Multiple vertebral compression fractures are a rare condition and are a serious consequence of postpartum osteoporosis. This report describes the case of a 35-year-old woman who had given birth to her first child 2 months before the onset of pain. Magnetic resonance imaging showed compression fractures of 7 vertebrae. The patient was treated with both conventional, and Korean medicine methods including acupuncture and herbal prescriptions. The patient's progress was assessed using self-reported symptoms, scale scores and laboratory test results. Her pain was gradually alleviated and biochemical inflammation marker levels improved, but her functional status remained severely impaired. Clinical practitioners who treat women in the postpartum period must be aware of osteoporosis and potential vertebral fractures and need to consider Korean medicine as an alternative therapy to help such patients.
Objectives: This study was performed to analyze randomized controlled trial, which studied the effect of Saenghwa-tang treatment on postpartum prolonged lochia and uterine subinvolution. Methods: Researchers searched for randomized controlled trial of based on postpartum prolonged lochia, uterine subinvolution and Saenghwa-tang. The paper search was conducted through 6 online databases on August 10, 2023. Results: 8 studies were included after selection and exclusion criteria. 5 studies compared Saenghwa-tang alone with western medicine. 3 studies compared combined treatment of Saenghwa-tang and western medicine, with western medicine alone. Comparing with control group, the treatment group showed statistically significant improvement on total effective rate, uterine involution, serum fibrinogen, D-dimer, viscosity of blood and plasma, Erythrocyte aggregation, and various symptoms. Conclusions: This study suggests that Saenghwa-tang has benefit for treating prolonged lochia and uterine subinvolution. For reliable evidence, further research is needed to establish safety of Saenghwa-tang, standardize diagnosis criteria and specify the treatment course.
연구목적 : 산후우울증은 여러 요인이 관여하여 발생하며, 이들 위험요인에는 다양한 심리사회적 요인이 포함된다. 이전의 여러 연구결과들을 통해 산후우울증의 위험요인이 알려지고 있지만 아직까지 국내에서 산후우울증을 대상으로 위험요인을 연구한 경우는 별로 없다. 본 연구는 국내에서 산후우울증의 심리사회적 위험요인으로는 어떤 것이 있는지를 알아보기 위하여 시행되었다. 방법 : 일신기독병원을 방문한 출산 후 약 6~8주에 있는 산모 119명을 대상으로 하였다. 설문지를 이용하여 산후우울증의 위험요인에 대하여 조사하고 산후우울의 정도를 측정하였다. 설문지는 이전 연구들을 통해 알려진 위험요인들을 참고하고 산모들의 스트레스와 관계되는 요인들을 추가하여 작성하였다. 산후 우울증을 평가하기 위해 EPDS(Edinburgh postnatal depression scale)를 번안해서 사용하였고, 산후우울의 정도는 측정된 EPDS 접수에 따라 판단하였다. 결과 : 총 대상 산모 119명 중 EPDS에 의해 산후우울증으로 진단될 수 있는 고위험군은 16명으로, 13.45%의 빈도를 보였다. 고위험군에서는 과거 우울증상을 경험한 경우와 결혼생활에 대한 만족도가 낮은 경우가 저위험군에 비해 유의하게 많았다, 또 과거 우울증상을 경험하였던 군, 임신중 우울과 불안증상을 경험한 군, 임신 및 산욕기동안 스트레스 생활사를 경험하였다고 보고한 군, 결혼생활에 대한 만족도가 낮은 군, 그리고 나이가 많을수록 EPDS 접수가 유의하게 높았다. 반면, 산모의 교육수준, 종교, 직업의 유무, 임신과 출산에 따른 직업중단 희망임신 여부 분만방법 수유방법, 유아의 입원여부, 그리고 기대했던 유아의 성별 및 실제 유아의 성별은 우울증상에 영향을 미치지 않았다. 결론 : 이상의 결과로 볼 때 산후우울증은 산후에 비교적 높은 빈도로 발생 가능하며, 우울증의 과거력, 임신동안 우울과 불안증상의 경험, 스트레스 생활사, 결혼생활에 대한 낮은 만족도, 그리고 산모의 연령과 같은 요소들이 산후우울증의 위험요인이었다. 임신과 산욕기 동안 산모에 대하여 적절히 정신의학적으로 개입함으로서 조기에 산후우울증을 발견하고 치료할 수 있으며, 산후우울증으로 인하여 산모와 유아에게 미치는 다양한 손실을 예방할 수 있을 것으로 생각된다.
