Influence of Prescribed Herbal and Western Medicine on Patients with Abnormal Liver Function Tests: A Retrospective Quasi-Experimental Study

  • Objectives: The aim of this study was to investigate the safety and the efficacy of Korean herbal, western and combination medicine use in patients with abnormal liver function tests. Methods: We investigated nerve disease patients with abnormal liver function tests who were treated with Korean herbal, western and combination medicine at Dong-Eui University Oriental Hospital from January 2011 to August 2011. We compared aspartic aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP) and total bilirubin (T-bil) levels before and after taking medicine and excluded patients who had liver-related disease when admitted. Results: AST and ALT were decreased significantly in patients who had taken herbal, western medicine. AST, ALT and ALP were decreased significantly in patients who had taken combination medicine. Compare to herbal medicine, AST, ALT and ALP were decreased significantly in patients who had taken western medicine, and ALT and ALP were decreased significantly in patients who had taken combination medicine. There were no significant differences between western and combination medicine. Conclusions: This study suggests that prescribed Korean herbal medicine, at least, does not injure liver function for patients', moreover, it was shown to be effective in patients with abnormal liver function tests.

A Case of Cerebellar Infarction Patient with Ataxia and Vertigo prescribed by Chukdam-tang (운동실조와 현훈을 주소로 한 소뇌경색 환자의 척담탕 치험 1례)

  • Ataxia and vertigo are frequent clinical findings in cerebellar infarction patients. In severe cases, they cannot achieve normal activity in daily life. A 65-year-old man was admitted with ataxia and vertigo diagnosed as acute cerebellar infarction. We prescribed for Chukdam-tang to treat the phlegm disease. After this treatment, ataxia and vertigo improved. We suggest Chukdam-tang is significantly effective on the treatment of cerebellar infarction.

  • Cough is not only serves to maintain normal function of respiratory tract, but also many diseases may lead to this symptom. In Korean medicine, Zhudanxi(朱丹溪), was the first to classify cough according to the different stages of time. In this research, I investigated the stage of Nocturnal Cough(夜嗽) through 26 oriental medical books. As a result, deficiency of Yin(陰虛) was the most common causes of Nocturnal Cough and indigestion(食積) is the next. Liuweidihuang-tang(六味地黃湯) prescribed as the treatment for Yin Xu(陰虛) and Xeibaisan-jiawei(瀉白散加味) prescribed as the treatment for indigestion(食積).

EFFECTS OF HWA GAE SAN EXTRACT ON THE CONTRACTION OF ISOLATED GUINEA PIG TRACHEA SMOOTH MUSCLE (화개산(華蓋散)이 GUINEA PIG의 기관지(氣管支) 평활근(平滑筋)에 미치는 영향(影響))

  • In order to study the effects of HWA GAE SAN known clinically for their effects of treatment for cough and asthma, the study was carried out to investigate the effect of HWA GAE SAN extract on the contractile force of isolated guinea pig trachea smooth muscle and elucidate its mechanism. The results were obtained as follows. 1. Preparation of isolated guinea pig trachea smooth muscle was suspended in the oxygeneted Kreb's Henseleit bicarbonate buffer solution at $37^{\circ}C$ and recorded the developed tension by the drug with the isometric transducer (Nacro F-60). The restin tension was approximately 0.5g. 2. The trachea smooth muscle in normal state showed a significant atony to the increase of density of HWA GAE SAN. 3. The atonic effect of the trachea smooth muscle was restricted after prescription of Pyrilamine& Cyproheptadine. Hireceptor broker, which were prearranged. 4. After 5, 15, 50& $150{\mu}l/ml$ of HWA GAE SAN were prescribed, the atony of trachea smooth muscle was caused by Histamine. 5. After 50& $150{\mu}l/ml$ of HWA GAE SAN were prescribed, the atony of trachea smooth muscle was remarkably dwindled which was caused by Acetylcholine. 6. After $150{\mu}l/ml$ of HWA GAE SAN were prescribed, the atony of trachea smooth muscle was remarkably dwindled which was caused by 5-Hydroxytryptamine.

Consequences of Doses of Herbal Medicine Including Buja and Cheono on Electrocardiography (부자(附子) 및 천오(川烏)를 포함한 처방이 심전도 변화에 미치는 영향)

  • Objectives: Buja and Cheono, containing aconitine known for its cardiotoxicity, are not commonly prescribed, but they are necessary for some clinical conditions. Hence, consequences for electrocardiography (EKG) after short- and long-term dosage of herbal medicine including Buja and Cheono are here reported.Methods: From September 2014 to April 2015, 36 patients belonging to the sixth internal medicine department of the Dong Eui Oriental Medicine Hospital took herbal medicine including Buja and Cheono, and after these doses, they underwent an EKG. Thirty-one patients who took the medicine for 7-8 days comprised the short-term group, and 18 patients taking the medicine for 17-106 days comprised the long-term group. The results of the EKGs for each group, before and after dosage with herbal medicine, are compared.Results: No significant changes in EKG appeared in either the short- or the long-term group after taking herbal medicine including Buja and Cheono.Conclusions: If Buja or Cheono are prescribed along with other suitable herbs by a doctor of Korean medicine, no cardiac trouble will occur on an EKG regardless of the term of the dosage.

