Meta-Analysis: Association Between Wrist Posture and Carpal Tunnel Syndrome Among Workers

  • Background: Carpal tunnel syndrome (CTS) is a common work-related peripheral neuropathy. In addition to grip force and repetitive hand exertions, wrist posture (hyperextension and hyperflexion) may be a risk factor for CTS among workers. However, findings of studies evaluating the relationship between wrist posture and CTS are inconsistent. The purpose of this paper was to conduct a meta-analysis of existing studies to evaluate the evidence of the relationship between wrist posture at work and risk of CTS. Methods: PubMed and Google Scholar were searched to identify relevant studies published between 1980 and 2012. The following search terms were used: "work related", "carpal tunnel syndrome", "wrist posture", and "epidemiology". The studies defined wrist posture as the deviation of the wrist in extension or flexion from a neutral wrist posture. Relative risk (RR) of individual studies for postural risk was pooled to evaluate the overall risk of wrist posture on CTS. Results: Nine studies met the inclusion criteria. All were cross-sectional or case-control designs and relied on self-report or observer's estimates for wrist posture assessment. The pooled RR of work-related CTS increased with increasing hours of exposure to wrist deviation or extension/flexion [RR = 2.01; 95% confidence interval (CI): 1.646-2.43; p < 0.01: Shore-adjusted 95% CI: 1.32-2.97]. Conclusion: We found evidence that prolonged exposure to non-neutral wrist postures is associated with a twofold increased risk for CTS compared with low hours of exposure to non-neutral wrist postures. Workplace interventions to prevent CTS should incorporate training and engineering interventions that reduce sustained non-neutral wrist postures.

PNF 어깨뼈-골반 패턴이 편마비 환자의 보행에 미치는 영향 -증례보고- (The Influence of Scapular-Pelvic Patterns of Proprioceptive Neuromuscular Facilitation on Hemiplegic Gait -A Case Report-)

  • Purpose: This study examined changes in gait speed and stride length after an intervention involving simultaneous scapular and pelvic patterns of proprioceptive neuromuscular facilitation in a hemiplegic patient. Methods: A 58-year-old woman with left hemiplegia who had complained of slowness of gait speed and weakness of leg strength took part in an intervention involving scapular postdepression patterns on the affected side and pelvic postdepression patterns on the nonaffected side. The intervention was performed with the patient lying on her left side, in a half kneeling position, and in a standing posture. Rhythmic initiation was used for teaching the movements to the patient and improvement of kinesthesia, and a combination of isotonic was employed for increasing strength and irradiation of the scapula and pelvic movement. The intervention took place for 30 min. It was implemented twice a day, 5 days a week, for 3 weeks. After three repetitions, the average time taken to complete the 10-m walk test (10 MWT), in addition to stride length, was measured to determine gait speed. Results: After the 3-week program, the patient's performance in the 10 MWT improved from 21.7sec to 17.1sec, and her stride length improved from 31.4cm to 38.7cm. Conclusion: The results showed that trunk movement exercise, especially coordinative movements of the scapula and pelvis can improve gait speed and stride length by increasing trunk stability and mobility. A combination of pelvic and scapular patterns can facilitate trunk rotation, thereby improving gait speed and stride length.

Statistical Parametric Mapping을 이용한 시상면에서의 양발 착지와 외발 착지의 전략 차이 (Analysis of the Differences of the Shock Attenuation Strategy between Double-leg and Single-leg Landing on Sagittal Plane using Statistical Parametric Mapping)

  • Objective: The purpose of this study was to investigate differences of shock attenuation strategies between double-leg and single-leg landing on sagittal plane using statistical parametric mapping. Method: Nine healthy female professional soccer players (age: 24.0±2.5 yrs, height: 164.9±3.3 cm, weight: 55.7±6.6 kg, career: 11.2±1.4 yrs) were participated in this study. The subjects performed 10 times of double-leg and single-leg landing from the box of 30 cm height onto force plates respectively. The ground reaction force, angle, moment, angular velocity, and power of the ankle, knee, and hip joint on sagittal plane was calculated from initial contact to maximum knee flexion during landing phase. Statistical parametric mapping was used to compare the biomechanical variables of double-leg and single-leg landing of the dominant leg throughout the landing phase. Each mean difference of variables was analyzed using a paired t-test and alpha level was set to 0.05. Results: For the biomechanical variables, significantly increased vertical ground reaction force, plantarflexion moment of the ankle joint, negative ankle joint power and extension moment of the hip joint were found in single-leg landing compared to double-leg landing (p<.05). In addition, the flexion angle and angular velocity of the knee and hip joint in double-leg landing were observed significantly greater than single-leg landing, respectively (p<.05). Conclusion: These findings suggested that negative joint power and plantarflexion moment of the ankle joint can contribute to shock absorption during single-leg landing and may be the factors for preventing the musculoskeletal injuries of the lower extremity by an external force.

