STATEMENT OF PROBLEM: The improvement in oral function and comfort from the dental implant appears to depend on the particular type of implant support used with the denture. The number and positioning of implants have an influence on the force transfer and subsequent stress distribution around implants. Nevertheless, a quantitative comparison has not been made between the types of implant prosthesis used with different materials compared to conventional complete denture. PURPOSE: The objective of this study is to assess the masticatory performance, bite force and impact of two different type of implant supported prostheses on oral health-related quality of life compared to conventional complete denture with GOHAI, validated oral-specific health status measures, the sieving method, and the Prescale Dental System. MATERIAL AND METHODS: From the years 1999 to 2006, a total of 30 completely edentulous patients in a single arch were selected from the Yonsei University Dental Hospital, Department of Prosthodontics and Implant Clinic in Seoul, S. Korea. Patients were divided into 3 groups of 10 each. Group HR was restored with fixed-detachable hybrid prostheses with resin teeth. Group FP had fixed dentures with porcelain teeth while Group CD had a complete denture. The masticatory performance was compared between 3 groups. RESULTS: The results showed a significant improvement in oral health-related quality of life with dental implants compared to a conventional denture in GOHAI comparison. Overall, implant prostheses showed a higher masticatory performance ($S_{50}$) and maximum bite force compared with conventional dentures (P < .05) but no differences between different implant supported prostheses (P > .05). CONCLUSION: Within the limitation of this study, the numbers of implant and material of implant prostheses does not appear to impact patient satisfaction, masticatory performance or bite force.
The purpose of this study is to evaluate possibility of using indirect composite resin instead of porcelain through the measurement of shear bond strength between zirconia core and indirect composite resin under treatment of $Rocatec^{TM}$ system for improving the adhesion of indirect composite resin. 20 cylindrical zirconia core specimens were divided into 2 groups, according to zirconia surface treatment and attached materials: 1) treated with sandblast and attached with indirect composite resin, 2) treated with sandblast + $Rocatec^{TM}$ system and attached with indirect composite resin. The shear bond strength of each experimental group was measured by MTS and the changes of zirconia core surface according to surface treatments were obtained by SEM observation and measurements of surface roughness. The mean shear bond strength values are $0.55\;{\pm}\;0.11MPa$(Group SC) and $1.16\;{\pm}\;0.46MPa$(Group SRC). The mean Ra values for the surface treatments were follows: $0.39\;{\pm}\;0.13$($100{\beta}_{{\mu}m}$ sandblast) and $0.50\;{\pm}\;0.03$($100{\beta}_{{\mu}m}$ sandblast + $Rocatec^{TM}$ system). In the analysis of EDS, Si element was detected in the Group SC. The shear bond strength between zirconia core and indirect composite resin was improved significantly by using $Rocatec^{TM}$ system.

PURPOSE. To evaluate the adhesion to CAD/CAM feldspathic blocks by failure analysis and shear bond strength test (SBSt) of different restorative systems and different surface treatments, for purpose of moderate chipping repair. MATERIALS AND METHODS. A self-adhering flowable composite (Vertise Flow, Kerr) containing bi-functional phosphate monomers and a conventional flowable resin composite (Premise Flow, Kerr) applied with and without adhesive system (Optibond Solo Plus, Kerr) were combined with three different surface treatments (Hydrofluoric Acid Etching, Sandblasting, combination of both) for repairing feldspathic ceramics. Two commercial systems for ceramic repairing were tested as controls (Porcelain Repair Kit, Ultradent, and CoJet System, 3M). SBSt was performed and failure mode was evaluated using a digital microscope. A One-Way ANOVA (Tukey test for post hoc) was applied to the SBSt data and the Fisher's Exact Test was applied to the failure analysis data. RESULTS. The use of resin systems containing bi-functional phosphate monomers combined with hydrofluoric acid etching of the ceramic surface gave the highest values in terms of bond strength and of more favorable failure modalities. CONCLUSION. The simplified repairing method based on self-adhering flowable resin combined with the use of hydrofluoric acid etching showed high bond strength values and a favorable failure mode. Repairing of ceramic chipping with a self-adhering flowable resin associated with hydrofluoric acid etching showed high bond strength with a less time consuming and technique-sensitive procedure compared to standard procedure.
This study was peformed to evaluate the dynamic fatigue characteristics of the dental ceramics for all-ceramic crown. A feldspathic porcelain of VMK68. glass ceramic of IPS-Empress, and glass infiltrated alumina ceramic of In-Ceram were used. Disc specimens were prepared to the final dimensions of 12 mm in diameter and 1 mm in thickness. The biaxial flexure test was conducted using a ball-on-three-ball method. 240 specimens were tested in $37^{\circ}C$ water by testing 20 samples at each of four loading rates:0.05, 0.2, 1, and 5mm/min. 60 specimens were tested in a moisture-free environment by testing 20 samples at 5mm/min. The inert strength of VMK68 was 80.25MPa, and the fatigue parameters were n=29.1, ${\sigma}_{fo}=52.90MPa$. The inert strength of IPS-Empress was 104.76MPa, and the fatigue parameters were n=32.46, ${\sigma}_{fo}=67.52MPa$. The inert strength of In-Ceram was 429.33MPa, and the fatigue parameters were n=31.46, ${\sigma}_{fo}=258.36MPa$. 10-year failure stresses of VMK68, IPS-Empress, and In-Ceram were 20.3MPa, 24.8MPa, and 93.6MPa, respectively. Failure strength and fatigue life showed the highest value in In-Ceram, and then, IPS-Empress and VMK68.
