Comparison of shear bond strengths of different types of denture teeth to different denture base resins

  • PURPOSE. To determine the shear bond strengths of different denture base resins to different types of prefabricated teeth (acrylic, nanohybrid composite, and cross-linked) and denture teeth produced by computer-aided design/computer-aided manufacturing (CAD/CAM) technology. MATERIALS AND METHODS. Prefabricated teeth and CAD/CAM (milled) denture teeth were divided into 10 groups and bonded to different denture base materials. Groups 1-3 comprised of different types of prefabricated teeth and cold-polymerized denture base resin; groups 4-6 comprised of different types of prefabricated teeth and heat-polymerized denture base resin; groups 7-9 comprised of different types of prefabricated teeth and CAD/CAM (milled) denture base resin; and group 10 comprised of milled denture teeth produced by CAD/CAM technology and CAD/CAM (milled) denture base resin. A universal testing machine was used to evaluate the shear bond strength for all specimens. One-way ANOVA and Tukey post-hoc test were used for analyzing the data (α=.05). RESULTS. The shear bond strengths of different groups ranged from 3.37 ± 2.14 MPa to 18.10 ± 2.68 MPa. Statistical analysis showed significant differences among the tested groups (P<.0001). Among different polymerization methods, the lowest values were determined in cold-polymerized resin.There was no significant difference between the shear bond strength values of heat-polymerized and CAD/CAM (milled) denture base resins. CONCLUSION. Different combinations of materials for removable denture base and denture teeth can affect their bond strength. Cold-polymerized resin should be avoided for attaching prefabricated teeth to a denture base. CAD/CAM (milled) and heat-polymerized denture base resins bonded to different types of prefabricated teeth show similar shear bond strength values.

Assessment of vertical root fracture using cone-beam computed tomography

  • Purpose: The aim of this study was to investigate the accuracy of cone-beam computed tomography (CBCT) in the diagnosis of vertical root fractures in a tooth with gutta-percha and prefabricated posts. Materials and Methods: This study selected 96 extracted molar and premolar teeth of the mandible. These teeth were divided into six groups as follows: Groups A, B, and C consisted of teeth with vertical root fractures, and groups D, E, and F had teeth without vertical root fractures; groups A and D had teeth with gutta-percha and prefabricated posts; groups B and E had teeth with gutta-percha but without prefabricated posts, and groups C and F had teeth without gutta-percha or prefabricated posts. Then, the CBCT scans were obtained and examined by three oral and maxillofacial radiologists in order to determine the presence of vertical root fractures. The data were analyzed using IBM SPSS 20.0 (IBM Corp., Armonk, NY, USA). Results: The kappa coefficient was $0.875{\pm}0.049$. Groups A and D showed a sensitivity of 81% and a specificity of 100%; groups E and B, a sensitivity of 94% and a specificity of 100%; and groups C and F, a sensitivity of 88% and a specificity of 100%. Conclusion: The CBCT scans revealed a high accuracy in the diagnosis of vertical root fractures; the accuracy did not decrease in the presence of gutta-percha. The presence of prefabricated posts also had little effect on the accuracy of the system, which was, of course, not statistically significant.

Esthetic Restoration of Deciduous Teeth Using Prefabricated Zirconia Crown in Children with Early Childhood Caries: A Report of Three Cases

  • Early childhood caries is a widespread condition that requires attention; however, its treatment remains a challenge in terms of child behavior management. This study describes the usefulness of customized zirconia crowns for the restoration of deciduous teeth through the evaluation of some cases. Three cases are described: a 29-month-old girl who presented with severe early childhood caries affecting anterior tooth, a 50-month-old boy who presented with extensive caries of his anterior tooth, and 70-month-old girl who presented with extensive caries of his primary posterior dentition. These prefabricated, zirconium-based ceramic crowns ($Nusmile^{TM}$ NuSmile) are available in various sizes, shapes and colors. Before the treatment, radiographs and intraoral photographs were taken to evaluate the appropriateness of treatment. Our findings indicate that customized zirconia crowns may be appropriate for restoring the deciduous teeth. Further long-term clinical studies are required to clarify the usefulness of this restorative method.

