A Model for Community Based Mother Infant Care Center - TMIC(transitional mother infant care center) using a Sanhujoriwon - (산후조리원의 모자건강관리 현황과 제도화방안 - 지역사회중심의 모자건강관리센터(TMIC) 개발을 위한 전략 -)

  • The purpose of this study was 1) analyze the current state of Sanhojoriwon; and 2) to suggest the new model for the community based mother infants health care delivery system: strategies of TMIC are related to Public Health policy, cost-effectiveness, mother infant care provision of medical professionalism, and so on. Method: Forty-seven workers from seventeen Sanhojoriwon participated to analyze several aspects of Sanhojoriwon. Using a questionnaire developed at Korean Sanhojori Research Forum (KSARF), such as the traditional and medical concept of the Sanhojori, postpartum care, Korean traditional postpartum care, job description on women and infant care at Sanhojoriwon, professional management, health care policy and the educational need. Results: Based on the descriptive study results, the TMIC, the community based transitional mother infants care center was suggested as a new model for the cyclic public health care system related on the reproductive health, using an already existing related center, Sanhojoriwon. Also, several strategies were presented on the TMIC.

A Study on the Effect of Omibaekchul-san on Postpartum Edema - Comparison with Saenghwa-tang (오미백출산이 분만 후 산모의 부종에 미치는 영향 - 생화탕과 비교하여)

  • Objectives : The aim of this study was to investigate the effect of Omibaekchul-san on postpartum edema in women hospitalized at korean medical postpartum care center in comparison with Saenghwa-tang. Methods : The body composition data was collected from 102 women, who took postpartum care in postpartum care center attached to Won-kwang University Jeonju Korean Medical Hospital, from March 2012 to July 2013. The data was analyzed by Body Composition Analyzer (Inbody 720). Specifically the data was composed of the Extra Cellular Fluid/Total Body Fluid (ECF/TBF), Total Body Water (TBW), Intra Cellular Water (ICW), Extra Cellular Water (ECW), Body Weight, Body Mass Index (BMI), Waist Hip Ratio (WHR), and the ECF/TBF of each body parts such as the upperlimb, lowerlimb and trunk. The body composition was measured twice during 2~8.5 days from childbirth and once again after 7~13.5 days from childbirth by Body Composition Analyzer. Results : The Omibaekchul-san -treated group showed a significant decrease of ECF/TBF, ECW, TBF, Body weight, BMI, ECF/TBF of each body parts and increase of WHR in comparison with the data of the Saenghwa-tang-treated group. In two groups, only the change of ICW was unsignificant. Conclusions : Statistically this study suggests that the Omibaekchul-san will help the quality of life of women who are in postpartum edema.

Effects of Newborn Care Education for First-time Fathers on Their Knowledge and Confidence in Newborn Care at Postpartum One Month (첫 아기 아버지에 대한 신생아 돌보기 교육이 산후 1개월 돌보기 지식과 자신감에 미치는 효과)

  • Purpose: This study was conducted to identify the effects of newborn care education for fathers on their knowledge and confidence in newborn care at postpartum one month. Methods: A nonequivalent control group pretest posttest design was used. The participants were 53 first-time fathers of newborns, 27 in experimental group, and 26 in control group. They were recruited at the nursery of one municipal hospital in Seoul. For the experimental group, a 50-minute education on newborn care using video, verbal education, demonstration and practice were provided prior to discharge. Fathers' knowledge and confidence in newborn care and their satisfaction with the education program were measured at postpartum one month. Results: The fathers in the experimental group showed significantly higher knowledge (t= -4.51, p<.001), and confidence in newborn care (t= -2.29, p=.026) compared to the control group at postpartum one month. Fathers in the experimental group had a satisfaction score of $27.37{\pm}2.73$ immediately after the education, and $25.30{\pm}3.40$ at postpartum one month. Conclusion: Results indicate that newborn care education for first-time fathers is an effective method in enhancing the level of knowledge and confidence in newborn care. It can be used in the nursery department before discharge as a useful nursing intervention.

