Comparison of Hip Joint Strength between Young Woman Patient with Chronic Low Back Pain with Lumbar Instability and Normal Subjects

  • Objective: The purpose of this study was to compare the hip joint muscle strength of patients with chronic back pain with lumbar instability and normal subjects. Design: A case control study. Methods: Five types of lumbar instability test were conducted on forty young women with chronic low back pain for more than six months, and those who had 3 or more positive tests were selected as subjects. To select chronic back pain patients with lumbar instability group, aberrant movement patterns during lumbar flexion test (FMT), prone instability test (PIT), posterior-anterior mobility test (PAT), passive lumbar extension test (PLE), and pressure bio-feedback (PBF) were applied. In addition, a digital muscle strength meter was used to measure the hip flexor, extensor, adductor, and abductor muscles of chronic low back pain patients with lumbar instability group (n=20) and normal subject group (n=20). Results: As a result of comparing the hip joint muscle strength between the chronic back pain patient group with lumbar instability and the normal group, there were significant differences in the hip extensor, abductor, and adductor muscles (p<0.05). Conclusions: Patients of chronic back pain with lumbar instability were found to have weak hip joint muscle strength. Therefore, this study suggest that include hip joint strength exercise for functional recovery of chronic back pain patients.

Three-column reconstruction through the posterior approach alone for the treatment of a severe lumbar burst fracture in Korea: a case report

  • Generally, patients with severe burst fractures, instability, or neurological deficits require surgical treatment. In most cases, circumferential reconstruction is performed. Surgical methods for three-column reconstruction include anterior, lateral, and posterior approaches. In cases involving an anterior or lateral approach, collaboration with general or thoracic surgeons may be necessary because the adjacent anatomical structures are unfamiliar to spinal surgeons. Risks include vascular or lumbar plexus injuries and cage displacement, and in most cases, additional posterior fusion surgery is required. However, the posterior approach is the most common and anatomically familiar approach for surgeons performing spinal surgery. We present a case in which three-column reconstruction was performed using only the posterior approach to treat a patient with a severe lumbar burst fracture.

Craniovertebral Junction Tuberculosis with Atlantoaxial Dislocation : A Case Report and Review of the Literature

  • Craniovertebral junction (CVJ) tuberculosis is a rare disease, potentially causing severe instability and neurological deficits. The authors present a case of CVJ tuberculosis with atlantoaxial dislocation and retropharyngeal abscess in a 28-year-old man with neck pain and quadriparesis. Radiological evaluations showed a widespread extradural lesion around the clivus, C1, and C2. Two stage operations with transoral decompression and posterior occipitocervical fusion were performed. The pathological findings confirmed the diagnosis of tuberculosis. Treatment options in CVJ tuberculosis are controversial without well-defined guidelines. But radical operation (anterior decompression and posterior fusion and fixation) is necessary in patient with neurological deficit due to cord compression, extensive bone destruction, and instability or dislocation. The diagnosis and treatment options are discussed.

Effect of Ankle Stabilization Training Using Biofeedback on Balance Ability and Lower Limb Muscle Activity in Football Players with Functional Ankle Instability

  • Purpose: This study focuses on influence of ankle stabilization training on balance ability and lower limb muscle activation of soccer player with functional ankle instability. Methods: Subjects were grouped into ankle stabilization training group using biofeedback comprised of 15 subjects and general exercise group of 15. The training was conducted for 30 minutes, 3 times a week for 8 weeks in total. All 30 football players conducted plyometric training for 30 minutes before main training. To evaluate balance ability, biorescure was used to measure whole path length and surface area and surface electromyography (EMG) system was used to measure tibialis anterior, tibialis posterior, and soleus to evaluate lower limb muscle activation. Results: The experiment group showed significant difference to the comparison group in regard of whole path length and surface area which represents balancing capability and muscle activation of tibialis anterior, tibialis posterior, and soleus. Conclusion: Therefore, ankle stabilization training using biofeedback is more effective in enhancing balance ability and lower limb muscle activation than general exercise.