  • Implant-supported overdenture is a reliable treatment option for the patients with edentulous mandible when they have difficulty in using complete dentures. Several options have been used for implant-supported overdenture attachments. Among these, bar attachment system has greater retention and better maintainability than others. SFI-Bar$^{(R)}$ is prefabricated and can be adjustable at chairside. Therefore, laboratory procedures such as soldering and welding are unnecessary, which leads to fewer errors and lower costs. A 67-year-old female patient presented, complaining of mobility of lower anterior teeth with old denture. She had been wearing complete denture in the maxilla and removable partial denture in the mandible with severe bone loss. After extracting the teeth, two implants were placed in front of mental foramen, and SFI-Bar$^{(R)}$ was connected. A tube bar was seated to two adapters through large ball joints and fixation screws, connecting each implant. The length of the tube bar was adjusted according to inter-implant distance. Then, a female part was attached to the bar beneath the new denture. This clinical report describes two-implant-supported overdenture using the SFI-Bar$^{(R)}$ system in a mandibular edentulous patient.

Effect of ferrule on the fracture resistance of mandibular premolars with prefabricated posts and cores

  • PURPOSE. This study evaluated fracture resistance with regard to ferrule lengths and post reinforcement on endodontically treated mandibular premolars incorporating a prefabricated post and resin core. MATERIALS AND METHODS. One hundred extracted mandibular premolars were randomly divided into 5 groups (n=20): intact teeth (NR); endodontically treated teeth (ETT) without post (NP); ETT restored with a prefabricated post with ferrule lengths of either 0 mm (F0), 1 mm (F1), or 2 mm (F2). Prepared teeth were restored with metal crowns. A thermal cycling test was performed for 1,000 cycles. Loading was applied at an angle of 135 degrees to the axis of the tooth using a universal testing machine with a crosshead speed of 2.54 mm/min. Fracture loads were analyzed by one-way ANOVA and Tukey HSD test using a statistical program (${\alpha}=.05$). RESULTS. There were statistical differences in fracture loads among groups (P<.001). The fracture load of F2 ($237.7{\pm}83.4$) was significantly higher than those of NP ($155.6{\pm}74.3N$), F0 ($98.8{\pm}43.3N$), and F1 ($152.8{\pm}78.5N$) (P=.011, P<.001, and P=.008, respectively). CONCLUSION. Fracture resistance of ETT depends on the length of the ferrule, as shown by the significantly increased fracture resistance in the 2 mm ferrule group (F2) compared to the groups with shorter ferrule lengths (F0, F1) and without post (NP).

반복하중하에서의 carbon fiber post의 파절강도에 관한 연구 (A STUDY ON THE FRACTURE STRENGTH OF TEETH RESTORED WITH A CARBON FIBER POST UNDER CYCLIC LOADING)

  • In the restoration of endodontically treated teeth, carbon fiber post was recently introduced. The purpose of this in vitro study was to investigate the fracture strength of teeth restored with a pre-fabricated carbon fiber post in comparison with teeth restored with a prefabricated titanium post & custom cast gold post after cyclic loading in the different environment. A total of 30 recently extracted human central incisors of similar dimension with crowns removed were used. All teeth were placed into acrylic blocks and every steps for post and core fabrication were made accord-ing to manufacture's instruction. The post length and core dimensions were standardizd. All teeth were divided into 6 groups: 1) carbon fiber post / atmosphere, 2) titanium post / atmosphere, 3) gold post / atmosphere, 4) carbon fiber post / wet, 5) titanium post / wet, 6) gold post / wet. Carbon fiber post and titanium post were cemented in place using resin cement and cores were fabricated with Ti-Core. Custom cast gold post was made from Duralay pattern resin and cemented using resin cement, too. All specimens were thermocycled 10,000 times. After 50,000 cyclic loading, failure strength was measured using Instron testing machine. Kruskal-Wallis test followed by Mann-Whitney test was used to compare the mean fracture strength. Results were as follows : 1. All specimens showed lower fracture strength in wet environment after cyclic loading than in atmosphere condition, but did not reveal a significant difference. 2. There was no significant difference between carbon fiber post specimen and titanium post specimen in the same environment. 3. Gold cast post specimen showed significant different greater fracture strength than those of others in the same environment. 4. Carbon fiber post specimen showed no root fracture.