A Prospective Observational Study on Symptoms of the Postpartum Women with Korean Medicine Treatment during Six Weeks after Childbirth (한의치료를 받은 산모의 산후 6주간 증상에 대한 전향적 관찰 연구)

  • Objectives: This study aims to investigate the changes of women's postpartum symptoms, including their weights, edema index and quality of life (QOL) over the first six weeks after childbirth. Methods: The study participants were 31 postpartum women treated in the Dept. of Obstetrics & Gynecology, at Woo-Suk University Korean Medicine Hospital, from May 20th to August 5th, 2016. In the first 2 weeks of the postpartum period, the main data collection method was to interview each patient and write down all of their complaints, and additionally referring to each participant's PHR (Personal Health Records). At the 3rd, 4th, 5th and 6th week, the researcher interviewed participants by phone once a week. The participants' weight, edema index (ECW/TBW) and the quality of life (by EQ-VAS) were measured at 1st and 2nd week after childbirth. And, EQ-VAS was assessed once more at the 6th week after childbirth. As a last step, the participants responded to a survey on satisfaction regarding their postpartum care with Korean Medicine. Results: 1. In the 1st one week after childbirth, edema was the most frequent complaint. It was about joint pain of the upper limbs in the 2nd and 3rd weeks, sweating in the 4th week, and joint pain of the upper limbs in the 5th and the 6th weeks. 2. In the 1st two weeks of the postpartum period, the weight of the participants decreased from $66.33{\pm}9.30kg$ to $62.60{\pm}8.92kg$ (p<0.001) and the edema index ECW/TBW decreased from $0.399{\pm}0.010$ to $0.385{\pm}0.0 4$ (p<0.001). The EQ-VAS significantly improved from $61.77{\pm}17.72$ to $73.51{\pm}14.67$ (p<0.001). In the last 4-weeks of the postpartum period, the EQ-VAS decreased from $74.30{\pm}14.25$ to $73.63{\pm}13.35$, but this difference was not statistically significant (p=0.749). 3. Regarding the satisfaction with postpartum care with Korean Medicine, 60% of the participants responded that it was 'Excellent' and 40% said it was 'Good'. Conclusion: Over the entire postpartum period, the most frequent complaints were about musculoskeletal symptoms. Postpartum care with Korean Medicine treatments in the early postpartum period improves various postpartum symptoms and the quality of life for postpartum women.

A Survey on Health Management of during Pregnancy, Childbirth, and the Postpartum of Immigrant Women in a Multi-cultural Family (다문화가정 이주여성의 임신과 출산 건강관리 실태)

  • Purpose: The purpose of this study was to investigate health management state during pregnancy, childbirth, and postpartum of immigrant women. Methods: A descriptive research design was employed. Data were collected from 182 immigrant women who lived in four provinces and D city. Subjects were completed the following questionnaires: demographic, medical service use, pregnancy and childbirth, and health management during the ante-postpartum. Results: The average age of subjects was 28.96 and 83 subjects (45.6%) were coming from Vietnam. 55 Subjects (30.2%) were within the low-income group with less than 4 million won per month. So, 62 subjects were uninsured women because of the expensive costs. Subjects mainly visited a health care center with their husband. On the using a health care center, subjects complained about communication difficulties and transportation problems. 42 Subjects were pregnant but 21.4% of pregnant subjects did not receive ante-natal care. Subjects got information about pregnancy and childbirth through their husbands and husband's family. Conclusion: Nursing intervention to manage the pregnancy, childbirth, and the postpartum of immigrant women need to be developed and should include strategies to take care of themselves after delivery and provide knowledge and information about ante-postpartum.

Clinical Study of Oriental Medical Treatments for Neonates in Oriental Medicine Hospital Postpartum Care Center (한방병원 산후조리센터 내 신생아의 한방적 처치에 대한 임상적 고찰)

  • Objective : The aim of this study is to investigate the effects of oriental medical treatments for neonates in the Postpartum care center. Methods : This clinical study was carried out with the 186 neonates who admitted to the Postpartum Care Center of Conmaul Oriental Medicine Hospital, from November, 2003 to May, 2004. The data of sex, delivery method, gestation age, birthweight, symptoms, herbal prescription, improved condition and other treatments was collected via the medical charts. Results : Among 186 newborn babies, 48 cases had diarrhea. We used Gwakhyangjeonggi-san for 31 cases, Bulwhangumjeongnggi-san for 5 cases, compound prescription of the two for 8 cases. The effect of treatment was excellent for 25 cases, good for 11 cases, not good for 8 cases. In fever, there were 39 cases. We used Gwakhyangjeonggi-san and venesection of blood for 17 cases, only venesection of blood for 20 cases, and only Gwakhyangjeongg-isan for 3 cases. The effect of treatment was excellent for 37 cases, good for 3 cases. In cold, there were 4 cases. We used Samsoum for 1 case, Socheongryong-tang for 3 cases. The effect of treatment was excellent for 2 cases, good for 1 case, not good for 1 case. Conclusions : This study shows oriental medical treatments like acupucture, herbal medicine, and venesection of blood for neonates have significant effects. we will need further prospective studies about effects of these oriental medical treatments.