한 개의 신선동결 동종아킬레스건을 이용한 후방십자인대 및 후외방구조의 동시 재건술 (Combined Reconstruction of Posterior Cruciate Ligament and Posterolateral Corner with a Fresh Frozen Achilles Tendon Allograft)

  • 목적: 슬관절의 후방 및 후외측 불안정성을 동종 아킬레스건 1개를 나누어 동시에 재건하는 수술 방법을 보고하고자 한다. 대상 및 방법: 슬관절의 후방 및 후외측 불안정성으로 수술받은 42명(45예)을 조사하였다. 남자 38명, 여자 4명이었고, 평균 나이는 39세였다. 수술 방법은 이식건으로 동종 아킬레스건을 2:1 넓이로 나누어 큰 것은 (${\emptyset}$ 10 mm) 후방십자인대, 작은 것은 (${\emptyset}$ 8 mm)은 후외측 재건술에 사용하였다. 먼저 후방십자인대에 대하여 관절경적으로 10 mm 신선 동결 동종 아킬레스건을 이용하여 후방십자인대 잔유물을 보존한 채로 경경골 접근법, 단일절개, 단일가닥방법으로 재건술을 시행하였으며, 후외측 불안정성에 대해서는 작은 이식건을 사용하여 변형된 8자 모양으로 고정하여 외측부 인대 및 슬와비골 인대를 재건하였다. 평균 추시 기간은 25개월이었다. 임상적 평가는 운동범위, 후방전위검사, 내반 부하 검사, 복와위 외회전(dial) 검사, Lysholm 점수와 Tegner 점수 및 후방전위 방사선검사로 평가하였다. 평균 추시기간은 25개월 이었다. 결과: 후방 전위 검사에서 수술 전 Grade II 5예, Grade III 40예, 추시관찰 시 정상 22예, Grade I 18예, Grade II 5예로 호전되었다. Dial 및 내반 부하 검사는 수술 전 45예 모두 양성에서 수술 후 36예에서 정상, 9예에서 양성을 보였다. 수술 후 10도 이상 굴곡제한을 보이는 경우가 3예 있었으나 나머지는 모두 정상 관절 운동 범위를 보였다. Lysholm 점수는 수술 전 평균 50점에서 수술 후 83점으로 호전되었다(p<0.05). Tegner 활동 점수는 수술 전 평균 2.1에서 수술 후 4.6점으로 호전되었다. (p<0.05). 슬관절 90도 굴곡위에서 시행한 후방 긴장 방사선 소견상 수술 전 평균 16 mm에서 수술 후 4.1 mm로 호전되었다. (p<0.05). 후방 전위 검사, 내반 부하 검사, dial 검사 등의 임상적 검사와 방사선 사진에서 후방 전위 정도를 비교하였을 때 모든 환자에서 수술 전보다 나은 결과를 보였다. 결론: 슬관절의 만성 후방 및 후외측 불안정성을 동종 신선 동결 아킬레스건을 이용하여 동시에 재건해 좋은 결과를 얻을 수 있다.

Vertical Reduction Using Atlantoaxial Facet Spacer in Basilar Invagination with Atlantoaxial Instability

  • Although posterior segmental fixation technique is becoming increasingly popular, surgical treatment of craniovertebral junctional disorders is still challenging because of its complex anatomy and surrounding critical neurovascular structures. Basilar invagination is major pathology of craniovertebral junction that has been a subject of clinical interest because of its various clinical presentations and difficulty of treatment. Most authors recommend a posterior occipitocervical fixation following transoral decompression or posterior decompression and occipitocervical fixation. However, both surgical modalities inadvertently sacrifice C0-1 and C1-2 joint motion. We report two cases of basilar invagination reduced by the vertical distraction between C1-2 facet joint. We reduced the C1-2 joint in an anatomical position and fused the joint with iliac bone graft and C1-2 segmental fixation using the polyaxial screws and rods C-1 lateral mass and the C-2 pedicle.

