18F-2-Deoxy-2-Fluoro-D-Glucose Positron Emission Tomography: Computed Tomography for Preoperative Staging in Gastric Cancer Patients

  • Purpose: The use of 18F-2-deoxy-2-fluoro-D-glucose positron emission tomography-computed tomography as a routine preoperative modality is increasing for gastric cancer despite controversy with its usefulness in preoperative staging. In this study we aimed to determine the usefulness of preoperative positron emission tomography-computed tomography scans for staging of gastric cancer. Materials and Methods: We retrospectively analyzed 396 patients' positron emission tomography-computed tomography scans acquired for preoperative staging from January to December 2009. Results: The sensitivity of positron emission tomography-computed tomography for detecting early gastric cancer was 20.7% and it was 74.2% for advanced gastric cancer. The size of the primary tumor was correlated with sensitivity, and there was a positive correlation between T stage and sensitivity. For regional lymph node metastasis, the sensitivity and specificity of the positron emission tomography-computed tomography were 30.7% and 94.7%, respectively. There was no correlation between T stage and maximum standardized uptake value or between tumor markers and maximum standardized uptake value. Fluorodeoxyglucose uptake was detected by positron emission tomography-computed tomography in 24 lesions other than the primary tumors. Among them, nine cases were found to be malignant, including double primary cancers and metastatic cancers. Only two cases were detected purely by positron emission tomography-computed tomography. Conclusions: Positron emission tomography-computed tomography could be useful in detecting metastasis or another primary cancer for preoperative staging in gastric cancer patients, but not for T or N staging. More prospective studies are needed to determine whether positron emission tomography-computed tomography scans should be considered a routine preoperative imaging modality.

  • Lung cancer has become a loading cause of cancer death in Korea. Positron emission tomography was introduced in clinical nuclear medicine in the early 1990s and many studios using this functional imaging technology were performed for evaluation of its clinical utility. I review the current role of positron emission tomography in the diagnosis, staging, and therapy monitoring of lung cancer.

  • In this paper, an iterative method is proposed to perform attenuation correction and image reconstruction simultaneously for positron emission tomography, by using the time-of-flight information. Numerical simulation results are presented to demonstrate an improved performance of the proposed method in attenuation correction and image reconstruction.

F-18 fluorodeoxyglucose positron emission tomography/computed tomography in the infection of heart

  • Infections involving the heart are becoming increasingly common, and a timely diagnosis of utmost importance, despite its challenges. F-18 fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) is a recently introduced diagnostic tool in cardiology. This review focuses on the current evidence for the use of FDG PET/CT in the diagnosis of infective endocarditis, cardiac implantable device infection, left ventricular assist device infection, and secondary complications. The author discusses considerations when using FDG PET/CT in routine clinical practice, patient preparation for reducing physiologic myocardial uptake, acquisition of images, and interpretation of PET/CT findings. This review also functions to highlight the need for a standardized acquisition protocol.

  • Recent advance in electronics and scintillators makes it possible to utilize the time-of-flight (TOF) information in improving image reconstruction of positron emission tomography(PET). In this paper, we propose a TOF-based fast image reconstruction method for PET. The proposed method uses the deconvolution of TOF data for each angle view and the rotational averaging of deconvolved images. Simulation results show an improved performance of the proposed method, as compared with filtered backprojection (FBP) method, TOF-FBP, and TOF version of expectation-maximization(EM) methods. Simulation results also show a great potentiality of the proposed method in limited angle tomography applications.

근골격계 악성 종양 환자의 림프절 전이 발견을 위한 양전자 방출 컴퓨터 단층 촬영기(Positron Emission Tomography/Computed Tomography)의 유용성 (The Efficacy of Detecting a Sentinel Lymph Node through Positron Emission Tomography/Computed Tomography)

  • 목적: 악성 종양 환자에서 림프절 전이를 발견하는 것은 초기에 정확한 병기 및 예후와 관련된 정보를 알 수 있고 수술 후 보조치료(adjuvant therapy)의 필요성을 평가해서 조기에 치료를 할 수 있도록 해주기 때문에 중요하다. 본 연구를 통해 정형외과 영역의 악성 종양 환자를 평가함에 있어 양전자 방출 컴퓨터 단층 촬영기(positron emission tomography/computed tomography, PET/CT)가 sentinel lymph node biopsy와 비교하여 유용한지를 알아보고자 한다. 대상 및 방법: 2008년부터 2018년까지 악성 근골격계 종양으로 진단받고 PET/CT를 촬영한 환자 251명 중 72명의 환자를 선별하여 후향적 분석을 시행하였다. 림프절 전이가 의심되는 환자군은 조직검사를 시행하였다. 분석은 의무기록, 임상정보, PET/CT 영상 및 판독 소견, 병리 결과를 통해 시행하였으며, 임상경과 및 병리검사 결과를 PET/CT 결과와 비교하여 PET/CT의 정확도를 평가하였다. 결과: 환자의 나이는 14세에서 88세까지 분포하였고, 평균 추시 기간은 2.4년이었으며, 22명에서 림프절의 전이가 확인되었다. 육종 및 비육종 종양에서 PET/CT 영상의 민감도, 특이도, 양성예측도, 음성예측도는 전문가의 소견을 함께 고려할 경우 상당히 상승하는 것을 확인하였다. 결론: 본 연구를 통하여 전문가의 소견을 함께 고려한다면, 림프절의 전이를 발견하는 데 PET/CT의 유용성은 증가할 것으로 생각된다.

Radiolabeled Benzamide Derivatives for Development of Malignant Melanoma Imaging Agents

  • Malignant melanoma has an aggressive nature and high metastatic potential that result in one of the highest cancer mortality rates. Over the past three decades, primary and metastatic melanoma incidence has rapidly increased. The recent advances in diagnostic technology have shown promise, but there is still an enormous need for specific detection methods to diagnose malignant melanoma. Positron emission tomography can visualize a particular biomarker of malignant melanoma and promise a noninvasive image of micrometastases. However, the development of PET radiopharmaceuticals remains necessary for diagnosing malignant melanoma by using positron emission tomography. In this review, the history and a general overview of PET radionuclide labeled benzamide derivatives, including their radiosynthesis, in vivo characterization, and evaluation, are provided as imaging agents for malignant melanoma.

Novel zirconium-89-based radiopharmaceuticals for medical application

  • Since utilization of metal is attractive research area applicable to medical applications, several metals have been studied by many scientists. Zr-89 is known as one of important radioisotopes nowadays. Novel ligands for Zr-89 have been developed, and Zr-89-based radiopharmaceuticals have been employed for positron emission tomography (PET) study. In this highlight review, recent advances on Zr-89 utilization for radiophramaceuticals are illustrated.

  • Breast cancer is the most prevalent woman cancer, and its prevalence rate is increasing rapidly. It is necessary to develop the breast-dedicated positron emission tomography (PET) or positron emission mammography (PEM) systems with high resolution and sensitivity to improve the accuracy of diagnosis of breast cancer. Therefore, it is necessary to develop the high-end PET system technologies, such as the measurement of depth-of-interaction (DOI) and fully digitalized data acquisition, to establish high performance breast imaging systems. In this paper, recent technical advances in breast-dedicated PET imaging systems will be reviewed.
