The Effect Analysis of Aging Preparation Between National and Private Medical Workers (국가 · 민간의료기관 근로자간의 노후준비 영향 분석)

  • The purpose of this study is to find out the differences in between national and private medical institutions' perception towards aging, and to analyze how their different perceptions effect each of their later life preparation. A total of 783 questionnaires were distributed among national and private medical institution workers. Frequency analysis, t-test, hierarchical regression analysis was conducted from the collected data via SPSS 21.0. The results were the following. Firstly, there was a partial difference in their perception of aging, government policy satisfaction and later life preparation. National medical institutions were better than private medical institutions in all three terms. Also, perception of aging and government policy satisfaction were higher-than-average in both institutions, but in later life preparation, both scored lower-than-average. Secondly, the factors affecting preparation for aging were significantly different between these two institutions. Also, how each factors affected later life preparation was partially different. In conclusion, effects of factor was difference in aging preparation between national and private medical institutions workers. We need to provide as basic data for education and policy making for aging preparation.

The Preparation of Medical Institutions in Seoul for Attracting Foreign Patients (서울시 의료기관의 외국인환자 유치 준비실태 - 병.의원을 중심으로 -)

  • The purpose of this study is to examine the preparation of medical institutions for medical tourism and its effects. The sample of this study were 99 case which were selected from Korea Health Industry Development Institute list. Data were collected through the mail questionnaire survey from Sept. 15 to Oct. 30 in 2010. The collected data were analyzed using Chisquare test, t-test. The main findings of study are as follows: Using Chisquare test, we found statistically significant differences in resources prepared for medical tourism between hospitals and clinics. In general, hospitals were well prepared in human resources(e.g., proportion of employees with foreign language capability) and physical resources(e.g., helpdesk, brochures) compared to clinics.

  • The purpose of this study is to examine the preparation of medical institutions for Russian medical tourists. The data for this study include 44 cases which were selected from Korea Health Industry Development Institute list. Data were collected through the e-mail survey from Feb. 24 to Jul. 1 in 2014. The results of the study are as follows. Overall, tertiary hospitals were well prepared in hardware resources and structure(e.g. the number of coordinators and marketers, accreditation)compared to hospitals and clinics. On the other hand, hospitals were well prepared in service process protocol.

The Influence of Organizational Culture on Organizational Conflict in Medical Institutions (의료기관의 조직문화가 조직갈등에 미치는 영향)

  • Objectives: The purpose of this study was to examine the awareness of medical institution employees of their organizational culture and conflicts, as well as the influence of organizational culture on organizational conflict, in an attempt to provide some information on the preparation of organizational conflict management methods for medical institutions. Methods: Structured questionnaires and basic hospital data were used to gather data from the employees of medical institutions. The collected data were analyzed, using SPSS 24.0. Results: First, workers whose length of employment was longer were less aware of the characteristics of their organizational culture, while employees who were older, whose length of employment was longer, and who were middle managers experienced greater organizational conflict. Second, in terms of conflicts among different kinds of occupations, conflicts with nurses were the most common. Third, there was a negative correlation between organizational culture and organizational conflict. Conclusions: As stronger awareness of the characteristics of organizational culture leads to less organizational conflict, the organizational culture of each medical institution should be created and strengthened.

Survey of Activated Charcoal Administration for Poisoning Patients Visited in Emergency Medical Centers and Emergency Staff's Perception in Korea (국내 응급의료센터의 중독 환자에 있어 활성탄 투여 현황과 응급실 의료진의 인식)

  • Purpose: Activated charcoal (AC) has been widely used as a universal antidote. Currently, emergency medical centers in Korea cannot administer AC due to discontinuation of the supply of commercial ready-mixed AC suspension. This study was conducted to investigate the proportion of emergency medical centers that administer AC to poisoning patients and provide basic information for emergency physicians and toxicologists. Methods: A prospective telephone survey of all of the included emergency medical institutions was conducted. The type of emergency medical institution, average annual number of patients admitted to the emergency department, annual average number of patients who were poisoned and whether the hospital currently utilizes gastric lavage and administration of AC were determined. Results: AC was administered to poisoning patients in 40% of regional emergency medical centers, 59.3% of local emergency medical centers, and 45.9% of local emergency medical rooms. Overall, 37% of total emergency medical institutions did not administer AC due to discontinuation of the commercial ready-mixed AC suspension. Additionally, 77% of emergency physicians in institutions without AC knew AC is necessary for poisoning patients. The rate of vomiting experienced by the medical staff according to types of charcoal showed that the average rate of vomiting was 33% for commercial ready-mixed activated charcoal suspension and 51% for self-prepared charcoal powder (p=0.02). Conclusion: AC should be secured promptly in emergency medical institutions. Before the supply of commercial ready-mixed AC suspension becomes again it is essential to develop a standardized regimen for self-preparation of charcoal powder and to educate emergency physicians and toxicologists to its use.