Effects of Kinesio Taping and Massage Application to Calf Muscle on Ankle Stability

  • PURPOSE: This study was conducted to investigate the effects of Kinesio taping and massage application to the calf muscles on ankle stability. METHODS: The study subjects were 66 healthy adults (male: 32, female: 34) who had no instability in their ankles. Subjects were randomly assigned to a Kinesio taping group or a massage group. The research tool used the Y-balance test to measure instability of the ankle. The distance between the right and left foot was measured from the center in the anterior, posterior-medial, and posterior-lateral directions. Massage was applied to the calf muscles three times over two weeks and Kinesio tape was attached to the calf muscle and tibialis anterior for 10 hours twice during the experimental period. RESULTS: The results showed that application of Kinesio taping resulted in increases in the distance between the feet increased in some directions. After the massage, the distance extended from the anterior and posterior-lateral directions was prolonged, and the ankle stability on the right and left sides was partially improved by massage application. CONCLUSION: Kinesio taping and massage applied to the calf muscles are appropriate interventions for the improvement of ankle stability. Based on the application times of massage, Kinesio taping appears to work more effectively for ankle stability.

Surgical Treatment of Craniovertebral Junction Instability : Clinical Outcomes and Effectiveness in Personal Experience

  • Objective : Craniovertebral junction (CVJ) consists of the occipital bone that surrounds the foramen magnum, the atlas and the axis vertebrae. The mortality and morbidity is high for irreducible CVJ lesion with cervico-medullary compression. In a clinical retrospective study, the authors reviewed clinical and radiographic results of occipitocervical fusion using a various methods in 32 patients with CVJ instability. Methods : Thirty-two CVJ lesions (18 male and 14 female) were treated in our department for 12 years. Instability resulted from trauma (14 cases), rheumatoid arthritis (8 cases), assimilation of atlas (4 cases), tumor (2 cases), basilar invagination (2 cases) and miscellaneous (2 cases). Thirty-two patients were internally fixed with 7 anterior and posterior decompression with occipitocervical fusion, 15 posterior decompression and occipitocervical fusion with wire-rod, 5 C1-2 transarticular screw fixation, and 5 C1 lateral mass-C2 transpedicular screw. Outcome (mean follow-up period, 38 months) was based on clinical and radiographic review. The clinical outcome was assessed by Japanese Orthopedic Association (JOA) score. Results : Nine neurologically intact patients remained same after surgery. Among 23 patients with cervical myelopathy, clinical improvement was noted in 18 cases (78.3%). One patient died 2 months after the surgery because of pneumonia and sepsis. Fusion was achieved in 27 patients (93%) at last follow-up. No patient developed evidence of new, recurrent, or progressive instability. Conclusion : The authors conclude that early occipitocervical fusion to be recommended in case of reducible CVJ lesion and the appropriate decompression and occipitocervical fusion are recommended in case of irreducible craniovertebral junction lesion.

Short-term effects of joint mobilization with versus without voluntary movement in patients with chronic ankle instability: A single-blind randomized controlled trial

  • Objective: Joint mobilization for arthrokinematics altered by the positional fault of chronic ankle instability (CAI) is an effective intervention for stabilization. In this study, we compared the effects of ankle dorsi flexion range of motion (DFROM) and dynamic balance ability (DBA) in CAI patients via passive joint mobilization (PJM), a method traditionally performed in previous studies, and active joint mobilization (AJM), a method that can have a greater effect on cortical excitability with spontaneous movements. Design: Single-blind two-arm randomized controlled trial Methods: A total of 30 participants were registered: 15 each to the PJM and AJM groups. Each participant received a total of 10 intervention sessions, 10 minutes per session, 5 times a week for 2 weeks. PJM used Maitland's mobilization method to apply joint mobilization with talus in the posterior direction and AJM used an angular joint motion to induce patient's voluntary motion of medial malleolus anterior gliding and lateral malleolus posterior gliding, respectively. DFROM of the ankle was measured by using tape and DBA was evaluated by using the balance system. Results: Significant improvement was observed after intervention in both the PJM and AJM groups except for the DBA-anterior and DBA-right variables of the PJM group. There were statistically significant differences between the AJM and PJM groups in the DFROM, DBA-anterior, DBA-posterior, and DBA-right variables. Conclusions: The overall improvement of DFROM and DBA was found to be more effective in joint mobilization including voluntary movement. When it is accompanied by voluntary movement, it further affects the neuromuscular system of the ankle.