Effects of Ankle Joint Taping on Postural Balance Control in Stroke Patients

  • This study aims to examine the effects of taping of the ankle joint on the static and dynamic balance and gait ability of stroke patients. Twenty-six stroke patients receiving physical therapy at a hospital located in Gyeonggi-do were divided equally into a group that had taping in physical therapy and an ordinary physical therapy group. They exercised for 30 minutes each, 3 times per week for 8 weeks from June to August 2011. Romberg's eye open and eye closed tests, limits of stability(LOS), forward and back test, timed up and go test(TUG) and 10-meter gait velocity test were performed to evaluate static balance, dynamic balance, and gait ability, respectively, prior to and 8 weeks after the intervention. Differences within each group in relation to the lapse of time were compared by a paired t-test. Differences between the two groups were compared by an independent t-test. Regarding comparison of differences within each group, all tests resulted in significant changes in both groups after the intervention (p<.05). Comparison of differences between the two groups showed that taping in the physical therapy group had significantly better test results than the ordinary physical therapy group in all measured items(p<.05). The after effects of ankle taping on stroke patients are more efficient and effective than ordinary physical therapy alone in improving balance and gait ability.

Effect of strengthening and elongation exercises of upper extremity muscle to forward head posture correction

  • This study was designed to provide basic data for developing exercise program that helps correcting posture by knowing the effect of strengthening and elongation exercises of upper extremity muscle to forward head posture correction. In this study determined subjects whether they had forward head posture or not. On the basis of the New York state posture rating, if a subject's posture is match up with the normal standard posture, gives 5 points and if the posture is slightly get out of the normal standard posture, gives 3 points and if the posture is apparently get out of the standard, gives 1 points. When determining the forward head posture, if talus, humerus and outer ear center are on the same line, it is determined as normal and if outer ear center is off the line less than 1.0cm, it is a slight deformation and if outer ear center is off the line more than 1.0cm, it is a high deformation. In the study selected people who have more than 1 cm gap between two vertical lines start from outer ear center and acromion separately as subjects. Length between the ideal alignment line measured by using goniometer and temporal region showed statistically significant decrease as $2.36{\pm}1.07cm$ before the intervention and $1.06{\pm}0.88cm$ after the intervention. After 4 weeks of neck and chest extensor muscle exercise, the group who exercised both showed increase in range of neck joint motion and neck flexion of the forward head posture. Meanwhile the group who only exercised neck extensor muscle only and the group who only exercised chest extensor muscle didn't showed statistically significant result. That only the group who exercised both muscles showed significant result is the different with studies before. Because this study didn't target patient who had a lesion, couldn't compare effect of the conservative manner and exercise. However, this study provides the fact that the group who exercised both neck and chest muscle had more effect than the control group.

컴퓨터 작업 시 전방머리자세 교정장치의 효과 (The Effect of Forward Head Posture Correctional Device During Computer Work)

  • Work-related musculoskeletal disorder has been associated with long hours of computer work and prolonged periods of static posture. In clinical settings, postural correction is a common treatment approach for individuals with neck, shoulder, and back pain. This study was designed to identify the effect of Forward Head Posture Correctional Device during computer work. Twelve healthy adults (mean age, 27.4 yrs; mean height, 165.0cm mean weight, 65.8 kg) participated in the study. They had no medical history of neurological or surgical problems with their upper extremity. The subjects were asked to perform Head Forward Posture under the guidance of physical therapists and the measured angles were analyzed using a 3-D motion analysis system. Markers were placed on the C7 spinous process, tragus of the ear and forward head angle was between the line from the tragus to the C7 line and the Y-axis at the C7. The statistical significance of difference between, "without" and "with" correctional device was tested by paired t-test. A level of significance was set at ${\alpha}$=.05. In comparison of the computer work between "without" and "with" correctional device, Forward Head Angle was showed significant difference (p<.05). In conclusion, the range of Forward Head Angle was significantly decreased during computer work with the correctional device. Further research is needed to understand the nature of motor control problems in deep muscles in patients with neck, shoulder, and back pain.

소방대원들의 작업자세에 따른 족저압력 분석 (Analysis of Foot Pressure according to the Work Postures on Fire Fighters)

  • The purpose of this study is to find out the difference of foot pressure according to the firefighter's work postures for providing the basic information to prevent Musculoskeletal disorders. This study was conducted by 9 male firefighters. Work postures were selected for rescuee handling, fire hose and hydraulic rescue equipment work postures. These were divided into 3 position, "High", "Middle" and the postures of taking out and letting down hydraulic rescue equipment were analyzed as starting point and end point respectively. Foot Pressure was used to analyze contact area, peak pressure, and maximum force in terms of work postures, and compared between fire hose and hydraulic rescue equipment work postures. The results of foot pressure are as follows. According to the results of rescuee handling work postures, one person handling posture showed wide contact area and foot pressure showed the highest at right foot. Accoridng to the (High), (Middle), (Low) postures of fire hose, the results didn't show the difference among the contact area, peak pressure and maximum force. As the results of hydraulic rescue equipment work postures, (Low) postures showed the highest in terms of the right foot of contact area, peak pressure and maximum force and (High) postures showed the highest in left foot. The increase of foot pressure lead to be inconvenience of low extremity and muscle fatigue for maintaining postural control cause pain. Thus, it is necessary to design insole-equipped working shoe for reduce the impulse and effect of foot during the rescuee handling work which standing out as foot pressure.

