총의치의 교합면 형태에 따른 저작 효율 및 기능에 관한 연구 (A STUDY ON MASTICATORY PERFORMANCE AND FUNCTION BY POSTERIOR OCCLUSAL SCHEMES IN COMPLETE DENTURE)

  • This investigation was designed to determine the effectiveness of the posterior occlusal schemes on masticatory activity during mastication in complete denture. Twelve edentulous subjects were selected for this study. All subjects had no past history and no functional abnormality on masticatory system and TMjoint. And, they had residual ridge of favorable morphology, firm mucosa and Class I skeletal jaw relationship, Twelve experimental denture with interchangeable occlusions(0-degree teeth, 30-degree teeth, Levin teeth and S-A teeth) were constructed for this study. The masticatory performance was analyzed by means of standard sieve(10, 16, 20, 30sieve), and the electrical activity from selected muscles(Temporalis and Masseter muscle) was recorded simultaneously with electromyography (Bio-Pak system) as the subject masticated test foods (rice, peanut and gum) with four different occlusal schemes. Mandibular movement was, also, measured with Sirognathography(Bio-Pak system). These recordings were performed in immediately, after 1 week and after 2 weeks of insertion of complete denture. The results were as fellows; 1. The average masticatory performance of 0-degree artificial teeth was higher than any other artificial teeth. 2. Masticatory performance in denture wearer was affected preferentially by food and artificial occlusal schemes. 3. During chewing, there was a statistical difference of EMG activity between masseter and temporal muscle(p<0.01). Especially, EMG activity of working masticatory muscle was highly affected by food rather than by artificial occlusal schemes. 4. In denture wearer, the velocity of opening was not affected by food, whereas, the velocity of closing was faster in soft food chewing than in hard food chewing, and the amount of vertical displacement was grater in chewing of soft and large bolus than in chewing of hard and small bolus. However, the amount of lateral displacement showed conversely(p<0.05). 5. It was considered that masticatory performance in denture wearer is not affected by the condition of residual ridge. the history of denture wear, the preference, the adaptation to artificial teeth and the total mesiodistal length of artificial posterior teeth.

임플랜트로 수복된 하악 구치부에 대합되는 자연치 주변의 골변화 (ALVEOLAR BONE CHANGES AROUND THE NATURAL TEETH OPPOSING THE POSTERIOR IMPLANTS IN MANDIBLE)

  • Statement of problem: Alteration of tooth function is assumed to be changed by stress/strain on the adjacent alveolar bone, producing changes in morphology similar to those described for other load-bearing bones. When teeth are removed, opposing teeth will not be functioned. When edentulous area is restored by implant prostheses, opposing teeth will be received physiologic mechanical stimuli. Purpose: The aim of this study was to evaluate the bone changes around the teeth opposing implant restoration installed mandibular posterior area. Material and method: Eight patients who had mandibular posterior edentulous area were treated with implants. Radiographs of the opposing teeth were taken at implant prostheses delivery(baseline), 3 months, and 6 months later. Customized film holding device was fabricated to standardize the projection geometry for serial radiographs of opposing teeth. Direct digital image was obtained. Gray values of region of interest at each digital image were measured and compared according to time lapse. Repeated measured analysis of variance and post-hoc Scheffe's test were performed at the 95% significance level. Results: Alveolar bone changes around the natural teeth opposing the posterior implant in mandible showed statistically significant difference compared to control group(P<0.05). And gray values of alveolar bone around the teeth opposing implants were increased. There were no statistically significant differences of alveolar bone changes between crestal group and middle group and between mesial group and distal group according to time lapse(P>0.05). There were no statistically significant differences of alveolar bone changes among mesial-crestal group, mesial-middle group, distal-crestal group, distal-middle group, and control group(P>0.05). Conclusion: Alveolar bone around the natural teeth opposing the implant prosthesis showed gradual bony apposition.

  • This study was performed to investigate the influence of occlusal change on the masticatory muscle activity. 8 students without any symptom of T.M.J. dysfunction, any history of prosthodontic or orthodontic treatment on dental college of WonKwang Univ. were participated in this study. The activity of masseter and anterior temporal muscles were measured by bioelectric processor(EM2, Myotronics, Inc., U.S.A.) during voluntary maximal clenching on natural teeth, by splint with bilateral posterior surface contact, by splint with unilateral posterior surface contact, and by splint without unilateral posterior teeth contact. The obtained results were as follows ; 1. The loss of posterior contact on noe side resulted in change of the activity of anterior temporal and masseter muscle during clenching on ipsilateral side, but there was no change of muscle activity on contralateral side. 2. The activity of anterior temporal and masseter muscle during clenching were not affected by the pattern of occlusal contact. 3. There were no difference between the activity of anterior temporal and masseter miuscle during clenching by natural teeth and by occlusal splint.