A study was done to identify mothers' perception of fatigue in postpartum period. Questionnaires were collected from 267 normal postpartum mothers in Korea from July to December 2000. The questionnaires used to measure mothers' fatigue were "Subjective Symptoms of Fatigue Test" designed by Research Committee of Industrial Fatigue in Hygienic Association of Japan Industry. The collected data were analyzed by SPSS-Win. The results revealed that The average fatigue score of subjects was 1.70; Physical fatigue had the highest value with a mean of 1.89, followed by psychological fatigue with a mean score of 1.62, neuro-sensory fatigue was rated lowest with a mean of 1.58. Sorting out the subjects fatigue level per a week during Sweeks, 6th week after postpartum had the highest value with a mean of 1.85. Degree of fatigue was also compared with the respect to the general characteristics of mothers: There were statistically significant differences according to mothers' job (t=-2.021, p=.046), sleeping time (t=2.771, p=.006), having a nap (t=-2.132, p=.034), baby character (F=3.089, p=.047), baby sex (t=-2.054, p=.041), having a person who help domestic affairs (t=2.292, p=.023). Therefore variable nursing intervention should be provided to alleviate the fatigue according to the postpartum period. Especially the nursing intervention program to alleviate the physical fatigue is needed.
Objective : The aim of this study is to investigate rota virus infection of neonates In the Postpartum care center. Methods : This clinical study was carried out with the 205 neonates who admitted to the Postpartum Care Center of ${\bigcirc}{\bigcirc}$ Oriental Medicine Hospital, from July, 2004 to July, 2005. Stools were examined daily with Rotazyme ELISA tests(Bio Line Rota Virus, SD Standard Diagnostics, INC.) for 3 days from the day of admission. Supplementary test was examined when change was found in stools. Results : Among 205 newborn babies, 58 cases turned out to be positive. The positive rate of rota virus was not significantly related to sex, birth weight, gestation age. But, it was significantly related to delivery method and it was higher in caesarean section group. The manifestation rates of symptoms were 51.7% in Rotazyme positive neonates. In the Rotazyme positive 58 cases, 30 cases(51.7%) had diarrhea, 8 cases had fever(13.8%), 1 case(1.7%) had vomiting. The positive rate of rota virus was higher during from october to march. There were 190 cases(92.2%) of mixed feeding and 15 cases(7.8%) of artificial feeding. There was significance between breast feeding and the positive rate of rota virus. There was no significance between hospital of birth and the rate of positive rotazyme test. Conclusions : A further study of neonates in postpartum care center is necessary as preference of postpartum care center was increased. And the oriental medicine treatment of rota virus IS expected to be effective, so a further study is necessary.
Objectives: This study analyzed medical records and online survey of mothers who received herbal medicine treatment using the Jeollabuk-do Postpartum Health Support Project. Through this, we would like to discuss the proposal of a postpartum questionnaire of korean medicine for the initial examination, and the project direction. Methods: This study investigated the medical records of 46 mothers who received herbal medicine treatment using the Jeollabuk-do Postpartum Health Support Program at one korean medical hospital. The mother's general characteristics, past history, symptoms, and herbal medicine prescription were analyzed. In addition, through the 33 mothers who responded to the online survey questionnaire, 13 items including medication satisfaction, lactation, and side effects were investigated. Results: The average age of the subjects was 34.46±4.27 years. Musculo-skeletal pain was the most common in both medical records and online surveys. The date of visit from the date of delivery was 47.56±57.15 days. Boheo-tang was prescribed the most, and 84.8% of mothers answered positively about their health after taking herbal medicine. Conclusions: The satisfaction of mothers who received herbal medicine treatment through this project was very high. There were no significant side effects on the mother and the breastfeeding baby who took herbal medicine. In the future, support projects for postpartum korean medical treatment at the national or local level will be necessary.
