Inter-ovarian differences in ultrasound markers of ovarian size in women with polycystic ovary syndrome

  • Objective: The aim of this study was to establish whether differences in ovarian size exist between the right and the left ovary of the same individual in women with polycystic ovary syndrome. Methods: In total, 206 Korean women with polycystic ovary syndrome were included in this study. In all participants, a transvaginal or transrectal ultrasound examination was conducted in the early follicular phase of the menstrual cycle. Results: A significant linear correlation was found between the two ovaries with regard to antral follicle count and ovarian volume. The mean antral follicle count in the right ovary (26.75 ± 11.72) was significantly higher than that in the left ovary (23.98 ± 10.85), and the mean volume of the right ovary (11.06 ± 5.17 cm3) was significantly different from that of the left ovary (9.12 ± 4.89 cm3). Conclusion: Ovarian size is different between the right and the left ovary in women with polycystic ovary syndrome.

한우(韓牛)에 발생(發生)한 Polycystic Kidney (Pathological Observations of Polycystic Kidney in a Korean Native Calf)

  • This paper dealt with the macroscopical and histopathological observations on the polycystic kidney occurred in a Korean native calf aged about a year. The results summarized are as follows: 1. In macroscopical findings, numerous cysts in the bilateral kidneys were seen under the renal capsule, and the cysts were various in size and clear or cloudy in their contents. The cysts in the inner area of the renal cortex were smaller than those of the outer area of the renal cortex in size and in number. 2. In microscopical findings, marked dilatations of Bowman's spaces and convoluted tubules were prominant feature. Numerous cysts with or without eosinophilic materials were contained atrophic glomerulus. prolferation of fibrous connective tissue, atrophy of convoluted tubules were also observed. Inner walls of the cysts were surrounded by cuboidal cells, sguamous cells or fibrous connective tissue. 3. This case was regarded as congenital polycystic kidney belonged to type III of Osathanondh and Potter.

성인 다낭종신 환자 1례에 대한 임상적 고찰 (Clinical Study on 1 Case of patient with Adult Polycystic Kidney Disease)

  • One case of patient with adult polycystic kidney disease was reported in this clinical study. After the study, the results were as follows: The adult polycystic kidney disease(APCK) is inheritant disease in which cysts are distributed throught the cortex and medulla of both kidneys. It belongs to Hematuria in oriental medicine field. For this patient, it was effective to appliment acupuncture Hap-kok, Kok-chi, Oe-gwan, Chok-samni, Chok-imup, Sam-umgyo, Su-bun, Tae-chung and Hae-gye showed a great effect on decreasing the pain and increasing the volume of urine. For this patient, it was effective to dispense Gamiwuiryungtang(加味胃苓湯), Ohryungsan(五苓散) and Chungsimyeonjaeum(淸心蓮子飮), Dosubokryungtang(導水茯苓湯).

다낭성 난소 증후군의 한의학적 치료에 대한 국내 임상 연구 고찰 (A Review of the Clinical Study Trends on Korean Medicine Treatment for Polycystic Ovary Syndrome)

  • Objectives: The purpose of this study is to analysis the Clinical Study Trends on Korean Medicine Treatment for Polycystic Ovary Syndrome. Methods: The key words such as 'polycystic ovary syndrome', 'polycystic ovarian syndrome', 'korean medicine', 'herbal medicine', 'acupuncture' are used for the research through 'OASIS', 'KCI', 'KISS', 'RISS'. Results: Ten clinical studies with 73 patients were selected. All studies were the noncomparative studies, and 8 case reports, 1 case series, 1 retrospective chart review. Symptoms accompanied by PCOS were oligomenorrhea, amenorrhea, irregular menstruation, hirsutism, acne, obesity etc. The most used treatment was herbal medicine in all studies with 72 patients (98.6%). The most frequently used acupuncture and moxibustion point was respectively 合谷 (LI4) and 關元 (CV4). The duration of treatment was between 3 months and 10 months, the average 159 days. Outcome measurements were Recovery of menstrual cycle (97.6%), Hormone test (23.8%), Sonogram (61.9%), Indexes of obesity (23.8%). In terms of menstrual recovery, 35 patients (85.4%) have recovered from menstruation in 8 studies with 41 patients. In hormone tests, all 10 patients have decrease of LH/FSH ratio and LH in 3 studies with 10 patients. Through sonogram, Ovulation menstruation was confirmed in all 6 patients. In all 10 patients of 3 studies evaluating weight before and after treatment, weight was reduced. Conclusions: The effect on Korean Medicine treatment for Polycystic Ovary Syndrome had mostly positive results. However, Further large, well-designed clinical trials are needed to establish the foundation of Korean Medicine treatment for Polycystic Ovary Syndrome.

Mouse models of polycystic kidney disease induced by defects of ciliary proteins

  • Polycystic kidney disease (PKD) is a common hereditary disorder which is characterized by fluid-filled cysts in the kidney. Mutation in either PKD1, encoding polycystin-1 (PC1), or PKD2, encoding polycystin-2 (PC2), are causative genes of PKD. Recent studies indicate that renal cilia, known as mechanosensors, detecting flow stimulation through renal tubules, have a critical function in maintaining homeostasis of renal epithelial cells. Because most proteins related to PKD are localized to renal cilia or have a function in ciliogenesis. PC1/PC2 heterodimer is localized to the cilia, playing a role in calcium channels. Also, disruptions of ciliary proteins, except for PC1 and PC2, could be involved in the induction of polycystic kidney disease. Based on these findings, various PKD mice models were produced to understand the roles of primary cilia defects in renal cyst formation. In this review, we will describe the general role of cilia in renal epithelial cells, and the relationship between ciliary defects and PKD. We also discuss mouse models of PKD related to ciliary defects based on recent studies.

