Masticatory function is more important than esthetics in posterior fixed restoration. The usual technique-mounting the casts on a semi-adjustable articulator, etc. -cannot make all patients satisfied with their restorations. For example, functionally generated path technique can be an easier and more satisfactory method in the restoration of group function. These clinical reports describe various approaches for occlusal restoration of relatively simple posterior fixed protheses according to patients occlusal patterns. The 3-unit bridge restoration is one of the most popular treatment options in prosthodontics. Because dentists have much experiences of it, they restore a missing span of one tooth mechanically, that is, without special consideration. While esthetics is important in making an anterior 3-unit fixed prostheses, mastication is more focused on in posterior 3-unit bridge restoration. Many dentists are concerned about various aspects in esthetics, such as morphology of the tooth, value, chroma, hue, translucency, surface texture, etc. But they do not usually consider various methods to restore occlusion. They treat one-tooth-missing area in a similar way in spite of patients having variety of occlusal patterns. Three cases are presented here in 3 or 4-unit bridge restoration. They show some methods to restore patients' occlusal patterns.
The purpose of this study were to investigate the patterns of prosthetic restorations according to sex and age. We had surveyed the out-patents of dental clinics it Jeon-Ju, Iri and Gun roan area. The abtained result were as follow ; 1. The patterns of prosthetic restoration in female was higher than male and 20$\sim$30 age groups was higher than others. 2. The crown was higher in prosthetic restoration. 3. The procelain appliance area was high in male and female on 20$\sim$29 age. 4. The partial denture was higher than full denture in both sex. 5. The crown appliance area was not different in both sex. 6. The 3 unit bridge was higher than other bridge in both sex. 7. The porcelain appliance area was high in male and female on upper anterior. 8. The inlay appliance area of lower posterior was higher than upper posterior. 9. The upper posterior was high in male, and the lower posterior was high in female in partial denture. 10. The full dentur was high in old age group.
Current dental restorations present a relatively weak resistance to fracture. Owing to their unique mechanical properties, fibre-reinforced polymers are now being considered. Unidirectional or woven continuous fibres, made of glass, polyethylene, carbon or Kevlar, have been evaluated. This study focused on the use of glass fibre knitted fabrics to reinforce acrylate resins, in order to investigate the possibility to construct single crowns as well as three unit bridges. Some points affecting the final composite system were tested ; 1) static strength, with focus on the stress transfer under a occlusal contact point ; 2) modelling of a three nit bridge ; 3) fatigue strength as a posterior three unit bridge material. The study demonstrated that knitted fabric reinforcements are showing an interesting compromise between stiffness, static strength for single crown. For three unit bridge applications in the posterior arch, however knitted glass fabric reinforcements were not strong enough in fatigue An additional reinforcement in the posterior arch fixed partial denture design was recommended.
PURPOSE. To compare the clinical outcomes of two types of implant restoration for posterior edentulous area, 3-unit bridge supported by 2 implants and 3 implant-supported splinted crowns. MATERIALS AND METHODS. The data included 127 implant-supported fixed restorations in 85 patients: 37 restorations of 3-unit bridge supported by 2 implants (2-IB), 37 restorations of 3 implant-supported splinted crowns (3-IC), and 53 single restorations (S) as controls. Peri-implantitis and mechanical complications that occurred for 14 years were analyzed by multivariable Cox regression model. Kaplan-Meier curves and the multivariable Cox regression model were used to analyze the success and survival of implants. RESULTS. Peri-implantitis occurred in 28.4% of 2-IB group, 37.8% of 3-IC group, and 28.3% of S control group with no significant difference. According to the implant position, middle implants (P2) of the 3-IC group had the highest risk of peri-implantitis. The 3-IC group showed a lower mechanical complication rate (7.2%) than the 2-IB (16.2%) and S control group (20.8%). The cumulative success rate was 52.8% in S (control) group, 62.2% in 2-IB group, and 60.4% in 3-IC group. The cumulative survival rate was 98.1% in S (control) group, 98.6% in 2-IB group, and 95.5% in 3-IC group. There was no significant difference in the success and survival rate according to the restoration type. CONCLUSION. The restoration type was not associated with the success and survival of implants. The risk of mechanical complications was reduced in 3 implant-supported splinted crowns. However, the middle implants of the 3 implant-supported splinted crowns had a higher risk of peri-implantitis.

