중등도 이상의 목 통증을 동반한 앞쪽 머리 자세를 가진 중·고등학교 교사들을 위한 물리치료적 프로그램: 머리-목뼈 굽힘근 훈련과 일반적 훈련의 효과 비교 (A Physiotherapy Program for Secondary School Teachers with Forward Head Posture Accompanied by Moderate to Severe Neck Pain: Comparison of the effects of cranio-cervical flexor training and general training)

  • Purpose : This study applied general training (control group) or cranio-cervical flexor training (experimental group) using a pressure biofeedback unit along with general training for 4 weeks to secondary school teachers with moderate to severe neck pain and forward head posture. After that, we tried to compare the effects through differences in neck pain intensity (using numberical rating scale), functional performance (using neck disability index), and cranio-vertebral angle change. Methods : All 50 subjects were randomly assigned to either the "experimental group (n= 25)" or the "control group (n= 25)", and the measurements were evaluated in the same way before the intervention (baseline) and after the intervention (4 weeks). During the intervention period, the subject visited the physiotherapy center and made a reservation three times a week at a fixed time as much as possible, and each training session was thoroughly conducted under the 1:1 guidance of the therapist in charge so that the correct movement and number of times could be performed without compensatory action. Results : As a result of the homogeneity analysis on the general characteristics of the subjects, there were no significant differences between the groups in all variables (p>.05). Compared to the "control group", the "experimental group" showed significant improvement after intervention in all measured variables of neck pain intensity, functional performance, and cranial-vertebral angle (p<.05). Conclusion : For secondary school teachers with forward head accompanied by neck pain, cranio-cervical flexor training using a compression biofeedback unit is an excellent method to show superior pain reduction and functional performance improvement compared to general training alone. In addition, it can be presented as a more effective intervention method that can promote recovery of forward head posture, which is an essential element of the solution.

  • Background: The purpose of this study is to analyze the effect of shoulder and neck stabilization exercise using a gym ball for healthy women in their 20s with round shoulder and forward head posture, it is intended to try and find improvement for posture. Methods: The subjects of the study were 25 women who understood the purpose of the research and voluntarily agreed to participate in the experiment. They carried out the shoulder and neck stabilization exercise program using a gym ball, which was conducted for 35 minutes, 3 times a week for 4 weeks. The position of the round shoulder and shoulder bones was measured using an tape measurement and were recorded in both at pre, 2 weeks, and post intervention. The craniovertebral angle (CVA) and craniorotational angle (CRA) was measured using an image J. Results: The round shoulder posture left was statistically significantly different within group (p<.05); however, the round shoulder posture right wasn't statistically significantly different (p>.05). As a result of this contrast, test were significant different in both at pre, 2 weeks and post. The scapular position on the left and right were statistically significantly different within the group (p<.05). As a result of the contrast, test were significant different in both at pre, 2 weeks and post (p<.05). The CVA and CRA on the left and right were statistically significantly different within the group (p<.05). As a result of the contrast, test were significant different in both at pre, 2 weeks and post (p<.05). Conclusion: The gym ball exercises improved the posture of women in their 20s. Therefore, gym ball exercises can help improve the quality of life of those with shoulder and neck pain.

전방머리자세 성인을 대상으로 모바일 어플리케이션을 이용한 자세정렬 측정의 신뢰도 및 수렴 타당도 연구 (Reliability and Convergent Validity of Standing Posture Measurement Using a Mobile Application in Forward Head Posture)

  • Purpose : The aim of this study was to assess the intra- and inter-rater reliability and validity of measurements of head, neck, and pelvis posture using a mobile application in subjects with forward head posture. Methods : Forty-eight volunteers (22 men, 26 women) participated in this study. Two raters independently examined whole body picture images in a lateral standing posture with arms crossed using a CA-Smart Posture Reminder (CA-SPR), and a rater took and calculated posture images twice to assess reliability. We measured five parameters: craniovertebral angle 1 (CVA1), anterior shoulder translation (AST), pelvic tilt (PT), knee angle (KA), and ankle angle (AA) in the subject's sagittal plane using CA-SPR. We examined whole spine X-ray images in the same position to assess validity. We measured four variables in the subjects: CVA2, translation distance (AHT), anterior pelvic plane (APP), and sacral slope (SS). The intra- and inter-rater reliability were calculated using the intraclass correlation coefficient (ICC). Convergent validity was calculated using Pearson's correlation coefficient. Results : The intra-rater reliability (ICC=.889 -.989) and inter-rater reliability (ICC=.800 -.980) were excellent for all variables measured using CA-SPR. The variables measured using CA-SPR and X-ray were significantly positively correlated (r=.623, p<.01). However, the correlation of the variables in the pelvis was not statistically significant. Conclusion : This study shows that a mobile application (CA-SPR) is a useful tool for measuring head and neck posture in subjects with forward head posture. However, further study is needed to measure pelvic variables when using a mobile application.