유전치 기성 크라운의 형태 및 치질 삭제량 비교 (Comparison of Crown Shape and Amount of Tooth Reduction for Primary Anterior Prefabricated Crowns)

  • 이 연구의 목적은 임상에서 많이 사용되고 있는 3종의 기성 크라운 즉, 레진관, 레진피복 금속관, 지르코니아 크라운의 크기와 형태를 3차원적으로 계측하여 자연치와 비교함으로써 기성 크라운의 크기 선택과 치질 삭제량에 대한 지침을 얻고자 하는데 있다. 300개의 유전치 석고모델과 3종의 기성 크라운을 대상으로 3D 스캐너와 컴퓨터 이미지 프로그램으로 3차원적 모델을 만들었다. 3차원 모델에서 치관의 근원심 너비, 치관 높이, 치관 ratio, 순면의 곡률반경 등 4가지 평가 척도에 의해 각 기성 크라운별로 한국인 자연치 표준모형과 가장 유사한 형태를 고르고 이를 기준으로 치질 삭제량을 비교, 평가하였다. 유중절치의 경우 기성 크라운별로 레진피복 금속관은 2번, 지르코니아 크라운은 1번, 레진관은 2번이, 유측절치의 경우 레진피복 금속관은 3번, 지르코니아 크라운은 2번, 레진관은 3번이 가장 유사한 형태를 보였다. 치아 삭제량은 상악 유중절치와 유측절치에서 같은 양상을 보였다. 절단연에서는 지르코니아 크라운의 삭제량이 가장 컸다. 인접면은 지르코니아 크라운과 레진관이 비슷한 삭제량을 보였으며 레진피복 금속관에 비해서 컸다. 순면 역시 지르코니아 크라운이 유중절치와 유측절치에서 삭제량이 가장 컸다. 설면의 경우 순면에 비해 전체적으로 삭제량이 적었으며 3종류의 기성 크라운 간에 삭제량 차이도 크지 않았다. 크라운의 형태를 비교하기 위한 4가지 평가 척도 중 치관의 근원심 너비가 기성 크라운의 크기를 선택하는데 가장 중요한 척도로 나타났다.

심미적 포스트 코어의 종류에 따른 미세누출에 관한 연구 (Microleakage of endodontically treated teeth restored with three different esthetic post and cores)

  • 연구목적: 오늘날 심미적 요구의 증가로 치아색 포스트 코어에 대한 많은 관심이 모아지고 있으며 다양한 연구가 진행 중이다. 현재 화이버 포스트 (ParaPost fiber White, Coltene Whaledent, Cuyahoga Falls, USA), 지르코니아 포스트 (CosmoPost, Ivoclar Vivadent, Schann, Germany) 등에 관한 논문은 다소 나와 있으나 요즘 여러 분야에서 관심을 받고 있는 CAD/CAM (computer-aided design/computer-aided manufacturing) 방식으로 지르코니아를 가공한 절삭형 지르코니아 포스트 코어에 대한 연구는 많지 않은 실정이다. 이 연구의 목적은 CAD/CAM을 이용한 지르코니아를 비롯하여 근관치료된 치아에서 사용된 세 가지 다른 치아색 포스트 코어에 대해 미세누출 (microleakage)을 비교해 보는 것이다. 연구 재료 및 방법: 발치된 건전한 상악 절치 27개를 채택하여 백악법랑질경계 (CEJ)에서 절단하여 근관치료를 시행하고, 세군 (각 9개)으로 나누었다. 각군들은 화이버 포스트와 콤포짓트 레진 코어 (1군), 지르코니아 포스트와 열가압형 세라믹 코어 (2군), CAD/CAM 절삭형 지르코니아 포스트 코어 (3군)로 수복하였다. $5-55^{\circ}C$ 1000회 열순환 처리 (thermocycling)를 시행하고, 2% 메틸렌 블루로 $37^{\circ}C$에서 24시간 염색하였다. 치아장축과 평행하게 순설방향으로 절단하여 미세누출을 현미경과 컴퓨터 프로그램을 사용하여 분석을 하였다. 데이터는 one-way ANOVA와 Scheffe's multiple range test (${\alpha}=0.05$)로 분석하였다. 결과: 모든 군에서 미세누출을 보였으며, 지르코니아 포스트와 열가압형 세라믹 코어군 (2군)에서 치근계면 입구에서는 다소 많은 미세누출을 보였으나 포스트 끝에 이르러서는 거의 미세누출이 없는 결과를 보였다. 화이버 포스트와 콤포짓트 레진 코어군 (1군)과CAD/CAM 절삭형 지르코니아 포스트 코어군 (3군)에서는 부위별로 균등한 미세누출을 보였다. 결론: 화이버 포스트와 콤포짓트 레진 코어군, 지르코니아 포스트와 열가압형 세라믹 코어군, CAD/CAM 절삭형 지르코니아 포스트 코어군 모두에서 미세누출을 보였고, 이 중에서 지르코니아 포스트와 열가압형 세라믹 코어군이 좀 더 큰 미세누출에 대한 저항성을 보였으며 화이버 포스트와 콤포짓트 레진 코어군, CAD/CAM 절삭형 지르코니아 포스트 코어군은 유사한 양상을 보였으나, 각 실험군 간에 통계적으로 유의한 차이는 인정되지 않았다. A comparative evaluation of fracture resistance of endodontically treated teeth restored with different post core systems - an in-vitro study

