A Meta-Analysis of Korean Diabetes Patient Studies from 1977 to 2008 (메타분석을 이용한 당뇨중재의 효과분석)

  • Objectives: This study analyzed the effects of diabetes interventions published in journals of nursing, medical and public health, to suggest directions for future research. Methods: A comprehensive search of three databases was undertaken (Korean studies, Riss4U, RICHIS) using research terms such as "diabetes", "nursing", "public health", "intervention" and "program". One hundred fifty four original articles were retrieved and 21 articles were finally analyzed. The analyzed articles satisfied the inclusion criteria and their effect size was calculated. Results: Most studies about diabetes intervention were published form 2001. 2005 (57.1%). The psycho-social variables, 'self-efficacy' and 'self-care' presented good effects on exercise or exercise prescription intervention. In addition, the physiological variables, 'fasting blood sugar' and 'glycosylated glucose' presented good effects on the interventions using four more elements or the interventions held in a public health center. Conclusion: Further research should consider the essential elements that benefit diabetes patients' behaviors or physiological indicators.

  • Objectives : The discussion of enlargement of Korean Medical insurance has been existed since 1987. But it had less effects. For better discussion, we compared insured herbal extracts and clinical prescriptions. Methods : The database of insured herbal extracts and clinical prescriptions bas set up to compare the ratios of herbal weight and analyze indications of insured herbal extracts and diagnosis and chief complain of clinical prescriptions. Results & Conclusions : The most frequent insured herbal extract is Bojungiki-tang. Analysis about indications of insured herbal extracts and diagnosis and chief complain of clinical prescriptions is that Bojungiki-tang is used frequently for gastrointestinal diseases, pulmonary diseases, and not for diseases. Bojungikgi-tang is also used for muscloskeletal dieases, hemorrhage, and ischemia.

Koreans' Experience at Health Facilities in Thailand (한국인의 태국 의료기관 이용 경험)

  • Purpose: The purpose of this study was to identify experiences when Koreans who reside in Thailand visit Thai health facilities. Methods: The data were collected via direct interview from 36 Korean residents of Thailand. Collected data were analyzed by content analysis. Result: 1) 162 content analysis data were collected from the participants' statements. These data were categorized into 70 attributes and 9 higher attributes. 2) Out of the 70 attributes, the 3 attributes which were most commonly mentioned by the participants were the following: medical expense for foreigners were expensive(66.7%), difficulties in communication with health care providers(36.1%), kindness of nurses(33.3%). 3) The 9 higher attributes were 1) high medical expense to foreigner, 2) difficulties in communication, 3) distrust about medical service, 4) excessive prescription in oral medicine, 5) kindness, 6) good service, 7) difference in medical environment according hospital type, 8) irrational process in medical treatment for out patients, 9) cultural difference in postpartum care. Conclusion: When Korean residents in Thailand use Thailand health facilities, they experience high medical expense, difficulties in communication, low quality in medical service and kindness of the medical staff.

  • I. PURPOSE AND SIGNIFICANCE OF RESEARCH The presentation of research direction for compositive study and application of oriental medicine is urgently demanded. Since there is few investigation for the interest in basic and clinical field of oriental medicine, and we hope that compositive study and concentrative research between instruction in college and local clinics is established by this research. II. RESEARCH CONTENT ${\cdot}$ Analysis of survey in basic field of oriental medicine as instruction curriculum. ${\cdot}$ Analysis of survey in clinical field of oriental medicine as the classification of disease. III. RESULT The result of investigation shows that the most interested field is acupuncture and moxibustion in clinical field and diagnostics in basic field of oriental medicine. And oriental medicinal clinic doctors are concerned about prescription and a newly-developed techniques such as new acupuncture therapy, new machines. They hope that a study of oriental medicine in clinical field be accomplished with the special clinics as leader. According to the previous results, we can work out the foundation of compositive study and concentrative research between instruction in college and local clinics, and on this ground it is considered that we can grope for the presentation of research direction of local clinics.

  • 1. Objectives This paper was written in order to understand formative process of Taeumin's Dry-febrile Symptom. 2. Methods We analysis Taeumin's Dry-febrile Symptom through pathology and prescription in ${\ulcorner}$Dongyisusebowonchobongyun${\lrcorner}$, ${\ulcorner}$Dongyisusebowongabo-bon${\lrcorner}$, ${\ulcorner}$Dongyisusebowonsinchukbon${\lrcorner}$. And we also analysis before study on pathology of Taeumin. 3. Results and Conclusions SCM was controled by Good-Qi. There was explained the strong and weak of Good-Qi to the number of real life. Slightness and Severeness of disease are dependent on the Health Energy of small viscera. So the interior febrile disease of Taeumin is divided to Liver-febrile symptom and Dry-febrile symptom. Because Liver-febrile symptom is reasonable symptom but Dry-febrile symptom is contrary symptom.