Use of separate single-tooth implant restorations to replace two or more consecutive posterior teeth: a prospective cohort study for up to 1 year

  • PURPOSE. The aim of this study was to evaluate the periodontal and prosthodontic complications of multiple freestanding implants in the posterior jaws for up to 1 year of function. MATERIALS AND METHODS. Eight patients received 20 implants posterior to canines. Two or more implants were consecutively inserted to each patient. Single crowns were delivered onto the implants. Marginal bone loss, implant mobility, probing depth, and screw loosening were examined to evaluate the clinical success of such restorations for maximum 1 year of functional loading. RESULTS. All the implants performed well during the observation period. Neither periodontal nor prosthodontic complications were found except a slight porcelain chipping. While the marginal bone level was on average 0.09 mm lower around the implant after 6 months of loading, it was 0.15 mm higher after 1 year. CONCLUSION. Within the limits of this investigation, separate single-tooth implant restorations to replace consecutive missing teeth may clinically function well in the posterior jaw.

Posterior maxillary segmental osteotomy for management of insufficient intermaxillary vertical space and intermolar width discrepancy: a case report

  • Backgrounds: Insufficient intermaxillary space is caused by non-restoration following tooth extraction in the past, and this involves eruption of the opposing teeth and changes of the arch structure. Such cases are difficult just by a simple prosthetic approach, and diversified treatment plans should be established. Among these, posterior maxillary segmental osteotomy (PMSO) is an efficient treatment option than extraction of opposing teeth as it surgically repositions multiple erupted teeth and alveolar bone. PMSO can preserve the natural teeth; therefore, it is being regarded as a treatment method which can improve insufficient intermaxillary space significantly. Case presentation: In this case report, the first patient received PMSO in order to place an implant in the mandibular edentulous space after decreased vertical dimension is restored, and the second patient received PMSO along with orthodontic treatment to obtain the intermaxillary space and balance the interarch molar width. Conclusion: PMSO is the treatment of choice when occlusion is compromised in the presence of decreased vertical dimension or arch length discrepancy.

Treatment of anterior open bite by posterior maxillary segmental osteotomy and miniplates: a case report

  • Background: Anterior open bite is a challenging malocclusion to correct orthodontic treatment. Anterior open bite associated with over-erupted posterior teeth and long lower facial height should be treated by reduction of posterior dimension for esthetic results. Although the possibility of orthodontic treatment of an anterior open bite has increased with the introduction of skeletal anchorage, there are still cases requiring surgery for various reasons. Case presentation: This case report covers an anterior open bite of a 25-year-old man successfully treated with the posterior maxillary segmental osteotomy (PMSO) and miniplates. After the pre-surgical orthodontic treatment, the PMSO between canines and first premolars was performed under local anesthesia and miniplates were placed on the zygomatic buttress. As a result of 28 months of treatment, an impaction amount of 3.5 mm was obtained in the maxillary posterior teeth, and the facial esthetics improved at rest and smile. Conclusion: The impaction of the posterior dentoalveolar segment using the PMSO can be a good treatment option in patients with anterior open bite showing long lower facial height.

한국 성인 유치악자의 상악전치 위치에 관한 연구 (A Study on the Position of the Maxillary Anterior Teeth in Korean Adults with Natural Dentition)