  • In this study, it is experienced that Drug eruptions of Soyangin patient mistreated with Taeumin, The patient was 61 years old, female. She entered our hospital for Stroke and sicked Drug eruptions by mistreatment of Taeumin, After we prescribed Yanggyuksanhwatang(凉膈散火湯), had the general improvement of drug eruptions. Drug eruptions is related to Yangdokbalban(陽毒發斑) in Oriental medicine. The Yangdokbalban(陽毒發斑) of Soyangin is classed as Wisuyulliyulbyung(胃受熱裏熱病). This is a kind of dermatitis caused by Wiyul(胃熱). We classified this as Soyangin-Liyuuljeung(小陽人裏熱證) and prescribed Yangdokbaekhotang(陽毒白虎湯) and Yanggyuksanhwatang(凉膈散火湯). In the result, we had the general improvement of Yangdokbalban(陽毒發斑).

One Case Report of Middle Cerebral Artery Infarction Patient Prescribed Chungpyesagan-tang (청폐사간탕(淸肺瀉肝湯)을 투여한 중대뇌동맥영역의 뇌경색환자 치험 1례)

  • This study is clinical report of one patient on a middle cerebral artery territory infarction who improved with the administration of an herbal medication. We prescribed Chungpyesagan-tang to the patient, and then CAVI(cardio-ankle vascular index), NIHSS, and MBI were followed up 1, 2 and 3 weeks later. After 1, 2 and 3 weeks,, CAVI and NIHSS decreased and MBI increased. These findings suggest that Chungpyesagan-tang has an effect on improvement of neurologic deficit of MCA infarction.

  • Two Fructus Jujubae & three Rhizoma Zingiberis Rencns are usually used in parts of prescriptions in oriental medicine. In "Dongeuibogam", those are prescribed as drugs. However, those are not exactly same with original books, which "Dongeuibogam" had quoted. In original books, those are sometimes mentioned just as 'Fructus Jujubae & Rhizoma Zingiberis' or different amount of them. Therefore, authors try to compare with "Dongeuibogam" and its original books to find out the reason of discordance. As a result, we could conclude that Two jujubes & three pieces of ginger are more commonly prescribed than original books and are fixed to use more conveniently and to give standard for doctor.

  • The total 46 prescriptions of Ju Dal(酒疸), recorded in all the medical were investigated. 1. The most used prescription of Ju Dal(酒疸) is CHIJADAlHUANGTANG(梔子大黃湯). The nest ones are GALCHULTANG(葛朮湯), GALHWAHAIJEONGTANG(葛花解醒湯), DANGGUIBAICCHULTANG(當歸白朮湯). 2. The total 101 kinds of medicines were adminstered for the prescription of Ju Dal(酒疸). Fructus Aurantii Immaturus(枳實) was prescribed for the most popular medicine and Fructus Gardeniae(梔子), Radix et Rhizoma(大黃), Semen Sojae Praeparatum(豆鼓), Radix Glycyrrhizae(甘草), Herba Artemisiae Scopariae(茵蔯) follows. 3. Li Gi(利氣) mediemes were the most popularly prescribed ones, Chung Youl(淸熱) medicines, Hai Dok(解毒) medicines, Sam Sub Li Su(渗濕利水) medicines follows.

Nonsteroidal Anti-inflammatory Drugs Utilization Patterns among the Elderly with Osteoarthritis at Primary Ambulatory Care Units in Busan Metropolitan City, Korea (부산지역 의원급 외래 노인 골관절염환자의 비스테로이드소염제 사용양상평가)

  • Objectives : To investigate the utilization patterns of non-steroidal anti-inflammatory drugs (NSAIDs) among the elderly with osteoarthritis (OA) undergoing primary ambulatory care in Busan metropolitan city, Korea. Methods : OA patients, aged 65 years and over, were identified from the Korean National Health Insurance Review Agency drug prescription database. The subjects had at least one episode of claim for OA (ICD-10-CM: M15-M19) between August 1, 2000 and February 28, 2002. Trends in the determinations of NSAIDs utilization were identified using chi-squared tests for trend. Results : There were 47,711 osteoarthritic patients. The total number of visits by these patients was 177,443, with a total frequency for NSAID prescriptions of 214,952. Seventy-nine percent of the OA patients were female. NSAIDs were prescribed on 133,284 visits (75.1%) and the proportion of prescriptions was significantly increased with age. Only the proportion of visit when NSAIDs were prescribed decreased, from 65.1 to 43.5%, during the study period (p<0.001). However, the proportion of combined treatments with anti-ulcer drugs was increased. The use of NSAIDs injections was decreased. Of the individual NSAIDs, diclofenac (28.7% of total frequency of NSAID prescriptions), piroxicam (15.0%) and talniflumate (8.7%), were the most frequently prescribed. Among the NSAIDs prescribed OA visits, 45.7% used two or more NSAIDs. Conclusion : The total proportion of NSAIDs prescribed to the osteoarthritic patients was higher than in other studies. The decline in the use of NSAIDs during the study period, and the frequent selection of safer medications, such as combination therapy with anti-ulcer drug, may reflect the risk awareness of the use of NSAIDs.