  • HPLC analysis of a commercial herb drink marketed as a functional food revealed to contain an unauthorized substance similar to sildenafil, the active ingredient of the prescription drug Viagra$^{(R)}$ approved for the treatment of male erectile dysfunction. In order to identify the illegal additive, the herb drink was extracted with methylene chloride, and the extract was purified further using semipreparative HPLC. The chemical structure of the isolated substance was elucidated based on IR, LC/MS-ESI, and NMR spectroscopy, which showed the characteristics similar to sildenafil with minor modification. The only difference was the substitution of the methylpiperazine moiety of sildenafil to the hydroxyethylpiperazine group of the illegal additive.

  • The purpose of this study was to analyse scientific according to period of rehabilitation training of ACL patients. ACL patients seven subjects participated in this study. Gait (1.58 m/sec) analysis was performed by using a 3-D Cinematography, a Zebris system and a electromyograph system. The data were analyzed by paired t-test. The joint angles were recorded from the ankle, knee, hip joints. Peak max dorsi-flexion and peak max plantar-flexion identified significant differences (p<0.05). Another angles were no significant difference. Vertical force (Fz) and max pressure variables improved 6 month RTP better than 3 month RTP. EMG were collected from 4 muscles (rectus femoris, biceps femoris, gastrocnemius, tibialis anterior) with surface electrides in gait system. EMG signals were rectified and smoothed data. EMG signas were no significant difference but they also improved 6 month RTP better than 3 month RTP. More research is necessary to determine exactly what constitutes optimal rehabilitation training period for ACL patients.

Quantitative Analysis of the Seventeen Marker Components in Dangguisu-san Using Ultra-performance Liquid Chromatography Coupled to Electrospray Ionization Tandem Mass Spectrometry (LC-MS/MS를 이용한 당귀수산 추출물 중 17종 성분의 함량분석)

  • Dangguisu-san is a well-known traditional Korean herbal medicine prescription and has been widely used to treat ecchymosis, blood stagnation, and pain resulting from physical shock in Korea. In this study, an ultra-performance liquid chromatography-electrospray ionization-mass spectrometer method was established for the simultaneous determination of the 17 biomarker components in Dangguisu-san. All analytes were separated on an UPLC BEH $C_{18}$ ($100{\times}2.1$ mm, $1.7{\mu}m$) column and maintained at $45^{\circ}C$. The mobile phase consisted of two solvent systems, 0.1% (v/v) formic acid in water (A) and acetonitrile (B) by gradient flow. The injection volume was $2.0{\mu}l$ and the flow rate was 0.3 ml/min with detection at mass spectrometer. Calibration curves of the 17 biomarker components were acquired with $r^2$ values ${\geq}0.9951$. The values of limit of detection and quantification of all analytes were 0.02~6.32 ng/ml and 0.05~18.95 ng/ml, respectively. The amounts of the 17 components in Dangguisu-san sample were $3.17{\sim}13,224.50{\mu}g/g$.

Autophagy-enhancing and neuroprotective effects of Wonji-Gobon mixture (WGM) in a Parkinson's disease mouse model

  • The aim of this study was to evaluate autophagy-enhancing and neuroprotective effects of Wonji-Gobon mixture (WGM), a traditional Chinese prescription medication, in Parkinson's disease (PD) mouse models. Our investigation found that WGM increased the expression of both Beclin1 and LC3b-II proteins as measured with western blot in the BV2 cell line; both proteins play a role in autophagy. WGM also increased the autophagy expression as measured by fluorescence-activated cell-sorting analysis in the BV2 cell line. In 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine-induced PD models, WGM significantly increased the amount of dopamine in a striatum-substantia nigra suspension, produced notable results in the forced swim test, and increased serotonin as measured by high-performance liquid chromatography analysis; these results are indicative of neuroprotective effects. In summary, our findings indicate that WGM treatment has neuroprotective effects that are partially mediated by autophagy enhancement.

  • Objectives : This study aims to improve the diagnosis and treatment of contemporary insomnia by examining Zhang Jiebin's discussion on treating insomnia. Methods : The classical texts from the 'Insomnia' chapter of the Jingyue Quanshu were examined threefold in terms of symptom, treatment, and prescription analysis, after which the treatment discussion part was examined within the historical context of discussions on insomnia in major medical texts starting from the Huangdineijing. Results : According to Zhang, the cause of insomnia could be divided into two, after which criteria for diagnosis and treatment were set as excess pathogen and vital qi deficiency. He argued that insomnia could be naturally resolved through improvement of various pathogenic situations. Discussions on insomnia from various medical texts since the Huangdineijing suggest that pathology related to psychological function and emotions gradually increased and expanded over time. Conclusions : Zhang's discussion on symptom, treatment and prescriptions of insomnia suggests a new framework that could improve treatment effects through a Korean Medical Mind-Body approach, rather than the contemporary classification of organic insomnia and non-organic insomnia.