The purpose of this study is to investigate the production method of posterior bridges pontic ridge lap type which prevents the infection in bridge pontic base and is able to cleanse itself, in the process of producing final prothesis that maintains healthy mucous membrane of oral cavity and interproximal papilla, minimizing diastema, is aesthetic and has no effect on pronunciation. New technology is applied to make optimal pontic base which prevent inflammation and clean itself and its products were clinically evaluated in 10 places of dental clinics in busan and gyeongnam. The making of posterior 3 unit bridge pontic base, it was presented as the new technology of forming ridge lab type and to carry out clinical validation, existing conventional method and the new technology were compared. Pontic base made with the existing conventional method cause infection and other periodontal disease by 96% but the pontic base made with the new technology cause infection and other periodontal disease by 3%. Remains of food cause infection and other periodontal disease 100% by the existing conventional method and 91% by the new technology, showing a distinct difference. However, after a gargle, the new technology had low 13%. Additionally, the pontic base made with the existing conventional method showed 71% of chance, the new technology method showed 8% of chance in terms of self-cleansing.
State of problem: Zirconia all-ceramic restoration fabricated with CAM system is on an increasing trend in dentistry. However, evaluation of the marginal and internal fits of zirconia bridge seldomly have been reported. Purpose: This study was to evaluate the at of margin and internal surface in posterior 3-unit zirconia bridge framework fabricated with CAM system(DeguDent, Germany). Material and Method: Preparations of secondary premolar and secondary molar on artificial resin model were performed for fabrication of 3-unit posterior bridge framework. Fits of 5 zirconia bridge framework were compared with 5 precious ceramo-metal alloy framework(V-GnathosPlus, Metalor, Switzerland), and prepared margins were designed to chamfer and shoulder finishing line. Each framework was cemented to epoxy resin model with reinforced glass ionomer(FujiCEM, GC Co., Japan), embedded in acrylic resin and sectioned in two planes, mesio-distal and buccolingual. Samples were divided into six pieces by sectioning and had two pieces of each surface(i.e mesial, distal, buccal and lingual surface) per abutment, so there were eight measuring points in each abutment. External gap was measured at the margin and internal gaps were measured at the margin, axial and occlusal surface. Gaps were observed under the measuring microscope(Compact measuring microscope STM5; Olympus, Japan) at a magnification of $\times100$. T-test were used to determine the statistic significance of the different gaps between zirconia and metal framework. Results and Conclusion: 1. External and internal marginal gaps of zirconia and metal framework were in clinically acceptable range. External marginal gaps were not different significantly between zirconia$(81.9{\mu}m)$ and metal $(81.3{\mu}m)$ framework and internal marginal gaps of zirconia $(44.6{\mu}m)$ were smaller than those of metal framework $(58.6{\mu}m)$. 2. Internal axial gaps of zirconia framework$(96.7{\mu}m)$ were larger than those of metal frame-work$(78.1{\mu}m)$ significantly and adversely, internal occlusal gaps of zirconia frame-work$(89.4{\mu}m)$ were smaller than those of metal framework $(104.9{\mu}m)$ significantly. 3. There were no significant differences in external and internal marginal gaps between chamfer and shoulder finish line when zirconia frameworks were fabricated.

Conventional radiograph, computed tomograph (CT), magnetic resonance image (MRI) are commonly used methods for diagnosis of oro-maxillofacial cancer. MRI is an effective tool to verify soft tissue lesion however, metal produces black artifacts in the image. Therefore, metal structure should be removed before taking MRI to diagnose head and neck cancer patients. A 52-year-old female patient with adenocarcinoma in the posterior right soft palate was referred to take a MRI before surgery. She has 7-unit porcelain fused to metal bridge in the maxilla. Eight-unit Tagis-Vectris fixed partial denture was fabricated to replace her existing PFM bridge to take a MRI without any artifact before and after surgery. The patient satisfied with her restorations in terms of esthetics, function after 11 months. Even though minor staining was detected, Tagis-Vectris restoration fixed partial denture was intact during observation period.