The Response of the Seoul Municipal Hospitals against COVID-19 and Its Implications for Public Hospitals (서울시립병원의 코로나19 대응을 통해 본 공공병원의 시사점 고찰)

  • Purpose: The purpose of the study is to suggest the main functions and implications of public hospitals to effectively respond to the future epidemic crisis based on analyzing the accessibility to designated Coronavirus Disease 2019 (COVID-19) medical institutions of Seoul and examining the main features of the quarantine of Seoul municipal hospitals. Method: To analyze the response and function of Seoul municipal hospitals, we reviewed the Infectious Disease Control and Prevention ACT, 258 articles of Seoul Metropolitan Government press releases from January to the end of April, 48 articles of Seoul Metropolitan Government's daily newsletters, 2019 Health Bureau Budget report. We also referred to internal data of Seoul Children's Hospital, Seoul Seobuk Hospital, and Seoul Eunpyeong Hospital during the same period. Besides, the accessibility to medical institutions was analyzed by using the COVID-19 data which was announced daily basis. Results: The accessibility of COVID-19 patients living in the Southeastern part of Seoul to a medical institutions was 16.2km on a distance basis, and it was the lowest accessibility among four regions of Seoul since it took about 40 minutes by car. On the other hand, patients living in the Northeast part had the highest accessibility, as the access to medical institutions was 10.7km and 27 minutes by car. Also, the main functions of the municipal hospital of Seoul against COVID-19 were to shift the public hospital function to COVID-19 patients only hospitals, to perform the epidemiological investigation by medical doctors, and to support the operation of self-isolation facilities, community treatment centers and triage rooms of community health centers. Conclusion: Through the experience of COVID-19, we suggested that the functions of public hospitals will be reorganized as the reinforcement of infectious disease treatment and mental health for quarantined patients, cooperation with private hospitals, supporting for strengthening community health capacity and preparation for another epidemic.

Degree of Execution and Preparation for the Retirement of Hospital Employees (병원종사자들의 노후대책 준비내용 및 실행정도)

  • The main purpose of this research is to gain full understanding of how well people are aware and prepared for after retirement. To reach the goal to learn more about the upcoming aging society, professionals from variety of medical fields were employed as subjects to this research. In order to get a detailed result, and to provide an ideal suggestion, the research was conducted with 406 professional workers from 31 different medical institutions as its subjects. Obtained results were summarized as follows. First, in terms of government policies in the field, the idea of long term recuperation insurance gained more support than the belief that the public annuity is a sufficient economic countermeasure. Second, there is a causal relation between the economical preparation for retirement and the reason why people do not prepare for their after retirement. The more one feel short of money, the more one is obtuse towards the necessity of preparing for retirement, the more ignorant one is about preparing for retirement, the higher the chance that there will be no preparation done for his/her after retirement.

Study on the Difference of Urine Sediment Preparation for Microscopic Examination (현미경검사를 위한 요침사 표본제작에 따른 차이 연구)

  • Urinalysis is considered to be easier and simpler than other tests. It has been known to cause no burden to patients, while offering important information on diagnosing, treating, and determining the prognoses of kidney and urinary tract diseases. Urinary sediments are usually performed by microscopic examination of centrifuged urine by technologist. The guidelines proposed by the Korean Association of External Quality Assessment Service are actually different from those actually practiced by medical institutions and taught to biomedical students in textbooks. Therefore, we verified whether different sediment preparation methods lead different test results. Specimens that tested positive from the occult blood and leukocyte esterase in the urine dipstick test were randomly selected for a microscopic examination. The differences in the urine sediment preparation affected the sediment concentrations, which influenced the cell grade and cell number per HPF. The first factor in determining the sediment concentration is the centrifugal force. Many medical institutions use 1,500 rpm as the centrifugal speed without considering the radius of the centrifuge; such a value may not be accurate for 400 G. Consequently, there were differences in urine concentrations, which influenced the results. The second factor is the amount of sediment in urine. Different amounts of the remaining supernatant led to different sediment concentration factors, again, causing different results. Furthermore, not only by using a pipette to obtain an accurate amount as stipulated, but also by roughly obtaining a drop, the microscopic examination using such a volume of sediment examined affected the results. Therefore, this study highlights the importance of standardization of urine sediment preparation procedures to promote consistency and accuracy across institutions.

Improvement of the pre-hospital emergency medical service system in China (중국의 병원 전 응급의료체계 개선방안)

  • Purpose : Emergency medical services in China are increase in demand by people and under the greater pressure than ever before. So it is, necessary to advance the pre-hospital system in order to promote the development of emergency medical services. Methods : This is based on China-related articles, books, journals, reports, statistical data and other literature. Results : First, pre-hospital emergency medical care with the introduction of specialist training program should be established. Second, to strengthen pre-hospital emergency services and to develop the EMS guidelines. Third, the "120" reporting systems unification and awareness activation. Fourth, the preparation of the EMS facilities equipment system. Fifth, the rapid transport system establishment to the selected medical institutions. Conclusion : It is necessary to strengthen the emergency medical personnel at the scene, rapid transport, rapid patient triage and to improve the survival rate of the patients.

Changes in the characteristics of patients transferred to the emergency room through private institutions during inter-hospital transport after the COVID-19 outbreak : A retrospective study (COVID-19 발현 이후 병원간 이송 시 민간 이송업체를 통해 응급실로 전원된 환자들의 특성 변화 : 후향적 연구)

  • Purpose: The purposes of this study were to determine the changes in the pattern of patients who were transferred to the emergency room through inter-hospital private institutions and to determine how long transport takes following the COVID-19 outbreak. Methods: This retrospective observational study analyzed the emergency medical services reports of private institutions following the COVID-19 outbreak in South Korea. The study was conducted in Busan between January 19, 2019 and January 18, 2020, and between January 19, 2020 and January 18, 2021. Results: Upon comparing the patient transport times during the "Pre-COVID-19 period" and "COVID-19 period," a significant delay was noticed in the preparation for transfer of patients during the "COVID-19 period" (p<.05). There were significantly more patients with respiratory infections and patients who complained of general symptoms during the "COVID-19 period." Moreover, there was a higher frequency of patients who were transferred to a 'Level I' emergency room during the "COVID-19 period" compared to during the "Pre-COVID-19 period" (p<.05). Conclusion: Following the COVID-19 outbreak, there is a delay in patient transport to the emergency room through private institution inter-hospital transport and an increase in the number of patients complaining of respiratory infection symptoms. Thus, emergency medical services need additional administrative and economic support to transport infected patients.
