Purpose: Kimura's disease is a relatively rare head and neck tumor that frequently occurs in young orientals accompanied with eosinophila. We shared our experience of two cases of Kimura's disease, treated by radical resection and post operative steroid therapy, so we report the correlation of eosinophil counts and disease progression. Methods: A 25 years old male came to the clinic with a mass localized to the right cheek inferior to the right auricle. We could not resect the mass totally. During the follow up period, we checked the eosinophil counts, and steroid therapy was started 7 months after the surgery. A 34 year old female came to the clinic with a mass localized inferior to the left auricle reaching from the posterior portion of the auricle to the left temporal portion. We tried to remove as much tumor as possible, save the temporal region, in regard to the impairment of blood supply to the auricle. After operation, steroid therapy was started. Results: In the first case, the tumor was easily approachable, and radical resection of the tumor with post operative steroid therapy was an effective treatment. In tumors located at difficult regions to remove, as in the second case, optimal debulking and post operative steroid therapy was also effective in treating Kimura's. There were no recurrences in both cases. Eosinophil counts were always reduced after surgery and steroid therapy, and during the period with low eosinophil counts, there was no recurrence of Kimura's disease. Conclusion: Surgery and post operative steroid therapy were effective in treatment of Kimura's disease, and we could assume eosinophil counts as a good indicator for evaluation of the prognosis of Kimura's disease during the follow up period.
The purpose of this study was to evaluate the effect of canal filling methods and post-space preparation time on the apical seal of endodontically treated teeth. Seventy-six single-rooted human teeth were divided into 6 groups and each tooth was obturated and prepared post-space according to the purpose of this study. After all specimens were immersed in Indian ink, decalcified and cleared, the degree of dye penetration into the root canals observed by magnifying glass (X20) and measured by caliper. The results were as follows: 1. In the thermatic condensation, no significant difference in ink penetration occured when the post-spaces were created immediately after obturation or when they were made a week later. 2. In the lateral condensation, no significant difference in ink penetration existed between teeth whose post-space were prepared immediately or delayed. 3. In comparison of experimental groups and control groups, no significant difference showed when the groups were obturated with McSpadden compactor but when the teeth were laterally condensed, the experimental groups showed less ink penetration than controls. 4. Thermatic condensed teeth with McSpadden compactor were less ink penetration thn laterally condensed teeth.
The experimental study was made to evaluate the influence of retentive grooves on the canal wall upon the retention of post, using different type of cements. Ninety periodontally involved anterior teeth were extracted and divided into six groups of 15 teeth. The groups were labeled as follows: Group I: Dentatus post cemented zincphosphate cement into post hole with smooth wall. Group II: Dentatus post cemented with zincphosphate cement into post hole with retentive grooves. Group III: Dentatus post cemented with glass-ionomer cement into post hole with smooth wall. Group IV: Dentatus post cemented with glass-ionomer cement into post hole with retentive grooves. Group V: Dentatus post cemented with composite resin adhesive into post hole with smooth wall. Group VI: Dentatus post cemented with composite resin adhesive into post hole with retentive grooves. The tensile strength was tested by an Instron type tester with cross-head speed of 5mm per minute. The results were as follows: In the group without retentive grooves, three kinds of cement have little effect upon retentive capacity of Dentatus post. The Dentatus post cemented on the canal wall with retentive grooves is more retentive than Dentatus post without retentive grooves.
Many clinical and laboratory experiments have been developed to prevent or decrease post-operative pain. One of these methods is pre-operative administration of opioid. Recently there have been differing and debatable results reported of pre-operative treatment for post-operative pain management. It was our study to determine whether pre-operative epidural fentanyl prevented central facilitation or wind up of spinal cord from nociceptive afferent input through c-fibers. We evaluated the effect of epidural fentanyl 50 mcg 10 minutes before operation and 10 minutes before the end of surgery. 28 parturient women for Cesarean Section were randomly allocated to receive the epidural fentanyl either at 10 minutes before operation (Group 1, n=14) or 10 minutes before the end of surgery (Group 2, n=14). All of the 28 parturient women were anesthetized with epidural block using (22 ml of) 2% lidocaine supplemented with light general anesthesia ($N_2O$ 2 L/min-$O_2$, 2 L/min), we controlled post-operative pain with epidural PCA(patient controlled analgesia) infusion of meperidine and 0.07% bupivacaine. The action duration of epidural fentanyl from the end of surgery to the first requirement of analgesics with epidural PCA were not significantly different between the two groups. No significant differences between two groups were observed in VAS pain score at 1, 2, 3, 6, 12, 24, and 48 hours after the operation. The number of self administration of narcotics with PCA during 48 hours after surgery were the same between the two groups. The hourly infusion rates of demerol were the same. Pre-operative administration of fentanyl was not clinically effective compared to administration just before the end of surgery for postoperative pain control.