Dorsal Neck Muscle Fatigue Affects Cervical Range of Motion and Proprioception in Adults with the Forward Head Posture

  • Purpose: This study was to investigate the effect of dorsal neck muscle fatigue on the cervical range of motion (CROM) and proprioception in adults with the forward head posture (FHP). Methods: Thirty pain-free subjects were enrolled in this study. All subjects were measured the forward head angle by taking the capture of the sagittal plane of their upper body to determine the FHP. Subjects were distributed into two groups: the FHP group (n=14) and Control group (n=16). All subjects were measured the CROM and the Head repositioning accuracy (HRA) for joint proprioception before and after inducing muscle fatigue of the dorsal neck. The CROM and HRA were measured in neck flexion, extension, right-left lateral flexion, and right-left rotation. Sorenson's test was used to induce muscle fatigue of the dorsal neck. Results: Total CROMs were significantly decreased after dorsal neck muscle fatigue in both groups (p<0.05). Total HRAs were significantly increased after dorsal neck muscle fatigue in the FHP group (p<0.05), but there were no significant differences in the control group (p>0.05). Total CROM changes were not significant differences between groups (p>0.05), but total HRA changes were significant differences between groups (p<0.05) except for right and left lateral flexion (p>0.05). Conclusion: Immediate CROM and proprioception reduction after the dorsal neck muscle fatigue were observed in adults with the FHP. Therefore, FHP can significantly affect the CROM and positioning consistency of cervical proprioception.

  • This study developed a responsive healthcare system that users can easily use in real life to prevent turtle neck syndrome by posture correction. We propose a system that naturally induces direct posture improvement by adjusting the height with a responsive cradle through a turtle neck discrimination algorithm detecting the turtle neck posture in real time using a webcam. The turtle neck algorithm was developed based on machine learning, using the points that the distance relationship between the jaw line and the shoulder varies depending on the posture. For the younger age group, which is particularly problematic due to the increase in the use of IT devices, image data in different situations according to the height and posture of the cradle was collected and learned as a support vector machine classifier. In addition, a height-adjustable cradle that can support a laptop has been created and expanded into a responsive cradle that can be controlled with software by interlocking with the Arduino. Therefore, this service enables posture correction of many modern people suffering from turtle neck syndrome and will become an essential platform in the increasing online environment in the non-contact era.

전방머리자세 대상자에 대한 목운동과 골반운동이 척추 굽이에 미치는 효과 (Effects of Neck and Pelvic Exercise Using Swiss Ball on Spinal Curve in Adults with Forward Head Posture)

  • Purpose : The purpose of this study was to compare the effect of pelvic exercise on the CVA and spinal curve in adults with forward head posture compared to the group using only neck exercise when pelvic exercise was performed in parallel with conventional neck exercise. Methods : GPS 400 and Formetric were used to identify craniovertebral angle (CVA), thoracic kyphosis, lumbar lordosis, and pelvic torsion and were measured by an experienced research manager. Forward head posture (FHP) was selected for people whose angle between the line connecting the ear ball and the seventh cervical spine and the horizontal line is 50 degrees or less. The 30 selected students were randomly divided into 15 experimental groups and 15 control groups. Mackenzie exercise and sling exercise were performed for neck exercise in both the experimental group and the control group, and pelvic exercise using a Swiss ball was additionally performed in the experimental group. All data collected in this study were analyzed using SPSS statistics 21.0. Cervical vetebral angle (CVA), thoracic kyphosis, lumbar lordosis, and pelvic torsion were compared at 0 weeks, 3 weeks, and 6 weeks for each measurement factor using ANOVA with repeated measures. For the statistical significance test, the significance level of α was set to .05. Results : The changes of pelvic torsion, thoracic kyphosis and CVA due to pelvic exercise using Swiss Ball and neck exercise were changed over time, but lumbar lordosis were not changed. And there were no significant differences among the groups. Conclusion : In conclusion, the pelvic correction exercise is considered to be an effective exercise for correcting the FHP and requires regular pelvic correction exercises. We expect the results to be used in clinical trials.