Objective : To mitigate the risk of iatrogenic instability, new posterior decompression techniques able to preserve musculoskeletal structures have been introduced but never extensively investigated from a biomechanical point of view. This study was aimed to investigate the impact on spinal flexibility caused by a unilateral laminotomy for bilateral decompression, in comparison to the intact condition and a laminectomy with preservation of a bony bridge at the vertebral arch. Secondary aims were to investigate the biomechanical effects of two-level decompression and the quantification of the restoration of stability after posterior fixation. Methods : A universal spine tester was used to measure the flexibility of six L2-L5 human spine specimens in intact conditions and after decompression and fixation surgeries. An incremental damage protocol was applied : 1) unilateral laminotomy for bilateral decompression at L3-L4; 2) on three specimens, the unilateral laminotomy was extended to L4-L5; 3) laminectomy with preservation of a bony bridge at the vertebral arch (at L3-L4 in the first three specimens and at L4-L5 in the rest); and 4) pedicle screw fixation at the involved levels. Results : Unilateral laminotomy for bilateral decompression had a minor influence on the lumbar flexibility. In flexion-extension, the median range of motion increased by 8%. The bone-preserving laminectomy did not cause major changes in spinal flexibility. Two-level decompression approximately induced a twofold destabilization compared to the single-level treatment, with greater effect on the lower level. Posterior fixation reduced the flexibility to values lower than in the intact conditions in all cases. Conclusion : In vitro testing of human lumbar specimens revealed that unilateral laminotomy for bilateral decompression and bone-preserving laminectomy induced a minor destabilization at the operated level. In absence of other pathological factors (e.g., clinical instability, spondylolisthesis), both techniques appear to be safe from a biomechanical point of view.
Purpose: The aim of this retrospective chart review was to evaluate the four-year survival rate of a titanium implant system. Methods: A total of 352 sand-blasted, thermally acid-etched titanium implants were inserted into 181 partially or completely edentulous patients. Their cumulative survival rate was evaluated retrospectively. Associated factors, such as the implant distribution and treatment type were included in the evaluation. Results: The implants were equally distributed between the maxilla (52.3%) and the mandible (47.7%). 48 implants (13.6%) were placed in the anterior region and 304 implants (86.4%) in the posterior region. The majority of the implants were inserted into bone of type II and III quality (89.8%) and volume (quantity B and C, 87.2%). Most of the implants (70.7%) were restored as single crowns; 28.7% supported a bridge construction and 0.6% a full denture. Only one implant failed, resulting in a four-year cumulative survival rate of 99.7%. Conclusions: The implant system showed an excellent four-year survival rate. It proved to be a safe and predictable means for restoration of the dentition in partially or completely edentulous patients.

Statement of problem: Ceramics have been important materials for the restoration of teeth. The demands of patients for tooth-colored restorations and the availability of various dental ceramics has driven the increased use of new types of dental ceramic materials. Improved physical properties of theses materials have expanded its use even in posterior crowns and fixed partial dentures. However, ceramic still has limitation such as low loading capability. This is critical for long-span bridge, because bridge is more subject to tensile force. Purpose: The wire reinforced ceramic was designed to increase the fracture resistance of ceramic restoration. The purpose of this study was to evaluate the fracture resistance of wire reinforced ceramic. Material and methods: Heat pressed ceramic(ingot No.200 : IPS Empress 2, Ivoclar Vivadent, Liechtenstein) and Ni-Cr wire(Alfa Aesar, Johnson Matthey Company, USA) of 0.41 mm diameter were used in this study. Five groups of twelve uniform sized ceramic specimens(width 4 mm, thickness 2 mm, length 15 mm) were fabricated. Each group had different wire arrangement. Wireless ceramic was used as control group. The experimental groups were divided according to wire number and position. One, two and three strands of wires were positioned on the longitudinal axis of specimen. In another experimental group, three strands of wires positioned on the longitudinal axis and five strands of wires positioned on the transverse axis. Three-point bending test was done with universal testing machine(Z020, Zwick, Germany) to compare the flexural modulus, flexural strength, strain at fracture and fracture toughness of each group. Fractured ceramic specimens were cross-sectioned with caborundum disc and grinded with sandpaper to observe interface between ceramic and Ni-Cr wire. The interface between ceramic and Ni-Cr wire was analyzed with scanning electron microscope(JSM-6360, JEOL, Japan) under platinum coating. Results: The results obtained were as follows: 1. The average and standard deviation in flexural modulus, flexural strength and fracture toughness showed no statistical differences between control and experimental groups. However, strain was significantly increased in wire inserted ceramics(P<.001). 2. Control group showed wedge fracture aspects across specimen, while experimental groups showed cracks across specimen. 3. Scanning electron microscopic image of cross-sectioned and longitudinally-sectioned specimens showed no gap at the interface between ceramic and Ni-Cr wire. Conclusion: The results of this study showed that wire inserted ceramics have a high strain characteristic. However, wire inserted ceramics was not enough to use at posterior area of mouth in relation to flexural modulus and flexural strength. Therefore, we need further studies.