Statement of problem: Shade selection has traditionally been accomplished by visual examination, which is particularly relevant to the shade selection of anterior teeth, but the subjective nature of visual analysis can lead to errors in shade matching. Recently shade selection systems have been developed to provide a more objective and scientific approach to understanding and clarifying shade selection. Purpose : The purpose of this study was analysis of various shade analyzing equipment with the goal of providing a more objective shade selection. Materials and method: Visual shade matching system selection(Vita Lumin Vacuum shade guide, Vitapan 3D Master shade guide) and mechanical shade matching method($ShadeEye^{(R)}$-EX Chroma Meter, $Shadescan^{TM}$ System) used for this study. The shade guide tap specimens for 10 extracted maxillary anterior teeth were produced by selecting shades using each shade matching system. The accuracy of the selection of shades for the teeth and fabricated specimens were evaluated by analyzing the calculated shade difference(${\Delta}E^*$), using a spectrophotometer and calculating the output of value $L^*,\;a^*,\;b^*$. Results and conclusion: The results show that the average ${\{Delta}E^*$ value (difference of shade) of the shade tap specimens to the actual specimen decreased in the following order: Vita Lumin Vacuum Shade Guide(VL), $ShadeEye^{(R)}$-EX Chroma Meter(SE) Vitapan 3D Master Shade guide(V3), and $Shadescan^{TM}$ System(55) : and that there are significant statistical differences between the VL and SS (p<0.05). In the analysis of the ${\Delta}E^*$ (difference of shade) value,40% of the VL group was found to be less than 3.3 (limit value of shade tap specimens clinically acceptable), 60% in the V3 group, 50% in the SE group, and 80% in the SS group.
The purpose of this study is to analyze external elements of 'time series table serving' and to discover any problems in the table setting. We selected two traditional Korean restaurants in five-star hotels and two representative traditional Korean style restaurants and analyzed the tableware arrangement of their table settings. The results are as follows. In restaurant A, the arrangement of different styles of tableware made from a variety of materials seemed relatively natural. However, the dessert plates were 'Buncheong' ware made with 'Bakji' method; in this case, the pattern was larger than the size of the fruits served and made the food inconspicuous. Rather than using plates made with a 'bakji' method, using plates with small patterns in the 'johwa' method would complement the food. In restaurant B, the space between the tableware, spoon and chopsticks was narrow, and the spoon and chopsticks sets would not fit on the table pads when large dishes or multiple dishes were served. In this case, changing the dishes to smaller sized dishes, considering the size of the table pad, or not using the pad at all would solve the issue. In restaurant C, too many small dishes were laid on the table, making it look disorderly. We believe that recovering tableware that is no longer in use, laying dishes and cutlery inside of the table pads or not using the table pad would be better. In restaurant D, the same style and color of dishes were provided 9 times. The lack of variety could be tedious to customers. The forms and colors of dishes should be changed in accordance with the order of the meal served. However, when using tableware created by different methods such as 'Buncheong' ware or white porcelain, the same pattern should be chosen in order to elevate the dignity of the table menu through uniformity. These issues in tableware arrangement indicate that external forms of banquet table setup should be studied so that the culture of Korean traditional cuisine can be enriched.
PURPOSE. The purpose of this study was to evaluate whether surface treatments affect the translucency of laminate veneers with different shades and thicknesses. MATERIALS AND METHODS. A total of 224 disc-shaped ceramic veneers were prepared from A1, A3, HT (High Translucent) and HO (High Opaque) shades of IPS e.max Press (Ivoclar Vivadent) with 0.5 mm and 1.0 mm thicknesses. The ceramics were divided into four groups for surface treatments. Group C: no surface treatments; Group HF: etched with hydrofluoric acid; Group SB: sandblasted with 50-${\mu}m$$Al_2O_3$; and Group L; irradiated with an Er;YAG laser. A translucent shade of resin cement (Rely X Veneer, 3M ESPE) was chosen for cementation. The color values of the veneers were measured with a colorimeter and translucency parameter (TP) values were calculated. A three-way ANOVA with interactions for TP values was performed and Bonferroni tests were used when appropriate (${\alpha}=0.05$). RESULTS. There were significant interactions between the surface treatments, ceramic shades and thicknesses (P=.001). For the 0.5-mm-thick specimens there were significant differences after the SB and L treatments. There was no significant difference between the HF and C treatments for any shades or thicknesses (P>.05). For the 1-mm-thick ceramics, there was only a significant difference between the L and C treatments for the HT shade ceramics (P=.01). There were also significant differences between the SB and C treatments except not for the HO shades (P=.768). CONCLUSION. The SB and L treatments caused laminate veneers to become more opaque; however, HF treatment did not affect the TP values. When the laminate veneers were thinner, both the shade of the ceramic and the SB and laser treatments had a greater effect on the TP values.