The Effect of Postpartum Depression on Breast-Feeding Practice in Puerperium Mothers (산욕기 산모의 산후우울이 모유수유 실천에 미치는 영향)

  • The purpose of this study is to identify the relationship between postpartum depression and breast-feeding in puerperium mothers to practice healthy breast-feeding. Puerperium mothers at postnatal care center completed an anonymous survey. Postpartum depression severity was observed at ($x^2=19.556$, p = 0.000). Postpartum depression was found to negatively affect healthy breast-feeding at a rate of Exp (B) = 0.887, p = 0.014. Puerperium mothers' postpartum depression has shown significant effect on breast-feeding practice, thus, when postpartum depression rate decrease, regular breast-feeding practice rate increased. Despite lacking robust statistical evidence, these results indicate that postpartum depression leads to bottle-feeding.

The effect of Kami-Saenghwatang and Kami-Bohertang to postpartum weight retention (가미생화탕과 가미보허탕의 연속 투여가 산후 체중정체와 체성분 변화에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the relationships between the herbal medicines used for women in puerperium on postpartum care, Kami-Saenghwatang(SHT) and Kami-Bohertang(BHT) and postpartum body composition change. We also examined the influence of gestational age, the number of parity and the route of delivery for gestational weight gain and postpartum weight loss. Methods: 239 women followed by postpartum care center. Variety factor s were conformed and then checked body composition analysis. 8 weeks later, we followed up body composition of 36 women among the former checked. and additionally asked the body weight of another 42 former checked women by telephone call. Results: $Mean{\pm}S.D$(standard deviation) of pre-pregnancy weight is $55.22{\pm}8.98kg$, $Mean{\pm}S.D$ of gestational maximun weight is $69.26{\pm}9.77kg$, $Mean{\pm}S.D$ of weight loss in postpartum 8 weeks(means sub-tract postpartum 1 week weight from postpartum 8 week weight) is $57.86{\pm}8.60kg$. The number of parities had corrrelation with gestational weight gain. Age, number of parities and route of delive were not significantly correlated with postpartum weight loss during 8 weeks after delivery, and also it was not correlated with body water mass, body fat mass, body mass index and body weight on postpartum 8 weeks whether they treated with SHT and BHT. conclusion: It was not correlated with body water mass, body fat mass, body mass index and body weight on 8 weeks postpartum whether they treated with SHT and BHT.

A Study on the Postpartum depression in Postpartum Women. (산욕기 산모의 산후우울정도와 영향요인)

  • The purpose of this study was to provide the basic data for developing a program for effective prevention for postpartum depression (PPD) by investigate the level of PPD in postpartum women. The subjects were 104 women. The data were collected from march, 2003 to June, using a 36 item questionnaire and analyzed by SPSS program for t-test, ANOVA, multiple regression. The results were as follow 1. The score of Postpartum depression was 44.1. The level of PPD according to General Characteristics was significantly difference according to home care need. 2. The level of PPD according to obstetrical characteristics was significantly differences according to postpartum complication(p<.05). depression in pregnancy(p<.01), baby's health state(p<.01). 3. The variables to predict postpartum depression in postpartum women are depression related to depression during pregnancy, complication after delivery, and a baby's condition. As the result of multiple regression analysis, variables bringing about postpartum discomfort were depression during pregnancy, complication after delivery, and a baby's condition, and this model showed the explanatory power at 28.8%. In conclusion, it is necessary to care mothers belonging to a risk group more concentrically by taking the factors causing postpartum depression into account, and to keep on studying repeatedly in order to raise the number of objects and to find related variables because this study has more or less limited objects not enough to generalize a bit. It is also necessary to study to make a program of arbitration in nursing of postpartum depression actually.

Comparison of Knowledge and Confidence of Newborn Care between Mother and Father (어머니와 아버지의 신생아 돌보기 지식과 자신감 비교)

  • Purpose: This study was to compare the knowledge and confidence of newborn care between mother and father. In furthermore, to develop a nursing intervention based on the data. Methods: A descriptive design was used, the participants of this study were 85 couples of mother and father of newborn, hospitalized in postpartum care center. Knowledge and confidence of newborn care was measured from the two postpartum care center located in G province. Descriptive statistics, paired t-test, t-test and ANOVA were used to analyze the data using SPSS. Results: Comparing to the mothers', the scores for fathers' knowledge and confidence of newborn care were lower. Particularly, fathers of first-born showed lower score of knowledge and confidence of newborn care. Also, fathers were lacking of knowledge and confidence related to health problem management. Conclusion: The results showed that nursing intervention which encourage fathers to raise the confidence in involvement of newborn care is needed to be developed. Also evaluation of the effect of newborn care education for parents, and longitudinal study of effect on infant and child development is suggested.
