  • Premature loss of maxillary primary incisors often results from early childhood caries (ECC), trauma. While space maintenance in the posterior region is an important consideration when there is early loss of primary molars, the anterior segment appears to be stable, even with the early loss of several incisors, once the primary canines erupt. However, collapse of anterior arch integrity is evident in cases where incisor teeth are in a crowded dentition prior to extraction or lost before the eruption of the primary canines. So, when early loss of maxillary primary incisors, the aim of restoration is esthetics, speech problem, oral habit such as tongue thrusting than space maintenance. This paper reports that the esthetic problem due to premature loss of maxillary incisors can be successfully resolved by soldered open-faced stainless steel crown.

Estimating the Economic Burden of Premature Mortality Caused by Cancer in Iran: 2006-2010

  • Background: Cancer is currently one of the main public health problems all over the world and its economic burden is substantial both for health systems and for society as a whole. To inform priorities for cancer control, we here estimated years of potential life lost (YPLL) and productivity losses due to cancer-related premature mortality in Iran from 2006 to 2010. Materials and Methods: The number of cancer deaths by sex and age groups for top ten leading cancers in Iran were obtained from the Ministry of Health and Medical Education. To estimate the YPLL and the cost of productivity loss due to cancer-related premature mortality, the life expectancy method and the human capital approach were used, respectively. Results: There were 138,228 cancer-related deaths in Iran (without Tehran province) of which 76 % (106,954) were attributable to the top 10 ranked cancers. Some 63 % of total cancer-related deaths were of males. The top 10 ranked cancers resulted in 106,766,942 YPLL in total, 64,171,529 (60 %) in males and 42,595,412 (40%) in females. The estimated YPPLL due to top 10 ranked cancers was 58,581,737 during the period studied of which 32,214,524 (54%) was accounted for in males. The total cost of lost productivity caused by premature deaths because of top 10 cancers was 1.68 billion dollars (US$) from 2006 to 2010, ranging from 251 million dollars in 2006 to 283 million dollars in 2010. Conclusions: This study showed that the economic burden of premature mortality attributable to cancer is significant for Iranian society. The findings provide useful information about the economic impact of cancer for health system policy/decision makers and should facilitate planning of preventive intervention and effective resource allocation.

THREE-DIMENSIONAL SPACE CHANGES AFTER PREMATURE LOSS OF THE PRIMARY FIRST MOLAR: A LONGITUDINAL STUDY (제1유구치의 조기 상실로 인한 공간 변화에 대한 3차원적 분석)

  • The purpose of this study was to evaluate a 3-Dimensional laser scanner for the space analysis after loss of a primary first molar. Six children with premature loss of a primary first molar were examined using study models taken before and after the extraction. The results were as follows: 1. There was no change in primary molar space after the extraction of a maxillary primary first molar However, 2 out of 3 children experienced primary molar space loss in extraction side of a mandibular primary first molar. 2. Arch width and arch perimeter showed no difference between initial and final model. 3. All primary canines did not show any changes in inclination. Maxillary primary second molars had similar changes in both extraction and control side. However, 2 out of 3 mandibular primary second molars in extraction side showed more lingual tipping compared to control side. Mandibular permanent first molars tipped more lingually in extraction side. 4. In angulation, primary canines showed nothing of significance. Mandibular primary second molars tipped more mesially in extraction side than in control side. Maxillary permanent first molars have increased distal angulation after extraction of primary first molars in both side.

  • Purpose : Premature ovarian failure, characterized by amenorrhea or oligomenorrhea, estrogen defieciency and elevated gonadotropins in woman less than 40 years of age, affects approximately 1% of woman and is associated with autoimmune phenomena or genetic abnormalities, but mostly idiopathic. This disease is loss of ovarian function seems not to be always permenent. Method : We investigated herb medication was effective in patient of premature ovarian failure. Results: Signs and symtoms of estrogen deficiency were improved and mensturation occurred monthly and the elevated during treatment of herb medication. As a result of our case recovering of ovarian function, we hope more investigtions about treating premature ovarian failure by oriental medicine will be gained.