Adenoid cystic carcinoma is a rare primary tracheal tumor and this tumor behave slow growth, low-grade ma!ignancy, locally invasion and long-term host survival. Operation with the primary goal of complete excision is the treatment of choice but this tumor require excessive margins at surgical removal because of locally invasive cancer. A 45-years-old male patient had complained paroxysmal coughing from 1 year ago prior to admission and was diagnosed pre-operatively as endotracheal adenoma. He had been treated by operation, and combined with radiotherapy by 4 MeV. Lineal Accelerator. The tracheal mass was removed by tracheo-bronchotomy transpleurally and right total pneumonectomy was performed. There was post-operative course uneventfully and no post-operative complication. The patient Is free from cancer until post-operative 1 year clinically and alive with good healthy.
The clinical and statistical evaluation on the patients of maxillofacial deformities who were operated by author were analyzed after several month or years or se. Pre and post operative cephalometric radiographs of 45 orthognathic surgery patients were compared. The post operative radiographs had been taken at least 9 month to several years. To know the differences of proton MR spectroscopic features between recurrent rectal cancer and fibrosis in post-operative period, and to evaluate the possibility to discriminate recurrent rectal cancer from post-operative fibrosis by analysis of proton MR spectra.
Purpose: This study aimed to investigate the effect of physiotherapy on the range of motion (ROM) of patients with hemophilic arthropathy after total knee replacement (TKR) during treadmill gait. Method: Nineteen patients (age range, 30-61 years) who received physiotherapy at the Korea Hemophilia Foundation (KHF) Clinic in Seoul between 2011 and 2013 after TKR were recruited. Protocol rehabilitation was performed (KHF Clinic, Department of Physiotherapy) with an average follow-up of 70 days (range, 6-141 days). Result: Physiotherapy after TKR improved the ROM parameters, including the post-operative average ROM. The post-operative and follow-up ROM also significantly increased. Conclusion: Patients with hemophilia require different physiotherapeutic techniques and need dedicated post-operative care in comparison with the general population.
We performed post-operative hemodynamic evaluation and lung perfusion scan after left lung transplantation in dog. The heart & lungs were extracted from donor dog while the both lungs were flushed with 4oC Euro-Collin`s solution after heparinization & infusion of prostaglandin E1.In the recipient dog, anastomosis of the left atrial cuff was performed by continuous 4-0 Prolene everted suture, and bronchial anastomosis by telescope method with 4-0 Prolene continuous suture. The end-to-end anastomosis of left main pulmonary arteries was performed with continuous 6-0 Prolene suture. After closure of left thoracotomy incision, the lung perfusion scan was performed post operative 2 days for evaluation of the function of the transplanted lung which showed good perfusion. The dog was sacrificed in the post-operative 5 days for autopsy.
Purpose: This study was performed to identify and investigate the therapeutic effects of low-level laser (LLL) combined with a light-emitting diode (LED) on post-operative wound healing and functional recovery after hand orthopedic surgery. Methods: The study subjects were twenty patients who had passed the acute inflammatory phase after hand orthopedic surgery and were assigned equally to an experimental or a control group. Phototherapy was administered three times weekly for two weeks. Changes in wound length, edema, pain, and hand function were measured. Results: Significant differences in wound length, edema, pain, and hand function were observed between the experimental and control groups (p<0.05). However, no significant intergroup difference was observed (p>0.05). Nonetheless, a comparison of results showed changes in the experiment group over the two-week study period were significantly greater than in the control group (p<0.05). Conclusion: These findings show that combined LLL plus LED phototherapy positively influences post-operative hand rehabilitation.
