. Among the areas of stressors, school life put the most pressure on them(3.25). 2. The most dominant way for the dental hygiene students to handle stress was talking with friends(4.32). Out of the areas of stress-coping patterns, mental coping was most prevalent(2.94), and the least common stress-coping area was physical coping(2.23). 3. As for the relationship between the place where they were raised and stress-coping styles, physical coping and direct coping were more widespread among the students who were brought up in large urban communities than the others. Conclusion : As the dental hygiene students were different in stress-coping styles according to the place where they were grown, educators who nurture dental hygienists should offer customized education and counseling in consideration of where students were brought up.

The goal of this study was to examine how effective stroke prevention program is for body mass index, blood pressure, blood glucose, and serum lipids of the elderly in the rural areas. The stroke prevention program included health care education, nutrition guidance and exercise. The program was provided for 12 weeks(36 sessions) from May to August, 2014. Participants were 55 elderly people in the rural areas; 28 people in the experimental group and 27 in the control group. The results of before and after the experiment showed that body mass index(p<.001), TC(p<.001), TG(p=.047), and LDL(p=.010) were significantly better in the experimental group compared to the control group. Also, after the program, the figures of body mass index, fasting blood glucose, TC, TG, and HDL in the control group were decreased, while those of LDL were increased. Study findings indicate that the stroke prevention program is effective and can be recommended as a precaution against the stroke of the elderly in the rural area by preventing risk factors beforehand.