Reproductive Performance of Women with Uterine Anomalies (선천성 자궁기형 환자의 생식력에 관한 고찰)

  • A reveiw of 85 patients with uterine anomalies was made in respect to the incidence, chief complaints, the reason of infertility, fetal wastage rate, pregnancy complications, fetal presentations and obstetric outcome after metroplasty from 1980 to 1985. The results were summarized as follows: 1. Incidence of uterine anomaly was 0.18% among all outpatients (85/48,240). 2. Of the 85 patients, there were 36 with bicornuate deformities (42.3%), 21 septate (24.7%), 18 uterus didelphys (21.2%), 8 arcuate (9.4%) and 2 patients with unicornuate anomalies (2.4%). 3. Uterine anomalies were diagnosed by hysterosalpingogram (54.1%), pelvic examination (14.2%) and other operative procedures. 4. Chief complaints were primary infertility (41.2%), secondary infertility (15.3%), repeated pregnancy loss (12.9%), antenatal care (11.8%) and menstural disturbance (10.6%), etc. 5. Twenty-nine patients with uterine anomalies had primary infertility. The cause of infertility was proved nonuterine in 26 cases and remained unknown in 3 cases. 6. The obstetric outcome of 104 pregnancies was spontaneous abortion in 51.0%, premature delivery in 11.50/0 and fetal loss in 57.7%. 7. Complications of 41 present pregnancies were threatened abortion (22%), premature rupture of membrane (12%) and premature labor (10%), etc. The frequency of abnormal presentation was 35.3% and 64.7% of deliveries was made by Cesarian section. 8. Metroplasty was performed in 13 patients who didn't have a baby because of repeated miscarriage and unknown cause of infertility. Subsequently 8 patients had 9 successful pregnancies: 6 patients had 7 healthy babies and 2 patients are now in pregnancy without any complications.

  • Exfoliation of the primary teeth and eruption of the permanent teeth is a normal and predictable physiologic process. The accurate prediction of tooth development is of major importance in treatment planning for children. Norms that correspond to the child's age and development have been determined by many investigators. However, there are a variety of factors that may hasten, retard, or otherwise affect the normal developmental process. Among the factors that influence tooth eruption is the premature loss of primary teeth. An abscess of a primary molar, followed by bone resorption and early extraction or exfoliation may lead to premature eruprion of the permanent succesor, and a variety of clinical sequelae. I, hereby present the patients who have prematurely erupted permanent successor in Seoul National University Hospital Pediatric Dentistry, and report the clinical and radiologic characteristics, its sequelae, proper managements, and followed up findings.

The Socioeconomic Cost of Injuries in South Korea (우리나라 손상의 사회경제적 비용)

  • Objectives : This study was conducted to estimate the socioeconomic cost of injuries in South Korea. Methods : We matched claims data from national health insurance, automobile insurance and industrial accident compensation insurance(IACI), and mortality data obtained from the national statistical office from 2001 to 2003 by patients unique identifier. Socioeconomic cost included both direct cost and indirect cost: the direct cost was injury-related medical expenditure and the indirect cost included loss of productivity due to healthcare utilization and premature death. Results : The socioeconomic cost of injuries in Korea was approximately 1.9% of the GDP from 2001 to 2003. That is, 12.1 trillion KRW(Korean Won) in 2001, 12.3 trillion KRW in 2002, and 13.7 trillion KRW in 2003. In 2003, direct medical costs were 24.6%(3.4 trillion KRW), the costs for loss of productivity by healthcare utilization were 13.0%(1.8 trillion KRW), and the costs for loss of productivity by premature death were 62.4%(8.6 trillion KRW). Conclusions : In this study, the socioeconomic cost of injuries in Korea between 2001 and 2003 was estimated by using not only health insurance claims data, but also automobile insurance, IACI claims and mortality data. We conclude that social efforts are required to reduce the socioeconomic cost of injuries in Korea, which represented approximately 1.9% of the GDP for the time period specified.

  • Distribution systems is consist of network in physical and radial in electrical aspect. Therefore radial operation is realized by changing the status of sectionalizing switches, and is usually done for loss reduction in the system. In this paper, we propose a optimal method for distribution systems reconfiguration. Specifically we use genetic algorithm method to solve distribution systems reconfiguration for loss minimization problem. A genetic algorithm(GA) is set up, in which some improvements are made on string coding, fitness function and mutation pattern. As a result, premature convergence is avoided.

  • The author observed a case of ectopic erupted maxillary second premolars and impacted mandibular second premolars due to premature loss of the primary second molars. The treatments were done as follows: 1. In the maxilla, the teeth arrangement problem was corrected with removal of the ectopic erupted second premolars. 2. In the mandible, normal dentition was established with surgical removal of the left second premolar and removal of the permanent first molar in right side combined by orthodontic therapy.

  • Confusion of a patent ductus arteriosus (PDA) for the descending thoracic aorta is a fatal error occurring occasionally in infants or neonates. As a result, the left pulmonary artery (LPA) may be misconceived as the PDA, and ligated. This surgical mishap of other hospital leads to serious congestive heart failure and loss of left lung function due to the underdevelopment in the peripheral vascular and alveolar structures in neonates and premature infants. In this report, 3 cases of LPA ligation and subsequent treatment are presented.