PURPOSE. The purposes of this study are to evaluate the internal and marginal adaptation of two widely used CAD/CAM systems and to study the effect of porcelain press veneering process on the prosthesis adaptation. MATERIALS AND METHODS. Molar of a lower jaw typodont resin model was prepared by adjusting a 1.0 mm circumferential chamfer, an occlusal reduction of 2.0 mm, and a $5^{\circ}$ convergence angle and was duplicated as an abrasion-resistant master die. The monolithic crowns and copings were fabricated with two different CAD/CAM system-Ceramil and Zirkonzahn systems. Two kinds of non-destructive analysis methods are used in this study. First, weight technique was used to determine the overall fitting accuracy. And, to evaluate internal and marginal fit of specific part, replica technique procedures were performed. RESULTS. The silicone weight for the cement space of monolithic crowns and copings manufactured with Ceramil system was significantly higher than that from Zirkonzahn system. This gap might cause the differences in the silicone weight because the prostheses were manufactured according to the recommendation of each system. Marginal discrepancies of copings made with Ceramil system were between 106 and $117{\mu}m$ and those from Zirkonzahn system were between 111 and $115{\mu}m$. Marginal discrepancies of copings made with Ceramil system were between 101 and $131{\mu}m$ and those from Zirkonzahn system were between 116 and $131{\mu}m$. CONCLUSION. Marginal discrepancy was relatively lower in Ceramil system and internal gap was smaller in Zirkonzahn system. There were significant differences in the internal gap of monolithic crown and coping among the 2 CAD/CAM systems. Marginal discrepancy produced from the 2 CAD/CAM systems were within a reported clinically acceptable range of marginal discrepancy.
PURPOSE. The effect of core design on the fracture resistance of zirconia-lithium disilicate (LS2) bilayered crowns for anterior teeth is evaluated by comparing with that of metal-ceramic crowns. MATERIALS AND METHODS. Forty customized titanium abutments for maxillary central incisor were prepared. Each group of 10 units was constructed using the same veneer form of designs A and B, which covered labial surface to approximately one third of the incisal and cervical palatal surface, respectively. LS2 pressed-on-zirconia (POZ) and porcelain-fused-to-metal (PFM) crowns were divided into "POZ_A," "POZ_B," "PFM_A," and "PFM_B" groups, and 6000 thermal cycles (5/55 ℃) were performed after 24 h storage in distilled water at 37 ℃. All specimens were prepared using a single type of self-adhesive resin cement. The fracture resistance was measured using a universal testing machine. Failure mode and elemental analyses of the bonding interface were performed. The data were analyzed using Welch's t-test and the Games-Howell exact test. RESULTS. The PFM_B (1376. 8 ± 93.3 N) group demonstrated significantly higher fracture strength than the PFM_A (915.8 ± 206.3 N) and POZ_B (963.8 ± 316.2 N) groups (P<.05). There was no statistically significant difference in fracture resistance between the POZ_A (1184.4 ± 319.6 N) and POZ_B groups (P>.05). Regardless of the design differences of the zirconia cores, fractures involving cores occurred in all specimens of the POZ groups. CONCLUSION. The bilayered anterior POZ crowns showed different fracture resistance and fracture pattern according to the core design compared to PFM.
PURPOSE. To determine whether the fracture strengths and failure types differed between metal and zirconia frameworks veneered with pressable or layering ceramics. MATERIALS AND METHODS. A phantom molar tooth was prepared and duplicated in 40 cobalt-chromium abutments. Twenty metal (IPS d.SIGN 15, Ivoclar, Vivadent, Schaan, Liechtenstein) and 20 zirconia (IPS e.max ZirCAD, Ivoclar) frameworks were fabricated on the abutments. Each framework group was randomly divided into 2 subgroups according to the veneering material: pressable and layering ceramics (n=10). Forty molar crowns were fabricated, cemented onto the corresponding abutments and then thermocycled ($5-55^{\circ}C$, 10,000 cycles). A load was applied in a universal testing machine until a fracture occurred on the crowns. In addition, failure types were examined using a stereomicroscope. Fracture load data were analyzed using one-way ANOVA and Tukey HSD post-hoc tests at a significance level of 0.05. RESULTS. The highest strength value was seen in metal-pressable (MP) group, whereas zirconia-pressable (ZP) group exhibited the lowest one. Moreover, group MP showed significantly higher fracture loads than group ZP (P=.015) and zirconia-layering (ZL) (P=.038) group. No significant difference in fracture strength was detected between groups MP and ML, and groups ZP and ZL (P>.05). Predominant fracture types were cohesive for metal groups and adhesive for zirconia groups. CONCLUSION. Fracture strength of a restoration with a metal or a zirconia framework was independent of the veneering techniques. However, the pressing technique over metal frameworks resisted significantly higher fracture loads than zirconia frameworks.