Importance of family segregation in the American College of Medical Genetics and Genomics and Association of Molecular Pathology guidelines: Case of a Korean family with autosomal dominant polycystic disease

  • Since the American College of Medical Genetics and Genomics and Association of Molecular Pathology published their guidelines in 2015, most interpretations of genetic tests have followed them. However, all variants have only limited evidence along 28 interpretation standards, especially de novo variants. When de novo variants, which are classified as variants of uncertain significance (VUS) due to lack of evidence, are detected, segregation in the affected family could provide an important key to clarifying the variants. Autosomal dominant polycystic kidney disease is the most common inherited kidney disorder with pathogenic variants in the PKD1 or PKD2 genes. We detected a novel in-frame deletion variant in the PKD1 gene, c.7575_7577del (p.(Cys2526del)), which was interpreted as a VUS. We analyzed this variant in a Korean family to decide for segregation. Here, we report the variant as a likely pathogenic variant based on the evidence of segregation in three affected relatives and two unaffected members.

  • Polycystic ovarian syndrome (PCOS) is the most common endocrine disorder in women, which is characterized by the oligo/anovulation, hyperandrogenism (HA) and polycystic ovarian morphology which are diagnostic criteria. PCOS has diverse clinical aspects in addition to those diagnostic criteria including increased risk for cardiovascular diseases, metabolic syndrome, dyslipidemia, type 2 diabetes and impaired fertility. Because of the heterogeneity of the disease, the pathogenesis of the disease has not been elucidated yet. Therefore, there is no cure for the endocrinopathy. HA and insulin resistance (IR) has been considered two major pillars of the pathogenesis of PCOS. Recent advances in animal studies revealed the critical role of neuroendocrine abnormalities in developing PCOS. Several pathways related to neuroendocrine origin have been investigated such as hypothalamus pituitary ovarian axis, hypothalamus pituitary adrenal axis and hypothalamus pituitary adipose axis. This review summarizes the current knowledge about the role of HA and IR in developing PCOS. In addition, we review the results of recent genome wide association studies for PCOS. This new perspective improves our understanding of the role of neuroendocrine origins in PCOS and suggest a novel potential therapeutic target for the treatment of PCOS.

Estradiol Valerate로 유발된 다낭성 난소 증후군의 한약치료에 대한 국내 실험연구 고찰 (Review of Experimental Studies on Estradiol Valerate-induced Polycystic Ovarian Syndrome in Herbal Medicine)

  • Objectives: To observe the effects of Herb medication on Polycystic Ovary (PCO) in rats. Methods: Riss (Research Information Sharing Service) was searched for Oriental medicine about PCO on 2000~2014. So, Experimental studies using PCO model induced by Estradiol Valerate (EV) was selected to analyze how effects they were. Consequently, 10 studies published on 2000~2014 were selected to analyze about weight of model and both ovaries, the number aries, the number of follicle and corpus of follicle and corpus luteum, the level of serum Androstenedione (ADD) and total estrogen, the level of Nerve Growth Factor (NGF) and Corticotropin Releasing Factor (CRF), the crossover rate and the number of implantation. Results: Among them, there was significantly difference between treatment group and control group in weight of both ovaries, the number of follicle and corpora lutea, the crossover rate and the number of implantation. Conclusions: The effects of Herb medication was observed on making weight of ovaries with PCO to promote. The study about effect of Herb medication on NGF and CRF with PCOS had to be conducted. The further research have to focus on treatment of subfertility and infertility, that is field showing the advantage of Oriental medicine.

다낭성 난소 증후군 환자에서 Isoflavone 섭취에 따른 혈중 생화학적 지표의 변화 (The Effect of Isoflavone Supplementation on Plasma Biochemical Parameters of Women with Polycystic Ovary Syndrome)

  • Objective: This study was performed to evaluate the effect of isoflavone supplementation on hormone levels, lipid profiles and total antioxidant status in patients with polycystic ovary syndrome. Methods: Total 11 women with polycystic ovary syndrome were supplemented daily with 150mg of isoflavone for 6 months. Blood samples were collected 0, 3, and 6 months after supplement of isoflavone for analysis of LH, FSH, E2, testosterone, free testosterone, SHBG levels, serum lipid profiles, and total antioxidant status (TAS). Results: After 6 months isoflavone supplementation, the hormone levels did not change significantly. Serum lipid profiles did not show any significant change in total cholesterol, LDL-cholesterol, triglyceride, lipoprotein(a), and free fatty acid levels. However, there was significant increase in HDL-cholesterol (p<0.05) for 3 months. Total antioxidant status was increased significantly after isoflavone supplementation for 6 months (p<0.05). Conclusion: Isoflavone supplementation showed positive effects on the HDL-cholesterol and total antioxidant status. it is implicated that isoflavone supplementation will may have a effect on cardiovascular disease in patients with polycystic ovary syndrome.