  • PURPOSE. To compare the fracture resistance and the mode of failure of endodontically treated teeth restored with different post-core systems. MATERIALS AND METHODS. Root canal treatment was performed on 40 maxillary incisors and the samples were divided into four groups of 10 each. For three experimental groups post space preparation was done and teeth were restored with cast post-core (Group B), stainless steel post with composite core (Group C) and glass fiber post with composite core using adhesive resin cement (Group D). Control group (A) samples were selected with intact coronal structure. All the samples were prepared for ideal abutment preparation. All the samples were subjected to a load of 0.5 mm/min at $130^{circ}$.until fracture occurred using the universal testing machine. The fracture resistance was measured and the data were analyzed statistically. The fracture above the embedded resin was considered to be favorable and the fracture below the level was considered as unfavorable. The statistical analysis of fracture resistance between different groups was carried out with t-test. For the mode of failure the statistical analysis was carried out by Kruskal-Wallis test and Chi-Square test. RESULTS. For experimental group Vs control group the fracture resistance values showed significant differences (P<.05). For the mode of failure the chi-square value is 16.1610, which means highly significant (P=.0009) statistically. CONCLUSION. Endodontically treated teeth without post core system showed the least fracture resistance demonstrating the need to reinforce the tooth. Stainless steel post with composite core showed the highest fracture resistance among all the experimental groups. Teeth restored with the Glass fiber post showed the most favorable fractures making them more amenable to the re-treatment.

Effects of a Prefabricated Functional Orthodontic Appliance on Children with Class II Division 1 Malocclusion

  • Background: This study aimed to evaluate the effect of a prefabricated functional appliance (Myobrace®) on skeletal, dental, and soft tissue components in children with Class II, division 1 malocclusion. Methods: Thirteen patients with Class II, division 1 malocclusion (9 girls and 4 boys; mean age, 8.2±0.9 years at the start and 9.3±1.0 years at the end of the treatment) were treated with Myobrace® for a mean period of 12.9±4.0 months. Patients were instructed to use the appliance daily for 1 hour and overnight while sleeping. A control group of 10 patients with untreated Class II, division 1 malocclusion (3 girls and 7 boys; mean age, 9.0±1.6 years at the start and 10.4±2.1 years at the end of the observation) was included to eliminate possible growth effects. The mean observation period for this group was 17.7±11.2 months. Lateral cephalograms were taken at the start and end of the treatment, and findings from 41 measurements were analyzed using the V-CephTM program. The mean and standard deviation of cephalometric measurements were analyzed using paired and independent sample t-tests. Results: The treatment group showed significant changes in SNB, ANB, maxillary protrusion, ramus height, proclination of upper anterior teeth, interincisal angle, overjet, and upper lip protrusion compared with the control group. However, only decrease in ANB, maxillary protrusion, overjet, upper lip protrusion, and increase in interincisal angle were significantly higher in the treatment group than in the control group. Conclusion: The prefabricated functional appliance induced skeletal, dentoalveolar, and soft tissue changes, resulting in a significant reduction in anteroposterior discrepancy.