수술후 통증 관리의 Patient-Controlled Analgesia와 마약류의 전통적인 근육내 주사와의 비교 (A Comparison of Patient-Controlled Analgesia and Conventional Intramuscular Opioid Regimen in Relation to their Post-Operative Pain Control and Side Effects)

  • Using a visual analogue scale, we compare the effect of patient-controlled analgesia and conventional intramuscular opioid regimen in 68 patients undergoing lower abdominal or gynecological surgery. We also recorded the incidence of side effects. We checked visual analogue scale 4 hours interval for 30 cases managed by patient-controlled analgesia and 38 cases of conventional intramuscular opioid group managed by obstetrician. We maintained fentanyl $0.33{\mu}g/kg/hr$ and set self administrable bolus dose $5.0{\mu}g$(lockout interval: 15 min) in patient-controlled analgesia group. Conventional intramuscular bolus injection group were administered meperidine 50 mg for 4 hour interval. Mean visual analogue scale scores obtained by patient-controlled analgesia group and intramuscular bolus injection group were $2.49{\pm}0.67$ and $4.53{\pm}1.28$(p<0.05). Side effects such as; no significant incidence of respiratory depression, urinary retention, postural hypotension, nausea, vomiting and pruritus were developed by either group. These results suggest that patient-controlled analgesia was more effective method compared with conventional intraumuscular opioid injection regimen for post-operative pain management.

심리운동과 감각통합치료가 발달장애유아의 운동기술에 미치는 영향 (Effects of Psychomotorik and Sensory Integration on the Motor Skills of Children with Development Disabilities)

  • 본 연구는 또래와의 상호적으로 다양한 움직임을 이끌어 내는 그룹 심리운동과 기능적인 움직임을 이끌어내기 위한 개별 감각통합치료가 유아의 운동기술(motor skills) 향상에 미치는 영향을 비교하여 살펴보고자 하였다. 연구 대상은 본원에 재원 중인 만 4세 아동 2명을 대상으로 2017년 4월부터 2018년 1월까지 주 2회 40분 씩 각각 심리운동과 감각통합치료를 적용하였다. 연구 방법은 B-O test, DeGangi-Berk Test of sensory Integration(TSI), MOT4-6 등의 평가도구를 사용하였으며 그 중 대 동작을 필요로 하는 항목을 선별하여 사전 평가 결과와 프로그램 진행 후 사후평가 결과를 토대로 항목별 향상 정도를 그래프로 비교 분석하였다. 연구 결과는 심리운동과 감각통합치료 모두에서 운동기술 향상을 가져왔으며 특히 균형성 및 자세조절 등에서 두드러지게 향상 된 점수를 획득되어 두 영역의 프로그램이 운동기술에 긍정적인 효과가 있음을 알 수 있었다.

발바닥 체성 감각 저하에 따른 만성 발목 불안정성 환자군과 정상인 군의 Y-balance Test 능력에 미치는 효과 (The Effect of Diminished Plantar Cutaneous Sensation in Y-balance Test between Chronic Ankle Instability (CAI) Patients versus Healthy Individuals)

  • Objective: This study aimed to investigate the effect of diminished plantar sensation in Y-balance test between chronic ankle instability (CAI) patients versus Healthy individuals. Method: A total of 90 subjects and CAI group (N=45) (age: $24.49{\pm}2.52yrs$, height: $173.53{\pm}8.20cm$, weight: $69.62{\pm}12.92kg$) and healthy group (N=45) (age: $24.85{\pm}2.70yrs$, height: $170.27{\pm}7.70cm$, weight: $66.04{\pm}11.60kg$) participated in this study. Participants were tested on the anterior (ANT), posterolateral (PL), and posteromedial (PM) reach directions of the Y-Balance Test before and after a 10-minute of plantar cutaneous sensation application using ice ($2^{\circ}C$). Normalized reach distances were measured 3 times each direction. Results: We observed a decrease in reach-distance scores for the reach directions after diminished plantar cutaneous sensation in all reach directions (p<.01). Also, we observed a decrease in reach-distance scores for the PL, and PM reach directions between groups (p<.05). Conclusion: Our results indicated that dynamic postural control was adversely affected immediately after diminished plantar cutaneous sensation between CAI group and healthy group. Future research may suggest that determine the studies involving more realistic dynamic movement, such as walking or running, landing.