  • It is essential to establish the suitable position for artificial maxillary anterior teeth, because of not only esthetics, phonetics, mastication, but also optimal position of artificial posterior teeth for the construction of functional and esthetic prostheses. Anatomic landmarks have been used in the arrangement of artificial teeth. Such as incisive papilla and palatal rugae are useful landmarks for positioning occlusal rim and upper anterior artificial teeth because they are relatively stable and to be identified on master cast. Therefore, if average distance between maxillary anterior teeth and landmarks in dentate subjects are measured and applied, appropriate position of occlusal rim can be initially established. In this study, to present a guide to the position of the occlusal rim for upper anterior teeth of edentulous patients, horizontal distance between anatomic landmarks were measured. Maxillary casts were made in 72 Korean dentate subjects. Horizontal distance between central incisor and incisive papilla, between incisive papilla and intercanine line, and between primary palatine rugae and gingival margin of canine were measured on each cast. The results of this study were as follows ; 1. The mean distance from the incisal edge of central incisor to the posterior border of incisive papilla was 12.1 mm (Male 12.2 mm, Female 11.9 mm). 2. The mean distance between posterior border of incisive papilla and intercanine line was 3.5 mm (Male 3.4 mm, Female 3.6 mm / Left 3.6 mm, Right 3.4 mm). 3. The mean distance from the palatal gingival margin of canine to the lateral border of primary palatine rugae was 2.4 mm (Male 2.4 mm, Female 2.4 mm / Left 2.4 mm, Right 2.3 mm). 4. On all measured items, there were no significant differencies in measured values between male and female, and between left and right sides. (P>0.05).

Orthopantomogram상에서의 치아 장경 측정에 관한 연구 (A STUDY ON MEASUREMENTS OF TOOTH LENGTH IN ORTHOPANTOMOGRAM)

  • This study was performed to evaluate the accuracy of orthopantomogram by measuring the actual and radiographic tooth length and by analyzing the vertical magnification rate. For this study, total 90 teeth of the maxilla and mandible teeth and a dry skull were used. This experiment was attached with metal balls of 1±0.02㎜ at the root and the crown cusp tips of central, 2nd premolar, 1st molar of the maxilla and mandible and the teeth were embedded in dry skull, and then orthopantomogram was taken. The obtained results were as follows: 1. The average of tooth length in orthopantomogram was longer than that of actual tooth length. 2. The average of vertical magnification rate in the orthopantomogram to actual tooth length was 17-26%. 3. Vertical magnification rate of the maxilla teeth was 18-26% and that of mandibular teeth was 17-23%, and the magnification of maxillary teeth was larger than that of mandible teeth(P<0.0l). 4. Vertical magnification rate of posterior area was 22-26% and that of anterior area was 17-18%, and the magnification of anterior area was less than that of posterior area(P<0.01).

CT 이미지를 사용한 Micro-implants 식립을 위한 매부학적 연구 (An Anatomical Study using CT Images for the Implantation of Micro-implants)

  • 교정치료의 고정원으로서 수술용 Microscrew가 소개되고 사용되어 왔다. 몇몇 임상가들은 Miniscrew혹은 Microscrew를 치근의 손상을 피하기 위하여 치근 하방의 기저골에 식립하여 사용하여 왔다 그러나 저자는 Microimplant를 치근 손상없이 치근사이의 치조골에 식립하여 고정원으로 사용하여 왔다. 이렇게 하므로서 치근하방에 식립할때 생기는 문제점인 수직 분력이 커지고 수평 분력이 작아지는 것을 방지하여 상악의 경우 적절한 후상방 교정력을 가할 수 있다 그리고 치조골에 상악의 경우 치아 장축에 대하여 30-40도의 각도로, 하악의 경우 10-20도의 각도로 식립하여 교정력의 방향을 전술한 바와 같이 수평방향으로 바꾸고 긴 Micro-implant를 치근 손상의 위험성을 줄이며 식립하여 안정도를 높일 수 있었다. 그러나 Mciro-implant를 치근사이 치조골에 식립하는데 기준이 되는 어떠한 연구도 이루어 지지 않았다. 본 연구는 Micro-implant식립에 기준이 되는 즉 치근 손상 없이 비교적 안전하게 식립할 수 있는 부위의 선택을 위한 근거 자료를 얻기 위하여 시행되었다. 21명 환자를 대상으로 치조골로부터 5-7mm에 해당하는 CT 절단면을 선택하여 피질골의 두께, 피질골 표면과 치근과의 거리, 치근사이의 거리등을 측정하였다. 각 부위별 피질골의 두께, 골 표면과 치근사이의 거리, 인접 치근사이의 거리등을 구하였다. .피질골의 두께는 전치부에서 구치부로 이행할수록 두꺼웠다. 특히 하악골 구치부에서 가장 두꺼웠다. 인접 치근 사이의 거리 계측 항목에서 상악에서는 제2소구치와 제 1대구치사이, 하악에서는 제 1대구치와 제 2대구치 사이에서 가장 큰 값을 보였다.