관절가동술과 근력강화운동이 앞쪽머리자세 환자의 목기능과 근긴장도에 미치는 영향 (The Effects of Joint Mobilization and Muscle Strength Exercise on Neck Function and Range of Motion in Forward Head Posture)

  • Background: The purpose of this study was to investigate the effects of joint mobilization and muscle strengthening exercises on neck function, range of motion, and muscle tone in patients with forward head posture. Methods: A total 32 subjects were divided into muscle strengthening exercise (n=16) and joint mobilization (n=16) who met the diagnostic criteria for the forward head posture, and the study method used the neck disability index (NDI) and BPM Pro to compare the range of motion. Results: As a result of the study, there was no significant difference between each group, and when looking at the differences before and after each group, there was a significant difference in neck pain in both groups. There was a significant difference in muscle tension in the joint mobilization group, but there was no significant difference in the muscle strengthening exercise group. In the range of joint mobilization, there was a significant difference in the range of left joint mobilization of the joint mobilization group, there was no significant difference in the range of right joint mobilization, and there was no significant difference in the range of joint mobilization of the muscle strengthening exercise group. Conclusion: The above results revealed that joint mobilization and muscle strengthening exercises were effective on neck function, range of motion, and muscle tone in forward head posture patients.

The Effect of Exhalation Breathing Exercise on Respiratory Synergist Muscle Activity and Pulmonary Functions in Patients with Forward Head Posture

  • Purpose: The aim of this study is to suggest an intervention method for clinical use in the future by analyzing the effect of breathing exercise on activity of sternocleidomastoid muscle and scalenus anterior muscle, which are respiratory synergist muscles, and pulmonary functions in patients with forward head posture. Methods: Prior to the experiment, 12 patients (experimental group) performed feedback exhalation exercise along with conventional deep neck exercise, and 11 subjects (control group) performed feedback deep neck exercise along with conventional deep neck exercise. The intervention programs were performed for 40 minutes once a day (three times a week for four weeks). Results: Before intervention, %RMS was measured for surface electromyography (sEMG), and FVC, FEV1, and FEV1/FVC were measured using a spirometer. After four weeks, these items were re-measured under the same condition and analyzed. In within-group comparison of the experimental group, activity of sternocleidomastoid muscle and scalenus anterior muscle showed a significant decrease (p<0.05)(p<0.001), and forced vital capacity (FVC) showed a significant increase (p<0.05). In within-group comparison of the control group, activity of sternocleidomastoid muscle and scalenus anterior muscle showed a significant decrease (p<0.05), and in between-group comparison, there were significant differences in activity of sternocleidomastoid muscle and FVC (p<0.05). Conclusion: Long-term forward head posture restrains exercise performance of the neck and leads to exercise avoidance of the neck during daily activities, thus restraint factors might be created even while breathing. To cut off this link, a constant effort is required and diversified research on the correlation between neck functions and breathing should be conducted.

  • Objective : The purpose of this study was conducted to find treatment of forward head posture and shoulder instability with proprioceptive neuromuscular facilitation concept. Methods : This is literature study with books, seminar note and international course. Results : Forward head posture and shoulder instability has related each other. Forward head posture will make muscle instability, weakness and stiffness on neck and shoulder girdle. It will make pain also. Important muscle are suboccipital muscle, omohyoid muscle, sternoclaidomastoid muscle, scaleni, pectoralis minor, levator scapular and digastric. Conclusion : Treatment of the forward head posture and shoulder instability is provided. It is that treatment of stiff muscle with eccentric muscle work, muscle elongation, muscle strengthening at the structure level and at the functional level for daily activities.