전열침요법을 출산 후 장요인대 등 천장관절 부위의 인대 손상으로 인한 산후 요통을 호소하는 환자에 적용하여 호전된 3례를 통하여 전열침요법이 인대 손상으로 인한 산후 요통 환자 치료에 유효한 효과를 나타냄을 알 수 있었다. 이상의 결과로 미루어 전열침요법을 인대 손상으로 인한 산후 질환에 적용하면 양호한 임상 결과를 가져올 것으로 기대되며, 향후 더 많은 연구가 필요할 것으로 생각된다.
Background: Childbirth represents a significant transition for women, with physical and psychological sequelae. Reentry to the workplace during the postpartum period is understudied, with implications for maternal well-being and job-related outcomes. This study's aim was to examine selected pregnancy, childbirth, and return-to-work correlates of overall self-rated health within the first month of work reentry after maternity leave. Methods: Between December 2016 and January 2017, we surveyed women employed at a large, public Midwestern university who had given birth in the past five years (N = 249) to examine self-rated overall health in the first month of work reentry. Using ordinal logistic regression, we examined whether physical or psychological health problems during pregnancy, childbirth complications, length of maternity leave, and depression and anxiety at work reentry were related to overall health. Results: Women who experienced depression (odds ratio [OR] = 0.096 [95% confidence interval {CI} = 0.019 to 0.483, p = 0.004]) and anxiety (OR = 0.164, [95% CI = 0.042 to 0.635, p = 0.009]) nearly every day reported worse health at work reentry than those with no symptoms. Controlling for demographics and mental health, women who experienced medical problems during pregnancy (OR = 0.540 [95% CI = 0.311 to .935, p = 0.028]) were more likely to report poor health, while taking a longer maternity leave (OR = 14.552 [95% CI = 4.934 to 42.918, p < 0.001]) was associated with reporting better health at work reentry. Conclusion: Women who experience medical complications during pregnancy, return to the workplace too soon after birth, and experience mental health symptoms are vulnerable physically as they return to work.
Purpose. This study was conducted to investigate the effectiveness of pelvic floor muscle exercise using biofeedback and electrical stimulation after normal delivery. Methods. The subjects of this study were 49 (experimental group: 25, control group: 24) postpartum women who passed 6 weeks after normal delivery without complication of pregnancy, delivery and postpartum. The experimental group was applied to the pelvic muscle enforcement program by biofeedback and electrical stimulation for 30 minutes per session, twice a week for 6 weeks, after then self-exercise of pelvic floor muscle was done 50-60 repetition per session, 3 times a day for 6 weeks. Maximum pressure of pelvic floor muscle contraction (MPPFMC), average pressure of pelvic floor muscle contraction (APPFMC), duration time of pelvic floor muscle contraction (DTPFMC) and the subjective lower urinary symptoms were measured by digital perineometer and Bristol Female Urinary Symptom Questionnaire and compared between two groups prior to trial, at the end of treatment and 6 weeks after treatment. Results. The results of this study indicated that MPPFMC, APPFMC, DTPFMC were significantly increased and subjective lower urinary symptoms were significantly decreased after treatment in the experimental group than in the control group. Conclusions. This study suggested that the pelvic floor muscle exercise using biofeedback and electrical stimulation might be a safer and more effective program for reinforcing pelvic floor muscle after normal delivery.
Objectives : The purpose of this study is to report the effect of traditional Korean medicine therapy on Bell’s palsy during early puerperium. Methods : The patient who had Bell’s palsy during early puerperium were treated with traditional Korean medicine such as Gunggwijohyeol-eum, Saenghwa-tang, Gunggwijohyeol-eumhabBulsu-san and Sasang constitution medicine. The progresses of symptoms were evaluated by House-Brackmann facial nerve grading system and Numeric Rating System (NRS). Results : After treatment, the patient’s symptoms of Bell’s palsy and puerperium were improved. Conclusions : This case study shows that the postpartum care in traditional Korean medicine therapy and Sasang constitution medicine is effective for the patient who had Bell’s palsy during early puerperium.
